[The value of arthroscopic operations].

Between April 1976 and July 1983, 831 arthroscopies were performed in the orthopedic department of the Salzburg State Clinic, and 186 patients underwent arthroscopic surgery between 1979 and 1983. Thirty-two meniscectomies performed with arthroscopy were assessed with regard to postoperative duration of hospitalization and postoperative condition. One of the disadvantages of arthroscopic surgery is that the procedure is rather protracted, especially when the surgeon is unaccustomed to it; however, the duration can be considerably shortened as the surgeon gains practice and experience. The following advantages of arthroscopic surgery for the patient are mentioned: 1. it is a more accurate surgical procedure; 2. surgical trauma is reduced; 3. rehabilitation is achieved more rapidly.

Selective vulnerability of hippocampus and disturbances of memory storage after mild unilateral ischemia of gerbil brain.

The effect of selective injury of hippocampal neurons on the consolidation of memory traces was studied in gerbils (meriones unguiculatus) after production of mild cerebral ischemia. The right carotid artery was permanently ligated, and animals without gross neurological deficits ("symptom-negative" gerbils) were selected. Eight days and eight weeks after vascular ligation, cell counts of hippocampal neurons were carried out and correlated with regional blood flow and the acquisition of operant behaviour. Eight days after carotid artery occlusion, learning behaviour was significantly impaired although the number of hippocampal neurons had not changed and blood flow had even increased above normal. After eight weeks, learning behaviour and blood flow were normal but now a significant loss of pyramidal neurons was present in the CA1 and CA2 sectors of the hippocampus. Our observations demonstrate that it is possible to detect subtle functional disturbances by appropriate behavioural investigation before manifestation of selective injury of the hippocampus. Recovery of integrative function, despite persistent cellular damage, provides further evidence for central nervous plasticity.

Visual benefit of correcting higher order aberrations of the eye.

There is currently considerable debate concerning the visual impact of correcting the higher order aberrations of the eye. We describe new measurements of a large population of human eyes and compute the visual benefit of correcting higher order aberrations. We also describe the increase in contrast sensitivity when higher order aberrations are corrected with an adaptive optics system. All these results suggest that many, though not all, observers with normal vision would receive worthwhile improvements in spatial vision from customized vision correction, at least over a range of viewing distances and particularly when the pupils are large. Keratoconic patients or patients suffering from spherical aberration as a result of laser refractive surgery as it is presently performed would especially benefit. These results encourage the development of methods to correct higher order aberrations.
