Primary aldosteronism with bilateral adrenal adenomas.

An unusual case of primary aldosteronism with bilateral single adenomas is reported. The two tumors were revealed by computerized axial tomography and subsequently confirmed by surgical exploration. Spironolactone therapy prior to the operation induced the formation of spironolactone bodies in only one of the two adenomas. As it has been postulated that these cytoplasmic inclusions may reflect the activity of the adenomatous cells, the presence of the bodies in a single adenoma would indicate a unilateral source of the hyperaldosteronism. Thus, the existence of spironolactone bodies could corroborate the data of functional localizing tests more closely than the morphological findings of computerized tomography.

Computed tomography of the dilated main pancreatic duct.

The ducts of the pancreas undergo changes secondary to pancreatic disease. This report is based on computed tomographic studies of 13 cases with enlarged main pancreatic ducts. All patients showed severe pancreatic disease of tumoral or inflammatory etiology. The dilatation of the main pancreatic duct was confirmed by endoscopic retrograde cholangiography and pancreaticography, surgical intervention, biopsy, and/or autopsy in all but two cases.

Radiological findings in multiple hamartoma syndrome (Cowden disease): a report of three cases.

The radiological findings in three cases of multiple hamartoma syndrome (MHS) (Cowden disease) are reported. MHS was associated with a widespread distribution of polyps of varied morphology along the gastrointestinal tract. There was an isolated case of bladder carcinoma, and the one woman studied had breast masses. In view of the nonspecific clinical findings in this disease, which may also suggest carcinoma, the importance of the radiological recognition of MHS is stressed.

[Axial tomodensitometry in acute pancreatitis].

Early surgery for the treatment of acute necrotizing pancreatitis is important. With the CAT-scan we evaluated the extent of the lesions of the pancreatitis in 26 patients with acute necrotizing pancreatitis. Due to severe clinical symptoms and the CAT-results 12 of these patients had to be operated. We found a good correlation between intraoperative extent of disease and the CAT-scan.

Cerebral gigantism (Sotos syndrome) with juvenile macular degeneration.

A 6-year-old girl had an excessively rapid longitudinal growth of early onset (height age of 9 years), moderate obesity, large hands and feet, a large dolichocephalic head and facial features as described in Sotos syndrome or cerebral gigantism. In addition, she exhibited mental dullness, hypotonia and clumsiness. CT scanning of the head demonstrated major ventricular anomalies and absence of corpus callosum. Fundoscopy and electroretinography revealed an early stage of atrophic macular degeneration (AMD) with cone dysfunction, bilaterally. Wether this association of cerebral gigantism and AMD is fortuitous or not is unknown.

Structural organization of free and esterified cholesterol in human high density lipoproteins. A 100.6 MHz 13C NMR study.

The structural organization of free and esterified cholesterol in human high density lipoproteins has been studied by high-field 1H and 13C NMR. The measurements are consistent with free cholesterol being present in at least two different environments. Part of the free cholesterol is oriented in the outer surface layer of the high density lipoprotein particle in contact with phospholipid or apoprotein, or both. The rest is probably present in the liquid, hydrophobic core of the HDL particle.

Clustering of fatty acids in phospholipid bilayers.

From electrophoresis experiments it is concluded that acidic phospholipids incorporated in liquid crystalline phosphatidylcholine bilayers at neutral pH are randomly distributed. The same is true for spin-labelled fatty acids. In contrast, long chain fatty acids are not fully ionized at neutral pH and appear to be clustered, i.e. they segregate out into patches. Only at pH greater than 11 is the fatty acid-COOH group fully ionized and charge repulsion leads to a random distribution of the fatty acid within the plane of the bilayer.

Effect of stress, adrenocorticotropin or corticosteroid treatment, adrenalectomy, or hypophysectomy on hypothalamic immunoreactive adrenocorticotropin concentrations.

Despite variations in plasma and/or pituitary ACTH concentrations, no changes in median eminence or medial basal hypothalamic immunoreactive ACTH-like concentrations were seen in rats studied 6 weeks posthypophysectomy, 12 h or 28 days after bilateral adrenalectomy, 6 days after implantation of corticosterone pellets (75 mg), after dexamethasone administration (300 micrograms/100 g BW, ip, twice daily for 4 days), or after chronic immobilization stress. The lack of concordant variation of brain, pituitary, and plasma ACTH concentrations further supports the suggestion of a nonpituitary, central nervous system origin of some portion of brain immunoreactive ACTH-like activity.
