[Psychiatric controversies. Definition of the scientific position (author's transl)].

The history of psychiatry in the past 200 years shows that the basic position of this medical discipline is always influenced to a particular extent by trends of contemporary history. Such connections have in the past been the starting points of considerable psychiatric progress; but at other times they have -as in the Germany of National Socialism - also been the greatest dangers of psychiatry, even provoking its vilest misuse. Before such a background as this, comments are made on the present problems of psychiatry. Psychiatry today and in the future will only be able to cope with its tasks for the large number of mentally ill and mentally disturbed people if its foundation remains firmly anchored in medicine. And modern medicine will only be albe to overcome many problems in the near future if it always keeps itself open to the impulses and suggestions of a psychiatry fully integrated into the art of healing.

Effect of clonidine on growth hormone release in psychiatric patients and controls.

The stimulation of human growth hormone (HGH) release by clonidine (0.15 mg i.v.) was studied as a test of the postsynaptic alpha-receptor sensitivity of psychiatric patients. In this test, endogenous depressives showed a significantly reduced HGH response to clonidine as compared to normal controls, neurotic-reactive depressives, and schizophrenics. However, no differences were found between the endogenous depressives and a group of schizoaffective patients. HGH response to clonidine was not correlated with plasma levels of noradrenaline, serum cortisol, free fatty acids, or blood glucose. Within the group of normal control subjects, a reduced HGH response was found in most postmenopausal women and in some regular users of alcohol. Our findings suggest that patients with endogenous depression are characterized by a subsensitivity of postsynaptic alpha-receptors or of structures related to them. The clonidine test shows promise as an indicator of vulnerability to endogenous depression.

[On the practice of clinical therapy with psychotropic drugs -- retrospective investigation of physicians prescribing practices in a psychiatric hospital (author's transl)].

1. At the psychiatric hospital of the University of Munich physicians' practices in prescribing psychotropic drugs have been analyzed retrospectively for 1974. Case report of 2,100 patients from a total of 2,118 newly admitted inpatients in 1974 have been evaluated and various methods of analyzing data are compared. 2. 414 (19.7%) patients out of a total of 2100 have not been medicated by any drugs. On the average 2.7 psychotropic drugs/patient have been precribed in the remaining 1,186 patients (80.3%). 3. 37.4% of 4627 applications made have been allotted to neuroleptics. Clozapine has been found to be the drug most extensively used (37.4%). It was prescribed to 595 patients, followed by chloralhydrate (425 patients) and amitriptyline (374 patients). 4. Out of a total of 5373 prescriptions, 2,004 were allotted to a single substance and 3,369 to a combination of different drugs. With regard to combined medications, in 1,856 prescriptions two drugs were combined, in 1,015 three, in 404 four, in 70 five, in 18 six and finally in 6 prescriptions seven medications were applied to one patient. 5. Antidepressants and neuroleptics have been combined on the same patient in 768 prescriptions. As to neuroleptics most frequently clozapine and haloperidol have been prescribed together. 6. The mean time during which a defined combination of drugs was given continuously, never exceeded a quarter of the total time the individual had been admitted to the hospital. 7. It is evident that in clinical practice the combinations of psychotropic drugs are widely used. This pattern of psychopharmacological drug treatment is in conflict with the present accepted rules of psychotropic pharmacotherapy. Furthermore it is a remarkable result of the present studies that in therapeutical practice the single components of drug combinations are changed very often. Compounds with sedative effects are preferred within all classes of psychotropic drugs.

Antithymic immune factor in schizophrenia. A World Health Organization study.

Six WHO Collaborating Centre took part in the study of the antithymic activity of blood sera of patients suffering from schizophrenia. Blood serum specimens from 118 schizophrenic patients and 62 mentally healthy donors were investigated. Statistically significant differences between schizophrenic patients and the controls were found (p < 0.05). It is probable that as with other biological phenomena described in schizophrenia, antithymic activity is one of the biological factors, in combination with other factors, predisposing towards the development of the schizophrenic process.

[Drug monitoring in psychiatry (author's transl)].

The Study Group for Drug Surveillance in Psychiatry (AMUP) has been working since July 1978 as a task force group of the "Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie" (AGNP). A protocol was designed for the initiation of drug monitoring in psychiatric hospitals. With support of the Bundesgesundheitsamt, Berlin, the first part of this project was started as a practicability study in the psychiatric hospitals of the Universities of Berlin, Göttingen, and Munich and in the municipal hospital of Schleswig. The study includes 7650 in-patients. It is based on the following methods: intensive drug monitoring, organized spontaneous monitoring of adverse drug reactions and registration of drug applications. This drug monitoring system in psychiatry will be expanded to other psychiatric hospitals, and eventually to out-patient clinics as well as private practices within the next two years.

[Psychiatric diseases and driving fitness (author's transl)].

Mental factors frequently play a decisive role in the conditional setup of many accidents. Analysis of the mental factors determining the cause of accidents show these to be by far often mental factors of a non-pathological nature, such as personality factors, disordered concentration and alertness in conflict situations, overstrain and fatigue, than factors conditioned by illness, e.g. as associated with depression or schizophrenia. Psychological alterations in subjects of advanced age constitute a border area of high significance in medical science concerned with traffic safety. In the consulting room, particular attention should be paid to the recognition of potential risk constellations in traffic precipitated by alcohol or drugs.

Treatment of schizophrenia.

1. Progress in 25 years' history of the neuroleptics is briefly reviewed. 2. Development of certain butyrophenones more directly effective in "minus" symptoms of schizophrenia and introduction of depot neuroleptics is discussed. 3. Extrapyramidal motor side effects (EPMS) are still a serious problem in the treatment. 4. Clozapine does not have EPMS. This drug could therefore become the starting point of a series of less hazardous antipsychotic drugs. 5. The neuroleptics can be used as tools for exploration of the etiopathogenesis of schizophrenia. Some important pharmacological mechanisms, i.e. their antidopaminergic activity, are briefly outlined.

[Sleep problems and their treatment in psychosis (author's transl)].

Sleep disturbances in psychoses can mean hypo- as well as hypersomnia. In 90% of endogenous depressed patients sleep disturbances were seen, mostly as hyposomnia. In the group of schizophrenic psychotic patients only 30% had sleep disturbances. With polygraphical investigations in endogenous depressed patients a shortening of REM-latency and a disturbed sleep profile, in schizophrenic psychoses a shortened REM-rebound and a reduced amount of stages 3 and 4 were found. The treatment of choice for depressions are antidepressive drugs and sleep deprivation, for schizophrenic psychoses neuroleptic drugs. This treatments improved subjective and objective sleep disturbances with psychopathological remission at the same time. So far, only hypothetical considerations do exist about the relationship between psychopathology and sleep disturbances. It is suspected that etiological relations exist between depression and desynchronization of central sleep mechanisms and between schizophrenia and special disturbances of REM-sleep and stage 3 and 4.
