Molecular cloning of the complementary DNA for an additional member of the family of aortic aneurysm antigenic proteins.

PURPOSE: We have purified and partially sequenced a protein from the adventitia of the human aorta (aortic aneurysm antigenic protein 40 kDa; AAAP-40) that has homologies to bovine aortic microfibril-associated glycoprotein (MAGP-36). It is immunoreactive with immunoglobulin G (IgGs) purified from the serum and aortic wall of patients with abdominal aortic aneurysms. AAAP-40 and MAGP-36 have fibrinogen-like and vitronectin-like motifs. Screening an expression library constructed from human aortic adventitial messenger RNA has resulted in the cloning of three complementary DNAs whose gene products are immunoreactive with immunoglobulin G from patients with abdominal aortic aneurysms. Two strongly resemble each other and have been described separately. The purpose of this article is to report the third clone. METHODS: Messenger RNA from a specimen of human aortic aneurysmal adventitia was reverse-transcribed for insertion into the phagemid Uni Zap XR (Stratagene). A strain of Escherichia coli, engineered for expression (XL 1-Blue MFR', Stratagene), was transfected, and rabbit antihuman vitronectin antibody as used to identify positive clones. Sequencing of the positive clones was performed by the Core Laboratories at Columbia University. RESULTS: The hypothetical protein of rAAAP-CL4 (clone 4) shares sequence motifs with known microfibril-associated glycoproteins (MAGPs). The recombinant protein (rAAAP-CL4) is immunoreactive with serum from patients (three of four abdominal aortic aneurysm sera). In addition, similarities have been detected with immunoglobulins of the kappa family and with a protein from cytomegalovirus that is a potential molecular mimic. CONCLUSIONS: There may several members of a novel family of human aortic autoantigenic proteins implicated in abdominal aortic aneurysm disease.

Lifetime assessment by intermittent inspection under the mixture Weibull power law model with application to XLPE cables.

This paper proposes a new treatment for electrical insulation degradation. Some types of insulation which have been used under various circumstances are considered to degrade at various rates in accordance with their stress circumstances. The cross-linked polyethylene (XLPE) insulated cables inspected by major Japanese electric companies clearly indicate such phenomena. By assuming that the inspected specimen is sampled from one of the clustered groups, a mixed degradation model can be constructed. Since the degradation of the insulation under common circumstances is considered to follow a Weibull distribution, a mixture model and a Weibull power law can be combined. This is called The mixture Weibull power law model. By using the maximum likelihood estimation for the newly proposed model to Japanese 22 and 33 kV insulation class cables, they are clustered into a certain number of groups by using the AIC and the generalized likelihood ratio test method. The reliability of the cables at specified years are assessed.

Blood supply to the latissimus dorsi muscle and muscle performance during co- or counterpulsatile stimulation for circulatory assist.

For the application of the latissimus dorsi muscle (LDM) to circulatory assist, the muscle is stimulated with co- or counterpulsation during the cardiac cycle. The purpose of this study was to evaluate the blood supply to the LDM and its muscular performance during each respective stimulation. The origin of the LDM was connected to a tension gauge, a potentiometer, and 1 kg of weight in series. The LDM was stimulated at a ratio of 1:1 of heart to muscle contraction for 10 min. Copulsatile stimulation made thoracodorsal arterial flow (TDF) predominant during cardiac diastole. In counterpulsatile stimulation, TDF occurred predominantly during cardiac systole. Between the 2 patterns of stimulation, no significant differences were observed in the mean TDF rate during 1 cardiac cycle. The maximal force, maximal contraction length, and power of the LDM also did not differ significantly. These results suggest that despite the difference of the TDF profile, LDM performance may be comparable between co- and counterpulsatile stimulation for the application of the LDM to circulatory assist.

[Incidence of Kanagawa phenomenon-positive and -negative Vibrio parahaemolyticus strains isolated from traveller's diarrhea and their relation to tdh and trh genes].

A total of 1,319 strains of Vibrio parahaemolyticus isolated from traveller's diarrhea were analysed for Kanagawa phenomenon (KP) with the Wagatsuma blood agar test and the results were also compared with those of analyses of tdh and trh genes which encode thermostable direct hemolysin (tdh) and TDH-related hemolysin (trh). The majority of the strains (1,152 strains) counting 87.3% had positive KP, among which 1,049 and 103 strains were only tdh and both tdh and trh-positive ones, respectively. However, 167 strains counting 12.7%, which is quite high compared to the previous report, were found to have negative KP, among which 94 and 24 strains were only trh and both tdh and trh-positive ones, respectively.

Laryngeal adjustment in whispering magnetic resonance imaging study.

Previous studies confirmed that during whispering the glottis is kept open to prevent vocal fold vibration and the supraglottal structures are constricted. However, there has been no study exploring the exact contour of the laryngeal lumen in the frontal dimension during the production of whispering. In order to further elucidate the nature of the laryngeal adjustments regarding the contour of the laryngeal lumen in whispering, and the role of supraglottal constriction in particular, we conducted a physiological study using magnetic resonance imaging. According to the results, the supraglottal structures were not only constricted but also shifted downward, attaching to the vocal fold to prevent vocal fold vibration completely during whispering. The results suggested the underlying mechanism of suppression of vocal fold vibration during the production of whispering.

Increased lipogenic capacity of the islets of obese rats: a role in the pathogenesis of NIDDM.

The onset of NIDDM in obese Zucker diabetic fatty (fa/fa) rats is preceded by a striking increase in the plasma levels of free fatty acids (FFAs) and by a sixfold rise in triglyceride content in the pancreatic islets. The latter finding provides clear evidence of elevated tissue levels of long-chain fatty acyl CoA, which can impair beta-cell cell function. To determine if the triglyceride accumulation is entirely the passive consequence of high plasma FFA levels or if prediabetic islets have an increased lipogenic capacity that might predispose to NIDDM, the metabolism of long-chain fatty acids was compared in islets of obese prediabetic and nonprediabetic Zucker diabetic fatty (ZDF) rats and of lean Wistar and lean ZDF rats. When cultured in 1 or 2 mmol/l FFA, islets of both female and male obese rats accumulated, respectively, 7 and 15 times as much triglyceride as islets from lean rats exposed to identical FFA concentrations. The esterification of [14C]palmitate and 9,10-[3H]palmitate was increased in islets of male obese rats and could not be accounted for by defective oxidation of 9,10-[3H]-palmitate. Glycerol-3-PO4 acyl-transferase (GPAT) activity was 12 times that of controls. The mRNA of GPAT was increased in islets of obese rats. We conclude that, in the presence of comparable elevations in FFA concentrations, the islets of obese prediabetic rats have a higher lipogenic capacity than controls. This could be a factor in their high risk of diabetes.

Stroboscopic observation of the larynx after radiation in patients with T1 glottic carcinoma.

We studied laryngeal video stroboscopy (LVS) system for evaluation of patients with glottic carcinoma (T1N0M0) before and after radiotherapy. There were 10 patients with T1 glottic squamous cell carcinoma (9 men and 1 woman) who received radiotherapy at the Hitachi General Hospital. We performed LVS before and after radiotherapy. The presence or absence of mucosal waves (MW) was particularly noted. No MW were present before radiotherapy but at 1-6 months after, MW gradually appeared. One year after radiotherapy all patients showed MW on LVS. In patients with glottic carcinoma MW recovered after radiation therapy. LVS may be useful for the clinical follow-up of post-radiation patients for early detection of recurrence of glottic carcinoma.

[Studies on bacillary dysentery cases of overseas travellers--during 1979 to 1995].

A total of 36,780,440 overseas travellers during 1979-1995 (17 years) were quarantined at Osaka and Kansai Airport Quarantine Station, 84,777 travellers reported themselves suffer from diarrhoea. Stools from 29,587 persons were bacteriologically examined. Various enteropathogenic bacteria were isolated from 9,766 (33.0%) patients of the stools examined. Isolated species were as follows: Plesiomonas shigelloides (3,234 cases); Salmonella spp. (2,236 cases); enterotoxgenic Escherichia coli (1,621 cases); Vibrio parahaemolyticus (1,959 cases); and Shigella spp. (1,242 cases). 1,278 different Shigella strains were isolated from 1,242 cases who were thus diagnosed as bacillary dysentery patients. The suspected regions or countries for infection of these cases were analysed. The serovars and antibiotic-sensitivities of the isolated strains were examined. Colicine typing of S. sonnei strains were also done. The results can be summarized as follows. 1) The most cases (53.4%) were infected in India. 2) The percentage distribution of sub-species of the strains was as follows; S. sonnei (57.8%), S flexneri (29.8%), S. boydii (8.4%), and S. dysenteriae (4.0%), respectively. 3) The major colicine type of S. sonnei strains were type 6 and 0. 4) The percentage of Antibiotic-resistant strains of each sub-species was S. dysenteriae (92.2%), S. sonnei (89.4%), S. flexneri (87.1%), and S. boydii (84.9%), respectively. The percentage of Antibiotic-resistant strains of S. flexneri were increased annually.

Three-dimensional modeling and molecular evolution of Matrix Cell Adhesion Molecule #2: MAT-CAM 2.

Our research group has recently reported the partial amino acid sequence of an aortic adventitial collagen-associated fibrillar protein, which may be the target of an autoimmune response in patients with abdommal aortic aneurysms (AAA). Its apparent molecular weight is approximately 40 kDa, so we have named it aneurysm-associated antigenic protein-40 kDa (AAAP-40). It has similarities to microfibril-associated glycoprotein-36 kDa (MAGP-36), which was reported by Kobayashi et al. to have a tissue distribution limited to the aorta in pig; unlike other microfibrillar proteins which appear to distribute ubiquitously throughout the body with elastin. Accordingly, if AAAP-40 is the human homolog of MAGP-36, it would explain how an autoimmune reaction against this protein might have consequences more or less limited to the aorta and its branches.

[Aging of the blue mechanism studied by measuring the increment threshold on flashed background].

We studied aging of the blue cone mechanism by measuring increment thresholds for detection of a small blue flash of light (439 nm, 1 degree, 50 ms duration) on a second flashed light of identical color, (139 nm, 2 degrees, 500 ms duration) in 30 subjects. Simultaneously, a large yellow adapting field was presented. Subjects, consisting of 17 normal cases, 13 cases with refraction errors under 3 diopters, 11 myopes, and 2 hyperopes, with 1.0 or better corrected visual acuity, were divided into 4 groups by age. The control group consisted of 11 cases under 30 years, group I, 7 cases in their forties, group II, 6 cases in their fifties, and group III, 6 cases over 60 years. Best fitting exponential curves were calculated for this phase of the blue mechanism and averaged threshold curves for each group were evaluated. The averaged threshold curve in group I showed an upper shift, that is, an increase in the threshold alone. Curves in groups II and III demonstrated an upper right shift, with increased thresholds in both the blue flash and the yellow background. Together with decreased function of the blue cone mechanism, the shift of curves in the elderly groups is probably caused largely by yellowing of the crystalline lens.

[A case report of localized fibrous pleural mesothelioma].

A 47-year-old female with localized fibrous pleural mesothelioma arising from the visceral pleura of S2 of rt-upper lobe is reported. We demonstrated an indirect radiographic sign of pedunculation which was the change in position of the mass with the change in patient's position. The tumor was completely extirpated with a partial resection of the adjacent lung tissues through thoracotomy. Histopathologically, the tumor consisted of collagen fibers and small amount of fibroblast. Localized fibrous mesothelioma was recognized as benign with good prognosis. However, a few cases showing recurrence after resection were reported among them. So it was considered that this patient should be followed up carefully.

Blood sampling technique for measurement of plasma arginine vasopressin concentration in conscious and unrestrained rats.

A new technique for collecting blood from conscious and unrestrained rats is reported. Plasma arginine vasopressin (AVP) concentration has been known to be influenced by various procedures used for blood collection (e.g., manual contact and anesthesia). In the study reported here, the improvement of the routine jugular vein cannula method was undertaken by extending the connection tubing to allow use of a swivel mechanism. Plasma AVP, glucose, and corticosterone concentrations in virgin rats were measured in blood samples collected by use of the new method and of two generally accepted methods (decapitation and routine jugular vein cannula methods). All values of the three parameters obtained were the lowest when the new method was used, suggesting that this new swivel method induced the least stress in the rats.

[Surgical management of constrictive pericarditis].

Cases of constrictive pericarditis per a institute is few for decrease of this disease. Therefore for surgical management of constrictive pericarditis, it has not been clear whether cardiopulmonary bypass during operation is necessary or not, and the reports of long term results are few. We gathered information by questionnaires from thirty-four institutes, which constitute Tokai Cardiovascular Surgeons Conference in Shizuoka, Aichi, Mie and Gifu Prefecture, for surgical management of constrictive pericarditis, and investigated its results. Etiology of constrictive pericarditis is idiopathic (49%), tuberculosis (29%) or post open heart surgery (10%). Hospital death included operative death is 14%, and postoperative liver complications was many next to heart complications. The cases, whose NYHA functional class at discharge did not improve compared with preoperative state, was 33%. At one year after operation two third of them improved in NYHA functional class, but the others did not improved.

Quantification of prism induced metamorphopsia as a model for clinical retinal (and other) distortions.

Metamorphopsia (all causes) results in perceived distortions in the visual field. A simple technique utilizing hyperacuity judgements was developed in this laboratory some years ago to allow quantification of such distortions. We ask here, how good is this assessment method at detecting a distortion? To control better investigations of metamorphopsia, a model providing constant optical distortions and resultant perceptual metamorphopsia was devised using plane prisms of 8 delta and 15 delta. The instrument includes a beamsplitter and a CRT display controlled by an IBM PC computer and 'mouse'. In this study, three short vertical lines (perpendicular to the prism base-apex line) were used to assessed image distortion in the central retinal area. Positions of individual lines of this pattern were varied in order to determine perceived distortions induced by a prism. Normal observers were tested on multiple occasions. It is possible to measure accurately prism-induced directional biases (mean locations) using this method. We separated personal setting biases from the prism-related distortions. The test method devised mimics modest clinical metamorphopsia. Prism magnitude affected settings. We were able to make judgements to less than +/- 2% error.

[A case of esophageal leiomyoma found as mediastinal tumor, diagnosed with endoscopic ultrasonography].

A 20 year-old female was admitted to our department because of an abnormal shadow in chest roentgenogram on a physical check-up. The barium swallowing study revealed an oppression of the middle of the esophagus. Endoscopy, chest CT and MRI failed to distinguish submucosal tumor from mediastinal tumor. Endoscopic ultrasonography showed the mass originated from the esophagus and a diagnosis of esophageal smooth muscle tumor was obtained. The tumor was tried to resect by thoracoscopic surgery but was failed because of its shape like a tortoise shell. The tumor was resected through the right thoracotomy and the diagnosis was confirmed histopathologically.

[Binocular functions in amblyopia and strabismus].

Regarding the changing trends in the concept, definition, etiological classification, and criteria for diagnosis of amblyopia, we reviewed a total of 4,693 cases of amblyopia seen during the past 37 years. The amblyopia was divided into four types: strabismic, anisometropic, ametropic, and form vision deprivative. There was a definite trend for the incidence to decrease and for the diagnosis to be made during earlier age in recent years. Although favorable recovery of visual acuity is obtained after treatment of amblyopia and strabismus, there are difficulties in obtaining good binocular functions in early-onset amblyopia and strabismus. This feature was evaluated in regard to motion perception asymmetry (MPA) and binocular depth from motion (DFM). Many cases of early-onset amblyopia and strabismus showed no disparity stereopsis, or position stereopsis, in spite of the presence of DFM. The MPA appeared to be closely related to early-onset esotropia regardless of age, while it disappeared and motion perception became symmetric 4 to 5 months after birth in normal infants. The DFM seemed to play an important role in maintaining good motor alignment for several years after surgery. I developed a checkerboard pattern stimulator in 1978. This method proved to be useful in developing binocular functions and motor alignment by applying simultaneous bifoveolar stimulation and anti-suppression. Extensive exposure to the stimulation was essential for therapeutic success.

[The temperature dependency of the protective properties of the St. Thomas' Hospital cardioplegic solution, the University of Wisconsin solution, and Bretschneider buffered solution].

We studied three preservation solutions such as St. Thomas' hospital cardioplegic solution No. 2 (ST), the University of Wisconsin solution (UW) and Bretschneider's solution (BR) for protective properties, especially in terms of temperature dependence during the preservation. Rat hearts were studied and divided into three groups; ST. UW and BR. In each group, six hearts were carried out perfusion, cardioplegic infusion, ischemia and reperfusion for four different conditions. Using an isolated working rat heart model, the hearts were subjected to 5 minutes Langendorff perfusion followed by 20 minutes working perfusion. During this working period, cardiac functions such as aortic and coronary flow, aortic pressure, and heart rates were measured every 5 minutes. After working perfusion, 3 minutes of cardioplegic infusion was carried out either with UW, ST, and BR at 37, 20, 4, 1 degree C. The hearts were subjected to either 35 min at 37 degrees C, 120 min at 20 degrees C, 300 min at 4 degrees C, or 450 min at 1 degree C. The hearts were immersed in the same solution during ischemia. The hearts were reperfused at 37 degrees C, initially in the Langendorff mode for 15 min. Coronary effluent was collected for the measurement of CK leakage during reperfusion. The heart were then converted to the working mode for 20 minutes. During working reperfusion period, the variables of cardiac function were again measured and expressed as a percentage of their individual preischemic control values. Under conditions of 37 degrees C ischemia, % recovery of cardiac output (%CO) in ST group is 62.8 +/- 2.8%, although the hearts preserved with UW and BR were not recovered. Under conditions of 20 degrees C ischemia, %CO in UW, ST and BR were 46.8 +/- 2.7, 82.0 +/- 3.1 and 41.4 +/- 2.6%, respectively. Under conditions of 4 degrees C ischemia, %CO in UW, ST and BR were 66.2 +/- 8.0, 68.6 +/- 2.3 and 47.2 +/- 2.9%, respectively. Under conditions of 1 degrees C ischemia, %CO in UW, ST and BR were 70.6 +/- 3.3, 56.5 +/- 1.6 and 43.0 +/- 2.5%, respectively. In conclusions, ST revealed the best protective properties for the heart preservation between 37 degrees C and 20 degrees C ischemia. The properties of ST was similar to that of UW under conditions of 4 degrees C, although UW showed most excellent protective properties under conditions of 1 degrees C ischemia. These results suggested that, protective ability of preservation solution might be changed by the temperature during preservation period.
