[75-Se-labeled bile acid analog absorption in various gastrointestinal diseases using a whole body counter].

AIM: It is possible to detect disturbances of bile acid absorption using a whole body counter after administration of Se-75 labelled bile acid analogues. We scrutinized the benefit of a modification of the test method. METHODS: We investigated 77 patients with different forms of a gastrointestinal disease. After application of Se 75 homotaurocholic acid we measured patient-activity up to 7 days later including whole-body profile scans in the first 6 h. RESULTS: The fractional retention after 7 days was between 20 and 67%. In cases of impaired absorption it was below 12%. Patients with liver diseases and after cholecystectomy (without bile acid resorption disturbance) showed normal values. Patients with Crohn's disease of the ileum or with intestinal ileac by-pass or with colestyramine treatment or with disturbance of vitamin B12-absorption or with cystic fibrosis showed a disturbance of bile acid absorption. The normal whole-body half-life was more than 2.8 days. The 24 and 72 h values were 62 and 31% in cases with normal absorption. Smaller values are signs of bile acid malabsorption. Impulse rates measured with the whole body counter are of an order of magnitude that allows to reduce the usually administered dose of 37 kBq to 9.25 kBq. CONCLUSION: This is an efficient method to detect disturbances of bile acid absorption. The usually administered activity of 37 kBq can be reduced to 9.25 kBq.

[Endemic sprue: its first diagnosis based on bleeding complications].

HISTORY AND CLINICAL FINDINGS: A 47-year-old man without previously known illness was admitted to hospital because of acute haematomas in the legs: the history revealed no cause. The pale-looking patient reported having large and foul-smelling stools once or twice daily. There were large haematomas and swellings on both legs. His general and nutritional state was reduced. "Bleeding of unknown origin" was suspected at this time. INVESTIGATIONS: Haemoglobin concentration was 5.6 g/dl, while iron was normal and ferritin reduced. Quick value was below 5%, activated partial thromboplastin time prolonged to 180 s. Vitamin A and E concentrations were reduced; coumarin derivatives were not demonstrated in blood. Abdominal ultrasonography showed clearly thickened intestinal walls. TREATMENT AND COURSE: Four units of erythrocyte concentrate were immediately administered, together with 2000 IU factors II, VII, X and anti-haemophilic factor B (PPSB), and 10 mg vitamin K intravenously. As intestinal malabsorption was suspected, a vitamin A absorption test was performed: it indicated malabsorption. Upper intestinal endoscopy showed coeliac disease, as did a biopsy. The patient's state quickly improved after he had been given vitamins A, D, E and K and put on a gluten-free diet. CONCLUSION: Coeliac disease can take a clinically unremarkable course for a long time and may finally become manifest through an isolated abnormality, such as bleeding.

[Symptomatic therapy with famotidine in non-erosive gastro-esophageal reflux. Results of an open multicenter study].

180 patients suffering from frequent heartburn and endoscopically normal oesophageal mucosa or mild non-erosive oesophagitis entered an open, multicentre study to evaluate the 6-week safety profile and efficacy on symptom relief of famotidine (CAS 76824-35-6, Pepdul mite), a potent and long-acting H2-receptor antagonist. By week 6 the cumulative percentage of patients with defined response, that is complete relief of heartburn in 5 days and only mild discomfort in the remaining 2 days of a week, reached 68.9%, whereas the cumulative percentage of patients with complete relief of heartburn within a week reached 52.7%. Throughout the evaluation period famotidine relieved nighttime heartburn better than daytime heartburn. More than 75% of the responders remained without recurrence. Even the non-responders experienced a 60-70% reduction of heartburn severity assessed using scores. Antacid consumption was reduced from 18 tablets (median) in week 0 to 5 tablets in week 6. 90% of the patients reported at week 6 excellent (67.2%) or moderate (22.8%) symptomatic improvement. No serious adverse events attributable to famotidine occurred. It is concluded that in patients with non-erosive gastro-oesophageal reflux disease famotidine therapy, 20 mg twice daily, is highly effective in reducing reflux disease symptoms.

[Absorption and renal elimination of bismuth from 6 different bismuth salts after a single dosage].

The absorption and renal excretion of bismuth from seven pharmaceutical bismuth preparations was studied in five healthy volunteers. Following a single oral dose of colloidal bismuth subcitrate (CBS), basic bismuth salicylate (BSS), basic bismuth gallate (BSG), basic bismuth carbonate (BSC), bismuth aluminate (BA) and two different basic bismuth nitrate (BSN) preparations, ingested one hour prior to the breakfast, containing 350 mg bismuth in all but one bismuth subnitrate compound, the latter was given in a 1480 mg dose laid on granulated gum karaya, the bismuth serum concentrations (0 to 6 h) and urinary excretion (for 2 days) were measured. Slight amounts of bismuth were rapidly absorbed in all experiments with low transient mean peak concentrations between 0.6 (BSN) and 9.1 micrograms/l (CBS)--reached within 20-60 min--with a wide intra- und interindividual variability. The highest peak serum bismuth concentration was 18 micrograms/l in response to CBS. The median integrated 6-hour serum bismuth concentration after dosing with CBS and BSG was 49 and 32 ng h/ml, respectively, after dosing with BSC, BA, BSS, BSN and BSN1480 13, 11, 3, 0 and 5 ng h/ml, respectively. The values for CBS were significantly greater than after BSS and BSN350. There was a very strong overall correlation between 48-h urinary bismuth excretions and the corresponding integrated 6-hour serum bismuth concentrations (p < 0.001).

[Lipoprotein metabolism in menopause. Effect of hormonal substitution therapy].

Oral administration of conjugated estrogens, estradiol valerate and micronized estradiol--but not the percutaneous application--in the postmenopause modifies the plasmic lipoprotein profile by lowering, dose-dependently, LDL and elevating HDL (HDL2). In parallel, the cardiovascular mortality is decreased by 50-66%, with smokers also benefiting to the same extent. On account of the increased risk for endometrial carcinoma associated with postmenopausal estrogen monotherapy, combination with a lowest-dose gestagen is imperative. However, the very numerous synthetic gestagens can antagonize the favorable effects of the estrogen on lipoprotein metabolism. This applies in particular to the gestagens of the 19-nortestosterone type, such as norethisterone acetate and, in particular, levonorgestrel, but less so the 17-hydroxyprogesterone derivatives medroxyprogesterone acetate and medrogestone with their very low androgenic effect.

[Chemically prepared fats and Crohn disease. A pilot study of the occurrence of trans-fatty acids in the subcutaneous tissue of Crohn patients in comparison with healthy controls as a parameter of long-term fat intake].

In a pilot study the fatty acid pattern of subcutaneous adipose tissue from 22 patients with Crohn's disease and 22 subjects of a healthy control group was analyzed using glass capillary gas-liquid chromatography. Among all fatty acids amounting to at least 1% peak area of the chromatograms, only trans-octadecenoate differed significantly (p less than 0.05) between both study groups, the mean value being 2.39 +/- 0.83% in patients with Crohn's disease and 1.96 +/- 0.46% in healthy controls. Also the mean value of trans-hexadecenoate was significantly (p less than 0.05) higher in the Crohn group (0.25 +/- 0.07%) than in the control group (0.21 +/- 0.06%). There was a strongly positive linear correlation (p less than 0.001) between the trans-hexadecenoate and trans-octadecenoate values for the Crohn patients but not for the controls. Our results demonstrate that patients with Crohn's disease as a group consume more trans-monoene fatty acids than healthy controls, thus providing evidence for a higher intake of chemically processed fats like margarine, shortenings, frying and cooking fats. In further studies which are necessary to examine Guthy's hypothesis the fatty acid composition of adipose tissue should be followed up as an ideal marker of long-term dietary compliance.

High dosage desferrioxamine therapy in a female patient with acquired aplastic anaemia and transfusion siderosis.

A 32 year old woman with severe aplastic anaemia required frequent transfusions and consequently developed hyperferrioxaemia (54 microMol/l) and hyperferritinaemia (1,700 ng/ml). For the treatment of transfusion siderosis she was given 18 high dose courses each comprising 35 g of desferrioxamine. Because of pre-existing thrombocytopenia (platelet count 5 X 10(9)/l) the iron chelating agent was given by continuous intravenous infusion over 3 1/2 days. High dose desferrioxamine had to be abandoned because of severe bone pain. The desferrioxamine infusions achieved a negative iron balance, iron loss after each infusion being 100 to 200 mg in the urine and 400 mg in the faeces. Serum iron and ferritin concentrations fell almost to normal. This report shows that faecal iron excretion must be taken into account in assessing the balance of iron input and output during desferrioxamine treatment.

The effect of different fat supplemented diets on the erythrocyte ghosts and plasma lipid composition of geriatric subjects.

The responses of the lipid composition of erythrocyte ghosts and plasma phospholipid fatty acids of 20 female donors, aged older than 70 years, to different margarine and butter supplemented diets were investigated. On a low level of daily caloric intake (1900 kcal) there were no changes in phospholipid and cholesterol content of the red cell membranes, but statistically significant changes were found in erythrocyte and plasma phospholipid fatty acid patterns. These results demonstrate the influence of the fatty acid composition of the dietary fats with special reference to saturated, trans-isomeric monoene, and polyunsaturated acids.

Treatment of human fascioliasis with niclofolan.

We report a case of human Fasciola hepatica infection. For the first time evidence is presented of successful treatment of fascioliasis by oral administration of niclofolan, a biphenyl anthelmintic compound that is available for clinical testing in West Germany. Niclofolan therapy achieved elimination of eggs from the feces and normalization of eosinophil counts, liver enzymes and Fasciola titers, suggesting eradication of biliary flukes in our patient.
