[Oral treatment of continuous ventricular extrasystole with detajmiumbitartrate (Tachmalcor)--clinical studies on effectiveness and dose-time-effect relations including intraindividual therapy comparisons with ajmaline (Tachmalin)].

On 31 inpatients with ventricular permanent extrasystoles (12 females, 19 males) at the age of 17--17 years altogether 45 oral experiments of rhythmisation were performed, on them 29 times with the ajmalium derivate detajmium bitartrate (Tachmalcor) and 16 times with ajmalin (Tachmalin). In 14 cases the comparison of the effect of the two substances was performed in the intraindividual test. In ventricular extrasystoles the ajmalin derivation compared with the initial substance was significantly more effective antiarrhythmically already when the half dose was used. However, it led at the same time to a greater retardation of the intracardiac stimulus conduction. Heart rate and blood pressure were influenced only at the beginning of the treatment and only unessentially. The subjective tolerability was very good. In 9 cases with oral ambulatory long-term treatment the daily maintenance doses of 75--100 mg detajmium bitartrate proved to be fully effective in therapeutic respect. Practical therapeutic conclusions are derived from the results obtained.

Studies in man with a cold-recombinant live influenza B virus vaccine.

A cold recombinant live influenza B virus vaccine was tested in man. In comparison to a placebo, reactogenicity attributable to virus infection was slight or moderate. No revertant viruses were shed, and there was no evidence of transmission to the placebo group who were housed in close contact with the vaccinees. Serological responses to initial inoculation were moderate; 60% of vaccinees showing twofold increases in serum hemagglutination inhibition (HAI) titers gave a geometric mean titer (GMT) of 1:13. Three weeks after the first vaccination, both the vaccine and the placebo group were revaccinated with homologous live virus vaccine. The group previously given vaccine was resistant to reinfection as judged from clinical reactions and virus shedding and the GMT increased only slightly to 1:16.3. In contrast, the former placebo group responded; mild symptoms were seen, the majority shed viruses and 50% showed twofold increases in serum HAI titers to a geometric mean titer of 1:17.4.

Antibody response after application of a new live attenuated mumps vaccine (Pariorix) measured by enzyme-linked immunosorbent assay (ELISA).

Mumps virus specific antibody concentrations were determined by enzyme-linked immunosorbent assay (ELISA), hemolysis-in-gel (HIG) test, and hemagglutination-inhibition (HI) test in 17 adult volunteers before and after vaccination with a new live mumps vaccine, Pariorix. The results of the present study indicate that the strain Urabe-Am 9 of the mumps virus (Pariorix) induces antibodies in 100% of seronegative adults when measured by ELISA and in 70.6% and 64.7% in HIG and HI, respectively. ELISA is therefore a very sensitive and reliable method for the demonstration of sero-conversion after vaccination. The vaccine was well tolerated and clinically acceptable in the group of adult volunteers.

Preparation of gels from hyaluronate solutions.

Stable, transparent gels can be prepared from hyaluronate solutions by adding CuSO4. The best gelation was found using solutions of 2 mg/ml hyaluronate in glass-distilled water at a pH of 6.2 after the addition of 1 mg/ml CuSO4. Methylation of the carboxyl groups of the hyaluronate completely abolished the gelation, indicating the importance of the carboxyl groups for the gel formation with Cu2+ ions. Gelation also depends on the molecular size of the hyaluronate, since hyaluronate was not able to form a gel after depolymerization with hyaluronidase.

[Surgical method for the treatment of irreversible angle-closure glaucoma and aphakic glaucoma. Direct ciliary body coagulation with partial cyclodialysis].

Among 16 patients aged from 31 to 86 there were 13 cases of hemorrhagic glaucoma, three cases of secondary glaucoma from iritis and one case of secondary angle-closure glaucoma after trauma. Reduction in intraocular pressure was achieved by a surgical method which primarily aims to improve the outflow of aqueous humor. The ciliary body is laid bare by preparation of a scleral flap, a partial cyclodialysis is attached on both sides and the ciliary body is induced to stricture by coagulation, so that a large enough cleft is created. Before the operation, pressure ranged from 34 to 64 mm Hg; after the operation, during postoperative periods of observation lasting from two to 32 months, there were 14 cases in which the pressure was below 25 mm Hg and two cases with 30-40 mm Hg respectively. In view of the good results and the few complications, this method of operation can be recommended for treatment of both irreversible angle-closure glaucoma and aphakic glaucoma.

[Light and electron microscopy results following photocoagulation of ciliary processes with argon lasers].

The authors' own clinical observations have shown that a pressure-lowering effect in secondary glaucomas resistant to therapy can be obtained by photocoagulation of the pars plicata of the ciliary body using an argon laser. In order to be able to estimate the damaging effect of photocoagulation and thus to be able to administer it in a sensible dose, the authors applied this technique to four patients with tumorous eyes that had to be enucleated, with a subsequent histological and electron-microscopical scrutiny. The laser energy was varied from 750 to 2000 mW at a constant exposure time of 0.2 s and a dot size of 100 micrograms. With clear-refracting media an energy of 1000 mW brought about coagulative effects without any substantial side-effects. Damage was then generally restricted to the aqueous humorproducing formation, i.e., the ciliary epithelium and the adjacent stromal part.

[Treatment of gonorrhea in dermatological practice. Results of a survey].

A questionnaire on the treatment of gonorrhea was sent to 1,560 dermato-venereologists practicing in the Federal Republic of Germany. Nine hundred sixty dermatologists returned the questionnaire. More than 60% of them examine less than 50 patients with gonorrhea per year; 44% treated the gonorrhea with spectinomycin, 42% with penicillin. Single dose treatment is used by approximately half of the dermatologists, but only 4% use single dose treatment with penicillin/probenecid. One case of lethal allergic shock to penicillin is reported to have occurred in 1980.

[Surface reactivity of enosseous implanted bio-glass ceramics at varying lengths of implantation].

Chemical and structural surface changes at enosseous bioglass-ceramic-implants (Ap 40) were investigated by means of electron microprobe after an implantation period of 4 to 52 weeks. An intimate bonding between regenerated bone and bioglass-ceramic surface was evident at points already after 4 weeks. Initial degradation processes at implant surface came to a standstill after 12th week. A calcium and phosphorus rich layer of great importance for implant stability and biocompatibility was formed immediately contacting the bone. This surface layer had a thickness of nearly 25 micrometers -- 30 micrometers at 12 weeks and remains unchanged up to 52 weeks. The registered calcium and phosphorus impulse rate (cps) is equal to that of neighbouring bone. Chemical similarity of implant surface makes bioglass-ceramic Ap 40 a suitable biomaterial for durable bone replacement.

[Efficacy of specific immunoglobulin against TBE (author's transl)].

The efficacy of passive immunization against tickborne encephalitis (TBE) was evaluated in Vienna by means of a telephonic survey. It was learned that in 1979 after a tickbite in endemic areas, of TBE, approximately one out of 1,000 unvaccinated persons came down with the disease. Administration of TBE immunoglobulin after exposure reduced the incidence of the disease to 1:2,500. thus showing a rate of protection of approximately 60%. The survey also showed that in 1979 one third of those at risk (frequent walks in forests) had already undergone active immunization with the Austrian TBE vaccine.

[Gamma globulin as a prophylactic measure in an outbreak of hepatitis A in an institution for mentally retarded children (author's transl)].

An outbreak of hepatitis A was observed in an institution for mentally handicapped children. Within 2 months 21 cases occurred in 4 out of 7 groups. By means of gamma-globulin administration and measures of general hygiene, the outbreak, although not immediately stopped, was brought under control. The number of overt cases was reduced and further transmission to 3 other groups was prevented. The detection of IgM antibodies to hepatitis A virus proved to be very suitable for diagnostic purposes. Subsequently it turned out that the gamma-globulin used had a very low titre in comparison with other preparations. Recommendations are given to prevent similar outbreaks in the future.

[Serological diagnosis of virus hepatitis (author's transl)].

Hepatitis A is diagnosed by the detection of specific IgM-antibodies. The virus itself can be found in stool specimens. Gammaglobulin is useful in prophylaxis of Hepatitis A. In the course of Hepatitis A HBs- and HBe-antigen as well as HBc(IgM and IgG)-, HBs- and HBe-antibodies can be detected. By analysis of this parameters information is obtained about acute or chronic hepatitis as well as about the probability of infectivity of a blood sample. Hepatitis Non A Non B is most important after blood transfusion and shows a strong tendency to chronicity. So far no test for specific diagnosis exists in practice. Besides the agents mentioned a hepatitis also can be caused by cytomegalovirus, Epstein-Barr-virus and some tropical viruses.

[Shortening of interval between first and second TBE vaccination in asthmatic children (author's transl)].

37 children suffering from asthma had to be vaccinated against tick-borne encephalitis (TBE) with an interval of only 10 days between the first two vaccinations. Sufficient antibodies were detected in samples taken 14 days after the second injection. No differences were found between the results in this group of asthmatic children and in children who were vaccinated with the usual interval of 1 to 3 months elapsing between the first two injections. The asthmatic children tolerated the vaccination very well, moreover the indicence of side reactions was not different from that of the control group.

A method for the purification of bovine vitreous body hyaluronic acid.

Bovine vitreous body hyaluronic acid was treated with activated CH-Sepharose 4B after purification with cold 5% trichloroacetic acid. By this method a highly purified hyaluronic acid can be prepared. Contaminating serum proteins were completely removed. The remaining small amounts (0.3%) of the protein contaminants were found, by immunological methods, to be non-plasma proteins.
