[A clinical study of amino acid metabolism in cancerous hypoproteinemia--the role of branched chain amino acids (BCAA) and appropriate compositions of BCAA in parenteral nutrition].

In order to determine the nutritional effects of BCAA compositions in the treatment of cancerous hypoproteinemia, the appropriate ratio of I-leu: Leu: Val and the proportion of BCAA to Total Amino Acids were investigated. As for results, indices such as the serum albumin, the RBP and N-balance quickly recovered to normal levels when the ratio of I-leu: Leu: Val was 1.0:1.8:1.0 and the proportion o BCAA to TAA was 31%. These composition thus may be suitable for the treatment of cancerous hypoproteinemia.

[Therapeutic effect of stage III breast cancer--comparative study of preoperative intra-arterial infusion chemotherapy and non-infusion therapy].

Between January 1977 and March 1988, 111 patients of stage III breast cancer were treated in Sapporo Medical College. The median age was 51.6 years (range, 26-83 years). Menopausal status was 47 cases of premenopausal, 19 cases of perimenopausal and 45 cases of postomenopausal. Fifty-seven of these patients received intra-arterial infusion chemotherapy. The anticancer drugs were mainly given by two routes, infusion into the internal mammary artery and the subclavian artery. Continuous infusion of 5-FU and intermittent injections of MMC, ADR or 4'-epi-ADR were jointly or individually made in each artery. Clinico-histological response was studied in 53 out of 57 patients. Clinical response, defined as CR + PR, was noted in 33 patients (62.3%). Histological response according to Shimosato Criteria, defined as grade IIb or better, appeared in 31 patients (58.5%). Non-infusion group contained 27.7% of stage IIIb, against 72.2% in infusion group. The 5-year overall survival rates were; non-infusion group 62%, 5-FU infusion group 34.1%, and ADR MMC group 66.2%. A significant difference was seen between 5-FU infusion group and ADR-MMC group (p = 0.03). The median disease-free interval was 22.0 months (range, 1-77 months) in infusion group and 28.2 months (range, 1-124 months) in non-infusion group. MMC + ADR combination group revealed the most excellent results in terms of the 5 year survival rate and clinicohistological effect.

[Effects of anti-toxic agents in septic shock].

From 1975 to 1987, we had 56 patients of septic shock in the Department of Surgery. Multiple organ failure occurred in many septic patients. Glucocorticoids inhibited the secretion of chemical mediators (histamine, serotonin and bradykinin) and then prevented septic shock. Intravenous administration of dexamethasone showed no change in amounts of leukotrienes (LTC4, LTD4, LTE4) in venous blood in peritonitis rats. Dexamethasone treatment of septic rats corrected FDP and nearly normalized PEP values. When glucocorticoid was given intravenously at the time of cecal incision, PFKase, PKase, G6Pase and PEPCK were stimulated, respectively. Protease inhibitor FUT-175 was infused in 5% dextrose (0.1mg/ml/hr) in septic rats. Survival time was 12.1 +/- 2.3 hour in FUT-175 group and 6.6 +/- 1.1 hr without FUT-175. In FUT-175 injected rats G6P decreased by 20%, FDP increased 50% and lactate doubled. PEP levels increased 30% above peritonitis values. The amounts of leukotrienes (LTC4, LTD4, LTE4) in venous blood were gradually decreased by pretreatment with the specific 5-lipoxygenase inhibitor AA-861 after peritonitis. Specific treatments in septic shock should be instituted administration of glucocorticoid, antibiotics, protease inhibitor and lipoxygenase inhibitor. The importance of septic shock as a factor contributing to organ failure must be acknowledge. We believe that the prompt and efficacious treatment of septic shock is the best therapy.

[Intra-arterial infusion chemotherapy of advanced breast cancer--effects and side effects of adriamycin, 4'-epi-adriamycin and THP-adriamycin].

In 34 patients with primary advanced breast cancer, intra-arterial administration of ADR (50 mg X 3, total dose 150 mg, 10 cases), 4' epi ADR (50 mg X 3, 150 mg, 8 cases; 70 mg X 3, 210 mg, 10 cases) and THP-ADR (50 mg X 3, 150 mg, 6 cases) was performed, and its effects and side effect were analyzed. The clinical and histological response rate were superior in the ADR (150 mg) regimen and 4'-epi-ADR (150 mg) regimen. Signs of systemic toxicity such as gastrointestinal disorders, leukocytopenia and thrombocytopenia were the side effects in patients treated with THP-ADR, but the frequency of alopecia was lower. No cardiotoxicity was recorded in any of the patients. These results indicated that 4'-epi-ADR given the total dose of 150 mg in a single dosage of 50 mg was the most effective agent in intra-arterial infusion chemotherapy for advanced breast cancer.

[Pre- and postoperative nutritional management in gastric cancer patients].

Pre and postoperative metabolism of electrolytes and protein were studied in gastric cancer patients. Eight patients who underwent distal subtotal gastrectomy for gastric cancer, were treated with total parenteral nutrition (TPN) from the third day before surgery to the tenth day after surgery. Twelve patients who underwent total gastrectomy for gastric cancer were treated with enteral nutrition (EN, T-330, ED-AC) and TPN in the early postoperative phase. The results were as follows. 1) P which existed in intracellular space showed a decrease in the plasma level and a negative balance in the early postoperative phase. Thereafter, the plasma level of P ion increased and a positive balance was observed. 2) Ca showed a positive balance during the observation period. 3) The level of prealbumin and retinol-binding protein decreased to some extent after surgery, but came up to the preoperative level on the seventh postoperative day in enteral nutrition (T-330) after total gastrectomy. 4) Nitrogen balance was observed to show a positive balance on the seventh postoperative day. There was no statistical difference between the three groups. 5) There were significant relationships between the serum albumin level and the development of postoperative complications.

[Local treatment of locally recurrent breast cancers].

For the treatment of recurrent breast cancer, surgical treatment of the local lesion is only of limited significance, leaving a major role for systemic treatment such as chemotherapy and endocrine therapy. Therefore, a new method of local treatment other than surgical removal alone, or systemic treatment of these locally recurrent breast cancers is required. For the treatment of locally recurrent breast cancer, intra-arterial infusion chemotherapy was performed through both the internal mammary artery and subclavian artery or individually. Fourteen cases of locally recurrent breast cancer in our hospital were treated with this therapy. Continuous infusion of 5-fluorouracil and intermittent injections of mitomycin, adriamycin or 4'-epi-adriamycin were jointly or individually done into each artery. Regression of the locally recurrent mass was periodically measured from 4 to 6 weeks after infusion. The percentage of patients with PR was 35.7%. Intra-arterial infusion chemotherapy is a useful preliminary procedure for systemic treatment of local lymph node recurrence from breast cancer. A treatment involving local infusion of the non-specific immuno-activators, OK-432 and PSK, has also been performed for local recurrent lesions of breast cancers. We injected OK-432 and 5% PSK solution into patients with locally recurrent breast cancers and studied their suppressive effect on tumor proliferation. Macroscopically, PR was noted in 3 of 12 patients given OK-432. CR+PR were obtained in 9 of 12 patients given PSK. These results suggest that local injection of OK-432 and PSK solution is beneficial in the management of nodular or scattered-type locally recurrent tumors except for infiltrating type.

[Antibiotic susceptibility of bacteria isolated from surgical infection (second report)].

Isolated bacteria from infections in general surgery in 1984 and 1985 have been investigated to find bacterial composition and their susceptibilities to antibiotics in a joint research in which 6 university hospitals in Japan participated. A summary of findings from the investigation is as follows. 1. One hundred and seventy-two (1984) and 211 (1985) cases were included in the study. Cases in which bacteria were detected were 147 and 174 in the respective years. The detection rate was higher than 80% in either year. 2. Total numbers of strains isolated in 1984 and 1985 were 267 and 293, respectively; major sources of these strains were intraperitoneal infection exudates in either year. 3. The most frequent isolate from primary infection cases in both years was Escherichia coli (15-21%), followed by Bacteroides spp. and Staphylococcus spp., in that order. The most frequently isolated from postoperative infection cases were Enterococcus spp. (16-22%), followed by Pseudomonas spp. The diversity of isolated species, as well as the similarity of incidences of different species were noted in cases of postoperative infections. It is suspected that a certain species, even if its pathogenicity is essentially low, may become to be a causative organism once its number increases due to its survival through a perioperative prophylactic use of antibiotics, and also due to the decreased host resistance to infections caused by underlying diseases or surgical stress. 4. Staphylococcus spp. was the most frequent isolate from postoperative infections occurring after clean operations, while Enterococcus spp. and Pseudomonas aeruginosa were major isolates from infections after clean-contaminated operations. Isolates from infections occurring after contaminated operations included Enterococcus spp. greater than E. coli greater than Klebsiella pneumoniae, P. aeruginosa, Bacteroides spp. (1985). 5. In cases without the presence of clinical factors cause by depressed host defense, E. coli and Bacteroides spp. were major isolates, while in cases with the factors, a wide variety of bacterial population tended to be found. 6. Before an administration of antibiotics in primary infections, E. coli, Staphylococcus spp. Bacteroides spp. and Klebsiella spp. were most commonly isolated, while after a chemotherapy, Enterococcus spp. were the most frequent isolates, followed by P. aeruginosa during 1985. These findings reflected the antibacterial spectrum of cephems usually used in surgical field.(ABSTRACT TRUNCATED AT 400 WORDS)

[Intraarterial chemo-embolization with adriamycin containing albumin microspheres].

For local treatment of inflammatory breast cancer and recurrent breast cancers with liver metastasis, we applied intraarterial infusion chemoembolization therapy utilizing albumin microspheres containing adriamycin (ADM-ms), which was developed by Morimoto. In this study, there were 2 cases of inflammatory breast cancer and 3 cases of recurrent breast cancers with liver metastasis. Arterial canulation of primary breast cancer were made through both arteries, the internal mammary artery and lateral thoracic artery, which were feeding arteries of the breast. ADM-ms was injected into both arteries and its tumor effect was evaluated clinically and histologically. Remarkable reduction in tumor size was recognized. Histologic examination of resected specimens revealed extensive fibrosis in the center of the primary tumors. A few cancer cells, degenerated in various degrees, were found remaining in the periphery of the necrotic mass. Local side effects of the microspheres were skin disturbances such as pain, redness and blistering. ADM-ms was injected into the hepatic artery for the liver metastatic cases, and its antitumor effect was evaluated by CT scan or echography. Effective reduction in tumor size was only recognized in one case. No serious systemic side effects, not recognized in any case. Our report reviews the effectiveness of this therapy as a preliminary procedure for surgical treatment of inoperable inflammatory breast cancer.

[Adrenal circulation of rabbits in hemorrhagic and endotoxin shock].

Tissue circulation of the adrenal gland which is known as antishock organ was investigated by the hydrogen clearance method in hemorrhagic and endotoxin shock using rabbits. Mean blood pressure was maintained at 35 mmHg for two hours in the hemorrhagic group, while 2mg/kg of endotoxin was injected intravenously in the endotoxin group. The data obtained were as follows: Adrenal tissue circulation and renal circulation were reduced to 40% and 14.9%, respectively in the hemorrhagic group and reduced to 69% and 48%, respectively in the endotoxin group. Plasma corticosterone concentration was significantly elevated in both groups. Plasma beta-glucuronidase was significantly elevated in both groups and most animals died within 72 hours. From these data we concluded that adrenal circulation was distributed preferentially at the sacrifice of renal circulation in the shocked state.

[Clinical study of protein and amino acids metabolism in surgical infection--significance of administration of branched chain amino acids].

Anastomotic leakage and fistula of alimentary tract are accompanied by peritonitis because of exudation of contents in the digestive canal. In order to inquire about metabolism of protein and amino acids in surgical infection, this study was performed by the method of a time related cross-over trial, comparing high branched chain amino acids (BCAA) solution with low BCAA solution. The following results were obtained. When metabolism is physiologically compensated in infection, it was suggested that there appeared to be gluconeogenesis, ureagenesis and a liver disturbance accompanied in the early phase of sepsis. Administration of BCAA leads to an inhibition of muscle protein break down and a promotion of visceral protein synthesis in the case of severe infection. Administration of BCAA enriched amino acids solution have an important therapeutic measure under severe stressed condition and a significant role in medicine.

[Antibiotic susceptibility of bacteria isolated from surgical infections (first report)].

In vitro activities of several antimicrobial agents against bacterial pathogens isolated from patients with primary and postoperative infections were investigated in 1982 and 1983. Antimicrobial agents examined were as follows: sulbenicillin (SBPC), piperacillin (PIPC), cephalothin (CET), cefazolin (CEZ), cefmetazole (CMZ), cefotiam (CTM), cefoperazone (CPZ), cefotaxime (CTX), ceftizoxime (CZX), cefmenoxime (CMX), latamoxef (LMOX), lincomycin (LCM), gentamicin (GM) and amikacin (AMK). Specimens for bacterial isolation included plus, fluid drawn by centesis, or bile. Blood samples of septicemia were excluded. The antimicrobial activities of these drugs were determined by the agar plate dilution method of the Japanese Society of Chemotherapy. There were 123 strains obtained in the 1982 survey and 252 strains in the 1983 survey. Little or no differences were seen in frequencies of isolation between the isolates of principal species in 1982 and those in 1983. Isolation frequencies of pathogens in primary infections were, in an order of decreasing frequency, E. coli (25.6%), anaerobes (21.1%), Streptococcus sp. (14.3%), Staphylococcus sp. (11.3%); in postoperative infections, Streptococcus sp. was most frequent (28.6%), followed by Pseudomonas sp. (17.6%), anaerobes (12.6%), E. coli (10.9%), Staphylococcus (10.1%). Against S. aureus, CEZ, CTM, LCM and GM had similar degree of activity with CET being somewhat more active. CMX was the most active drug among the third generation cephems tested against S. aureus. No strain was CTM, CEZ, and LCM-resistant at the same time. Over 90% of E. coli, were sensitive to CTX, CZX and CMX, inhibited by 0.10 microgram/ml, while E. coli were slightly less susceptible to CPZ and LMOX. Penicillins were not very active against K. pneumoniae, and only 60% of K. pneumoniae were inhibited by PIPC at concentrations of 12.5 micrograms/ml. Third generation cephems, CTX, CMX and CZX, proved highly active against K. pneumoniae; over 90% of K. pneumoniae was inhibited by CTX, CMX and CZX at a concentration of 0.10 microgram/ml. About 60% of P. aeruginosa was inhibited by 3.13 micrograms/ml of PIPC and GM but was resistant to SBPC. This survey should be very useful for the selection of an appropriate drug for prophylaxis if the frequencies of incidences of pathogens in postoperative infections are taken into account in selecting the most active antibiotic agent(s) against the most frequent genus, genera and species of pathogens.

[Toxicological studies on (2''R)-4'-O-tetrahydropyranyladriamycin, a new antitumor antibiotic. Its acute toxicity in beagle dogs].

(2"R)-4'-O-Tetrahydropyranyladriamycin (THP), a new antitumor antibiotic of anthracycline derivative, was given intravenously to male and female Beagle dogs, followed by observations for 2 weeks. The doses were 0.125, 0.25, 0.5 and 1.0 mg/kg. Observations were performed on mortality, clinical signs, body weight and temperature, food and water consumption, ECG, ophthalmoscopy, testicular measurement, hematology, serum biochemistry, urinalysis, autopsy, histopathology and electron microscopy. The results were as follows. All animals of 1 mg/kg-treated group died but there were no deaths in other groups. All the levels of doses caused vomiting and diarrhea. Furthermore, the highest dose caused bloody diarrhea and decreases in food consumption, motility and body weight, followed by death. These effects may be related to histopathological changes in mucosa of digestive tract. Some changes in water consumption, urine volume and renal histopathology were sporadically observed after treatment, but they were not treatment-related. These results suggest that a treatment with THP does not significantly cause renal disturbance. There were no treatment-related changes in body temperature, ophthalmoscopical observation and testicular measurement. THP produced flattening of T wave and prolongation of QRS interval only at high doses of more than 0.5 mg/kg for males and of 1 mg/kg for females but did not cause any histopathological or electron microscopical changes in hearts. It seems that the abnormalities caused by THP in ECG traces are due to the deterioration of general conditions rather than the direct effect on heart. High doses produce the significant changes in platelet, WBC, Hb, Neutro. (Seg.), Mono., Hb, RBC and Lymph. and further regressive changes in thymus, spleen and bone marrow. These results suggest that THP causes some effects on hemopoietic tissues. The sporadic changes in serum biochemistry and urinalysis were neither related to the treatment nor to the changes in organ weight, autopsy, histopathology and electron microscopy. Therefore, it is likely that THP does not cause toxicological effect on the other tissues and organs except hemopoietic tissues.

[Intrinsic endotoxemia after massive hepatectomy--effect of intra-intestinal antibiotics].

It is known that intrinsic endotoxemia is caused by two main mechanisms, reticuloendothelial system dysfunction and intestinal circulatory failure. We investigated the escape of endotoxin from the intestine under occlusion of the superior mesenteric artery 3 days after 70% liver resection in rabbits. Twelve rabbits were given intra-intestinal antibiotics (Antibiotics group) and other twelve rabbits were given saline only (Control group). The results were as follows. Indocyanine green clearance and lipid clearance were deteriorated in both groups after hepatectomy but the antibiotics group showed a better clearance rate than the control group. Serum endotoxin level was elevated slightly after hepatectomy and then grossly elevated after superior mesenteric artery occlusion, but the antibiotics group showed a lower endotoxin level than that of the control group. Seventy-two hours survival rate was apparently better in the antibiotics group. From those data we conclude that the administration of intraintestinal antibiotics contributed to the maintenance of reticuloendothelial function of remnant hepatic lobe and suppressed the serum endotoxin level after superior mesenteric artery occlusion.

[Intra-arterial infusion chemotherapy with anticancer agents for advanced breast cancer--effect and toxicity of mitomycin C and adriamycin].

In 23 cases of primary advanced breast cancer, intra-arterial infusion chemotherapy of Adriamycin (ADR) and Mitomycin C (MMC), which were injected jointly or individually, was performed and its effects and side effects were studied. As for the clinical effects, the response rate (CR + PR) was 73.9% (17/23 cases) and the histological response rate (greater than grade IIb) was 82.6% (19/23 cases). ADR alone (100-150 mg) and MMC (28 mg) + ADR (42 mg) combined regimens were especially superior in both their clinical and histological effects. In metastatic lymph nodes, the histological response rate was 78.9% (15/19 cases). As for the side effects, in the cases treated with MMC, bone marrow suppression such as leukocytopenia and thrombocytopenia was remarkable and took a long time to recover. The above results suggested that the most effective regimen for primary advanced breast cancer using intra-arterial infusion chemotherapy is ADR alone, in single doses of 50 mg up to a total dose of 150-200 mg. Histological examination of the effective cases revealed that the central region of the tumor was more markedly recrotic than the periphery. It was suggested that the grade of the effects on tumor tissues is related to the mechanism of anticancer agents.
