Infection of terminally differentiated myotubes with Rous sarcoma virus: reduced synthesis of env and v-src proteins.

We studied Rous sarcoma virus (RSV) protein synthesis in RSV-infected, terminally differentiated chicken myotubes ('late-infected' myotubes), in which no viral DNA integration takes place but all three viral mRNAs (38S, 28S and 21S) are transcribed normally. With the use of specific anti-RSV protein antisera, we found that only the viral gag and pol proteins were synthesized at levels similar to those synthesized in RSV-transformed fibroblasts; the synthesis of env and v-src proteins was significantly reduced in these infected myotubes. We concluded that the viral RNA transcribed from the unintegrated RSV DNA was functional but that genes at the 3' end of the RSV genome were translated at a lower level. By contrast, when mononucleated replicating chicken myoblasts were infected with a mutant (tsNY68) carrying a temperature-sensitive v-src gene and maintained at the non-permissive temperature for this gene, they developed into myotubes with viral DNA integrated in their chromosomal DNA. These 'early-infected' myotubes expressed all four viral genes (gag, pol, env and v-src) at a level similar to that in infected fibroblasts. This result ruled out the possible presence of specific factor(s) in myotubes that preferentially inhibit the 3' genes of RSV, and suggested other translational control(s) of viral gene expression in late-infected myotubes.

5-S-cysteinyldopa in urine and tumors.

1) The urinary 5-S-CD contents in malignant melanoma subjects (n = 135) and non-melanoma subjects (n = 204) were measured by HPLC. These results suggest that, as a biochemical marker, periodic measurement of urinary 5-S-CD is quite useful for evaluating the determinations of stage classification (UICC, 1987), and the detection of metastases, the therapeutic efficacy of operation or immunochemotherapy against malignant melanoma. 2) Quantitative analyses of 5-S-CD values in tissues from primary malignant melanoma lesions (n = 24) and pigmentary tumors other than melanomas (n = 136) showed 80.6-821.4 ng/mg and N.D.-55.0 ng/mg respectively. In view of the above findings, it was suggested that the pigmentary tumors can be diagnosed as malignant melanoma if the 5-S-CD value in the tissues is higher than 100 ng/mg.

The inhibitory effect of the combination of antineoplaston A-10 injection with a small dose of cis-diamminedichloroplatinum on cell and tumor growth of human hepatocellular carcinoma.

The inhibitory effects of a combination of Antineoplaston A-10 Injection with a small dose of cis-diamminedichloroplatinum (CDDP) on cell and tumor growth was tested in vitro and in vivo settings. A human hepatocellular carcinoma cell line (KIM-1) was used for the cell growth and transplanted tumor growth studies. In the cell growth study, one-hour exposure of KIM-1 cells to CDDP in the medium at concentrations of 0.5, 1.0, and 2.0 micrograms/ml inhibited cell growth dose-dependently. Continuous exposure of cultured cells to Antineoplaston A-10 Injection at concentrations of 4, 6, and 8 mg/ml also inhibited tumor growth dose-dependently. The combination of 0.5 microgram/ml CDDP and 6 mg/ml A-10 Injection inhibited cell growth more than did each agent individually. Electron microscopic study showed well-maintained organelle structures in Antineoplaston A-10 Injection-treated cells compared to CDDP-treated cells. alpha-Fetoprotein (AFP) production by 10(4) cells in 48 h increased in the A-10 Injection-treated and A-10 Injection+CDDP-treated groups as the concentration of these agents increased. In the tumor growth study, daily administration of Antineoplaston A-10 Injection 75 mg with once a week administration of 20 micrograms of CDDP for 5 weeks inhibited transplanted tumor growth in athymic mice after 33 days of treatment, while administration of 75 mg of A-10 Injection or 20 or 60 micrograms of CDDP alone showed no significant inhibition of tumor growth.

Partial imitation and partial sensory agnosia in mentally normal children with convulsive disorders.

Mentally normal 6-9 year old children with or without convulsive disorders were examined using the procedures of the five soft signs (SS) selected from conventional soft neurological signs and Bergès' gesture imitation tasks. In comparisons of SS between the 60 children with convulsive disorders (CD group) and the 38 without (N group), the average number of SS detected in each case and the incidence ratio of the two signs--partial imitation (PI) and partial sensory agnosia (PSA)--were higher in the CD group than the N group. Examinations of relationships between age, IQ, error index (EI; an index taken from the Continuous Performance Test) and SS, and of correlations among the SS, found two kinds of SS: age-related and El-related signs. The former included right-left confusion, clumsiness and PI, which correlated with each other, while the latter included unstableness of lateral gazing and PSA, which did not correlate with each other. Even though several factors belonging to characteristics of convulsive disorders were evaluated concerning the presence or absence of PI and PSA, there was only one relationship: PSA was more common in children with febrile convulsions than in those with epilepsy. We speculated that PSA reflected an immaturity of the brain in children with febrile convulsions.

Regulatory factors of pheo- and eumelanogenesis in melanogenic compartments.

Melanogenesis, i.e., synthesis of melanin and melanosomes, is a "cascade" of event which is channelled by internal and external regulatory factors. The recognition and selection of this information and subsequent differentiation of melanogenesis (melanin type and melanosomal development) would be regulated significantly by melanosomal membrane. The melanogenesis type could be switched relatively easily by UV light, hormone, and availability of tyrosinase substrate. The role of sulphydryl compounds as a regulatory factor in melanogenesis type (in particular for pheomelanogenesis) may not be tied to its absolute presence or absence, but rather, to the effective concentration within the melanocyte at a given time. It is, therefore, probable that the morphogenesis of melanosomes may not follow immediately in response to melanogenesis-type changes, hence the melanocyte revealing more often mosaic forms of melanosomes in nature after exposure to non-genetic factors. The switch of melanogenesis would be significantly controlled by structural and functional availability of vesiculoglobular bodies which are encoded or associated with HMSA-5 (69 kDa) glycoprotein. This HMSA-5 protein shares a significant homology with gp75 "b-locus" protein. However, because of our hypothesis that vesiculoglobular bodies carry post- (and pre-) tyrosinase regulatory factors involving in both pheo- and eumelanogenesis, the term "b-protein" which focuses only on eumelanogenesis may not be applied to HMSA-5.

Renal effects of endogenous prostaglandin synthesis promoter ONO-3122.

The renal effects of a prostaglandin synthesis agonist, 1-iodo-3-aminomethyl-5,6,7,8-tetrahydro-2-naphthol (ONO-3122), were investigated in anesthetized rats. ONO-3122 (0.3 mg/kg + 0.3 mg.kg-1.h-1 iv) doubled the urinary excretion of the main metabolites of prostaglandin F, and induced transient increases in renal blood flow and glomerular filtration rate (GFR) with a marked, stable natriuresis. Indomethacin suppressed the natriuresis. When the diuretic fluid losses were replaced, micropuncture showed an unaltered reabsorption of sodium in the proximal tubule but reductions in the loop of Henle (86 +/- 1 vs. 76 +/- 1%) and in the more distal segments (98 +/- 1 vs. 83 +/- 3%) with comparable reductions in water reabsorption. Potassium secretion was seen in the distal and collecting tubules. Without fluid replacement, sodium reabsorption was reduced in the loop and more distal nephron but increased in the proximal tubule. Differences between proximal and distal nephron GFR were unaffected by systemic ONO-3122. Loop perfusion with ONO-3122 did not change tubuloglomerular feedback responses, which were, however, completely suppressed by furosemide. It is concluded that ONO-3122 stimulates renal prostaglandin biosynthesis, transiently dilates renal vasculature, and induces natriuresis mainly by suppressing sodium and water reabsorption in the loop of Henle and the more distal nephron. Luminal ONO-3122 does not affect the tubuloglomerular feedback.

Enantiomeric difference in percutaneous penetration of propranolol through rat excised skin.

The percutaneous penetration of R-(+)- and S-(-)-propranolol (PL) through rat excised skin was investigated in vitro. The flux of S-(-)-PL after application to normal skin was high compared with that of R-(+)-PL. On the other hand, in damaged rat skin, the flux of R-(+)-PL was almost equivalent to that of S-(-)-PL. It is suggested that there is an enantiomeric difference between S-(-)- and R-(+)-PL in terms of penetration through rat stratum corneum.

Neurochemical investigation on the effects of a new diphenylpiperazine calcium antagonist, KB-2796, on the central dopaminergic system of rats.

The effects of KB-2796, a new diphenylpiperazine calcium antagonist, on the striatal dopaminergic system of rats were investigated in comparison with various calcium antagonists and the dopamine antagonist chlorpromazine. The inhibiting effect of KB-2796 on [3H]spiperone binding to striatal membranes in vitro was weaker than those of chlorpromazine and the other diphenylpiperazine analogues, flunarizine and cinnarizine, and more potent than those of verapamil and nicardipine. Diltiazem and nifedipine were inactive. KB-2796 (30, 100 mg/kg, p.o.) had no effect on Kd and Bmax values of in vitro [3H]spiperone specific binding to striatal membranes obtained from the rat at 36 hr and 7 days after repeated administration for 18 days, whereas flunarizine (30 mg/kg, p.o.) and chlorpromazine (3 mg/kg, p.o.) increased Bmax values by 47% and 31%, respectively, at 36 hr, but not at 7 days after the final administration. At 1 hr after the single administration, KB-2796 (30, 100 mg/kg, p.o.) had no effect on the content of dopamine and its metabolites in the striatum, whereas flunarizine (30 mg/kg, p.o.) and chlorpromazine (3 mg/kg, p.o.) increased the level of homovanillic acid. These results indicate that flunarizine may affect dopaminergic neurotransmission by partially blocking dopamine D2 receptors, while KB-2796 has negligible in vivo effect on the dopaminergic system.

Thyroid function before and after induced abortion in normal pregnant women.

We assessed thyroid function before and after induced abortion in 25 normal pregnant women. Serum TSH was significantly increased (P less than 0.02), and serum hCG-beta was significantly reduced (P less than 0.001) 1 week after induced abortion, compared with the levels before induced abortion. There was a significant negative correlation between hCG-beta and TSH, and a positive one between hCG-beta and FT4 before induced abortion (P less than 0.02). No difference was observed in thyroid hormones before and 1 week after induced abortion. The results suggest that hCG stimulates the thyroid gland, gaining an advantage over TSH, in normal pregnant women.

Difference of physiological responses to swimming and running.

The purpose of this research is to investigate the range of response to the treadmill running and swimming. Cardiovascular changes and substrates changes in blood were examined before and after healthy male subjects (swimming group (SG) has swimming habits and running group (RG) has practiced basketball or baseball or running more than four days a week) swam and ran for 10 to 15 minutes voluntarily. Average heart rate was recovered more quickly in case of running than swimming in the RG, but in the SG there was no difference. Diastolic blood pressure recovered to the rest condition faster in the SG than RG. In case that subjects have done familiar exercise, free fatty acids increased a little more after 10 minutes than in case of unfamiliar exercise. These results suggest that response of habitual exercise showed lesser change of diastolic blood pressure and more increase of free fatty acids. There might be a difference in physiological responses between usual sports and other kinds, so that we had to be careful when we apply different styles of activity to use training.

Abnormalities of pulmonary function in patients with non-insulin-dependent diabetes mellitus.

To examine the possible association between the vascular complications of diabetes and changes in pulmonary function, we performed pulmonary function tests including assessment of the diffusing capacity (%DLco) in 80 patients with non-insulin-dependent diabetes mellitus (45 males and 35 females) without overt lung or heart disease. The mean age of the subjects was 57.9 years and the mean duration of diabetes was 10.8 years. The %DLco decreased significantly as the duration of diabetes increased (r = -0.38, p less than 0.01), and the same relationship was also observed in non-smoking subjects (N = 37). The reduction in %DLco was greater in patients with diabetic microangiopathy (especially nephropathy) and in those treated with insulin. Other pulmonary function tests (%VC, FEV1.0, PaO2 and PaCO2) showed no relationship to the duration of diabetes, the degree of microangiopathy or the type of treatment. These results suggest that diabetic microangiopathy may play an important role in the decrease of %DLco.

Modified De Vega's annuloplasty for functional tricuspid regurgitation--early and late results.

From 1978 through 1987, 321 patients who had functional tricuspid regurgitation (TR) associated with mitral or combined mitral and aortic valve disease underwent tricuspid annuloplasty with our modification of the original De Vega's annuloplasty technique. The modified De Vega's annuloplasty consisted of two separate semicircular sutures placed around the anterior and lateral aspect of the tricuspid valve. Preoperatively, 229 (71.3%) of the 321 patients were in New York Heart Association functional class III or IV. There were nineteen early deaths (5.9%), and 15 patients died during a follow-up period of 15 to 126 months (mean follow-up, 26 months). Five patients (1.5%) required reoperation because of biological mitral valve failure and recurrence of mitral stenosis. The severity of TR was evaluated by two-dimensional and pulsed Doppler echocardiography, and was classified on a scale of 0 to Grade 3 according to the maximal distance and the flow pattern of the regurgitant signals in the right atrium. Postoperative echocardiographic evaluation of TR was performed in 121 randomly selected patients late after operation. TR reduced to Grade 1 or less in 107 (88.4%) of the 121 patients after operation. Ninety-six percent of the survivors were in New York Heart Association functional class I or II, postoperatively. The actuarial survival rate for the 321 patients with the modified De Vega's annuloplasty including early deaths was 88.2% at 10 years and the actuarial rate of freedom from reoperation on the tricuspid valve was 97.6%. Our surgical experience indicated that the modified De Vega's annuloplasty, as the method of first choice, is a simple, reliable procedure and resulted in reduction of the severity of TR in 88.4% of the patients with functional TR.

Absorption of oligo-L-[35S]methionine after feeding of a low casein or a low soybean protein isolate diet in rats.

We studied the absorptive properties of oligo-L-methionine (OM), which is an enzymatically synthesized and slowly digestible peptide. Previously, we demonstrated that when OM was added to a low casein diet, the improvement of the body weight gain was higher than when OM was added to a low soybean protein isolate (SPI) diet and we suggested that the difference in the supplementary effect of OM depends on its absorptive rate. In the present study, the OM absorption estimated by the portovenous difference in radioactivity derived from 35S-labeled OM was higher in the casein diet than in the SPI diet in early stages of feeding after fasting. Absorbed OM was quantified by subtracting the radioactivity of [35S]OM remaining in the whole gut from the ingested [35S]OM, 90 and 180 min after feeding casein and SPI diets containing 3% [35S]OM. We also estimated the absorptive efficiencies by subtracting the amount of radioactivity remaining in the intestines from the amount of [35S]OM emptied from the stomach as percentages of the emptied OM. Both the amount of absorbed OM and absorptive efficiencies of OM were higher in the casein group than in the SPI group, and the higher absorptive efficiency in the casein group indicates a higher digestibility for OM when rats are fed a 3% OM diet after fasting. The digestibility of [35S]OM measured by fecal excretion of radioactivity of OM during normal feedings for diets containing 0.3% [35S] OM for 7 days was about 80% in the casein group and 60% in the SPI group. We conclude that the different supplementary effects of OM in the low casein and SPI diets depend on the difference in OM digestibility. The difference in the digestibility of OM may partly depend on the faster absorption rate of OM in the early stages of feeding.

[A case report of spermatocytic seminoma--clinical review of 17 cases in Japan].

A male aged 48 visited our department on October 17, complaining of swelling of his left scrotal content from July, 1990. Physical examination showed the testis to be swollen to the size of a chicken egg, and ultrasound examination disclosed the swelling to be solid and nodular. Left high inguinal orchiectomy was performed on October 29. The testis extirpated was 50 x 40 x 45 mm in size and weighted 80 g; the gross appearance of the cut surface of the testis was nodular with grayish-white color. The lesion was proved to be spermatocytic seminoma histopathologically, but no hemorrhage or necrosis was observed. Radiation in the dose of 36 Gy was given over the left hypogastrium and paraaortic region in 36 Gy each. The patient subsequently did will and has been followed up in our outpatient clinic without evidence of recurrence as of 10 postoperative months. This patient was the 19th reported case of spermatocytic seminoma in Japan. However, General Rules for Clinical and Pathological Studies on Testicular Tumors explain that spermatocytic seminoma is usually seen in a pure form, unassociated with other types of germ cell tumors, making our case actually the 17th case of this particular seminoma if spermatocytic seminomas combined with other germ cell tumors are exclude from the statistical analysis. Reports of spermatocytic seminoma thus defined are reviewed in this study.(ABSTRACT TRUNCATED AT 250 WORDS)

[Drug induced cystitis due to ketotifen fumarate--a case report].

Tranilast, an antiallergic drug, is well known as a causal drug of cystitis, and a report is made here of our experience of 1 case of drug-induced cystitis ascribable to ketotifen fumarate. A 13-year-old female had been taking anti-asthmatic drugs orally since the onset of athmatoid attacks, from age of 5. The attacks intensified from the age of 12, because of this she began to take various anti-asthmatic drugs orally. She visited another hospital, due to pollakisuria, in November, 1990, and received treatment for cystitis. However, the symptoms were not alleviated, and she visited our department on January 9, 1991. By urinalysis, large counts of leukocytes and erythrocytes were observed in a visual field of the sediment. Remarkable reddening was observed over the urinary bladder in the patient's cystoscopic findings. Treatment was given at our department, on an outpatient basis, with various antibacterial drugs for approximately one month, but her symptoms were not alleviated, pollakisuria and aseptic pyuria persisted. The patient had never taken tranilast; oral intake of ketotifen fumarate and saibokutou was discontinued on February 13, due to a suspicion of drug-induced cystitis, and her symptoms subsequently disappeared. On February 22, she took ketotifen fumarate orally again, on her own, due to asthmatoid attack, and her symptoms returned. The oral intake of ketotifen fumarate was again discontinued, and alleviation of the symptoms and normalization of the urinary findings were again observed. As a result, lymphocyte stimulation tests on all the drugs the patient had ever taken, only ketotifen fumarate turned out to be positive.(ABSTRACT TRUNCATED AT 250 WORDS)

[Bilateral multilocular cystic renal cell carcinoma].

A case of bilateral renal cell carcinoma with multilocular cysts is described. A 42-year-old female was admitted to our hospital for the purpose of further examination or right renal cystic mass discovered incidentally by ultrasonography. After she was examined by CT-scan and angiography, right nephrectomy was performed under the diagnosis of right renal cancer. Histological diagnosis was multilocular cystic renal cell carcinoma. One year later a small cystic legion was found in the left kidney. Under the diagnosis of left renal cancer, left partial nephrectomy was performed. Histological diagnosis was multilocular cystic renal cell carcinoma same as the right kidney. Multilocular renal cyst is classified into several classes by its style of development. This case of bilateral renal cell carcinoma which grew to be multilocular cysts is the first case in Japan as far as we can search.

[The successful treatment of G-CSF in autoimmune neutropenia with liver cirrhosis complicated by esophageal varices].

A 72 year-old woman was admitted to our hospital for hematoemesis. After admission, endoscopic examination showed esophageal varices with a red color sign which indicated endoscopic injection sclerotherapy (EIS). Concurrently, however, laboratory findings revealed severe neutropenia in peripheral blood, while bone marrow examination showed marked reduction of mature granulocytes with mild myeloid hyperplasia. As a result of those hematological abnormalities, EIS was halted. Concerning the pathogenesis of this neutropenia, immunofluorescence technique using flow cytometry disclosed the presence of anti-neutrophil autoantibody in the serum, giving a clinical diagnosis of autoimmune neutropenia (AIN). Thereafter, a conventional regimen of corticosteroids as an initial therapy and steroid pulse therapy as a succeeding maneuver were instituted, but in vain. As a last resort, 125 micrograms/body of rhG-CSF was given daily subcutaneously. As a consequence, significant increase in granulocyte count, though transient, was attained, which made EIS possible without any episodes of infections. It seems most likely that a high dose of rhG-CSF exerts beneficial effects as a prophylactic and therapeutic regimen against infections in patients with AIN.

[A case with Graves' disease whose TBII index was decreased after heart surgery].

This case report describes a patient with Graves' disease who underwent heart surgery and we studied changes in thyroid hormone and antithyroid autoantibodies. TBII index of this case, which was high value before surgery, was decreased after surgery for two years, and the patient did not need any antithyroid drug. It is suggested that these changes may be due to cardio-pulmonary bypass and administration of steroids during heart surgery. This case indicates that heart surgery may influence the clinical course of Graves' disease.
