[A case of chondroblastoma originating from the right 6th rib].

Chondroblastomas are rare neoplasms, and comprise 0.5-1% of histologically proved primary bone tumors. We report a case of the tumor originating form the right 6th rib. The patient, a 15-year-old female, was admitted because the abnormal shadow was found on routine chest X-ray film. It was suspected to be the posterior mediastinal tumor based on the chest X-ray, CT and MRI. In surgery, the tumor was 4.0 x 4.0 x 2.5 cm in size and fixed on the rib, but not invasive to other tissue. The content was rough and soft granuloma like. Histological examination confirmed the diagnosis of chondroblastoma of the rib. The patient is doing well without symptoms of recurrence fifteen months after surgery.

[A case of intrathoracic perineurial cyst].

A 67-year-old man was referred to our surgical clinic for evaluation of an abnormal shadow on the chest X-ray examination. CT scan and MRI revealed a tumor in the posterior mediastinum and destruction of the pedicle and intraspinal invagination by the tumor. The tumor in a paravertebral area are linked with a vertebral canal through a destructed intervertebral foramen. MRI showed that the tumor was a cyst which contained fluid. On thoracotomy a hen's egg sized cyst was found in the posterior mediastinum, the cyst was excised. Pathological diagnosis was a perineurial cyst. This case was clearly differentiated from an intrathoracic meningocele and a neurilemmoma microscopically. This case is the first intrathoracic perineurial cyst in Japan.

[A case of left atrial myxoma in the elderly (77-year-old)].

We reported a case of a 77-year-old female, whose left atrial myxoma with a cyst, which had been identified by 2D echocardiography, was successfully resected. Apart from echocardiogram-revealed tumor hemorrhages in atrial myxomas, reports of echo-lucency within cardiac tumors are considerably rare. She was successfully operated on, and the section through the tumor specimen showed a cyst with a diameter of 10 mm, that corresponded to the echo-lucencies noted on the 2D echocardiogram. Even in an elderly patient, surgical treatment of left atrial myxoma offers excellent results with low morbidity and low mortality.

The effect of left ventricular bypass on the right ventricular function: experimental analysis of the effects of ischemic injuries to the right ventricular free wall and interventricular septum.

The right ventricular (RV) function during left heart bypass (LHB) was examined in open-chest anesthetized mongrel dogs (average weight, 11.8 kg). The LHB was carried out by a left ventricle (LV) to femoral artery bypass using a centrifugal pump for 90 min, and the bypass flow was kept maximum to obtain almost complete decompression of the LV. The RV function was evaluated by hemodynamic parameters and pressure-dimension (sonomicrometry) relationship at pre-LHB (control) and 30, 60, and 90 min after LHB (LHB-30, LHB-60, and LHB-90). The materials were divided into three groups after LHB-30: intact heart (group 1, n = 5), RV free wall ischemia (group 2, n = 5), and interventricular septum (IVS) ischemia group (group 3, n = 8). No significant changes in mean right atrial pressure (mRAP), RV end-diastolic pressure (RVEDP), RV maximum derivative pressure, or RV fractional shortening (RVFS) were found between pre-LHB and post-LHB in groups 1 and 2. On the contrary, group 3 showed significant increases in mRAP and RVEDP, and a decrease in RVFS at LHB-90 compared to both pre-LHB and LHB-30. The RV end-systolic dimension (percentage of pre-LHB) showed significant increases at LHB-90 compared to LHB-30 in groups 2 and 3. These results indicate that the LHB itself does not depress the RV function in the intact heart and in the RV free wall ischemic heart, while the impairment of the IVS during LHB appears to lead to RV dysfunction.

[A study of voice mutation and physical growth--a longitudinal observation].

Past studies on the relationship between mutational voice change and body growth were generally made on grouped subjects and no exact longitudinal observation was performed. In the present report, a longitudinal study was made on 100 young male students in their puberty, in which voice recordings and measurements of physical parameters including body height and weight were performed twice for each subject with yearly interval. Information on subjective evaluation of voice abnormality was also obtained from each subject. The recorded voice samples were subjected to subsequent analysis for obtaining fundamental frequency (F0) and formant values. The following results were obtained. 1. A negative correlation in the rate of change was observed between F0 and physical parameters such as body height and weight, and sitting height. 2. It was suggested that the mutational period consisted of the rapid and slow phases. The rate of growth in body height and sitting height was more significant in the rapid phase. 3. Subjective voice abnormality and physical growths such as the development of the laryngeal prominence were often noted even before the rapid phase. After the rapid phase was over, all the cases showed secondary sexual characteristics including the laryngeal prominence. 4. Before and during the rapid phase, there was a tendency for the values of F1 and F2 to increase, while that of F3 to decrease. After the rapid phases was over, there was a trend that F1 and F2 increased, while F3 remained unchanged.

[Image analysis of the contour of paralyzed vocal cord during phonation].

The development of digital radiography (DR) has made it possible to analyze the contour of the laryngeal soft tissue structures in more detail than the conventional screen-film method. We analyzed the contour of the paralyzed vocal cord during phonation using motion subtraction technique. Forty four patients with unilateral recurrent nerve palsy were examined. The images were obtained by means of frontal tomography of the larynx during quiet inspiration and normal (expiratory) phonation. Tomography during inspiratory phonation was also examined in ten patients who can perform it. The thickness of the slices was 5.0 mm. The authors found that the free margin of the paralyzed vocal cord shifted by the aerodynamic power along the direction of the air flow, i.e. it shifted to the cranial direction during normal phonation and to the caudal during inspiratory phonation. The displacement phenomenon of the paralyzed vocal cord was significantly correlated with the degree of vocal cord atrophy (p less than 0.01). The result would indicate that the aerodynamic power during phonation is one of the factors eliciting the higher position of the paralyzed vocal cord during phonation, in addition to its anatomical displacement due to abduction of the paralyzed arytenoid. This notion should be taken into consideration in selecting the surgical approach for the treatment of unilateral recurrent nerve palsy.

[The development of visible light-cured composite resin. Kinds of fillers to be mixed with cyclophosphazene system monomers and its amount and its physical properties].

A visible light-cured composite resin was developed. Cyclophosphazene monomer, 4 PN-(TF)1-(EMA)7 was prepared as a monomer. The ratio of brush abrasion, mechanical properties, water sorption, thermal expansion coefficient and the surface of abrasion were examined after mixing with fillers of different particle size (R-972, OX-50 and VL-30). The ratio of brush abrasion showed a tendency to be small when more than 50 wt% of VL-30 with a large particle size was mixed with 4 PN-(TF)1-(EMA)7 monomer. However its abrasion surface was rough compared with that of the microparticle filler. When the microparticle filler (50 wt% of OX-50) was mixed with the monomer, its mechanical properties were good for the mixture with 50 wt% OX-50. In that case, the ratio of brush abrasion was 0.268, compressive and transverse strength, 124.3 and 86.3 MPa respectively, hardness, 43.2 Hk, water absorption 14.2 micrograms/mm3 and thermal expansion coefficient, 47.4 x 10(-6)/degrees C.

Suppressor T-lymphocyte dysfunction in MCNS: role of the H2 histamine receptor-bearing suppressor T lymphocytes.

To evaluate the number and function of suppressor T cells in children with minimal change nephrotic syndrome (MCNS), we performed an inhibition test of rosette formation and measured leukocyte procoagulant activity. The number of histamine H2 receptor-bearing T lymphocytes (histamine H2 R+ lymphocytes) was markedly decreased at the onset of MCNS but gradually increased and was normalized following steroid therapy. The production of leukocyte procoagulant activity by normal T lymphocytes was abolished by incubation with patient's lymphocytes. However, pretreatment of the normal T lymphocytes with cimetidine markedly decreased the suppression. The results suggest an abnormality in the histamine H2 receptors on the patient's suppressor T lymphocytes.

[Late results of CABG for postinfarction angina--relation to preoperative myocardial infarct size].

We evaluated the late results of coronary bypass grafting (CABG) in 85 patients. The patients were divided into two groups according to preoperative MI size estimated by the Selvester QRS score; 24 with MI size of larger than 20% of LV muscle (group A; average 28 +/- 11%), and 61 with MI size smaller than 20% (group B; average 10 +/- 9%). New York Heart Association classes of both groups following CABG improved significantly (from 2.8 +/- 0.7 to 1.3 +/- 0.4 in group A; p less than 0.01, from 2.5 +/- 0.6 to 1.2 +/- 0.5 in group B; p less than 0.01). There was higher incidence of serious ventricular arrhythmias in group A than in group B (83% vs. 21%, p less than 0.01). In Group A, LVEF and LVESVI did not improve following CABG (from 17 +/- 9 to 16 +/- 8 mmHg, from 39 +/- 15 to 40 +/- 15%, from 66 +/- 28 to 69 +/- 40 ml/M2), while in Group B, those improved significantly (from 13 +/- 6 to 11 +/- 5 mmHg; p less than 0.01, from 53 +/- 14 to 58 +/- 10%; p less than 0.01, from 39 +/- 23 to 32 +/- 14 ml/M2; p less than 0.05). The exercise-to-rest LVSWI ratios increased significantly following CABG in both groups (from 86 +/- 25 to 160 +/- 56% in group A; p less than 0.05, from 92 +/- 31 to 140 +/- 37% in group B; p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Experimental complete right heart bypass. Proposal of a new model and acute hemodynamic assessment with vasoactive drugs in dogs.

A new model of complete right heart bypass was devised in dogs. All systemic venous blood was directly led to the bilateral pulmonary arteries by end-to-side superior vena cava-right pulmonary anastomosis, diverting the inferior vena caval blood to the superior vena cava with a graft, ligating the inferior vena cava at its end and the pulmonary artery at its trunk, and shunting the coronary venous return from the right-ventricle to the left atrium. Nine consecutive dogs tolerated the procedures, and acute hemodynamic characteristics and responses to vasoactive drugs were observed for 5 hours throughout the following full studies. Cardiac output ranged from 66 to 102 ml/min/kg at a central venous pressure of 15 to 26 mm Hg. Norepinephrine, isoproterenol, and phentolamine were administered with the central venous pressure maintained constant at 18 mm Hg. Isoproterenol and phentolamine increased cardiac output while reducing pulmonary and/or systemic vascular resistance, with a possible concomitant inotropic effect in the former. Norepinephrine was detrimental, causing an increase in both pulmonary and systemic vascular resistance.

[A case of ruptured pseudoaneurysm of the aortic arch associated with hemoptysis and hoarseness].

We experienced a case of the ruptured pseudoaneurysm of the aortic arch. Patient complained left anterior chest pain, back pain and hoarseness. Considering the past history of pulmonary tuberculosis, emergency operation was performed median sternotomy approach and using aortic occlusion balloon catheter method and selective cerebral perfusion. Woven dacron graft was anastomosis intraluminal aneurysm. Postoperative course was good. Median sternotomy approach and AOBC method were better than intercostal approach method, because of a little bleeding, short operative time and method.

[Changes in the immune response in postoperative patients while running the ventricular assist device (VAD)].

We examined the immunological parameters in two patients who were assisted with LVAD in postoperative LOS. Both patients demonstrated transient consumption of complements immediately after running the LVAD. The serum concentrations of immunoglobulins (IgG, IgM, IgA) showed no significant decrease. Both patients had significant postoperative lymphopenia, but recovered in a week. A decrease in the number of T cells was observed in both patients. The OKT4/OKT8 ratio initially increased in one patient, but gradually declined postoperatively. In the other patient, the OKT4/OKT8 ratio decreased. The lymphocyte response to mitogen (PHA) remained significantly depressed when the number of lymphocytes returned to the normal level, suggesting that the lymphocytes were functionally impaired. Such findings are generally observed after cardiac surgery. Thus, LVAD does not affect the immune response.

Right ventricular myocardial function after atrial switch operation for transposition of the great arteries.

Postoperative right ventricular (RV) myocardial function was evaluated in 6 patients who underwent atrial switching for simple transposition of the great arteries (TGA). The average age at study was 5.5 years. RV function was evaluated at rest and during administration of methoxamine by cardiac catheterization and RV angiography. The data were compared with left ventricular function in a control group, which consisted of 6 patients, 3 with functional murmur and 3 with pulmonary valvar stenosis. During stress, the TGA group showed a significant increase in end-diastolic pressure, minute work index and end-diastolic and end-systolic volume indexes, along with a significant decrease in ejection fraction. The control group also showed an increase in these variables except for ejection fraction during stress, which did not change. The slope of the work-function curve for the TGA group was lower than that for the control group (p = 0.02). The TGA group had a lower slope of the peak systolic pressure-volume relation than the control group (p = 0.005). There was no significant correlation between the slope of the peak systolic pressure-volume relation and age at repair or study. This study shows that RV pump dysfunction observed in postoperative TGA patients may be caused by depressed myocardial function.
