Modulation of prostaglandin metabolism by K-MAP and prevention of toxic effect of cyclosporin on pancreatic islet cells.

The injection of 25 mg/kg i.p. cyclosporin (CsA) for 3 wk caused marked functional and morphological deteriorations of pancreatic islet cells in Wistar rats that were prevented by the combined administration of p-aminobenzoic acid-N-D-mannoside sodium salt (K-MAP). In this article, the toxic effect of CsA on pancreatic islet cells and the preventive effect of K-MAP on CsA-associated islet cell toxicity were investigated. Prolonged hyperglycemia and depressed insulin secretion after the glucose challenge observed in CsA-treated rats could be prevented by the combined administration of 300 and 900 mg/kg K-MAP. Cytoplasmic vacuolizations and a decrease in the number of mitochondria, intact endoplasmic reticula, secretory granules, and insulin-positive cells, as revealed by peroxidase-antiperoxidase staining, could also be prevented by the administration of 900 mg/kg K-MAP. This preventive effect of K-MAP on CsA-associated islet cell toxicity may suggest the combined use of K-MAP with CsA in pancreas transplantation and treatment of insulin-dependent diabetes.

Perceptual localization of visual stimuli flashed during saccades.

Subjects were asked to make a saccade to a visual target flashed in the dark during a prior primary saccade, and to report its apparent position by moving an adjustable light spot to that position. When targets were presented at the beginning of the primary saccade, subjects perceptually mislocated them in the direction of the saccade, whereas when targets were presented immediately before the end of the primary saccade, the flashed targets were mislocated in the opposite direction. The perceptually localized position of the target was primarily determined by its retinal position. However, at all actual and retinal positions of the target, the localized position shifted from the position that would be predicted if the location of the target was determined only by its retinal position to the prior primary saccade direction. The results were discussed in relation to extraretinal eye position signals. Subjects moved their eyes not to the actual position of the target, but to its apparent position. In some trials, there was a discrepancy between perceptual and oculomotor localization, which was interpreted as having been caused by the imprecise localization ability of the oculomotor system.

[Infectious diseases in kidney transplant recipients treated with cyclosporin].

Cyclosporin (CYA) is now recognized as an effective immunosuppressant to lead to a marked improvement in graft survival in organ transplant recipients. Although the incidence of infection in the CYA group has been decreased compared with that in the azathioprine group, infectious diseases in 400 kidney transplant recipients treated with CYA were noted in our single center. Treatment strategy for infectious diseases: Antibiotics and/or gamma-Globulin were administered to all recipients with bacterial infections. Aciclovir was added in recipients with herpes simplex virus (HSV) infection or varicella zoster virus (VZV) infection. Human interferon-beta (HuIFN-beta) was used in recipients who had life-threatening viral infection, especially cytomegalovirus (CMV) pneumonitis. Glycyrrhizin was used for acute hemorrhagic cystitis and nephropathy due to adenovirus (AV). Trimethoprim sulfamethoxazole and/or pentamidine were added in recipients complicated with Pneumocystis carinii (Pc) pneumonitis or in order to prevent Pc pneumonitis. Infectious diseases: One hundred and six recipients had infectious diseases 129 times in this series, seventy-six percent of all infections occurred during the first 4 months after the transplantation. Urinary tract infection (UTI), herpes zoster and pulmonary infection were the most common infectious diseases, occurring in 28.7%, 24.0% and 23.2%, respectively. Septicemia or bacteremia developed in 9 recipients, secondary to UTI in 8 and to surgical wound infection in one. Sixty-one symptomatic viral infections occurred in 57 recipients. A total of 5 recipients (1.3%) died of interstitial pneumonitis. Infectious organisms: Viral and bacterial infections were most common, occurring in 47.3% and 41.9%, respectively. Viral species detected in these recipients with the frequency were HSV 14 times, CMV 9 times, VZV 31 times and AV 7 times. 1) The incidence of viral infections in kidney transplant recipients treated with CYA is relatively high compared to bacterial infections. 2) HuIFN-beta therapy is effective in the treatment of serious opportunistic herpes virus infections, especially CMV pneumonitis. 3) Glycyrrhizin therapy is effective in the treatment of acute hemorrhagic cystitis and nephropathy due to AV and hepatic dysfunction. 4) Aerosolised pentamidine therapy is very useful for prophylaxis of Pc pneumonitis.

[Effect of endoscopical administration of OK-432 in 4 patients with advanced esophageal or gastric cancer].

OK-432 has been injected endoscopically into 4 patients with an unresectable esophageal or gastric cancer. Three months after the initial injection, a remarkable improvement in the symptoms and a reduction of the tumor size was recognized in all 4 patients. This study indicates that an OK-432 injection provides considerable benefits to patients with an advanced esophageal or gastric cancer.

Effects of Ca2+ antagonists on 45Ca2+ uptake by rat whole brain P1 and P2 fractions.

Voltage-dependent 45Ca2+ uptake into rat whole brain P1 (nonsynaptosomal) and P2 (synaptosomal) fractions was measured after 60-s KCl-stimulated depolarization to investigate possible inhibitory effects of Ca2+ antagonists, nitrendipine, diltiazem, verapamil and ifenprodil tartrate. All drugs (10(-5)M) had little influence on 45Ca2+ uptake by whole brain P1 fraction in both nondepolarizing and depolarizing mediums. Diltiazem (10(-5)M) had no influence on 45Ca2+ uptake by whole brain P2 fraction under both conditions. However, nitrendipine (10(-5)M) verapamil (10(-5)M) and ifenprodil tartrate inhibited depolarization-dependent 45Ca2+ uptake by whole brain P2 fraction, while nondepolarization-dependent 45Ca2+ uptake was not affected by these drugs. The inhibitory sequence of depolarization-dependent 45Ca2+ uptake by P2 fraction was: ifenprodil tartrate greater than nitrendipine greater than verapamil. These results suggest that ifenprodil tartrate, nitrendipine and verapamil specifically inhibit depolarization-dependent synaptosomal 45Ca2+ uptake, but that diltiazem has little influence on it.

Disease and pregnancy in adolescent girls.

The frequency of visits to the outpatient clinic, chief complaints, and pregnancies of adolescent girls were analyzed. The age distribution of adolescent girls who visited the clinic showed that the higher the age, the higher the frequency of menstrual cycle abnormalities. Recently, increased sexual activity has been seen in teenagers and has caused problems, especially the increase in teenage pregnancies, contributing to increases in induced abortion. To avoid unwanted pregnancy of unmarried teenagers, intensive sex education is essential, as well as provision of contraceptives.

Small bowel enteroclysis survey.

A survey questionnaire was sent to 125 radiology programs to determine the frequency and indications for the use of small bowel enteroclysis. Sixty-seven of the 84 respondents perform small bowel enteroclysis while 17 do not. The 3 most common indications for enteroclysis are detection of unexplained gastrointestinal bleeding, partial small bowel obstruction, and Crohn's disease. We conclude that although small bowel enteroclysis is a well-established examination, there are wide differences in the frequency with which it is performed, the techniques of examination, and indications for its use.

A facile synthesis of lanost-8-en-3 beta-ol-24-one (24-ketolanosterol). An inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A reductase.

A facile chemical synthesis of lanost-8-en-3 beta-ol-24-one (24-ketolanosterol) is described. This compound was found to be a potent inhibitor of 3-hydroxy-3-methylglutaryl (HMG) CoA reductase activity in cultured mouse L cells. The synthetic scheme developed in this study utilizes commercial lanosterol as a starting material and involves selective hydroboration of the C-24 double bond followed by oxidation of the carbon-boron bond at C-24 by pyridinium chlorochromate (PCC).

Renal sinus malignant lymphoma: a case report.

We report a case of renal sinus non-Hodgkin's lymphoma without contiguous adenopathy. Magnetic resonance and computed tomography images of this rare tumor were correlated with a pathologic specimen. On computed tomography the renal sinus lymphoma was evaluated as a well circumscribed, low-density mass owing to the mixture of the lymphomatous cells and fat tissues. Magnetic resonance showed the replacement of the renal sinus fat tissues with a spoke-like solid mass. The renal artery and vein were also evaluated thoroughly and were without tumor thrombosis.
