Purification and properties of a chromosomal beta-lactamase from Klebsiella oxytoca.

A beta-lactamase was purified from Klebsiella oxytoca strain GN10650. The enzyme was chromosomally-mediated and gave a single protein band on polyacrylamide gel electrophoresis. Its pI was 5.34 and its MW was approximately 27,000. The optimal pH and temperature were about 7.0 and 50 degrees C, respectively. The specific activity of the enzyme was 1,207 units per mg of protein for hydrolysis of penicillins and cephalosporins, including cefuroxime, cefotaxime, and aztreonam. The enzyme activity was inhibited by p-chloromercuribenzoate, iodine, ferrous ion, and by clavulanic acid. Rabbit antibodies raised against the purified K. oxytoca enzyme showed no cross-reactivity in neutralization tests with beta-lactamases produced by other species of Gram-negative bacteria.

Presence of human papillomavirus genome in human tumor cell lines and cultured cells.

A series of 47 human carcinoma cell lines and their cultured cells were examined for human papillomavirus (HPV) genomes with the use of an HPV detection kit (DNA-RNA hybridization, mixed HPV DNA probe of types 6, 11, 16, 18, 31, 33 and 35). Four of 8 cases of mild dysplasia, 3 of 9 cases of severe dysplasia, 3 of 7 cases of carcinoma in situ, 3 of 15 cases of uterine carcinoma and 5 of 6 cases of condyloma acuminatum were shown to contain the HPV DNA genome in primary cultured cells, while HPV was not detected in the third-passage cells except for the three cases of large cell, nonkeratinizing squamous cell carcinoma. HPV was also not detected in such normal tissues as uterine cervical squamous epithelium, uterine cervical columnar epithelium and endometrium. The presence of HPV DNA genomes was detected consistently in the passages of three lines (SKG-II, HKMUS and HKTUS; large cell nonkeratinizing squamous cell carcinomas of the uterine cervix) with the use of the Southern Blot method (DNA-DNA hybridization, mixed HPV probe of types 6, 11, 16 and 18). HPV type 16 DNA was detected in HKTUS, and HPV type 18 DNA was found in SKG-II and HKMUS. The other 44 cell lines, including ovarian carcinoma, endometrial carcinoma, sarcoma, gastric cancer, pancreatic cancer and rectal cancer, were negative for the HPV-6, HPV-11, HPV-16, HPV-18, HPV-31, HPV-33 and HPV-35 genomes under stringent hybridization conditions.

Features of the acute hypotensive action of carvedilol and its ameliorating effect on myocardial ischemia.

The objective of the present experiment was to investigate the following parameters concerning carvedilol: (a) the time course of acute hypotensive effect, (b) the influence of alpha 1-adrenoceptor blockade on the acute hypotension induced by carvedilol, and (c) the effect of carvedilol on myocardial ischemia. In conscious, catheter-implanted spontaneously hypertensive rats, carvedilol at the doses of 10 and 30 mg/kg, p.o., dose-dependently decreased mean arterial blood pressure (MAP). At 10 mg/kg, MAP was significantly decreased for 16 h, and at 30 mg/kg, it was significantly decreased until 24 h after administration. Thus, carvedilol shows long-lasting antihypertensive effects by a single oral administration. In anesthetized Wistar rats, intravenous administration of carvedilol (0.3 mg/kg) produced an immediate decrease in blood pressure. On the other hand, in phentolamine-pretreated rats whose blood pressure was restored by prostaglandin F2 alpha, the acute hypotension produced by carvedilol was abolished similarly to the case with labetalol (0.6 mg/kg) and prazosin (0.01 mg/kg). The hypotensive effect of either hydralazine (0.2 mg/kg) or nitroprusside Na (0.005 mg/kg) was attenuated by about 20%, whereas the effect of diltiazem (0.2 mg/kg) was not attenuated. These results indicate that alpha 1-adrenoceptor blocking action of carvedilol contributes to a large extent to the acute hypotensive effect of this drug. In anesthetized open-chest dogs, myocardial ischemia was produced by partial occlusion of the left anterior descending coronary artery. Myocardial pH of the ischemic region was decreased by about 0.7 units. Carvedilol at the doses of 0.1 and 0.3 mg/kg, i.v., dose-dependently attenuated the decrease in myocardial pH accompanied with decreases in MAP and heart rate.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--10. Effects of prostaglandin E1 on the hepatic and renal functions following prolonged surgery under total intravenous anesthesia].

Twenty one patients who underwent prolonged surgical procedures over 10 hours under total intravenous anesthesia with droperidol, fentanyl and ketamine were studied to evaluate post-operative hepatic and renal functions as judged by serum levels of GOT, GPT, BUN and creatinine. They were divided into two groups. Ten patients of the PGE1 group were given PGE1 at a rate of 0.035 micrograms.kg-1.min-1 during anesthesia, and the remaining eleven of the control group were not given PGE1. The two groups were comparable concerning, age, body weight, height, operation time and anesthesia time. In the PGE1 group, significantly more intraoperative fluid was given than in the control group. The blood loss was more but insignificantly in the PGE1 group than in the control group. There was no significant difference in urine output and the amount of blood transfused between the two groups. In both groups, post-operative s-GOT and s-GPT levels were increased significantly compared with pre-operative values, but there was no significant difference between the two groups. Serum BUN levels of the 7-10 the post-operative days were increased significantly in the PGE1 group, but those of the control group were not. These data suggest that our method of total intravenous anesthesia with droperidol, fentanyl and ketamine, when applied even for prolonged surgical procedure over 10 hours, would have beneficial effects on the post-operative hepatic and renal functions.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--12. Effects on plasma complement and immunoglobulin concentrations].

Complements and immunoglobulins in the plasma are the important humoral factors to maintain immunity. As there is no study on immune response to total intravenous anesthesia with droperidol, fentanyl and ketamine (DFK), twelve patients who underwent abdominal, neck dissection, or plastic surgery were studied to determine plasma concentrations of complements and immunoglobulins. In five patients of isoflurane group, anesthesia was induced with intravenous thiopental 5 mg.kg-1 and succinylcholine 0.8-1 mg.kg-1 and maintained with 1-2% isoflurane in nitrous oxide (50%) and oxygen (50%). The remaining seven patients of the DFK group received intravenous droperidol 0.25 mg.kg-1, fentanyl 1-2 micrograms.kg-1, ketamine 1-1.5 mg.kg-1 and succinylcholine 0.8-1 mg.kg-1 for the induction of anesthesia, and then they were given a total dose of fentanyl 5-15 micrograms.kg-1, ketamine 2 mg.kg-1.hr-1 and oxygen (30%) for the maintenance of anesthesia. Vecuronium was given intravenously as needed. Lactated Ringer's solution was used for intraoperative fluid replacement. A total of 40 ml of arterial blood was drawn on four occasions, just before the induction of anesthesia, at the recovery from anesthesia, on the third and tenth post-operative days. Plasma concentrations of complements (C3.C4) and immunoglobulins (IgG.IgA.IgM.IgD) were measured by immuno-turbidimetry. C3 concentrations in the plasma decreased significantly when the patients recovered from anesthesia, but they increased significantly on the third and tenth post-operative days in the isoflurane group. In the DFK group, they increased significantly on the tenth post-operative day only. No significant difference in the C3 concentrations was detected between two groups at any time of measurement.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--13. Application for pediatric patients].

Total intravenous anesthesia with droperidol, fentanyl, and ketamine (FK) was administered to 56 pediatric surgical patients ranging in ages from 5 to 15 years to evaluate their hemodynamics during anesthesia, post-operative hepatic as well as renal functions, and post-operative sedation as well as analgesia. These data were compared with those of the patients who underwent almost the same surgical procedures under enflurane-N2O anesthesia. The post-operative s-GOT, s-GPT, BUN, creatinine levels were not elevated significantly as compared with pre-operative levels in the FK group. As compared with those patients who received enflurane anesthesia, the blood pressure in the FK groups was higher by 15-30 mmHg, but it was stable during anesthesia without any complications. Their post-operative sedation and analgesia were better in the FK group than in the enflurane group and the complications such as nausea and vomiting were observed less frequently in the FK patients than in the patients who received anesthesia with ketamine alone reported in literatures. The data described above suggest that this method of anesthesia deserves further detailed clinical trials for pediatric patients.

Significance of sinking particles in the distribution of motile Aeromonas during the winter season.

The role of sinking particles in the distribution of motile Aeromonas species was studied during the winter season. Various environmental parameters and microbial populations were investigated to elucidate the relationship with motile aeromonads. Motile Aeromonas species were enumerated by most probable number technique with alkaline peptone water as the enrichment medium and modified pril-xylose ampicillin agar as the plating medium. Aeromonas species were isolated in a water column in any one of the two procedures but sediment and plankton samples exhibited an irregular isolation pattern for these organisms. Aeoromonas species were continuously isolated in sinking particles with the highest counts during January. Of the 206 isolates identified, three known motile Aeromonas species were observed of which A. caviae accounted for 51.4% of the total. Toxin characterization showed that 20% of the strains produced haemolysin as well as cytotoxin, and A. hydrophila was highly toxigenic. Statistical analyses revealed that nutrients govern the distribution of Aeromonas. It may be that riverine discharge influences the distribution of motile aeromonads in this environment.

Characterization and classification of Actinobacillus (Haemophilus) pleuropneumoniae plasmids.

Actinobacillus (Haemophilus) pleuropneumoniae plasmids were characterized and classified. They were isolated from A pleuropneumoniae strains different in serotype, year isolated, or location from which isolated. Six of 8 plasmids encoded streptomycin (Sm) and sulfonamide (Su) resistance (SmSu). One of the other plasmids, pVM105, encoded ampicillin (Ap) resistance and another, pHM0, encoded no drug resistance. All SmSu plasmids were transferred to Escherichia coli strains by transformation. Among them, pABO and pMS260 were 8.1 kb and incompatible with each other; they were stable in E coli. The other SmSu plasmids, pHM1, pVM104, pVM106, and pKD25, were 4.3 kb and did not replicate stably in E coli. The former SmSu plasmids were mobilized in E coli strains by a plasmid RP4, which belonged to incompatibility (Inc) group P, but the latter plasmids were not. Further, each 8.1-kb SmSu plasmid and each 4.3-kb plasmid had the same respective restriction pattern. These results indicated that there were at least 2 types of SmSu plasmids in A pleuropneumoniae. The 2 types were classified in 2 groups: H1(pMS260 and pABO) and H2(pHM1, pVM104, pVM106, and pKD25). The H1 and H2 plasmids belonged to different Inc groups, and H2 plasmids belonged to a different Inc group from that of pHMO and pVM105.

[Evaluation of two quantitative tests for salivary secretion--the chewing gum test and the Saxon test in normal subjects and in patients with Sjögren's syndrome].

A cooperative multicenter study was performed to evaluate two salivary secretion methods-the chewing gum test and the Saxon test by a crossover method. Twenty healthy volunteers and 27 patients with Sjögren's syndrome were examined. In healthy controls the amount of saliva collected per minute was larger in the Saxon test. Both tests revealed a significant decrease of salivary secretion in patients with Sjögren's syndrome, but the amount of saliva per minute was also larger in Saxon test. By repeated trials of both tests a notable increase of saliva was always observed at the second trial. Since this phenomenon may be due to the learning effect of the tests, one should perform at least one practice before starting formal tests. Saxon test could be performed in a patient having dental prostheses who was unable to masticate chewing gum. The Saxon test, thus, may be an efficient method, to evaluate the salivary secretory function.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--7. Effects on natural killer cell activity].

Twenty four patients who underwent ophthalmic surgery were studied to evaluate activities of natural killer (NK) cells during and following total intravenous anesthesia with droperidol, fentanyl and ketamine. They were divided into three equal groups according to anesthetic methods employed. In enflurane group, anesthesia was induced with thiopental 5 mg.kg-1 and succinylcholine 1 mg.kg-1, and maintained with 1-2% enflurane, nitrous oxide (50%) and oxygen (50%). In original NLA group, anesthesia was induced as above and maintained with droperidol 0.15 mg.kg-1, fentanyl 5-10 micrograms.kg-1, nitrous oxide (70%) and oxygen (30%). The patients of total intravenous anesthesia group received droperidol 0.15 mg.kg-1, ketamine 2 mg.kg-1 and succinyl-choline 1 mg.kg-1 for induction of anesthesia, and then were given fentanyl 5-15 micrograms.kg-1, ketamine 2 mg.kg-1.hr-1 and oxygen (30%) for the maintenance of anesthesia. Vecuronium was given to every patient of the three groups for intraoperative muscle relaxation. Hartmann's solution was used at a speed of 5 ml.kg-1.hr-1. Peripheral venous blood 10 ml was drawn on six occasions during and after the surgery for the measurement of NK cell activities and endocrine response as judged by plasma catecholamine and cortisol levels. NK cell activities decreased significantly on the first post-operative day in enflurane group, but no significant differences were found among three groups in NK cell activities. The data suggest that inhaled anesthetics should not be easily employed for patients with depressed immune response, malignant disease and prolonged surgery.

Vibrio virulence factors and the quantitative analysis of cytotoxicity elaborated by environmental isolates.

Vibrio strains isolated from freshwater environments were screened for the elaboration of haemolysin, cytotoxin and enterotoxins. While characterizing for various toxins, nine strains elaborated 10(3) and more haemolytic units (HU) per ml crude toxin with a maximum of 4.1 x 10(3) HU. About 41.7% of the strains which were tested produced a factor in the culture supernatant which was active in the suckling mouse assay. Cytotoxin studies revealed that all the Vibrio strains did not respond uniformly in the three cell lines tested. The elongation factor of the CHO cell line which was reported to have correlation with the enterotoxic activity, did not with some exceptions correspond to high fluid accumulation ratio in conventional suckling mouse assay. A modified, simple, rapid, sensitive procedure is described to quantify the elaboration of cytotoxic units (CU). When the HeLa cell line was used to estimate the CU production, 29% of the strains produced 10(2) CU with a maximum of 169 CU per 50 microliters of crude toxin.

[Fibromyalgia (fibrositis) syndrome--a case report].

A 25-year-old woman was admitted in our hospital with back pain and both hip joint pain. Pain was abruptly occurred from the beginning of March 1990. Physical examination revealed wide spread pain (occipital area, both shoulder, lumber area, bilateral gluteal area, inguinal area, both Achilles-plantar area) and more than 12 tender points (occiput, trapezius, second rib, supraspinatus, gluteal, greater trochanter, hip joints, pubic bone). Laboratory examination showed no abnormal findings except ANF (1:160). Any examination including X-ray, bone scintigraphy, CT and MRI did not disclose spondylitis, sacroiliitis and enthesopathy. She was diagnosed as primary fibromyalgia/fibrositis syndrome. Treatment with maprotine hydrochloride (30 mg/day) and phenobarbital (120 mg/day) brought approximately 1/3 reduction of pain and tenderness. Psychoanalysis revealed that she had psychological conflicts against her parents and her colleagues at the work. EEG showed a borderline record with irregular basic pattern and 14 & 6 Hz positive burst at the sleep stage. Although the newly proposed criteria for the classification of fibromyalgia was proposed by ACR, fibromyalgia/fibrositis syndrome has been seldom discussed in the Japanese literature. As this syndrome is frequently associated with various rheumatic diseases, hypothyroidism and malignant diseases, we should pay much more attention to understand this syndrome.

The effects of thyroid hormones on potassium-stimulated release of 3H-GABA by synaptosomes of rat cerebral cortex.

Depolarization-induced release of neurotransmitters from nerve endings is triggered by the rapid entry of Ca through voltage-sensitive channels. Low (1 to 10 nmol/L) concentrations of L-triiodothyronine (T3), but not D-T3, reverse T3 (rT3) or L-thyroxine (T4), enhance fast-phase depolarization-induced 45Ca uptake by rat brain synaptosomes. In the present study the effects of thyroid hormones on the release of 3H-gamma-aminobutyric acid (3H-GABA) by cerebral cortical synaptosomes from adult male Sprague-Dawley rats were determined by a superfusion technique. It was found that low (1 to 10 nmol/L) concentrations of T3, but not D-T3, rT3, or T4, significantly enhanced Ca-dependent 3H-GABA release by synaptosomes exposed to a high K depolarization buffer for 5 sec. Low concentrations of T3 did not enhance basal 3H-GABA release or release produced by exposure to depolarization buffer for 30 seconds. Much higher concentrations (10 mumol/L) of both T3 and T4, but not rT3, D-T3, or D-T4, stimulated 3H-GABA release produced by exposure to depolarization buffer for 30 seconds.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--8. Hepatic and renal functions following prolonged surgical operation of over 10 hours].

Forty-four patients were studied to evaluate their postoperative hepatic and renal functions on 2nd to 4th and 7th to 10th postoperative days as judged by serum GOT, GPT, BUN and creatinine levels. The patients were divided into two groups. Twenty two patients received total intravenous anesthesia with droperidol, fentanyl and ketamine (FK group). The remaining 22 patients were given conventional enflurane-nitrous oxide anesthesia. The two groups were comparable concerning age, body weight, sex distribution, performed operation, operation time and anesthesia time. In the total intravenous group, fluid given and urine output were significantly larger than those of the enflurane group, and the amounts of blood loss and blood given tended to be greater but insignificantly in the total intravenous group than in the enflurane group. In both groups, postoperative S-GOT levels increased significantly and those of the enflurane group were significantly higher than those of the FK group on 2nd to 4th postoperative days. In the enflurane group, postoperative S-GPT levels were significantly higher, but those of the FK group were not. S-GPT on 2nd to 4th postoperative days of the enflurane group were significantly higher than those of the FK group. As to serum BUN and creatinine, no significant differences were observed between the two groups. These data suggest that FK is much more beneficial than enflurane anesthesia to protect hepatic functions, particularly when it is applied for prolonged surgical procedures.
