Effects of dihydropyridines on cerebral blood vessels.

AIM: To evaluate the effects of dihydropyridine calcium antagonists on the cerebral circulation in stroke patients. DESIGN OF STUDIES: Mean cerebral circulation parameters were studied in chronic stroke patients with hypertension and treated acutely or chronically (2 weeks) with nicardipine or nifedipine retard. The effects of nicardipine on local cerebral blood flow were also studied in patients with a subacute brain infarction. RESULTS: Mean cerebral blood flow was significantly increased after a single oral administration of nicardipine but not after nifedipine retard. Both calcium antagonists reduced mean arterial pressure. Nifedipine retard acted mainly by reducing blood pressure and nicardipine mainly by reducing cerebrovascular resistance. CONCLUSION: Dihydropyridines have a beneficial effect in the control of hypertension in stroke patients.

[A 47-year-old woman with meningeal signs and consciousness disturbance].

We report a 47-year-old woman with SLE, who developed meningeal signs and consciousness disturbance. She noted an onset of fever, and swelling and pain in her face, hands and feet in 1990. She was seen in another hospital and the diagnosis of SLE was made. She was treated with prednisolone with marked improvement in her symptoms. She was well with 5 mg of oral prednisolone daily until January of 1991, when she developed fever, myalgia and weakness in her legs. She was admitted to the medical service of our hospital on August 5. She was receiving 15 mg of prednisolone daily. Gram positive rods were cultured from her blood on August 5. She became incoherent 2 days later, and had a convulsive episode on August 8. After the convulsion, she lost consciousness from which she did not recover. Her CSF contained 304/3 microliters cells, 29 of which were neutrophils, 6 lymphocytes, 90 others, and 179 destructed cells. The CSF protein content was 345 mg/dl, and glucose 23 mg/dl. A neurological consultation was asked on August 9. Physical examination at that time revealed a semicomatous woman. Respiration was 30/min and regular. BP 132/82 mmHg, heart rate 122/min and regular, and BT 39.6 degrees C. General physical examination was unremarkable. Pertinent neurologic findings were positive Kernig sign and spasticity in all four limbs. Brain stem reflexes were retained. Upon painful stimulation, withdrawal response was elicited both lower extremities. She was treated with pipiracillin, latamoxef and phenobarbital, however, she had frequent seizures. She was deeply comatose on December 10. She became flaccid and no more meningeal signs were observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Clinicopathological study on craniopharyngioma with sebaceous differentiation.

We presented 2 cases of craniopharyngioma, which showed sebaceous differentiation. Both cases shared the following features: 1) neuroradiological detection of a cystic tumor accompanied by calcification, 2) clear yellowish fluid detected in the cyst during surgery, 3) histological rating as an adamantinomatous type, 4) the presence of a nest-like distribution of multivacuolated cells in the innermost layer of the cyst.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--20. Summary of three thousand cases and the future of this anesthetic method].

Total intravenous anesthesia with droperidol, fentanyl and ketamine (DFK) was given to over three thousand patients during four years from April 1989 through March 1993. The patients ranged in age from three months to eighty seven years. They underwent surgical, orthopedic, gynecological, thoracic, plastic and otolaryngeal surgeries, but patients who underwent craniotomy and obstetric operations were excluded. None of them developed any serious complications primarily due to DFK. DFK has many advantages such as the broad safety margin for three agents employed in DFK, no accident by N2O, no air pollution, empty bowels, no increase in middle ear pressure etc, while this has disadvantages such as high blood pressure, slow awakening from anesthesia and unpleasant dreams. Calcium channel blockers are very effective for antagonizing high blood pressure, and rapid recovery from anesthesia can be easily obtained by reducing ketamine dose given and also by application of epidural block. Intraoperative dreams may be avoided by concomitant use of benzodiazepines. Thus we are convinced that DFK can be a good as well as convenient anesthetic method for clinical anesthesia.

Characterization of newly isolated plasmids from Actinobacillus pleuropneumoniae.

The genetic basis of drug-resistant strains of Actinobacillus pleuropneumoniae in Japan was studied. The A pleuropneumoniae strains AV277 and AV281 that belong to serotype 2 were resistant to streptomycin (SM) and sulfonamide (SA). Both strains had an 8.1-kilobase (kb) SM-SA plasmid that was previously classified in the H1 group. The AV177 (serotype 1) strain was resistant to SM, SA, ampicillin, and kanamycin (KM), but did not have any plasmids. The AV319 and AV324 (serotype 1) strains were resistant to SM, SA, tetracycline (TC), and chloramphenicol (CP). The AV318 (serotype 12) strain was resistant to SM, SA, TC, minocycline, and CP. These 3 strains (AV319, AV324, and AV318) had a 4.3-kb SM-SA plasmid and a 5.2-kb CP plasmid. The 4.3-kb plasmid was classified in the H2 group. The AV263 (serotype 1) strain was resistant to SM, SA, KM, TC, and CP. It had a 5.2-kb CP plasmid and a 6.6-kb SM-SA-KM plasmid. Both plasmids did not replicate stably in Escherichia coli strains. The former 5.2-kb plasmid was mobilized in E coli strains by plasmid RP4, which belonged to incompatibility P with broad host range, but the latter 6.6-kb plasmid was not so mobilized. Three 5.2-kb CP plasmids isolated from strains AV319, AV324, and AV318, had the same restriction endonuclease pattern after digestion with Ava I and EcoRI. They coexisted with H1 group plasmids in the incompatibility test, and coexisted also with H2 group plasmids of the original A pleuropneumoniae strains. Results indicated that the 5.2-kb CP plasmids could be classified in a new incompatibility group, H3. In this study, 4 types of plasmids were isolated, but no plasmids encoded TC and minocycline resistance.

Reactive arthritis induced by Vibrio parahaemolyticus.

We describe a case of reactive arthritis after Vibrio parahaemolyticus infection. V. parahaemolyticus is the principal bacteria involved in the causation of foodborne gastroenterocolitis outbreaks in Japan. Several bacteria have been recognized as being involved in the development of reactive arthritis and this is the first report of reactive arthritis induced by V. parahaemolyticus.

Survival in synovial sarcoma. A multivariate study of prognostic factors with special emphasis on the comparison between early death and long-term survival.

A retrospective study of 56 patients with synovial sarcoma was done to search for possible prognostic factors affecting survival. In a univariate analysis, age (> or = 20 years), site (proximal), large tumor size (> or = 5 cm), mitotic rate of more than 15 per 10 per high-power fields (hpf), high nuclear grade, tumor necrosis of more than 50%, the presence of rhabdoid cells, a small number of mast cells (< 20/10 hpf), and a high stage according to the criteria of the American Joint Committee (AJC) staging of soft tissue sarcoma, significantly decreased survival (log-rank test; p < 0.05). Moreover, when comparing the 10-year survival (11 patients) and the deaths within 1 year (12 patients), size, high nuclear grade, presence of rhabdoid cells, tumor necrosis, and stage were all considered to be significant prognostic factors (two-sided chi-square test; p < 0.01). In a multivariate analysis (Cox's model), stage was the only strong predictor of a poor prognosis (p = 0.011). We therefore concluded that large tumor size, high nuclear grade, the presence of rhabdoid cells, extensive tumor necrosis, and a high AJC stage were adverse prognostic factors in synovial sarcoma.

HLA-DP positive T cells in patients with polymyositis/dermatomyositis.

The examination of HLA-DP+ T cells in 21 patients with polymyositis/dermatomyositis revealed marked increases [28.5% (SD 16.6%)], compared with the HLA-DQ+ and DR+ T cells. The HLA-DP molecule was expressed on both CD4+ and CD8+ T cells. The majority of HLA-DP+ CD8+ cells were cytotoxic T cells. There was no significant correlation between the proportion of HLA-DP+ T cells and the level of myogenic enzyme, although a decrease in HLA-DP+ T cells after therapy was accompanied by a decrease in myogenic enzymes. However, the proportion of HLA-DP+ CD8+ cells was significantly higher in those patients with active pneumonitis.

[A case report of successful removal of entrapped IABP by Lambert's method].

A 68-year-old man who necessitated IABP support for acute myocardial infarction suffered from IABP entrapment after its perforation. We washed intra-balloon space several times with heparinized normal saline, and finally could successfully remove it without surgery. It appears that the washing method (Lambert's method) for IABP entrapment could be done with lower risk especially for severe cases.

Effect of sodium chloride, pH and organic nutrients on the motility of Vibrio cholerae non-01.

Using a simple method of semi-solid agar (0.2%) in U-tubes, the effect of temperature, NaCl, pH and nutrients on the motility of environmental Vibrio cholerae non-01 was elucidated. Under aerobic respiration succinate and pyruvate, which are metabolic intermediates, increased the motility rate, with succinate having the greatest influence. Glucose and benzoic acid reduced the motility rate of most of the strains. Optimum motility was obtained at pH 8.6 and 9.1 in all strains examined. Acidic pH 6.1 and alkaline pH 10.0 reduced motility while the strains showed no growth at pH 4.1. The addition of sodium chloride to the media enhanced motility. The incubation temperature also influenced motility with higher rates recorded at 30 and 37 degrees C than at 25 degrees C.

[Lymphangiomyomatosis involving the kidney: a case report].

A 58-year-old female was admitted with a chief complaint of gross hematuria. A diagnosis of carcinoma of the left kidney with lymphnode metastasis was made on the basis of computerized tomography and angiography. Left nephrectomy and lymphnode dissection were performed. Histological examination revealed lymphangiomyoma, and the final clinical diagnosis was changed to lymphangiomyomatosis with pulmonary, retroperitoneal, and left renal lesions. Lymphangiomyomatosis with renal lesions is extremely rare, and differentiation from renal carcinoma is important, though difficult.

Prognostic significance of histologic parameters of soft tissue sarcomas.

BACKGROUND AND METHODS: A univariate and multivariate analysis for the correlation between histomorphologic factors and prognosis was made using data from 1116 patients with soft tissue sarcoma, including 1005 cases available with complete histologic and follow-up data. RESULTS: The overall 5-year survival rate was 43.6%. The univariate analysis using Kaplan-Meier survival curves showed that tumor differentiation, cellularity, nuclear atypia, cellular pleomorphism, mitotic activity, amount of fibrous stroma, extent of myxoid areas, extent of tumor necrosis, and histologic grading (determined by the estimated range of malignancy for each type of sarcoma using a reported guideline) were all significant prognostic factors in the overall soft tissue sarcoma group. By a multivariate analysis using a procedure based on the Weibull model to failure data, the histologic grading and extent of tumor necrosis were proved to be prognostically significant in the overall sarcoma group. However, after additional analysis on each histologic type of sarcoma, it was determined that both tumor necrosis and histologic grading were applicable in only two types: malignant fibrous histiocytoma and leiomyosarcoma. Liposarcoma, synovial sarcoma, and malignant schwannoma each also showed some determinable factor of prognostic significance. CONCLUSIONS: It was concluded that there seemed to be no single universal prognostic parameter that could be applied to each soft tissue sarcoma type. These results suggest that the predictive significance of the various histologic parameters should be based on each specific type of sarcoma.

A reassessment of histologic classification and an immunohistochemical study of 88 retinoblastomas. A special reference to the advent of bipolar-like cells.

BACKGROUND: Despite perpetual efforts of investigators, the histogenesis of retinoblastoma is still in dispute and histologic classification satisfactorily predictive of prognosis does not seem to be in use. METHODS: The authors studied 88 cases of retinoblastoma clinicopathologically and immunohistochemically, paying special attention to the presence of a "bipolar-like cell" element that would be used as one of the criteria for the diagnosis of differentiated retinoblastoma. RESULTS: Twelve cases of retinoblastoma with the bipolar-like cell element in the absence of rosettes and 41 cases of the tumor with rosettes were classified as differentiated retinoblastomas. The other 35 cases without rosettes or bipolar-like cells were classified as undifferentiated tumors. Tumor cells forming rosettes usually had positive results for synaptophysin and neuron-specific enolase (NSE) and negative results for glial fibrillary acidic protein (GFAP) and S-100 protein; however, undifferentiated cells had negative results for these four antibodies. The bipolar-like cells had positive results for synaptophysin and NSE but negative results for GFAP and S-100 protein. Twelve tumors with bipolar-like cells that lacked rosettes showed no optic nerve invasion, and the patients had a significantly better prognosis (100% 5-year survival rate) than 35 patients with undifferentiated tumors (71% 5-year survival rate) (P < 0.01). CONCLUSIONS: The findings support a neuronal origin of the tumor and indicate that, not only the rosettes symbolizing the photoreceptor differentiation, but also other neuronal elements, such as bipolar-like cells, can be used as criteria for histologic classification of retinoblastoma.

[Sarcomatoid liver carcinoma diagnosed clinically as hemangioma].

This is a case report of a 69-year-old woman with sarcomatoid hepatocellular carcinoma (HCC), which was diagnosed clinically as hemangioma. She was first admitted to our university hospital, complaining of general fatigue in December, 1988, and cholelithiasis and liver cirrhosis with hepatic tumor in Segment 8 were diagnosed. The serum AFP level was within normal range, and the tumor was diagnosed as hemangioma radiologically. She underwent only cholecystectomy and was well without any therapy for the liver tumor up until March in 1991 when she was readmitted to our university hospital due to rapidly progressive liver dysfunction. The size of the liver tumor was unchanged. Despite intensive care, she died of hepatic failure due to cirrhosis in a decompensation state. At autopsy, a well defined yellowish white tumor of 3 cm in maximum diameter was seen in the cirrhotic liver. Although the largest part of the tumor revealed necrosis and hyalinization, a sarcomatoid part composed of spindle-shaped cells was noted in the peripheral portion. In addition, some necrotic ghost cells, probably hepatocellular carcinoma, were also noted. Low molecular cytokeratin, which is always found in HCCs, was seen in spindle-shaped sarcomatoid cells. The liver tumor was diagnosed as sarcomatoid HCC from these pathological findings. We report this histologically unusual HCC with an immunohistochemical study.

Identification of the prostacyclin receptor by use of [15-3H1]19-(3-azidophenyl)-20-norisocarbacyclin, an irreversible specific photoaffinity probe.

The prostaglandin I (PGI2) receptor of mouse mastocytoma P-815 cells was characterized by photo-affinity labeling with the stable PGI2 analogue [15-3H1]-19-(3-azidophenyl)-20-norisocarbacyclin ([3H] APNIC) used as a potential photoaffinity probe for the receptor. [3H]APNIC bound to the mastocytoma membrane with high affinity and in a saturable manner. Scatchard plot analysis indicated a single binding site with a Kd of 4.7 nM and a Bmax of 0.58 pmol/mg protein. The binding of [3H]APNIC was dose dependently inhibited by APNIC and iloprost, another stable PGI2 agonist, and to a much lesser extent by PGE2. The binding of the radioligand showed sensitivity to the guanine nucleotide guanosine 5'-O-(3-thio-triphosphate) (GTP gamma S). Photolysis of [3H]APNIC-prelabeled membranes resulted in incorporation of radiolabel into a protein of approximately 43 kDa. Photolabeling was inhibited by PGI2 agonists and other prostaglandins with specificity for the PGI2 receptor and was modulated by GTP gamma S. A protein of approximately 45 kDa was also labeled by [3H]APNIC in the membrane of porcine platelets, membranes that are known to be abundant in PGI2 receptors. These results demonstrate that [3H]APNIC specifically labels a protein that may represent the PGI2 receptor and that this radioprobe will be a useful reagent for further characterization and purification of the PGI2 receptor.
