[Influence of vasoactive substances on blood sugar and serum insulin in normal and diabetic carbohydrate metabolism (author's transl)].

The effect of the following vasoactive substances, which are used in the treatment of peripheral arterial occlusive diseases, was investigated in a randomized study in 36 patients with normal and 52 patients with diabetic carbohydrate metabolism by intravenous infusion on the behaviour of blood sugar and serum insulin (IMI) during simultaneous oral glucose tolerance tests (100 g oligosaccharides). The substances used and the doses given were as follows: protein-free calf-blood extract (Actihaemyl, 0,5 ml per kg body weight), bencyclane (Fludilat, 200 mg), naftidrofuryl (Dusodril, 200 mg, pentoxifyllin (Trenal, 200 mg). The results obtained with the simultaneous treatment and oral glucose tolerance test were compared with a second OGTT carried out at an interval of 3-4 days under the same conditions but without administration of the substances (in a cross-over procedure) and the results of these experiments were compared with those obtained from an untreated control group. In subjects with a diabetic metabolic state, Actihaemyl led to a significant reduction of the blood sugar after oral glucose load (p less than 0,05) without producing any change in serum insulin. The same behaviour was exhibited by Fludilat for the total area integral and by Trental for the first 60 min after the oral glucose load. The change in the blood sugar behaviour was only significantly different from the untreated controls with Actihaemyl (p less than 0,05). In subjects with a normal metabolic state neither blood sugar nor serum insulin (IMI) were altered by any of the substances investigated.

[Repetitive syncopale loss of consciousness by properly functioning pacemaker. Ophthalmodynamographic, ophthalmodynamometric, and cerebral-scinitigraphic investigations (author's transl)].

In spite of an intact pacemaker function, 3.8% of 444 patients with an implanted cardiac pacemaker taking part in a regular follow-up program had repetitive syncopes. The investigation thereafter, whether or not these attacks were symptoms of occlusive vascular disease of the carotid arteries, included a thorough physical and angiological examination, ophthalmodynamometry (ODM), ophthalmodynamography (ODG), and radionuclide angiography. The results were compared to those of a group of 46 cardiac pacemaker patients without syncopes after the implantation of the pacemaker: 29% of the patients with syncopes and 15% of the patients without syncopes showed ODM-, ODG-, and radionuclide angiography results compatible with carotid artery insufficiency; statistically, however, the group differences were insignificant. The reasons are discussed why ODM, ODG, and radionuclide angiography appear to be ineffective for the diagnosis of carotid artery occlusion in patients with an implanted cardiac pacemaker.

[Decrease of disaccharidase activity following application of bencyclan-hydrogenfumarate (author's transl)].

The disaccharidase activity of the small bowel mucosa of the rat is decreased by intraperitoneal application of Bencyclan-hydrogenfumarate. The doses applied were 10, 50 and 100 mg/kg body weight. The decrease rate after 100 mg/kg attained up to 50% of the control values. Furthermore, a reduction of villous height and an increase of the crypt depth has been observed in correlation with the enzyme activity decrease. Though functional and morphological alterations show a correlation, they cannot be explained by an antimetabolic-like effect because of the fact that the enzyme function recovers 72 h after application of the drug. Whilst our results do not sustain a cytostatic effect of Bencyclan-hydrogenfumarate, there is no doubt that the intraperitoneally applied drug influences the disaccharidase activity. These facts have to be considered in clinical therapy.

The effect of pentoxifylline on central and peripheral haemodynamics--an experimental clinical study.

The effect of an i.v. injection of 100 mg pentoxifylline on cardiac output, stroke volume, heart rate, atrial pressure, pulmonary arterial, pulmonary capillary and aortic pressure in normal myocardial function and in myocardial insufficiency was investigated in 8 patients with peripheral-arterial occlusive disorders. In normal myocardial function there was a brief, statistically significant increase in cardiac output of +9.6% and a simultaneous reduction in total peripheral resistance of -7.6%. No increase in cardiac output was recorded in patients with myocardial insufficiency. None of the other parameters monitored showed any statistically significant change.
