Actuarial analysis of reoperation in patients undergoing open mitral commissurotomy for mitral stenosis.

Reoperation is one of the most seriously problematical events in postoperative follow-up of patients undergoing open mitral commissurotomy (OMC) for mitral stenosis. In 217 patients with pure mitral stenosis, even when associated with severe subvalvular changes, the actuarial rate of freedom from reoperation was 94% 12 years after OMC. In contrast, in 85 patients with MS combined with regurgitation, the rate was 66%. In postoperative management of patients undergoing OMC, it is mandatory to know preoperative anatomical findings of the stenosed mitral valve.

Response to isoproterenol infusion in pediatric patients following atrial repair of transposition of the great arteries.

The postoperative function of the right (systemic) ventricle (SV) was evaluated in eight pediatric patients who had undergone intra-atrial switching for simple transposition of the great arteries (TGA). SV function was evaluated at rest and during isoproterenol (ISP) infusion and compared with left ventricular function in the control group. The control group consisted of five patients including two with postoperative pulmonary stenosis, two with mild pulmonary stenosis and one with postoperative partial anomalous pulmonary venous drainage. SV-graphy was performed at rest. During ISP infusion, the TGA group showed a significant increase in cardiac index (3.50 +/- 0.68 (mean +/- SD) L/min/m2 to 4.94 +/- 1.53 L/min/m2, p less than 0.05) and no significant changes in stroke volume index (SVI). Similar changes were observed in the control group. Resting SV ejection fraction was 0.37 +/- 0.05 in the TGA group. In regard to the relationship between SV end-diastolic pressure and SVI, all but one of the TGA patients showed normal response to the stress. These results indicated that the right ventricle in TGA showed an acceptable response to ISP stress and acted as a systemic ventricle for at least two years after surgery.

Reconstruction of the anterior cruciate ligament by allogeneic tendon graft. An operation for chronic ligamentous insufficiency.

We describe the use of allogeneic human tendon as an intra-articular replacement for the anterior cruciate ligament. Depending on the type and degree of functional instability we recommend the addition, in some cases, of supplementary extra-articular procedures. We have reviewed 31 patients at least two years after operation and have found that 30 of them had been able to return to full sporting activities. The indications for operation and the techniques are discussed and the use of allogeneic tendon is recommended.

[Evaluation of left ventricular function and coronary sinus blood flow during exercise after aortocoronary bypass surgery. Influence of revascularization of the left anterior descending artery].

Left ventricular function and coronary sinus blood flow at rest and during exercise were evaluated in 27 patients after aortocoronary bypass surgery and in 13 normal subjects (G-C). Twenty patients (G-1) had successfully revascularized left anterior descending artery (LAD). In 7 patients (G-2), the revascularization for LAD was not complete. There was no difference among 3 groups in stroke work index (SWI), left ventricular end-diastolic pressure (LVEDP) and coronary sinus blood flow (CSF) at rest. SWI during exercise in G-2 (44.7 +/- 14.7 g. M/beat/m2) was significantly lower than that in G-1 and G-C (67.9 +/- 15.0, 77.2 +/- 17.0 g.M/beat/m2) (p less than 0.02, p less than 0.002). LVEDP during exercise in G-2 (32 +/- 6 mmHg) was significantly higher than that in G-1, G-C (17 +/- 7, 13 +/- 3 mmHg) (p less than 0.001, p less than 0.001). CSF during exercise in G-2 (160 +/- 64 ml/min) was significantly lower than that in G-1, G-C (357 +/- 79, 290 +/- 113 ml/min) (p less than 0.001, p less than 0.002). These data indicated that left ventricular function during exercise and coronary sinus blood flow reserve for exercise in patient with completely revascularized LAD was significantly better than that in patients with incompletely revascularized LAD.

Long-term results in surgical treatment of children 4 years old or younger with coronary involvement due to Kawasaki disease.

Problems in myocardial revascularization for coronary impairment due to Kawasaki disease were assessed in five patients 1 to 4 years old. Each patient had significant stenosis or complete obstruction with aneurysm formation. There were no operative or late deaths in the follow-up period of 1 to 10 years after operation. The coronary arteries, including the first diagonal branch and obtuse marginal branch, were 1 to 1.5 mm in diameter. Eight saphenous vein grafts used were 2.5 to 3.5 mm in diameter and the internal mammary arteries used in two patients, ages 2 and 4 years, were 1 mm in diameter. All nine grafts examined in the early postoperative period were patent. However, only three of eight saphenous vein grafts were patent in the late postoperative studies. One saphenous vein graft to the right coronary artery was found patent 7 years after surgery in a patient who had been operated on at the age of 3 years. Internal mammary artery grafts used in the last two patients in the series were both patent throughout the late postoperative period. This experience suggests that myocardial revascularization is surgically feasible and beneficial even in young children and that further study is warranted.
