[Maximal force measurements of lip and tongue pressures and their significance for the diagnosis of orofacial dyskinesias].

The functioning of the orofacial muscular system essentially determines the shape of the jaw and of the dental arch as well as the position of the axis of the anterior teeth. Disturbances of the normal functioning can cause anomalies of the position of teeth or malformations of the bone structure. With the help of the newly developed device "Myometer 160" we now have the possibility to measure the intra- and extraoral forces. We examined 107 persons aged 8 to 37 years. The maximal force was measured by ventral tongue pressure, when the lips were pressed against each other and when a brass button was pulled. In addition clinical results as well as a functional status were ascertained in order to record orofacial dyskinesias. A significant connection between the age respectively the sex of the persons and the determined force was found. However, no clear relationship between the occlusal position respectively the functional anomaly and the results of maximal force was determined. Thus it does not seem sensible to use maximal force measurement within the framework of the determined. Thus it does not seem sensible to use maximal force measurement within the framework of the diagnosis of orofacial dyskinesias.

[Orthodontics and implantology. Their course and results after joint planning].

The therapeutical repertoire for treating patients with missing teeth was significantly expanded by the development of modern implant methods. We have performed a retrospective analysis covering all children who between 1977 and 1993 had been considered for implants as a method for gap management. All patients were pretreated up to early adolescence using standard orthodontic procedures. Unfortunately, it turned out that an originally planned implant was not suitable in approximately 2/3 of the patients because of inadequate alveolar conditions. These patients were subsequently treated by conventional orthodontic gap closure or adhesive prosthetics.

Microvascular anastomosis of interpositional vein grafts with sutures and a new mechanical device--a histologic and scanning electron microscopic study.

In an experimental study, a new mechanical device for microvascular anastomosis was compared with suture anastomosis. In 14 rabbits, seven end-to-end and seven end-to-side anastomoses of the jugular vein to interpositional vein grafts were done with a ring-pin device and by the conventional suture method. There was no significant difference between the patency rates of mechanical and suture anastomoses. The completion of mechanical anastomoses took one-third the time required for suture anastomoses. After 10 days, histologic and scanning electron microscopic examination revealed an uneventful reparative process at the anastomotic site with media atrophy and a continuous endothelial layer. At 4 weeks, occasional foreign body giant cells were seen between the polyethylene rings and surrounding connective tissue. The study confirms that the ring-pin device is safe and provides a faster way to do microvascular anastomoses than by suturing.

[Validating rehabilitation after stroke].

Evaluation of therapeutic efficacy in neurologic rehabilitation is methodologically limited by the well established polypragmasia and the lack of untreated controls. Controversial discussion is generated by the following topics: superiority of specific therapy versus spontaneous recovery, higher efficacy of in-patient-setting versus out-patient-setting and the value of neurorehabilitation treatment in latency of more than 1 year after the event. To develop a practicable method for the evaluation of efficacy, we surveyed retrospectively 30 patients after stroke in a sequential study design on 4 scheduled visits: first admission to in-patient rehabilitation, discharge to out-patient treatment, admission and discharge of second in-patient rehabilitation. Impairment, disability and handicap were documented by NIH-Stroke Scale, Barthel-Index, CGI, WWK-Pflegeskala N and Risk-Profile. A descriptive data-analysis implies a therapeutic gain of stroke rehabilitation in-patient setting which is apparently superior to out-patient treatment. A measurable therapeutic effect was evident even more than one year after the stroke.

Prognostic value of PCNA and cytokeratins for radiation therapy of oral squamous cell carcinoma.

In a retrospective pilot study we compared morphologically the first biopsies of 20 patients suffering from oral squamous cell carcinoma (OSCC) where preoperatively performed radiation therapy [gamma rays (cobalt-60)] was successful with 20 patients who underwent the same therapy without the expected success. All specimens were formalin fixed and paraffin embedded. We performed haematoxylin and eosin staining and immunohistochemistry (ABC-method) applying broad spectrum cytokeratin, cytokeratin (CK) 1 + 2, 3 + 6, 13 and anti-proliferating cell nuclear antigen (PCNA, PC10). The specimens of the patients with success of the radiotherapy showed statistically higher levels of PCNA positive tumour cells, measured by a computerised morphometric analysis system (VIDAS). These specimens showed significantly less CK 3-6 positive tumour cells than the specimens of the patients with failure of this therapy. The difference in the content of proliferating cell nuclear antigen might explain the different results of the performed radiation therapy. The difference in cytokeratin 3 + 6 expression might be linked with the different amount of benign hyperproliferation. Prospective studies are planned to prove the results.

[Diagnosis and differential diagnosis of sialo-odontogenic (glandular-odontogenic) cyst].

Sialo-odontogenic (glandular-odontogenic) cyst is a new entity in the classification of developmental epithelial odontogenic cysts. Differentiation of this type of odontogenic cysts from dentigerous cysts and keratocysts and also from cystic mucoepidermoid carcinoma is essential. A sialo-odontogenic (glandular-odontogenic) cyst is likely to show aggressive growth, so that complete resection is essential. We demonstrate sialo-odontogenic (glandular-odontogenic) cyst by presenting four new cases and differentiate it from a special type of odontogenic keratocyst and a typical cystic mucoepidermoid carcinoma.

[Gas in the portal vein system of the liver. Value of ultrasound].

We report on 9 patients presenting with different diseases of varying severity, who were found to have hepatic portal venous gas (HPVG) identified by diagnostic ultrasonography. Intrahepatic gas was a shortlasting phenomenon, which could not be seen radiologically in most cases. Earlier beliefs that HPVG is a poor prognostic sign associated with high mortality, should be revised.

[The "white hole" phenomenon--a potential functional parameter in non-gated 201Tl-myocardial scintigraphy].

In 50 patients with a "white hole" in their scintigram--among them 44 patients with a history of myocardial infarction--the relation between its relative volume and the ventriculographically determined ejection fraction was studied retrospectively. A relative volume of the "white hole" larger than 13.7% showed a specificity of 93% and a sensitivity of 45% for a decreased ejection fraction and had a significant correlation of r = -0.54 (alpha = 0.01) with the ejection fraction. Thus the relative volume of the "white hole" derived automatically from 201Tl-SPECT is an easy-to-determine additional parameter which allows an at least semiquantitative statement about the function of the left ventricle.

[The radioiodine-concentrating oncocytic thyroid carcinoma: a case report].

A patient with radioiodine-positive metastases to bone and lung of an oncocytic thyroid cancer is reported. A left-sided thyroidectomy had been performed nine years earlier. Histologically it had been a large capsular Huerthle-cell adenoma with regressive changes without signs of malignancy. During a recent operation for a disc prolapse in the lumbar spine a metastasis of an oncocytic thyroid cancer was found. A radioiodine scan showed high accumulation of 131I in the thyroid region, the lung and the lumbar spine. The rare finding of radioiodine uptake in oncocytic cancer is discussed.

Modulation of arachidonic acid metabolism by olsalazine and other aminosalicylates in leukocytes.

We investigated the action of the new aminosalicylate olsalazine (disodium azodisalicylate) on arachidonic acid metabolism in comparison with 5-aminosalicylic acid (5-ASA) and sulphasalazine (SASP) by in vitro incubation of cellular homogenates from human polymorphonuclear (PMNL) and mononuclear (MNL) leukocytes with 14C-labelled arachidonic acid. Olsalazine reduced the synthesis of leukotriene B4 (LTB4), 5-hydroxyeicosatetraenoic acid (5-HETE), 11-HETE, 12-HETE, and 15-HETE in PMNL and MNL slightly less than SASP. 5-ASA was significantly less inhibitory than olsalazine and SASP on the formation of lipoxygenase products in PMNL and on LTB4 synthesis in MNL. In contrast, in MNL the formation of 5-HETE was unaffected, and the production of 11-HETE, 12-HETE, and 15-HETE was even slightly activated by 5-ASA. Total prostaglandin synthesis was dose-dependently reduced by the aminosalicylates (SASP greater than olsalazine greater than 5-ASA), but only SASP markedly altered the prostaglandin (PG) profile, with an increase in PGE2 and PGF2 alpha at the expense of other cyclooxygenase products. It may be concluded that olsalazine resembled SASP with regard to the inhibition of the lipoxygenase but had effects intermediate between the other salicylates on cyclooxygenase. Furthermore, the alteration of the prostaglandin profile by SASP points to an overlying cofactor effect of this drug.

Characteristics of Israeli women studying nursing compared to women studying education and engineering.

This study examined the demographic characteristics, vocational personality, and sex-role orientation of Israeli women studying nursing compared to women studying education and engineering. The convenience sample was 176 university students. The instrument included a demographic inventory, Holland's Self-Directed Search (SDS) questionnaire, and the Sex-Role Orientation Attitude questionnaire. Nursing and education students had Holland's "social" personality types and engineering students were more "realistic" or "investigative". Nursing and engineering students were significantly more feminist in their orientation than education majors. Nursing students were nontraditional women who had traditional family backgrounds, yet were nontraditional in their feminist orientation. With nursing's move into colleges and universities, the need for academically qualified applicants has intensified. Developing a better understanding of the unique nature of nursing and nursing students is a challenge.

[Pseudo-ascaris in ultrasonography of the epigastrium].

A sonographically demonstrated vermicular structure in the region of the gall bladder was suspected to be an ascaris, which is to find in the biliary system occasionally. However under cholecystectomy there was not to detect a worm, only a stone in the cystic duct. In accordance to anamnesis we think it being the caudal part of a ventriculo-peritoneal drain implanted for hydrocephalus therapy.

[Synthesis and pharmacologic testing of 1-oxo-pyridazine and 1-oxo-phthalazine-2-acetic acid derivatives].

The title compounds, starting from variously substituted beta-benzoylpropionic or beta-benzoylbenzoic acids, were prepared by cyclocondensation with alpha-ethylhydrazinoacetate monohydrochloride and subsequent alcaline hydrolysis or aminolysis of the 2-carbethoxymethyl-1,2,5,6-tetrahydro-1-oxo-pyridazines or 1,2-dihydro-1-oxo-phthalazines respectively. The 1-oxo-phthalazines 42 and 34 have a weak anticonvulsive effect. Like a large number of the other synthezized compounds, they show a central sedative component and are relatively nontoxic.

[Thermal sterilization in the revision of the sterilization section of the East German medical reference].

The G-value estimating sterilization-process is offered and compared with other scales selected from bibliography. Based on the kinetic of destruction of microorganisms the G-value considers every parts of sterilization-processes especially with reference to peak-sterilization. It takes into account the resistance of microorganisms, the safety index and also the influence of germ reducing measures, e.g. cleaning, before the sterilization-processes. Finally a formula is presented to transform G- and F-values each other.

Effect of antacid treatment on endogenous prostaglandin synthesis in human antral and duodenal mucosa.

Using 14C-labeled arachidonic acid as precursor for in vitro prostaglandin synthesis, the effect of an antacid containing Al (OH)3, Mg(OH)2 and CaCO3 on endogenous prostaglandin synthesis was investigated in antral and duodenal mucosa of healthy volunteers. After three weeks of treatment with a high-dose antacid, there was no detectable change in the total capacity of the mucosa for prostaglandin synthesis, but the prostaglandin profile was markedly altered. The relative amounts of PGE2 and PGF2 alpha synthesized by antral and duodenal mucosa increased at the expense of the prostaglandins A2/B2, thromboxane A2, and prostacyclin. In a short-term study, this change was not observed following a single antacid dose within 1 hr after application. It is concluded that long-term antacid treatment may alter the prostaglandin pattern formed by gastroduodenal mucosa and this may be related to its therapeutic effect.

Alimentary tract and pancreas. Stimulation of mucosal prostaglandin synthesis in human stomach and duodenum by antacid treatment.

The effect of a low dose antacid treatment on mucosal prostaglandin metabolism was studied in 15 healthy volunteers. A daily dose of 46 mmol (= 138 mval) Al(OH)3 and 42 mmol (= 84 mval) Mg(OH)2 with a titrated in vitro neutralising capacity of 272 mval of H+ was given for three weeks. Total prostaglandin formation and the prostaglandin profile as well as the degradation of PGE2 were assayed by incubating homogenates of endoscopic biopsies from antral and duodenal mucosa with the precursor (14C)arachidonic acid. Total prostaglandin synthesis in antrum (623 (110) pmol/mg protein) and duodenum (432 (72) pmol/mg) was stimulated after three weeks administration of low dose antacids by 176% (p less than 0.05) and 154% (p less than 0.05), respectively. An untreated control group exhibited no significant changes. In contrast, the prostaglandin profile showed only a small increase of the prostacyclin metabolite 6-keto PGF1a (p less than 0.05) at the expense PGD2. PGE2 catabolism was unaffected. This enhanced activity of mucosal prostaglandin cyclooxygenase might represent one possible mechanism of action of a low dose antacid treatment.
