[Experimental traumatic epiphysiolysis (author's transl)].

Two experimental series of epiphysiolysis, with and without displacement, were performed on 100 young rabbits in each group. The histological site of the epiphysiolysis was demonstrated, and the healing process studied. The basic healing process is induced by the vascular metaphyseal penetration of the separation line, as shown in the epiphysiolysis without displacement. A compensative mechanism appears in the series of epiphysiolysis with displacement, consisting of an epiphyseal vascular penetration of the epiphyseal plate, performing then the embryonal anatomical pattern. The vascular epiphyseal channels close progressively after they reach the metaphyseal vessel. If they do not join the metaphyseal vessels quickly enough, or in sufficient number, they are invaded by osteoblastic cells and fibrous or bony bridges will be created through the epiphyseal plate. These bridges are responsible for growth retardation, axial deviation and even early closure of the epiphyseal plate.

New clinical observations in the trichorhinophalangeal syndrome.

Two unrelated Jewish families are reported with new clinical and radiographic findings observed in the trichorhinophalangeal syndrome (TRPS). These new observations are mainly of a skeletal nature and emphasize the wide range of expressivity found in the TRPS. Furthermore, these bony defects along with the characteristic hair changes suggest that the basic defect in this syndrome involves some developmental alteration in the normal growth and maturation of bone and hair.

Rigid fixation in diastasis of symphysis pubis.

Two cases of Peltier's Type II C fracture of the pelvis without any internal organ involvement are presented. Lack of hemodynamic or respiratory complications and failure of conservative treatment enabled open reduction and internal fixation. The resultant rigid fixation allowed an early mobilization and contributed to the rapid recovery of these aged patients.

An unusual fracture of the base of the fifth metatarsal bone.

An unusual fracture of the base of the fifth metatarsal bone is described. The type of fracture and the displacement of the proximal fragment was unlike that of most other fractures of this region of the foot. It requires open reduction and internal fixation which was successfully carried out by means of tension wires.

Persistent dislocation of the shoulder in elderly patients.

Four elderly patients with longstanding unrecognized dislocation of the shoulder were treated by physiotherapy only. Good functional results, associated with a greatly increased range of pain-free motion, demonstrate the usefulness of conservative treatment in cases of persistent dislocation of the shoulder in the elderly.

Sequential use of technetium 99m MDP and gallium 67 citrate imaging in the evaluation of painful total hip replacement.

Fourteen patients with 20 total hip joint replacements were studied for 14 painful prosthetic hips. Clinical examination, plain film radiographs and 99Tcm-MDP bone scans failed to differentiate between infection and mechanical loosening of a prosthesis. Sequential use of 99Tcm-MDP and 67Ga-citrate bone scans were performed in an attempt to discover underlying infectious process. Increased focal uptake of both radiopharmaceuticals over the same hip indicated an infectious process responsible for prosthetic loosening. There were no false positive gallium examinations. Sequential use of 99Tcm-phosphate compounds and 67Ga-citrate is recommended for differentiation between mechanical loosening of a prosthesis and loosening of a prosthesis secondary to an infectious process.

Distal transfer of the greater trochanter in cox vara.

Seven patients (nine hips) who suffered from coxa vara were treated by distal transfer of the greater trochanter. The operation eliminated or markedly diminished the preoperative Trendelenburg sign, thus improving gait. In three patients (four hips) operated upon at the aged of 9 and 12 years, the initial coxa vara was converted into an almost normal valgus kemoral neck. There was one poor result due to technical failure.

Fracture healing in rabbits after osteotomy using the CO2 laser.

Fracture healing was studied in 28 rabbits after their femurs had been osteomized using the carbon dioxide laser. Twenty rabbits whose femurs were osteotomized by means of a Gigli saw served as the control group. Fracture healing was initially delayed in the laser cut femurs, yet after 60 days no significant difference between the groups could be detected. The initial delay was caused by thermal damage to the laser cut bone edges. Further fragmentation of the damaged bone occurred during the immediate postoperative period.

Leg lengthening in achondroplastic children.

Bilateral simultaneous elongation of the lower limbs was performed in 3 achondroplastic children. A total of 10 bones were elongated with Wagner's distraction apparatus, mainly by a modified Wagner technique. Maximal elongation of 34 to 44% of the original bone length resulted in relatively minor complications. Elongation of the lower limbs in achondroplastic children improves trunk-to-lower-limb proportions.

Subluxation of the hip joint during femoral lengthening.

Three cases of subluxation of the hip joint occurred during femoral lengthening using the Wagner technique. This infrequent complication, which may result from a conflict of force vectors at the hip joint during lengthening, can, when left untreated, nullify the benefits of such a procedure. We suggest that increased awareness and closer monitoring can improve the prognosis for patients undergoing this course of therapy, especially where a previous hip operation has been performed.
