[Treatment of 61 patients with acute myelogenous leukemia at first relapse].

Treatment results in the 61 adult patients with AML in first relapse were analyzed to establish a better strategy for this group of patients. These patients received reinduction chemotherapy during 1979-1988. Complete remission (CR) was obtained in 57.4% of the cases, and the probability of survival and remaining in CR at five years was 11.9% and 17.9%, respectively. The longer duration of initial remission was favorable factor for achieving second CR. Type of reinduction regimens which were different from those used in the initial induction phase did not influence the second CR rate. The use of different consolidation regimen appeared to favorably affect the survival and probability of remaining in CR.

Laryngeal epithelial changes on effects of smoking and drinking.

The human larynx is essential not only for speech, but also for swallowing and respiration. Its tissues are affected by the presence of alcohol and cigarette smoke. In this report we present our findings on the effects of smoking and drinking on histological changes in laryngeal tissue. The larynges studied were those of autopsy patients: cases with damaged tissue were excluded. We studied the larynges of 84 men (non-smoker and non-drinker, 22 cases; smoker and drinker, 62 cases) and 48 women (non-smoker and non-drinker, 40 cases; smoker and drinker, 8 cases) between the ages of 12 and 88. The squamous metaplasia of laryngeal epithelium was examined using the gross staining method (Pyronin Y) of STELL et al. (J. Laryngeal. Otol. 86: 589-594, 1972). The squamous epithelium is unstained, whereas the respiratory epithelium is a brilliant red. The area of metaplasia in the supraglottic region increased with aging, tobacco and alcohol consumption. Microscopic changes of the laryngeal epithelium were investigated using the hematoxylin-eosin staining method. The thickness of epithelium of the supraglottic region and vocal cord increased with age. There is a significant difference in the thickness of the epithelium of the supraglottic region and vocal cords of smoking and drinking patients, when compared with the same tissues of non-smokers and non-drinkers. Furthermore, there is a significant difference in those tissues of the supraglottic region of heavy smokers, when compared with light smokers, but no significant difference when compared with drinkers. However, there is no significant difference in the thickness of vocal cord tissues when comparing light smokers and drinkers on the one hand, and heavy smokers and drinkers on the other.

[Changes in content of excitatory amino acids in the brain and cerebrospinal fluid].

Recently epilepsy is analysed on terms of two different conditions, seizure susceptibility and seizure induction. Much attention has been paid to excitatory amino acids in these conditions. An examination was made of changes of glutamate and aspartate in the brain and cerebrospinal fluid (CSF) in a cat amygdaloid kindling model so as to determine whether excitatory amino acids are possibly involved in seizure susceptibility and seizure induction. Twenty crossbred adult cats were divided into four groups: a sham operation group (Sh) as the control, just after stage 4 seizure group (S4), just after stage 6 seizure group (S6), and stage 6 group 2 weeks after the last stimulation (S6-2W). CSF, blood and 13 individual brain regions were taken at 9 + 4 degrees C, and their glutamate and aspartate contents were measured by high performance liquid chromatography (HPLC). Glutamate concentration was significantly higher in the S6 group than in the Sh group in CSF and less in the S6 group than in the Sh group in the left and right amygdala, hippocampus and piriform cortex. No aspartate could be detected in the CSF of any group, nor did it change significantly in the blood or brain. Based on the above results, the content and release of glutamate and aspartate apparently do not change in seizure susceptibility and glutamate is released from kindled tissues in seizure induction, thus showing it to be involved in seizure induction in a cat amygdaloid kindling.

[Malignant lymphoma with Adams-Stokes syndrome as the first manifestation].

A case of malignant lymphoma with Adams-Stokes attack which was the first manifestation was reported. A 56-year-old female admitted our institute because of unconsciousness attack. After implantation of pacemaker she had high fever and abdominal mass. Ga scintigraphy revealed many accumulation on the hepatic hilus and the heart. Abdominal Computed Tomography and echography revealed abnormal mass in the hepatic hilus. The biopsy of this abnormal mass was performed. The histological examination of this specimen showed diffuse middle sized type malignant lymphoma.

Fibrosarcoma of the neck in an adult female.

This paper is a case report of a 53-year-old housewife with fibrosarcoma of the neck. On 27 August 1986, she was admitted to our hospital with discomfort on the right side of her throat and difficulty in swallowing. She had no history of radiation therapy to the neck. The tumor was below the right submandibular gland and was movable. No enlarged lymph nodes were palpated in the neck. The mean CT number of the tumor was 59.5 H.U. The tumor was almost circular and its surface was smooth. Upon operating, the tumor was observed to be connected to the greater cornu of the hyoid bone and the superior cornu of the thyroid cartilage. The tumor measured 50 x 42 x 35 mm and was well circumscribed but not encapsulated. The cut surfaces were generally fleshy, glistening and grayish-white. The tumor was composed of small amounts of collagen and reticulin, producing fairly uniform spindle cells arranged in interlacing bundles, forming a herring bone pattern. The tumor cell showed rather little atypism, but did show obvious mitotic activity. Thus, the tumor was diagnosed as well-differentiated fibrosarcoma. When the patient was last seen, in April 1989, there was no evidence of recurrence.

[Clinical study of cortical and brainstem evoked potentials in head injury].

Three types of evoked potentials (EPs) auditory brainstem response (ABR), somatosensory evoked potential (SEP), and visual evoked potential (VEP) were recorded among 100 cases of head injuries within three days after the trauma had occurred. In order to assess these EPs, the normal wave patterns of 20 healthy subjects were used for comparison. For indices, wave I, III and V were used for ABR, N1, N2 and N3 for SEP, and N70, P100 and N125 for VEP. On this basis, five EP grades were constructed, from normal (grade I) to highly abnormal (grade V). Furthermore, an EP pattern classification was devised to integrate the respective EP grade. Namely, pattern A (PA), consisting of grade (G). I-III of the 3 types of EP; PB, composed of one type of EP or both ABR and VEP at G. IV-V; PC, consisting of both SEP and VEP at G. IV-V; PD, comprising both ABR and SEP at G. IV-V; and PE, covering all three types EPs at G. IV-V. In this EP pattern classification, PA signifies no severe damage, PB localized damage, PC severe cerebral damage, PD severe brainstem damage, PE severe diffuse damage. The significance was studied for an understanding of the pathological state, and for making a prognosis. The following conclusions were reached. 1. In severe head trauma, primary brainstem damage is very rare, and in cases where brainstem damage is shown, it is accompanied by extensive cerebral damage. To assess the pathological state of such primary cerebral damage EP in the acute stage is useful, and by performing further EP, successively, it becomes also possible to evaluate the secondary cerebral damage.(ABSTRACT TRUNCATED AT 250 WORDS)

Study of chronic alcoholic pancreatitis by means of serial pancreozymin-secretin tests.

Fourteen patients with suspected chronic alcoholic pancreatitis (CAP) and 21 patients who had been shown to have the disease were followed up by the pancreozymin-secretin test in order to clarify the serial changes in exocrine pancreatic function in alcoholic pancreatitis. The initial and final test data for secretory volume, maximal bicarbonate concentration, bicarbonate output (BO), and amylase output (AmO) of exocrine secretion were compared in these two groups. Patients with suspected CAP showed a significant serial decrease only in AmO; definite CAP developed in 3 of them during the follow-up period. In definite CAP, a significantly progressive decrease in BO as well as AMO was observed. It is suggested that in the earlier stage of CAP, AMO is initially affected, whereas decreased secretion of both bicarbonate and enzyme becomes apparent in the later stage.

Exocrine pancreatic function in asymptomatic chronic alcoholics without structural pancreatic disease.

In order to elucidate the functional states of the pancreas in the asymptomatic latent stage of chronic alcoholic pancreatitis, 45 chronic alcoholics with no obvious pancreatic structural abnormalities judged by endoscopic pancreatography were studied by pancreozymin-secretin test. We found three patterns of exocrine pancreatic function in alcoholics with or without cirrhosis: normal secretion (40%), hyposecretion (29%) and hypersecretion (31%). In the hyposecretory group, the amylase output proved to be impaired more frequently. In the hypersecretory group increases were observed in one or more of the following parameters, volume, bicarbonate output and amylase output. However, increase in volume was a fundamental condition in this hypersecretion. This study confirmed that exocrine pancreatic hyposecretion and hypersecretion were almost equally frequent in asymptomatic chronic alcoholics with no obvious pancreatographic abnormalities. These results suggest that ongoing exocrine pancreatic dysfunctions exist in the subclinical stage of chronic alcoholic pancreatitis.
