[Selective percutaneous thermocoagulation of the glossopharyngeal nerve in intractable glossopharyngeal neuralgia].

Percutaneous radiofrequency thermocoagulation of glossopharyngeal nerve at the jugular foramen was employed for the treatment of intractable glossopharyngeal neuralgia in three cases, one with essential and two with symptomatic pain from malignant tumor of the oropharyngeal area. Under radiological control, the thermocoagulation electrode was inserted through the lateral cervical route, and the electrode reached the jugular foramen with the tip toward the pars nervosa. The correct position of the electrode was confirmed by radiography and by electrophysiological stimulation test. Once the electrode was properly positioned, lesion was made with temperature of 60 degrees C to 70 degrees C for 1 approximately 2 minutes. Surgical results were satisfactory and no neurological and cardiovascular complications were noted. Percutaneous radiofrequency thermocoagulation of glossopharyngeal nerve is thought to be very useful for the treatment of intractable glossopharyngeal neuralgia.

[Quantitative evaluation of myocardial blood flow and myocardial fluoro-deoxyglucose uptake determined by PET before and after aorto-coronary bypass operation in patients with ischemic heart disease].

Absolute myocardial blood flow (MBF) and myocardial FDG uptake (MFU) in the fasting state were determined in 5 patients who underwent aorto-coronary bypass operation, using O-15 water, F-18 fluoro-deoxyglucose and dynamic PET before and after the operation. In the patent graft region, MBF was increased from 0.59 +/- 0.17 ml/min/g to 0.77 +/- 0.14 ml/min/g (p less than 0.05). Mean MBF was increased from 0.69 +/- 0.22 ml/min/g to 0.83 +/- 0.18 ml/min/g (p less than 0.05). MFU in the fasting state was significantly decreased in high MFU region compared with low MFU region (p less than 0.005). Quantitative evaluation of MBF and MFU before aorto-coronary bypass operation was quite useful to determine adequate indication of the operation.

[A fast technique to estimate local cerebral blood flow and partition coefficient using dynamic PET of H(2)15O: a new approach to weighted integration method based on time integration of Kety-Schmidt equation].

We developed a new technique for fast estimation of local cerebral blood flow and partition coefficient based on time integration of Kety-Schmidt equation. This technique has two advantages concerning statistical noise. Firstly, it does not need a Ci(T) image which contains high statistical noise because of low activity in brain. Secondly, statistical noise is minimized by time integration of Kety-Schmidt equation before multiplied by weighting function. The error analyses in flow and partition coefficient caused by statistical noise were performed using computer simulation, and dynamic PET human study using H(2)15O bolus injection. Both simulation and PET study indicated that uncertainties of flow and partition coefficient were confirmed to be lower in this technique than in the original weighted integration method.

Maternal age, fertility and failure of teratogenicity by trypan blue in Jcl:SD rats.

The study compared the fertility of nulliparous Jcl:SD rats, young mature (12-week-old) versus middle aged (47-week-old) or old animals (64-week-old), both groups near the termination of their reproductive capability. Trypan blue (TB) was given subcutaneously at 0 (control), 10 or 50 mg/kg on days 7 through 9 of gestation to increase the malformation rate. However, no clear teratogenic action of TB was observed. The dose of 10 mg/kg TB was well tolerated, while 50 mg/kg was of severe maternal toxicity and 14 of 53 females died. In comparison with the young control group, the average per litter from middle aged or old dams was significantly decreased for implantation sites, live fetuses and fetal weight, while the percentages of pre-implantation loss and dead implants were extremely high. No increase of the malformation rate occurred with maternal aging. Histologically, middle aged and old rats exhibited age-related lesions of the pituitary, adrenals and reproductive organs with pituitary adenomas, cystic endometrium and sclerotic atrophy of uterine glands being significantly increased in non-pregnant, old rats.

[MR imaging of adenomyosis].

MR imaging characteristics of adenomyosis were studied in nine women (five diffuse type, four focal type) who underwent hysterectomy. In all cases, MR images were correlated with surgical/pathologic findings. On T2-weighted images, diffuse type appeared as a thickening of the junctional zone, because it was often isointense with this zone. Focal type appeared as a low signal intensity, round or oval shaped mass poorly marginated from the adjacent myometrium.

Calcification of spinal cord tumors: report of two cases.

Two cases of calcification of spinal cord tumors were detected by computed tomography and magnetic resonance imaging. The first; a 27-year-old male, with a very rare astrocytoma accompanied by calcification of the thoracic spinal cord, and the second; a 62-year-old female, with a rare calcified neurilemmoma in the upper cervical extradural space. The purpose of this paper is to draw attention to the fact that some spinal cord tumors occasionally contain areas of calcification, and review the literature on this subject.

Localization of wheat-germ agglutinin-binding sites in the Golgi complex of cultured rat atrial myocytes.

In the Golgi region of cultured rat atrial myocytes, condensed secretory protein was seen in Golgi-associated tubules or cisternae which lay beyond, and often separated from, the remainder of the Golgi stacks. These structures appeared to be involved in packaging of condensed secretory protein into atrial granules. Binding sites of HRP-conjugated wheat-germ agglutinin (WGA) in saponin-treated cultured atrial myocytes were examined by electron microscopy with special reference to atrial granules and the tubular structures associated with the Golgi stacks. HRP reaction products were observed in both trans-cisternae of the Golgi stacks and the associated tubular structures. While the majority of atrial granules were devoid of reaction products, some granules, which were connected to the WGA-positive tubular structures in the vicinity of the Golgi trans-cisternae, showed HRP reaction products at their connected necks. Similar results were obtained when sections of the cells embedded in Lowicryl K4M were labeled with WGA coupled to colloidal gold (G-WGA); the Golgi complex was G-WGA positive, whereas no specific binding of G-WGA to atrial granules was observed. These results suggest that glycoproteins and/or glycolipids with oligosaccharides recognized by WGA in the Golgi transcisternae, may be separated from atrial natriuretic peptides which are packaged into atrial granules.

Atlanto-axial subluxation in Down's syndrome.

Atlanto-axial subluxation, which was causing symptoms in eight patients with Down's syndrome, was treated by operation. In every case various congenital bony anomalies were found. Marked myelopathy was present in four cases of ossiculum terminale, and mild myelopathy in one with anteroposterior spondyloschisis. Successful posterior arthrodesis was performed from C1 to C2 in six cases, and from the occiput to C2 in the other two patients with anteroposterior spondyloschisis. Excellent bony union and an overall improvement in symptoms was achieved. The follow-up period was from one year to five years and six months. Early surgical stabilization is the treatment of choice in those cases of symptomatic atlanto-axial subluxation in Down's syndrome where there are congenital osseous anomalies and generalised ligamentous laxity, and where the spinal cord is at risk.

Instrumentation surgery for primary tumors of the spine.

We performed instrumentation surgery on ten patients with the following rare primary spinal tumors; three solitary plasmacytomas, an aneurysmal bone cyst, a chordoma, a giant-cell tumor, a hemangioma, a solitary bone cyst, a benign osteoblastoma, and a chondrosarcoma. These tumors were found in the cervical spine (one case), thoracic spine (five cases), lumbar spine (two cases), and sacrum (two cases). The patients consisted of five men and five women, and their average age was 43.1 years. Harrington instrumentation was used in six cases, Zielke instrumentation in two, a combination of both in one case, and a Kirshner wire in one. In two cases, iliolumbar fusion was performed with Harrington instrumentation, after total resection of the sacrum. Average duration of the follow-up period was 5 years. This report describes these rare cases and emphasizes the effectiveness of instrumentation surgery in the reconstruction of the spine after extensive surgery.

Kaolin-induced writhing in mice, a new model of possible bradykinin-induced pain for assessment of analgesic agents.

Kaolin induced a clear and reproducible writhing reaction when intraperitoneally injected into mice. A simultaneous injection (i.p.) of soybean trypsin inhibitor (SBTI) significantly suppressed the kaolin-induced writhing reaction. This writhing reaction was markedly potentiated by a simultaneous injection (i.p.) of captopril. In an in vitro experiment kaolin caused kinin-release in mouse plasma, possibly through the activation of prekallikrein. This activation of plasma prekallikrein and kinin-release were inhibited in the presence of SBTI. Some non-steroidal anti-inflammatory agents inhibited the kaolin-induced writhing reaction dose-dependently. These results suggest that kaolin-induced writhing reaction may be caused by the released bradykinin through activation of the plasma kallikrein-kinin system. This model is a novel and simple tool for assessment of analgesic agents.

Cyclophosphamide, tetrahydropyranyl-adriamycin, and cis-platinum in the treatment of head and neck adenocarcinoma.

Twelve patients with advanced or relapsed head and neck adenocarcinoma received a combination chemotherapy regimen of either cyclophosphamide (C), tetrahydropyranyl-adriamycin (T), and cis-platinum (P) (CTP) or cyclophosphamide, adriamycin (A), and cis-platinum (CAP). Cyclophosphamide (300 mg/sq m), either etrahydropyranyl-adriamycin (30 mg/sq m) or adriamycin (30 mg/sq m), and cis-platinum (50 mg/sq m) were administered intravenously in a single day. Nine patients received the CTP regimen, and three patients, the CAP regimen. Prior to chemotherapy, five patients had received surgery or radiation therapy, and the other seven patients received no special treatment. A response rate 75% was achieved (9/12); there were 7 complete responses, whose duration was a mean 6.8 months, ranging from 2 to 18 months, and 2 partial responses, whose duration was 2 months. Virtually all patients experienced nausea and vomiting. Alopecia developed in 7 patients; however, the patients with the CTP regimen experienced less alopecia, if any. Leucopenia and anemia of either a slight or moderate degree were observed, but there was no patient for whom it was necessary to discontinue the treatment. Both CTP and CAP regimens appear to be of significant value in controlling head and neck adenocarcinoma.

A determination of the regional brain/blood partition coefficient of water using dynamic positron emission tomography.

In order to investigate the validity of the single compartment model in measuring CBF with the use of 15O-labeled water (H2 15O), dynamic positron emission tomography (PET) was performed following bolus injection of H2 15O. Careful attention was paid to accuracy in the measurement system (especially for the input function). In the region of the putamen, which includes the smallest mixture of gray and white matters in addition to the smallest contamination of cerebrospinal fluid (CSF) spaces, the partition coefficient obtained was 0.88 +/- 0.06 (ml/g). The discrepancy from the prediction estimated from the brain/blood water content ratio was only 7%. This finding suggests that there is no more complicated model than the usual single compartment one to describe the physiological behaviour of 15O water. On the other hand, in the other cortical regions, the discrepancy was larger (e.g., about 12% for the insular cortex and 26% for the frontal cortex) than in the region of the putamen, and a significant fit-interval dependence was observed in the calculated parameters. These observations suggest a significant effect of tissue heterogeneity and/or contamination with nonperfusable spaces in actual clinical PET data.

Surgical treatment for symptomatic spinal adhesive arachnoiditis.

This report concerns the outcome of 36 operations for spinal adhesive arachnoiditis (SAA) performed at Kyoto University Hospital since 1956; the follow-up period ranged from 1 to 25 years (average, 4 years 8 months). The original procedure involved intradural neurolysis after extradural decompression, but more recently microlysis was performed. The percentage of cases in which good results were obtained by nonmicroscopic lysis and microlysis was only 54.8%, however, when microlysis was followed by spondylodesis, the ratio increased to 80%, clearly showing the benefits of this new procedure. The authors therefore emphasize the value of spondylodesis following microlysis for symptomatic SAA.

Determinants of atrial natriuretic peptide secretion in cultured atrial myocytes.

Atrial natriuretic peptide (ANP) secretion by atrial myocytes in 7- to 8-day primary cultures from adult rats was measured by radioimmunoassay under conditions designed to separate primary effects on secretion from effects caused by contractions. Abolishing contraction with 10 microM tetrodotoxin significantly reduced ANP accumulated in 2 h. Raising external Ca2+ concentration from 0.2 to 1.2 mM in the presence of tetrodotoxin did not increase ANP secretion. Substantial ANP secretion persisted in a nominally Ca2+-free medium containing 10 mM ethylene glycol-bis(beta-aminoethyl ether)-N,N,N',N'-tetraacetic acid (EGTA) and was not diminished by 100 microM ryanodine. In the presence of EGTA, 100 nM 12-O-tetradecanoylphorbol 13-acetate (TPA) significantly increased ANP secretion; this increment was unaffected by 100 microM ryanodine. These experiments suggest 1) that in absence of contractions, ANP secretion requires neither transplasmalemmal Ca2+ influx nor ryanodine-inhibitable Ca2+ release by sarcoplasmic reticulum (SR) and 2) that TPA stimulates ANP secretion without requiring Ca influx or ryanodine-inhibitable SR Ca release.

Delivery of lectin-labeled membrane to the trans-Golgi network and secretory granules in cultured atrial myocytes.

To examine whether and how internalized plasma membrane components are routed to the compartment of the biosynthetic-exocytic pathway in cultured atrial myocytes, the plasma membrane labeled with wheat germ agglutinin conjugated to horseradish peroxidase (WGA-HRP) was traced electron microscopically by cytochemical detection of HRP. The WGA-HRP label was internalized via a coated pit-small vesicle pathway and reached vacuoles and endosomes by 3 min. Labeled endosomes comprised vacuoles and tubular elements containing reaction product. By 15 min, similar tubular structures containing reaction product accumulated in the area of the trans-Golgi network (TGN). The labeled TGN consisted of interconnected tubular elements, which often connected to atrial granules containing reaction product. In contrast, neither native HRP nor Lucifer Yellow reached Golgi elements or atrial granules. These results suggest that a proportion of the plasma membrane labeled with WGA-HRP is delivered to endosomes, from which tubules might bud off to transfer the tracer molecules to the TGN, where the lectin conjugate and associated membranes are packaged into atrial granules.

A new writhing model of factor XII activator-induced pain for assessment of non-steroidal anti-inflammatory agents. I. Kaolin-induced writhing in mice.

The writhing reaction in mice induced by kaolin, a factor XII activator, was studied. An intraperitoneal injection of kaolin clearly induced a writhing reaction in a dose-dependent fashion, and the reaction disappeared about 10-15 min later. The writhing reaction reached a peak at 5-10 min after the injection of kaolin (0.5 ml/mouse, i.p.; 5 mg/ml saline). A simultaneous intraperitoneal injection of soybean trypsin inhibitor (SBTI, 2.5 mg/mouse) almost completely suppressed the writhing reaction caused by kaolin (2.5 mg/mouse) for the first 10 min. The kaolin-induced writhing reaction was markedly potentiated by a simultaneous intraperitoneal injection of captopril (50 micrograms/mouse). At 60 min after kaolin injection during the disappearance of the writhing reaction, the reaction reappeared when captopril was injected, but reactions observed at this later stage were completely blocked by SBTI. Indomethacin, ibuprofen and alminoprofen inhibited the writhing reaction dose-dependently. Kaolin thus induces a clear and reproducible writing reaction, which might be mainly dependent on the action of bradykinin via activation of factor XII, and should prove to be a simple and convenient model of bradykinin-induced pain for the assessment of analgesic actions.

Pharmacological studies on EB-382, a new non-steroidal antiinflammatory agent: mode of action of the antiinflammatory effects.

The mode of action of (+/-)-2-[p-[(2-methylallyl)amino]phenyl]propionic acid (EB-382) was studied in terms of its antiinflammatory effect. EB-382 displayed a more potent inhibitory effect than that of ibuprofen on carrageenin-induced paw edema in rats, and its inhibitory action was unaffected by adrenalectomy. EB-382 had a less potent inhibitory effect than that of ibuprofen on the prostaglandin E2 biosynthesis of 3T6 mouse fibroblast cells. EB-382 displayed a much more potent inhibitory effect than that of ibuprofen on carrageenin-induced pleurisy in rats. EB-382 exhibited a dose-dependent and significant inhibitory effect on the bradykinin and prostaglandin contents released into the pleural cavity in rat pleurisy induced by carrageenin, and its potency was much higher than that of ibuprofen. EB-382 strongly inhibited the leukocyte emigration into the pleural cavity, and its potency was almost equal to that of indomethacin. EB-382 did not affect the kinin-forming enzyme and kininase activities of rat plasma in vitro. The above results suggest that the antiinflammatory effect of EB-382 was probably due to inhibition of the production of bradykinin and prostaglandins derived from emigrating leukocytes in the local inflamed tissue besides a weak inhibitory effect on prostaglandin biosynthesis.
