The kinetics of the body's center of gravity during level walking were analyzed in 50 normal subjects and 47 patients. The three-dimensional displacements of the center of gravity were computed by the integration of force plate data. The energy levels and the power requirements of the center of gravity were also calculated, and the average and standard deviation of these variables were determined for normal and pathological gaits. The sex-related variation in normal gait, as suggested by previous force plate studies, was clearly demonstrated in our study. The parameters obtained from the displacements and the energy variations of the center of gravity are considered useful in the evaluation of stability and efficiency for pathological gaits.
A 31-year-old patient, with a relatively uncommon benign osteoblastoma of the sixth cervical vertebra, exhibited symptoms of spinal cord compression. Computed tomographic metrizamide myelography was very useful in the preoperative planning, particularly for locating the position and extent of the lesion. The tumor was totally removed via posterior and anterior approaches in a two-stage operation. Roentgenograms, taken 1 year and 6 months postoperatively, showed good bony union with no evidence of tumor recurrence.
The mode of action of the inhibitory effect of CPZ on 13,14-dihydroprostaglandin F2-alpha (13,14H2-PGF2-alpha) formation in rat ovary was examined. The inhibition of 13,14H2-PGF2-alpha formation and of ovulation induced by a proestrus were completely recovered by an injection of hCG (25 IU/rat) or LH-RH (500 ng/rat) at 15:00 on the same day. 13,14H2-PGF2-alpha formation and ovulation were not inhibited by a single injection of prolactin (PRL:6 IU/rat) at 13:00 on the day of proestrus. Repeated injection of PRL inhibited cyclic ovulation and 13,14H2-PGF2-alpha formation. The estrus cycle of PRL treated animals showed a continuous state of diestrus. Although 13,14H2-PGF2-alpha formation and ovulation were inhibited by the repeated injection of CPZ, the repeated-simultaneous injection of CPZ and bromocriptine at 10:00 once a day for 3 days from the first day of diestrus partly restored both and entirely reversed the suppression of the cyclic-changes in the in the vaginal smear pattern. These results indicate that the inhibition of 13,14H2-PGF2-alpha formation induced by a single injection of CPZ probably occurs via the suppression of LH-RH release from the hypothalamus, whereas PRL secretion may participate in the inhibitory effects of repeated injections of CPZ.
We synthesized the 13S mRNA-encoded protein of the early region 1a (E1a) of human adenovirus in Saccharomyces cerevisiae under the control of the yeast GAL7 gene promoter. Similar to the case in HeLa cells, the E1a protein in yeast was phosphorylated and formed multiple bands on sodium dodecylsulfate-polyacrylamide gel electrophoresis. These bands migrated more slowly than expected from the Mr calculated on the basis of the nucleotide sequence of the gene. Synthesis of the E1a protein caused induction of a specific family of heat-shock proteins (Hsp70), which, however, did not confer heat resistance to the yeast. In addition, the E1a production resulted in an elongation of the generation time of yeast from 2.4 h to 3.9 h, which was attributed specifically to elongation of the G1 interval in the cell cycle. In the light of these findings, we suggest that the E1a protein synthesized in yeast exerts a specific function.
A system for CBF measurement using an H215O autoradiographic method and positron emission tomography (PET) has been designed and installed as a clinical tool. Following an intravenous injection of H215O, a radioactivity accumulation in the brain tissue for 60 s and a continuous record of radioactivity in arterial blood were measured by a high counting speed PET device and a beta-ray detector, respectively, and CBF was calculated by a table-lookup procedure. First, this method was compared with the C15O2 inhalation steady-state method on 17 cerebrovascular disease patients and four normal subjects. The two values for CBF agreed with each other when H215O autoradiographic method was applied by correction for the dispersion in the measured arterial radioactivity-time curve. However, without the correction, the CBF by the H215O autoradiographic method revealed substantial overestimation by 30.6 +/- 17.5%. A reduced gray/white ratio of CBF was also observed in the H215O autoradiographic method. Second, simulation was performed in order to determine optimal accumulation time by PET scan; the result was that errors due to dispersion and time mismatch became critical as the accumulation time was shortened to less than 60 s.
Two hundred and forty-four patients with various subtypes of DSM-III anxiety disorder were found among 3,059 outpatients who visited our clinic consecutively for evaluation for five years. They included 53 patients with panic disorder comprising an outstanding number of patients, about a quarter of patients with anxiety disorder and 1.7% of the whole outpatient population, and 78 with generalized anxiety disorder. Differentiation between these two groups was difficult to make besides their specific clinical features whether the patient had panic attacks or chronic generalized anxiety. The patients with anxiety disorder were divided into two major groups--panic-generalized anxiety-simple phobia and social phobia-obsessive compulsive-agoraphobia--in respect to their age of onset of the illness, the duration of episode, separation from their parents, psychosocial stressors and response to pharmacotherapy. Social phobia and obsessive compulsive disorder comprised those patients with similar qualities to each other in terms of their demographic data and their social backgrounds, forming a distinct group apparently different from the panic-generalized anxiety group. They consisted predominantly of young male patients with an earlier onset of the illness, less separation experienced and a longer duration of the episode, while without meaningful psychosocial stressors preceding the present episode. These findings indicate that the prevalence of panic disorder in the Japanese population is as high as previously reported, although its discriminant factors from generalized anxiety disorder are ambiguous except for panic attacks.
When Saccharomyces cerevisiae cells grown at 23 degrees C were transferred to 36 degrees C, they initiated synthesis of heat shock proteins, acquired thermotolerance to a lethal heat treatment given after the temperature shift, and arrested their growth transiently at the G1 phase of the cell division cycle. The bcy1 mutant which resulted in production of cyclic AMP (cAMP)-independent protein kinase did not synthesize the three heat shock proteins hsp72A, hsp72B, and hsp41 after the temperature shift. The bcy1 cells failed to acquire thermotolerance to the lethal heat treatment and were not arrested at the G1 phase after the temperature shift. In contrast, the cyr1-2 mutant, which produced a low level of cAMP, constitutively produced three heat shock proteins and four other proteins without the temperature shift and was resistant to the lethal heat treatment. The results suggest that a decrease in the level of cAMP-dependent protein phosphorylation results in the heat shock response, including elevated synthesis of three heat shock proteins, acquisition of thermotolerance, and transient arrest of the cell cycle.
Two adult cases of membranoproliferative glomerulonephritis (MPGN) associated with chronic hepatitis B were reported. Serum HBsAg, HBeAg and anti-HBc were positive. Glomerular changes were essentially the same in both patients and consistent with MPGN type III. Immunofluorescence microscopy revealed diffuse granular and lumpy deposits of IgG, IgA, IgM, C1q and C3 along glomerular capillary walls and in mesangial areas. Granular deposition of HBsAg was observed along capillary walls and in mesangial areas, and the staining patterns were similar to those of immunoglobulins and complements in both patients. Glomerular deposition of HBeAg, however, was negative in one case, and only slight and segmental in the other case. These findings suggest that HBsAg rather than HBeAg may play a role in the pathogenesis of MPGN associated with hepatitis B virus infection in adults.
The effect of the antiplatelet agents, ticlopidine and dipyridamole, on nephrotoxic serum nephritis (NTN) was evaluated in rats. An accelerated form of NTN was induced with preimmunization of rabbit IgG 7 days before injection of rabbit antirat nephrotoxic serum. Twelve animals received a peroral dose of 20 mg of ticlopidine daily, and 12 animals received a peroral dose of 10 mg of dipyridamole twice daily for 7 days. It was shown that both ticlopidine and dipyridamole were able to significantly reduce urinary protein excretion. There was, however, no significant difference in glomerular changes between treated animals and nontreated controls. Ticlopidine and dipyridamole were found to suppress the development of proteinuria in accelerated NTN. Although the antiaggregative action of dipyridamole was relatively weak in vivo, dipyridamole was found to be rather effective on protein excretion in the present study. These results support the significant role of platelets in the development of proteinuria in glomerulonephritis, and it is suggested that dipyridamole may have an antiproteinuric effect other than inhibition of platelet aggregation.
We have evaluated the effect of the antihyperlipidemic agent probucol on hyperlipidemia in patients with nephrotic syndrome. Twelve patients with long-standing nephrotic syndrome received 500 mg of probucol daily for 12 weeks. Serum total cholesterol, triglyceride, HDL-cholesterol and LDL-cholesterol were significantly lowered with probucol treatment. There were no differences in urine protein, serum total protein, serum albumin and renal function before and after probucol treatment. No drug-related side effects were observed during our study. These results indicated that probucol was effective against hyperlipidemia and free from side effects in patients with persistent nephrotic syndrome. The use of probucol is therefore suggested to be advisable when antihyperlipidemic treatment is required in some subgroups of nephrotic syndrome.
