Assessment of the antigenic structure of tick-borne encephalitis virus by the use of synthetic peptides.

The feasibility of using synthetic peptides for the identification of individual monoclonal antibody (MAb)-defined epitopes was assessed on the basis of a structural model of the tick-borne encephalitis (TBE) virus envelope glycoprotein E. For this purpose a series of 19 synthetic peptides was prepared, covering most of the E protein sequence. Each of the peptides was tested by ELISA for reactivity with 19 protein E-specific MAbs raised against TBE virus strain Neudoerfl. Specific reactivity was observed with three MAbs and two peptides (representing amino acids 1 to 22 and 221 to 240, respectively), thus providing new information on the location of the corresponding epitopes. Specificity was confirmed in a competition ELISA by the ability of the peptides to block MAb binding to TBE virus antigen. However, in contrast to the other MAbs, these peptide-reactive MAbs were not blocked by native virus particles in the competition ELISA, indicating that they do not recognize the native conformation of the E protein. These three MAbs also showed increased reactivity with denatured forms of the virus in a dot blot assay. Additionally, they reacted only in ELISA systems in which the virus was directly coated to the solid phase and thereby presumably partially denatured, but not when a capture antibody was used, which preserves the native antigen conformation. We have thus identified two classes of MAbs, those which recognize the native form and those which recognize the denatured form of protein E. The latter may be useful for the analysis of sites probably involved in protein folding and oligomerization.

Streptococcal and staphylococcal superantigens (ETA, SEB): presentation by human epidermal cells and induction of autologous T cell proliferation in vitro.

Streptococcal and staphylococcal toxins are responsible for skin-related clinical conditions, e.g. scarlet fever and toxic shock syndrome. Skin involvement may result from a hypersensitivity reaction to these toxins; however, their precise mode of action has still to be elucidated. The aim of the present study was to investigate the capacity of human epidermal cells to present streptococcal erythrogenic toxin A (ETA) or staphylococcal enterotoxin B (SEB) to autologous T-lymphocytes in vitro. We found a significant T-lymphocyte proliferation response to minute amounts of ETA (p < 0.01) and SEB (p < 0.02) when co-cultured with freshly isolated autologous human epidermal cells. We suggest that human skin may serve not only as an entry for microbial toxin-producing strains but also as an important target for streptococcal and staphylococcal toxin-binding and subsequent T cell proliferation.

Effect of the dose of ultraviolet radiation on the pigment formation by human melanocytes in vitro.

Human melanocytes were cultivated under different conditions with phorbol ester (TPA), or with bovine pituitary extract (BPE). The cells altered their morphology with the different culture conditions. With TPA they were predominantly bipolar, while with BPE most of the cells had a dendritic cell shape. In order to investigate the effect of UV irradiation, the cells were irradiated with 50, 100 and 200 mJ/cm2 UVA/B. After irradiation with 200 mJ/cm2 UVA/B the cells cultured with TPA also showed a dendritic shape. We determined the tyrosinase activity, the cellular melanin content and the cell number 3 days after irradiation. In all cases the number of cells decreased depending on the UVA/B doses. In melanocytes we found a marked increase in tyrosinase activity and melanin content after irradiation with 200 mJ/cm2. The UV-induced effect on tyrosinase activity was higher in melanocytes cultured with BPE than in those cultured with TPA. The results were compared with two human melanoma cell lines. Only little pigment formation could be measured in the tested melanoma cell lines without change after UV irradiation.

Carbonic anhydrase--a marker for fenestrated capillaries in psoriasis.

A strong correlation between capillary fenestrations and the demonstration of carbonic anhydrase (CA) has previously been shown. In the present histochemical study we sought to determine whether CA could serve as a marker for fenestrated capillaries in psoriasis. In normal human skin capillary staining for CA was found only in the fenestrated capillaries of the perifollicular and periglandular plexus. In psoriatic skin lesions, however, the intrapapillary capillaries also reacted for CA. From ultrastructural investigations it is known that these capillaries are fenestrated. Our findings have shown that there is a strong correlation between fenestrated and CA-positive capillaries in normal human skin as well as in the lesional skin of psoriatics. Therefore, the demonstration of CA activity may serve as a specific and sensitive marker for fenestrated capillaries in psoriasis using an uncomplicated method, which makes it possible to detect numerous fenestrated capillaries in a single histological section.

Molecular epidemiology of tick-borne encephalitis virus: cross-protection between European and Far Eastern subtypes.

Tick-borne encephalitis virus isolates from widely separated geographic regions of the USSR, six isolates from Hungary and one from France were compared with the European and Far Eastern prototype viruses. Peptide mapping by limited proteolysis yielded similar patterns for five selected isolates from the USSR. All isolates from Hungary and France exhibited the same reactivity pattern with a panel of 16 protein E-specific monoclonal antibodies, whereas 10 out of 12 isolates from the Soviet Union showed minor differences at certain epitopes. However, no correlation between geographic origin of the isolates and their antigenic structure was observed. No statistically significant difference in the degree of protection was detected when mice were immunized with the European prototype vaccine and challenged with three selected Asian isolates and one from the European part of the USSR.

Detection of tick-borne encephalitis (TBE) virus in Liechtenstein.

In this study, we present the first detection of a focus of tick-borne encephalitis (TBE) virus-infected ticks in Liechtenstein. The focus is located on a much-used forest path near Vaduz, the capital of the principality. The virus isolated is a representative of the Western subtype of the TBE virus. It is thus closely related to or identical with the other strains isolated in western Europe.

[Immunogenetic studies of familial occurrence of progressive systemic scleroderma and circumscribed scleroderma].

Two different forms of scleroderma in one family are described: the mother suffers from systemic sclerosis and her daughter from linear morphoea. The observed HLA antigens indicate that systemic sclerosis and morphoea have various features in common. The immunogenetic data can be used to calculate the aetiological and preventive fractions, which together with environmental hazards and other risk factors describe the HLA-associated potential for provocation of scleroderma.

[Detection of the Köbner phenomenon of the skeleton of patients with psoriasis].

Psoriasis is a humorally controlled systemic disease. The degree of "eruptive strength" of manifestation results from hereditary factors (disposition) and environmental factors (provocation). We were able to demonstrate that the well-known Köbner phenomenon of the skin also occurs on the skeleton of patients suffering from psoriasis. We analysed 83 patients in whom bone scans were carried out. Our results indicate that provocation factors such as bacterial foci and/or trauma correlate with a significantly higher number of pathological scintigraphic findings, ranging up to "hot spots". Furthermore, not only did bone fractures remained scintigraphically positive for an unusually long time, traumas of the end phalanx could be demonstrated in 70% of psoriatic patients compared with 21% of a control group. Obviously, one factor alone or a combination of factors triggers the involvement of the skeleton as a "deep Köbner phenomenon". In psoriatic patients the response of bone metabolism to disturbance differs from that of non-psoriatic patients in that there is a long-lasting dysregulation. This explains the high correlation between skin and skeletal manifestation in psoriatics. Therefore the manifestation of psoriatic disease is due not to a single-stranded linear causal interrelation but to a multicausal "network pathogenesis". Bone scintigraphy is the diagnostic method of choice in patients with psoriatic osteoarthropathy and allows an objective evaluation of therapeutic success.

[The possible nephrotoxicity of a bituminous coal dermatologic agent].

Nephrotoxic effects are discussed as a possible risk of dermatological coal tar preparations. Therefore, we have performed a renal tolerance study with a modern coal tar preparation for the scalp, Berniter. 15 healthy volunteers, randomly subjected to 3 different modes of application, applied the preparation: twice weekly (mode I) or daily (mode II) for a period of 8 weeks. In both modes, application time was 15 min. In mode III, the preparation was used under occlusion for 30 min every second day for a period of 4 weeks. Before, during and after treatment, renal functions and urinary phenol levels were assayed. No pathological change or impairment of renal functions was detected. A relationship between urinary phenol content and the coal tar treatment was not observed, either. We conclude that the coal tar preparation investigated here has no nephrotoxic properties.

[Immunology of leprosy. Review of data from the 1980-1990 time period].

The classification used for leprosy so far distinguishes between two polar forms (pauci-bacillary and multi-bacillary), with different influences on the immune cells. The identification of specific antigens of the germ surface with monoclonal antibodies allows finer differentiation in classification, which now extends to transitional forms of clinical relevance. The current status of knowledge about immunological phenomena triggered by Mycobacterium leprae is complex, and controversy is rife at present. The bacillus is comparable to other mycobacteria in antigenicity. Owing to the complicated composition of the cellular envelope, few epitopes are accessible for immunological reactions. Investigation on the behaviour of T-lymphocytes in combination with analysis of the bacterial surface structures led to the development of a new pathogenetic concept. In spite of different immunological and inflammatory reactions in patients (reflecting the individual pathology), the discrimination between a partial and a total immunological defect is made. So far, no histocompatibility-antigen type has been found that allows recognition of the predisposition to infection. Immunity against the bacteria is achieved after BCG in some cases. All this suggests that the development of a vaccine via biotechnical synthesis of idiotypes and anti-idiotypes may be possible.

Sequence of the genes encoding the structural proteins of the low-virulence tick-borne flaviviruses Langat TP21 and Yelantsev.

The structural protein coding regions of the genomes of Langat virus (strain TP21) and Yelantsev virus, which was originally described to be a low virulence natural isolate of tick-borne encephalitis virus, were cloned and sequenced. These viruses had both been used as experimental live vaccines against tick-borne encephalitis in Czechoslovakia and Russia, respectively. Peptide mapping and monoclonal antibody binding experiments yielded identical reaction patterns for Langat virus and Yelantsev virus which were distinct, however, from the pattern obtained with tick-borne encephalitis virus. Sequence analysis confirmed this distinctiveness and proved that the vaccine strain Yelantsev was also Langat virus. The envelope protein E of both viruses exhibits an 88% amino acid sequence homology with that of tick-borne encephalitis virus. Assessment of the antigenic reactivity and sequence comparison with the E protein of tick-borne encephalitis virus revealed several differences affecting epitopes involved in virus neutralization. These observations suggest that Langat-like virus-based vaccines may not represent the most effective means to achieve protection against tick-borne encephalitis virus.

The relationship between the flaviviruses Skalica and Langat as revealed by monoclonal antibodies, peptide mapping and RNA sequence analysis.

The flavivirus Skalica was isolated from a bank vole in Czechoslovakia in 1976. It can be serologically distinguished from prototype strains of tick-borne encephalitis (TBE) virus and has a decreased virulence for adult mice. We have further defined the relationship of Skalica virus to other members of the TBE serocomplex (TBE European and Far Eastern subtypes, Langat and louping ill virus) by using a panel of 22 monoclonal antibodies, peptide mapping and RNA sequence analyses. By these criteria Skalica virus proved to be distinct from TBE virus and to be very closely related to Langat virus, differing by only two bases among a total of 416 nucleotides compared. The sequence of 22% of the Langat genome was determined and the encoded amino acid sequences were derived. Comparison of these with the corresponding amino acid sequences of TBE virus revealed a similarity of 85%, as opposed to 93% similarity between the European and Far Eastern subtypes of TBE virus.

Fusion activity of flaviviruses: comparison of mature and immature (prM-containing) tick-borne encephalitis virions.

The fusion activity of flaviviruses [tick-borne encephalitis (TBE) virus and Japanese encephalitis virus] was assessed by inducing fusion from without of C6/36 mosquito cells with purified virus preparations. Membrane fusion and polykaryocyte formation was observed only after incubating the viruses at acidic pH. Two groups of monoclonal antibodies reacting with distinct non-overlapping antigenic domains on the TBE virus protein E inhibited fusion from without. One of these domains contains the most highly conserved and putative fusion-active sequence of the flavivirus protein E. Of five TBE virus monoclonal antibody escape mutants, each defined by a single amino acid substitution in the envelope protein E, one revealed a reduced fusion activity and another one a lower pH threshold. TBE virus grown in the presence of ammonium chloride as well as Langat virus purified from the supernatant of infected chick embryo cells contained the precursor of protein M (prM) rather than M itself. These 'immature' virions did not cause fusion from without, suggesting that the proteolytic processing of prM may be necessary for the generation of fusion-competent virions.

The flavivirus envelope protein E: isolation of a soluble form from tick-borne encephalitis virus and its crystallization.

By the use of limited trypsin digestion of purified virions, we generated a membrane anchor-free and crystallizable form of the tick-borne encephalitis virus envelope glycoprotein E. It retained its reactivity with a panel of monoclonal antibodies, and only subtle structural differences from the native protein E were recognized. Treatment with the bifunctional cross-linker dimethylsuberimidate resulted in the formation of a dimer. Crystallization experiments yielded hexagonal rod-shaped crystals suitable for X-ray diffraction analysis.

[Treatment of recurrent herpes labialis using a herpes simplex Type-I subunit vaccine. A prospective randomized double-blind multicenter study].

A herpes simplex type I subunit vaccine developed by Behring-Werke (Marburg/Lahn, FRG) was compared with placebo for therapeutic effectiveness in the treatment of recurrent orolabial herpes simplex in a multicentre study involving the major hospitals in Frankfurt/Main (n = 34), Hamburg (n = 18), Giessen (n = 17) und Kassel (n = 14). The effectiveness of the vaccine was judged on the reduction in the frequency of recurrences. Within the period of the 8-month study there was no statistically significant difference in the frequency of recurrences with vaccine or placebo. Both placebo and verum led to a significant decrease in the number of recurrences of orolabial herpes simplex, but the effect of placebo was slightly more pronounced.

[Congenital poikiloderma syndrome with early childhood blister formation (Brain syndrome) and unusual associated neurologic symptoms].

We report on an 8-year-old girl with manifestations of congenital poikiloderma during her first year of life. Macroscopically, there was reticular teleangiectasia on cheeks and thighs, generalized de- and hyperpigmentation, dry skin with pityriasiform scaling and milia as a result of former blister formation. Histologically and ultrastructurally cytoid bodies, probably of keratinocyte origin, were observed. Associated findings were leucocytopenia, hyperlipoproteinaemia, spastic ataxia and lack of teeth. There is consanguinity in the family, but 3 sisters, the parents and the ancestors were completely healthy. Because of transient blister formation on the face, upper arms and elbows we would like to classify our case as Brain syndrome.

[Porphyria cutanea uraemica: an obligate systemic disease in chronic kidney insufficiency?].

Patients with chronic renal failure, especially those receiving maintenance haemodialysis, have a number of dermatological alterations. Some of them are similar to those seen in porphyria cutanea tarda (PCT). Whereas early studies showed normal plasma porphyrin levels, a striking elevation of plasma porphyrins, and particularly of uroporphyrin, has recently been found. We measured the concentrations of porphyrins in the plasma and erythrocytes of 55 patients with chronic renal failure and receiving maintenance haemodialysis and also in 7 patients with PCT and 100 healthy volunteers. The mean concentration of porphyrins in plasma was 2.7-fold, and the maximum concentration, 6-fold the highest value measured in plasma of controls. The mean plasma concentration of uroporphyrin was 6-fold higher up to a maximum value of 37-fold the upper limit of the controls. The plasma porphyrin values of 3 of 7 patients with PCT were on the same high level as those measured in patients undergoing haemodialysis. The mean porphyrin concentration in the erythrocytes of haemodialysis patients were 1.5-fold the control values. Because of the known pathophysiological effect of uroporphyrin, especially its stimulation of the collagen synthesis and the activation of the complement system, we suppose that the porphyria cutanea tarda like skin lesions in patients with chronic renal failure are due to the highly increased uroporphyrin concentration.

[The significance of hot spots in psoriatic skeletal involvement].

Bone scintigraphy is a modern method of low specificity but very high sensitivity in determining early changes in bone metabolism. There is a relationship between excessive skeletal metabolic disorders of the bone and the appearance of foci of maximum radionuclide uptake, or "hot spots". This distribution of radionuclide uptake has a specific character. In a retrospective study, we investigated the distribution of hot spots in 92 patients with various forms of psoriatic disease. Hot spots were found at and near to joints only. In enthesopathies hot spots are not found, because of lack of substance. They do, however, show signs suggesting a major disorder in collagen metabolism. Hot spots indicate a rather acute degree of skeletal involvement in psoriatic disease and are also frequently found in very active phases of psoriatic disease. Therapy-induced regression is possible. Spontaneous regression was observed only in the case of hot spots of short duration. The correlation of scintigraphic findings with X-rays shows that hot spots are signs of maximal focal metabolic activity. In a high percentage of cases they are leading up to a state of destruction, which can be shown on X-rays later. Among other parameters, they indicate the likelihood of skeletal involvement in psoriatic disease. Moreover, they indicate a high risk of "psoriatic arthritis".
