Diagnostic value of captopril test in hypertensive patients with renal artery stenosis.

To examine the utility of the single-dose captopril test in detecting renovascular hypertension (RVHT), the authors measured peripheral plasma renin activity (PRA), before and thirty and sixty minutes after an oral dose of captopril (25 mg), in 28 patients with RVHT and 22 patients with high-renin essential hypertension (EHT) without renal artery stenosis who were consuming 8 grams of sodium chloride per day. There was considerable overlap of individual values in basal PRA between the two groups. Sixty minutes after captopril, PRA was higher in RVHT than in EHT patients (74.8 +/- 63.9 versus 15.1 +/- 11.9 ng/mL/hr, P < 0.01). With the cutoff point set at 16 ng/mL/hr, RVHT was detected with a sensitivity of 96% and a specificity of 77%. The discriminating power was also superior to that based on blood pressure response to angiotensin II analogue under sodium depletion, rapid-sequence intravenous pyelography, or renography. These results show that captopril-stimulated peripheral PRA is an adequate screening tool for detecting RVHT in a population with high-renin hypertension.

Functional difference between Ad4BP and ELP, and their distributions in steroidogenic tissues.

Ad4BP, a zinc finger DNA-binding protein, was identified as a transcription factor regulating steroidogenic P-450 genes in a cAMP-dependent manner. Immunochemical and immunohistochemical studies with steroidogenic tissues, adrenal, ovary, and testis, were performed using the antiserum to Ad4BP. Ad4BP was expressed to the same extent in the three zones of the adrenal cortex. Immunohistochemical examination of ovarian follicle and corpus luteum showed the expression of Ad4BP. The granulosa and thecal cells, the two distinct types of the steroidogenic cells in the follicle, gave Ad4BP signals, which were stronger than in the latter cells than in the former. Immunoblot analyses of mature and regressed corpora lutea indicated a parallel expression of Ad4BP and side-chain cleavage P-450, and both proteins significantly decreased in the regressed tissues. Leydig cells surrounding seminiferous tubules gave clear immunostaining signals for Ad4BP. ELP, a mammalian counterpart of Drosophila FTZ-F1 detected in EC cells, and are isoforms transcribed from the same gene. The Ad4BP and ELP forms recognize same nucleotide sequences. Reverse transcriptase-polymerase chain reaction with specific primers for ELP revealed that steroidogenic tissues contained ELP as well as Ad4BP. The effects of the two proteins on the transcription of the CYP11B gene were compared using the expression vectors of Ad4BP and ELP. ELP did not activate transcription and showed a weak inhibitor effect on the Ad4BP-dependent transactivation of the CYP11B gene promoter when transfected simultaneously. A gel shift analysis using in vitro synthesized Ad4BP and ELP revealed that the binding activity of ELP is significantly weaker than that of Ad4BP.

Lung retention of Pu following inhalation of PuO2 in rats measured using a whole body counter.

A lung retention function on the amount of PuO2 in rats was determined. Five rats were exposed to polydisperse aerosols of PuO2 having a size of 0.47 micron activity median aerodynamic diameter. The initial lung burden was between 1990 Bq and 2960 Bq. Instead of serial-sacrifice study, in vivo counting of low energy L X-rays with thin NaI(Tl) scintillation detectors was used to follow the lung retention of Pu at various intervals up to 468 days after inhalation. The calibration of this counting system was made by measuring lung activity of rats sacrificed for other experimental purposes. It was confirmed that the potential skin contamination and the Pu translocated to the other organs was not counted at the present counting geometry. Our results showed that 77% of PuO2 deposited deep in the lung was cleared with a half-time of 53 days, whereas the residual 23% stayed there for a longer period (a half-time of 795 days).

High incidence of malignant lung carcinomas in rats after inhalation of 239PuO2 aerosol.

Female Wistar strain rats were exposed to a single inhalation of a submicron-size aerosol of high-fired 239PuO2 to investigate pulmonary carcinogenesis during lifespan periods. The absorbed lung doses of the exposed animals ranged from 0.6 to 12 Gy and were well correlated with the initial lung deposition (ILD) of 0.1 to 2.3 kBq. Survival and induction of primary lung tumors in 116 exposed rats were compared with those in 56 untreated control rats in respect to lung doses received. Mean survival time was greatly reduced, and the cumulative incidence of total lung tumors was markedly increased to 90-100% in rats that received more than 4 Gy, whereas of the controls only one animal (1.8%) died of primary lung tumors. Primary but benign adenomas were present in exposed animals given 1.0 Gy or less, and the incidence of adenomas was 22-25% at 4-5 Gy, but decreased sharply to 3-5% at 6-8 Gy. In contrast, no malignant carcinomas, including adenocarcinomas, adenosquamous carcinomas and squamous cell carcinomas, developed at a dose of less than 1.0 Gy, whereas they were present in 75% or more of the rats given 4-10 Gy, but only in 55% at 12 Gy. Although there were no clear differences in the dose and time required for induction among the carcinoma types, all tended to develop in earlier periods after inhalation than adenomas. Despite the limited number of exposed animals that received lower doses, results suggest that malignant lung carcinomas are highly and early induced and have a different dose-effect relationship than benign adenomas at doses of more than 1 Gy after inhalation exposure to 239PuO2.

Differential induction of bone and hematopoietic tumors in C3H mice after the injection of 239Pu citrate.

Although alpha-emitting plutonium is easily distributed in the skeleton via circulation and subsequently induces bone tumors, there is little evidence that hematopoietic neoplasias are highly induced even though bone marrow stem cells are irradiated internally by alpha particles. We injected groups of female C3H strain mice with doses of 239Pu citrate from 500 to 10000 Bq to investigate the dose-related spectrum of tumor types induced during a lifetime. Survival time was reduced strikingly in all the injected mice due to much earlier induction of bone and lymphoid tumors as compared to the control animals that showed a variety of soft tissue tumors after a longer period of survival. Induction of osterosarcomas was dose-dependent, being maximal in 70% of the animals that received a mean skeletal dose of 10 Gy or less, but was 48% or less at 20 Gy or more. Non-thymic lymphomas accompanied by lymphocytic leukemia were observed in only 4-6% of the animals that received a dose of 10 Gy or less whereas it was maximal in 17-19% at 20 Gy or more. In contrast, there were no bone tumors in the control animals, rather thymic lymphomas or histiocytic lymphomas were found very late in 20% and other soft tissue tumors, including lung, liver and ovary tumors, were noted in 60%. Neither myeloid leukemia nor other myelogenous neoplasias were found in the control and 239Pu-injected animals that received a mean skeletal dose of 3 Gy or more. These results indicate that the differential induction of bone tumors and hematopoietic tumors in mice depends on the dose range and the time after the injection of plutonium.

[Endo-tracheal intubation technique in rats using a guide wire--application for trachea transplantation].

Sometimes endo-tracheal intubation on rats is needed for experiments on the chest, but usually it is difficult. A new oral intubation technique on rats was developed. Under pentobarbital anesthesia, a tracheostomy was carried out, and a smooth coated guiding wire was inserted retrogradely up to the mouth. The tube was then inserted into the trachea from mouth along the guiding wire. This technique is applied to trachea transplantation, and subsequently reduced operative mortality markedly from 54% to 2%.

Changes in histomorphometric values of iliac trabecular bone and serum biochemical constituents related to bone metabolism in beagle dogs during growth.

Changes in histomorphometric values of iliac trabecular bone and serum biochemical constituents related to bone metabolism in normal beagle dogs during growth were examined. A total of 90 dogs (male; 46, female; 44) aged 3, 6, 9, 12 and 24 months, were used. Each dog was injected with tetracycline hydrochloride and calcein for double bone labeling in order to quantitatively measure dynamic as well as static parameters in bone histomorphometry and a part of the ilium was later removed. The measurements on cancellous bone areas of undecalcified iliac sections were performed with a semiautomatic image analyzer. There were no sex significant differences in histomorphometric values except for mineralization lag time at the age of 3 months. In the static parameters, bone volume and mean trabecular thickness increased from 3 to 9 months of age. Bone surface area and mean osteoid thickness increased up to 6 months. Osteoid volume and osteoid surface area decreased up to 6 months of age. As to the dynamic parameters, the mineral apposition and bone formation rates decreased from 3 to 12 months of age. Mineralization lag time increased from 3 to 9 months of age. However, high bone surface area values in both sexes and low osteoid thickness values in females were seen at the ages of 9 and 12 months. Low values of trabecular thickness in both sexes, osteoid thickness and mineralization lag time in males and mineral apposition rate in females were also noted at the age of 12 months. Serum total calcium did not show any specific tendency.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case report of primary fibrosarcoma originated in the heart invading adjacent organs ].

A rare case of cardiogenic fibrosarcoma is reported. A 66 y.o. male with dyspnea, cough and fatigue was examined. B-mode echo, MRI, and angiography disclosed a large tumor in the left atrium. A large fibrosarcoma and 2 cm of the left atrial wall around the tumor pedicle were removed. The atrial defect and right lower pulmonary vein were repaired with a patch. Four months later, the tumor recurred in the right lung, mediastinum, and left atrium. The tumor, middle and lower lobes of the right lung, and part of the left atrial wall were excised. One year after the first operation, the patient died of air way obstruction due to another recurrence of the fibrosarcoma in the left lung, esophagus, and mediastinum. Cardiogenic fibrosarcoma invading outward for the mediastinal organs is rare.

[Coronary artery bypass grafting in a patient with right aortic arch associated with retroesophageal anomalous left subclavian artery].

A rare case of coronary artery bypass grafting in a patient with right aortic arch was presented. A 62-year-old man was admitted to our hospital with angina pectoris. Coronary angiography revealed total obstruction of left anterior descending artery and jeopardized collateral from right coronary artery. Aortography disclosed right aortic arch associated with retroesophageal anomalous left subclavian artery. As the symptom of neither the vascular ring nor the subclavian steal syndrome was recognized, left internal thoracic artery along with the saphenous vein was used for bypass conduit. The patient is doing well without any troubles 7 months after the operation. Attention should be made to use left internal thoracic artery in a patient with right aortic arch.

[A case report of surgical repair of a ruptured aneurysm of the sinus Valsalva in an aged person with cardiac cachexia].

A 71-year-old woman underwent surgical repair of a ruptured aneurysm of the sinus Valsalva. She had an aneurysm of the right coronary sinus ruptured into the right atrium. There was no VSD. The patient suffered from heart failure for 38 years and went into cardiac cachexia. Direct closure from the right atrium was performed and the patient has resumed full physical activities. As far as we know, this is one of the oldest patients who underwent successful surgery for this disease in this country.

[Longitudinal analysis of glucose metabolism in recurrent meningioma].

We repeatedly measured kinetic rate constants and glucose metabolic rate (kinetic rCMRG1) using dynamic positron emission tomography (PET) and autoradiographic rCMRG1 in a patient with recurrent meningioma. A 50-year-old woman who presented with a left visual disturbance was admitted to our hospital. MR images revealed a mass lesion occupying the left middle fossa. The patient underwent Simpson grade IV surgery. The histological diagnosis was meningothelial meningioma. One year later the tumor had grown back to almost the same size as before treatment and was removed again by Simpson grade IV procedure. Postoperatively, the patient underwent radiation therapy (54 Gy). Two years after the second operation, the tumor was found to have invaded the left orbit and was resected by Simpson grade IV procedure. After additional radiation therapy, the patient was discharged. The rate constants were analyzed preoperatively and whenever the tumor recurred according to the three compartment 18F-fluorodeoxyglucose (FDG) model. Preoperative PET indicated tumor k1 and k2 values higher than in the contralateral gray matter, suggesting high permeability due to absence of the blood-tumor barrier and an abundant blood supply. The tumor k3 value, an indicator of hexokinase activity, was as high as in the contralateral gray matter. When the tumor recurred, the tumor k1, k2 and k3 values remained consistently high, indicating high proliferative activity. In contrast, the contralateral gray matter k1, k2 and k3 values decreased to some extent, suggesting effects of surgery or radiotherapy. Tumor rCMRG1 values, both autoradiographic and kinetic, were enhanced markedly.(ABSTRACT TRUNCATED AT 250 WORDS)

Quantitative mapping of regional cerebral blood flow using iodine-123-IMP and SPECT.

UNLABELLED: A method was developed to calculate functional images of regional cerebral blood flow (rCBF) from a single scan using SPECT following intravenous 123I-N-isopropyl-p-iodoamphetamine (IMP) infusion. METHODS: A two-compartment model that includes two parameters of rCBF and regional distribution volume of IMP (Vd) was employed to correct for clearance of IMP from the brain. Using a given input function and a fixed Vd value (30 ml/ml according to an analysis on dynamic SPECT data), a unique value of rCBF was calculated for each pixel of the SPECT image according to the table-look-up procedure. This technique was applied to 15 human subjects, and the calculated rCBF values were compared with those measured by PET. RESULTS: A set of simulation studies demonstrated an optimal SPECT midscan time at 30 to 40 min postinjection of IMP, providing the minimal error sensitivity to the individual difference of the input function (rCBF values with an accuracy of +/- 10%). Another set of simulation suggested validity of fixing the Vd values, i.e., errors in calculated rCBF values were around +/- 7% for a change of Vd of +/- 10%. The measured rCBF values obtained from 15 human subjects were independent on the SPECT scan time. The calculated rCBF values also agreed well with those obtained by the nonlinear least-squares fitting analysis that were obtained from the dynamic SPECT scan and the frequent arterial blood sampling and measurement of lipophilic fraction for each sample (0.54 + 0.88x, r = 0.86), suggesting the validity of the simplified procedures in this method. CONCLUSION: These observations suggested the validity of this method as a clinical tool for quantitative measurement of rCBF.

[Secondary leukemia after etoposide treatment involved MLL gene rearrangement].

The 11q23 chromosomal abnormality is frequently observed in infantile leukemia and secondary leukemia, and the translocation associated gene in infantile leukemia is called mixed-lineage leukemia (MLL) gene. A 50-year-old man was admitted because of left axillary lymphadenopathy and IBL like T cell lymphoma was diagnosed by lymph node biopsy. The patient responded to the LSG-9 protocol with complete remission. After 10 months he was readmitted because of fever and was diagnosed acute myeloblastic leukemia by bone marrow aspiration. Chromosome analysis revealed 11q23 abnormality, suggesting that the leukemia was induced by etoposide treatment. Southern blot analysis demonstrated DNA rearrangement in the MLL gene at 11q23. It was suggested that the breakpoint region of the MLL gene in secondary leukemia is the same as that of infantile leukemia.

Rapid freezing replica studies of membrane specializations in the proximal tubules of mouse kidney.

The membrane specializations of the fresh unfixed kidney cortex of adult and neonatal ICR mice were examined by using rapid freezing replica methods. In proximal tubular cells, numerous apical intracellular tubules exhibited helical patterns on the E face with a pitch of about 12 nm. This regular pattern was often continuous with similar striped indentations on the edge of the vacuoles connecting with the tubules. On the luminal surface (ES) of these vacuoles, membrane surface particles were arranged regularly in striped patterns with a center-to-center spacing of about 12 nm. We could not identify differentiations on the PF or PS of the same membrane systems. Another membrane specialization was a plaque or patch of clear pits in tilted lattice alignments on the P face of the large vacuoles with a center-to-center spacing of about 20 nm. This type of specialization was often observed in the neonatal mice proximal tubular cells. These membrane specializations may indicate the active membrane functions in the proximal tubules and suggest the functional continuity and structural relationship of these apical endocytic membrane systems.

A case of renal cell carcinoma with metastasis to the thyroid gland and concomitant early gastric cancer.

Metastatic lesions from renal cell carcinoma are commonly recognized in the lung and bone, however, thyroid metastasis has rarely been reported in the literature. We present herein a case of a man with lung and thyroid metastatic tumors from renal cell carcinoma. Although making a preoperative diagnosis of thyroid metastasis is difficult because there is no established diagnostic criteria, we were able to identify this thyroid lesion as a metastatic tumor from renal cell carcinoma by the pathological findings. Interestingly, hypercalcemia was recognized in this patient whose serum parathyroid hormone (PTH) level was not increased. Moreover, this patient also had early gastric cancer simultaneously, being a so-called 'double cancer'. After surgical resection of the right kidney and stomach, interferon was administered without any efficacy against the metastatic lesions of the lung and thyroid gland.

Characterisation and localisation of a 67 kDa calcium binding protein (p67) isolated from bovine hearts.

OBJECTIVE: A 67 kDa Ca2+ binding protein (p67) from bovine hearts has recently been purified. It bound Ca2+ in vitro and localised at the sarcolemma of both striated and smooth muscle cells. In this study, further biochemical and morphological experiments were carried out to examine whether the p67 is a member of the annexin family of proteins and how the protein is associated with the sarcolemma membrane. METHODS: Phospholipid and biological membrane binding assays were used to determine if p67 has the properties of the annexin family of proteins. Extraction experiments with Triton X-114 and Na2CO3 were carried out to examine potential association of p67 with sarcolemma membrane vesicles isolated from bovine hearts. For characterisation of the protein, actin binding assays and immunoelectron microscopy were performed. RESULTS: The p67 bound to acidic phospholipids and biological membranes in a Ca2+ dependent manner. The protein was exclusively recovered in the aqueous phase after Triton X-114 phase separation and it was released from sarcolemma membranes by Na2CO3 treatment. It bound to membrane cytoskeletons insoluble in Triton X-100 but failed to bind to F actin at physiological free Ca2+ concentrations. Immunoelectron microscopy showed that p67 seemed to be localised at the cytoplasmic side of the cardiac sarcolemma. CONCLUSION: The p67 that was purified from bovine hearts could be a member of the annexin family of proteins. It might be a peripheral membrane protein associated with the cytoplasmic side of sarcolemma membranes.
