[A case of acute empyema who underwent omentopexy and was found to have gastric cancer during operation].

A 76-year-old man had undergone omentopexy for empyema. He was found to have gastric cancer and revealed Borrmann III by gastroendoscopy intraoperative period. Segmental gastrectomy was performed without dissection of lymph nodes, because his general condition was poor. After operation, the patient showed septic shock, ARDS and hepato-renal failure but his condition was getting improved by conservative therapy. After 5 years later, he was examined gastro-endoscopy, abdominal and chest CT and revealed no recurrence sign.

[Diagnosis and treatment of pulmonary tuberculosis complicated with chronic liver disease].

Immunocompromised or malnutritional hosts are high risk group of pulmonary tuberculosis. Chronic liver disease especially decompensated cirrhosis of the liver is one of the risk group for this infection. When ascites or pleural effusion developed in patient with hepatic cirrhosis, complication of pulmonary tuberculosis must be considered. In such condition, drug metabolism was impaired so that anti-tuberculous drugs should be used carefully, but in almost cases except decompensated cirrhotic patients are tolerable for standard anti-tuberculous combination therapy and they could be cured. Hepatitis C virus infection is common in patients with old pulmonary tuberculosis because many of them were infected Hepatitis C virus at the time of blood transfusion for pulmonary resection or thoracoplasty. In such condition recurrence of pulmonary tuberculosis is rare but probability of recurrence must be considered when they developed decompensated cirrhosis.

Long-term effect of alpha-glucosidase inhibitor on late dumping syndrome.

Dumping syndrome commonly occurs after gastrectomy. The late dumping, which is one of the dumping syndromes, is due to postprandial hypoglycaemia caused by an excessive insulin secretion after a sharp rise in plasma glucose. Several treatments, including operation, dietary fibre and somatostatin, have been attempted to relieve dumping symptoms. These treatments take effect through modulation of plasma insulin and glucose levels, but their efficacy is still under consideration. Alpha-glucosidase inhibitor attenuates the postprandial increase of plasma glucose levels and is widely used for treatment of non-insulin-dependent diabetes mellitus (NIDDM). The acute effect of alpha-glucosidase inhibitor on late dumping syndrome has been reported by some studies with test meals. The purpose of this study was to evaluate a long-term effect of alpha-glucosidase inhibitor treatment with ordinary meals in late dumping patients with NIDDM because administration of alpha-glucosidase inhibitor is only ethically allowed for diabetic patients in Japan. Six late dumping patients with NIDDM were orally administered alpha-glucosidase inhibitor, acarbose (50 or 100 mg), three times a day before each meal for 1 month. Diurnal changes of plasma glucose, insulin and pancreatic glucagon levels were compared before and after the alpha-glucosidase inhibitor treatment. All patients had late dumping-related symptoms, such as weakness, palpitation and dizziness before the induction of alpha-glucosidase inhibitor treatment. Patients suffered from a rapid fall in plasma glucose levels from hyperglycaemia at the same time as dumping symptoms. These late dumping-related symptoms disappeared and a rapid change of plasma glucose and insulin levels were attenuated after the alpha-glucosidase inhibitor treatment. These data suggest a long-term therapeutic efficacy of alpha-glucosidase inhibitor for late dumping patients.

Hypertrophic obstructive cardiomyopathy due to a novel T-to-A transition at codon 624 in the beta-myosin heavy chain (beta-MHC) gene possibly related to the sudden death.

Many missense mutations in the beta-myosin heavy chain have been reported in patients with hypertrophic obstructive cardiomyopathy (HOCM). However, the controversy is present whether the mutation accompanying the change of electric charge is related with poorer prognosis. The proband, a 48-year-old female, of the family was diagnosed clinically as HOCM, and a structural analysis of the cardiac beta-MHC gene showed that the proband and her junior daughter had a novel mutation with T to A transition in codon 624 replacing tyrosine with asparagine, which was not present in her husband, elder daughter and son. The proband's husband, son and two daughters were healthy except that the ECG of junior daughter (15-year-old) showed complete right bundle branch block. Proband's mother died suddenly after the delivery of the proband and the proband also collapsed suddenly. The occurrence of sudden death in proband and her mother suggested that HOCM with this novel mutation might be associated with a high risk of sudden death irrespective of the absence of charge alteration.

Keratinocytes become terminally differentiated in a process involving programmed cell death.

Oral keratinocytes originate from basal cells, differentiate during migration to the surface, and finally are shed. Apoptosis occurs at the end of differentiation, but the precise relationship between terminal differentiation and apoptosis is not clear. In the present study, Bcl-xL was expressed in the basal cell and spinous cell layers, and Bax was expressed in the spinous cell and granular cell layers. In cultured keratinocytes, Bcl-xL was expressed under conditions of 0.1 mM calcium (low Ca2+) but disappeared under conditions of 1.0 mM calcium (high Ca2+); the latter induces keratinocyte differentiation. Bax was not expressed in keratinocytes with low Ca2+ but was expressed in cells with high Ca2+. Finally keratinocytes with high Ca2+ underwent apoptosis, which was detected by the TUNEL method and by 180-bp DNA fragmentation. These results suggest that the process of terminal differentiation in gingival epithelium is a pathway to apoptosis.

BH3 domain of BAD is required for heterodimerization with BCL-XL and pro-apoptotic activity.

BAD interacts with anti-apoptotic molecules BCL-2 and BCL-XL and promotes apoptosis. BAD is phosphorylated on serine residues in response to a survival factor, interleukin-3. Phosphorylated BAD cannot bind to BCL-XL or BCL-2 at membrane sites and is found in the cytosol bound to 14-3-3. We report here that deletion mapping and site-directed mutagenesis identified a BH3 domain within BAD that proved necessary for both its heterodimerization with BCL-XL and its death agonist activity. Substitution of the conserved Leu151 with Ala in the BH3 amphipathic alpha-helix abrogated both functions. The BAD Leu151 mutant was predominantly in the cytosol bound to 14-3-3. The BH3 domain of BCL-2 also proved important for BCL-2/BAD interaction. These results establish a critical role for a BH3 domain within BAD and provide evidence that BAD may function as a death ligand whose pro-apoptotic activity requires heterodimerization with BCL-XL.

Cloning of mouse FGF10 and up-regulation of its gene expression during wound healing.

We cloned the mouse homolog of FGF10, which was recently reported as a new member of the FGF family. The predicted molecular mass of this molecule is 23.6 kDa, and both nucleotide and amino acid sequences show high degrees of similarity with those of the rat. Examination of mouse FGF10 mRNA expression in various tissues and developmental stages by Northern hybridization revealed tissue- and developmental stage-specific expression of the gene. Similarly to the rat counterpart, mouse FGF10 mRNA (4.5 kb) was expressed relatively abundantly in embryos and the lung, and at much lower levels in brain and heart. In addition, a shorter transcript (1.3 kb) is expressed only in testis. Considering the high similarity in primary structure between FGF10 and FGF7 (known as keratinocyte growth factor; KGF), we also examined the gene expression of FGF10 during wound healing using a mouse model. FGF10 mRNA was highly induced 1 day after injury and decreased rapidly by 3 days. This suggests that FGF10 is a primary factor in the process of wound healing similarly to other growth factors such as TGF alpha and FGF7.

Hepatoma-derived growth factor belongs to a gene family in mice showing significant homology in the amino terminus.

Hepatoma-derived growth factor (HDGF) is an acidic polypeptide with mitogenic activity for fibroblasts performed outside the cells despite the presence of a putative nuclear localization signal (NLS). We have now cloned three related mouse cDNAs: one for a mouse homologue of human HDGF and two for additional HDGF-related proteins provisionally designated HDGF-related proteins 1 and 2 (HRP-1 and -2). Their deduced sequences have revealed that HDGF belongs to a new gene family with a highly conserved 98-amino-acid sequence at the amino terminus (hath region, for homologous to the amino terminus of HDGF). HRP-1 and HRP-2 proteins are 46 and 432 amino acids longer than mouse HDGF, respectively, and have no conserved amino acid sequence other than the hath region. HRP-1 is a highly acidic protein (26% acidic) and also has a putative NLS. HRP-2 protein carries a mixed charge cluster, a sharp switch of positive-to negative-charge residues, which is often found in some nuclear proteins. Northern blotting shows that mouse HDGF and HRP-2 are expressed predominantly in testis and skeletal muscle, to intermediate extents in heart, brain, lung, liver, and kidney, and to a minimal extent in spleen. HRP-1 is expressed specifically in testis. These findings suggest that the HDGF gene family might play a new role in the nucleus especially in testis.

Isolation, partial purification, and ultrastructure of taste bud cells from rabbit foliate papillae.

A method is described for obtaining large numbers of isolated taste bud cells from lingual epithelium of rabbit foliate papillae. The isolation method is based on isopyenic sedimentation in a Percoll gradient. The purification of taste bud cells was evaluated by electron microscopy and by immunohistochemistry using CK 20 antibody. The cytology of the isolated taste bud cells remained very similar to in situ cells. The type III cells, which are regarded as gustatory cells, retained their characteristic dense-cored granules in the cytoplasm. This method will permit study of various parameters of taste bud cell biology.

Arabidopsis thaliana ECP63 encoding a LEA protein is located in chromosome 4.

DCECP63 is a carrot embryogenic cell protein. To perform its genetic analysis, we isolated an Arabidopsis thaliana (At) ECP63 cDNA. The cDNA encoded a polypeptide of 449 amino acids (aa). Its deduced aa sequence showed extensive similarity with DCECP63, DCDC8 and BPBP8 which are embryonic proteins isolated from carrot and birch, respectively. The aa sequence contained eight well-conserved tyrosine phosphorylation sites and 16 repeats of 11 aa. Southern analysis showed that the AtECP63 gene might belong to a small multigene family. The AtECP63 transcripts accumulated specifically in mature seeds, and exogenous abscisic acid (ABA) induced its expression in immature siliques, but not in vegetative tissues. These results suggested that the AtECP63 gene encoding a putative phosphotyrosine protein belonging to late embryogenesis abundant (LEA) protein in group 3 might be involved in maturation and desiccation tolerance of seeds. Restriction fragment length polymorphism (RFLP) and cleaved amplified polymorphic sequence (CAPS) mapping showed that AtECP63 gene was present in the South part of chromosome 4.

Development of a portable multipurpose recorder using a 24-hour ambulatory recorder: application to polysomnography.

With the advance of chronobiology, demands for simultaneous long-term monitoring and recording of various biosignals are increasing. To meet these demands we have developed a portable multipurpose recording system using a 24-hour ambulatory recorder. This technical achievement of less restrictive round-the-clock simultaneous recording of biosignals from not only the circulatory system, but also from the neurological and digestive systems of a subject in his/her daily life will contribute to clinical research on circadian variations of biosignals and EEG analysis during sleep.

Acute toxicity, accumulation and excretion of organophosphorous insecticides and their oxidation products in killifish.

Acute toxicity, accumulation and excretion of four organophosphorous insecticides (diazinon, malathion, fenitrothion and EPN) and their oxidation products (diazinon oxon, malaoxon, fenitrothion oxon and EPN oxon) were studied for killifish (Oryzias latipes). The 48-hr LC50 was 4.4 mg l-1 for diazinon, 1.8 mg l-1 for malathion, 3.5 mg l-1 for fenitrothion, 0.58 mg l-1 for EPN, 0.22 mg l-1 for diazinon oxon, 0.28 mg l-1 for malaoxon, 6.8 mg l-1 for fenitrothion oxon, and 0.16 mg l-1 for EPN oxon. The bioconcentration factors (BCF) of diazinon oxon 0.5, malaoxon 1.1, fenitrothion oxon 2.3 and EPN oxon 11 in the whole body of the fish were much lower than those of diazinon 49, malathion 11, fenitrothion 122 and EPN 1124. As reference data, partition coefficients between n-octanol and water (Pow) were measured for these chemicals. The BCF values of each pesticide and its oxidation product were consistent with the Pow values. The excretion rate constants (k) from the whole body of the fish were 0.12 hr-1 for diazinon, 0.27 hr-1 for malathion, 0.11 hr-1 for fenitrothion, 0.02 hr-1 for EPN, 0.30 hr-1 for fenitrothion oxon and 0.59 hr-1 for EPN oxon. The rates of diazinon oxon and malaoxon could not be measured, but were presumed to be as rapid as or more rapid than those of fenitrothion oxon and EPN oxon. The results suggest that the contamination of fish and other aquatic organisms by the oxidation products in the environment is very low.

Induced expression of Thy-1 molecules on dermal endothelial cells in skin allografts.

We obtained evidence in a foregoing study that inducible Thy-1 on vascular endothelial cells functions as a possible vascular permeability modulator in the rat. We now report on the regulation and function of endothelial Thy-1 in skin allograft rejection in the rat. While no obvious expression of Thy-1 antigen on the vasculature can be seen in normal organs, dermal endothelial cells do express Thy-1 during allogeneic skin graft rejection and Freund's complete adjuvant (FCA)-induced inflammation. The antigen was weakly induced on tubular epithelial, but not on endothelial cells during kidney allograft rejection, and not in FCA-induced inflammation of the kidney. In contrast, Thy-1 antigen was not induced in the rejected lung or in FCA-induced inflammation of the lung. This pattern of Thy-1 regulation was transcriptionally regulated. Administration of anti-Thy-1 antibodies generated increased vascular permeability in skin allografts, although this procedure did not modulate survival of the grafts.

A hamartoma-like mass on the palate? A possible discussion regarding the components of a pigmented naevus and hyperplastic salivary gland.

An extremely rare mass lesion arising on the hard palate is presented. The patient was a 49-year-old Japanese male. He had a painless, semi-globular, smooth-surfaced and partially pigmented mass located on the left hard palate with no evidence of growth for approximately 20 years. The resected specimen included hyperplastic salivary gland tissue, nerve fibres and vessels surrounded by adipose tissue. All constituent tissues showed excessive growth for this location. Also spiralling nests of naevus cells representing inactive intramucosal naevus were included. We consider the whole lesion to be a hamartoma.

Plasma concentration of endothelin-1 does not reflect renal vasoconstriction as estimated by duplex ultrasonography in cirrhosis.

Endothelin, a potent vasoconstrictor, is thought to play a role in liver cirrhosis-related functional kidney failure. Our aim was to investigate the correlation between renal vasoconstriction, as detected by a Doppler ultrasound technique, and plasma concentrations of endothelin in cirrhotic patients. Fifty cirrhotic patients underwent Doppler examinations to detect renal vasoconstriction. The plasma concentration of endothelin was measured in 10 patients who exhibited vasoconstriction of the renal microvessels diagnosed by Doppler waveform analysis and was compared to that of patients in whom there was no sign of such vasoconstriction. No difference was observed in the plasma concentration of endothelin between patients in whom renal vasoconstriction was diagnosed and those in whom it was not. Our results suggested that the circulating endothelin does not reflect renal vasoconstriction, at least in the early phase of the functional renal failure associated with cirrhosis of the liver.

Intraductal secretin test is as useful as duodenal secretin test in assessing exocrine pancreatic function.

We assessed the clinical usefulness of the intraductal secretin test in order to ascertain whether it can substitute for the conventional duodenal secretin test. Duodenal juice was obtained with a triple-lumen tube and pure pancreatic juice was obtained by retrograde cannulation of the main pancreatic duct using a duodenofiberscope. Pancreatic secretion was stimulated by a bolus intravenous injection of secretin (100 units). The two tests showed comparable interindividual coefficients of variation, significantly good correlations, and comparable diagnostic efficiencies. The intraductal secretin test showed no less reproducibility than that of the duodenal secretin test as reported in the literature. In the intraductal secretin test, secretory volume, peak flow rate, bicarbonate output, and lipase output yielded the best diagnostic efficiency, followed by amylase output and maximal bicarbonate concentration. In the intraductal secretin test, a 10-min collection provided as much information as a 20-min collection. We conclude, therefore, that the 10-min intraductal secretin test is as useful as the conventional duodenal secretin test in assessing exocrine pancreatic function and that the most discriminatory parameters are secretory volume, bicarbonate output, and amylase (or lipase) output.
