Bloom's syndrome complicated by myelodysplastic syndrome and multiple neoplasia.

The patient, a Japanese male born to a highly consanguineous family, was diagnosed as Bloom's syndrome at the age of 33 when he presented with diabetes mellitus and refractory anemia with excess blasts. Chromosome abnormalities of bone marrow cells included 5q-, -7/7q-, and unusual translocations. During the ensuing years, he developed squamous cell carcinoma of the external auditory meatus, adenocarcinoma of the colon, and squamous cell carcinoma of the tonsil. The patient died of pneumonia at the age of 38. Autopsy revealed intestinal polyposis and hemochromatosis secondary to massive blood transfusions.

Vestibular neuronitis in aged patients: results from an epidemiological survey by questionnaire in Japan.

An epidemiological survey of vestibular neuronitis in Japan was done using a questionnaire. Answer sheets were obtained from 619 patients with vestibular neuronitis. In order to evaluate data from aged patients, 74 cases were singled out. The following results were obtained: i) There was no sexual difference and no laterality of affected side; ii) There was no case of bilateral vestibular neuronitis; iii) Ten cases were reported as recurrent. Aged patients had a relatively high rate of recurrence; iv) About 10% of 74 cases had had an upper respiratory tract infection, and this rate was lower than that for patients under 65 years of age; v) Thirty-five cases had complications. Hypertension was the most common complication; vi) The caloric test was re-used in 28 cases. The continued existence of caloric CP was observed in 23 cases upon re-examination.

Serum viral antibody titer in vestibular neuronitis.

Fifty-seven cases of vestibular neuronitis were evaluated for viral infection by means of serum antibody titer. The viruses tested were herpes simplex virus, varicella-zoster virus, cytomegalovirus, EB virus, adenovirus, influenza virus A, influenza virus B, parainfluenza virus 3, mumps virus, rubella virus and measles virus. Paired sera were examined in 49 cases among 57 cases, 26 cases showed significant change (four-fold or greater change) in viral antibody titer. Only one case (53-year old female) showed high HSV 1 IgM antibody level by ELISA method, so the vestibular neuronitis in this case was assumed to have a close relation to viral infection.

The otolithic organ in the developing chick embryo. Scanning electron microscopic study on the utricular macula.

The long and short axes of utricular maculae were measured to calculate growth curves using an ocular micrometer under stereomicroscope. The mean lengths of the long and short axes on the 6th incubation day were 0.06 mm and 0.05 mm, respectively. Thereafter: 1.00 mm and 0.74 mm on the 11th incubation day; 1.34 mm and 1.03 mm on the 15th incubation day; and on hatching 1.51 mm and 1.07 mm, respectively. The surface structure, particularly the otoconia and the sensory hairs of the utricular maculae, on the 4.5th incubation day to the 7th post-hatching day of White Leghorn chicks were observed with a scanning electron microscope (SEM). A number of microvilli and primary cilium on the inner surface of the otocyst were found between the 4.5th and the 5th incubation day. Immature sensory hairs appeared on the 5th incubation day. The polarity of the sensory hairs became evident on and after the 9th incubation day. Mature sensory hairs were seen all over the utricular maculae between the 1st and the 3rd post-hatching day. Mature sensory hairs were mainly seen near the striola, and immature ones peripherally. Then, the various forms in the maturation phase of sensory hairs were seen between the striola and the periphery. These arrangements suggest some process of maturity from the peripheral region to the central striola region. Immature otoconia were first seen on the 6th incubation day. Mature otoconia were recognized on and after the 11th incubation day, and they were observed on the whole surface of the utricular maculae. Global substances on the macula with a rough surface and spongy structure were observed suggesting otoconial precursors transforming themselves into otoconial crystals.

Role of gastric digestion in the absorption of slowly digestible peptide, oligo-L-methionine, in rats.

Absorption of a slowly digestible peptide, oligo-L-methionine (OM), added to a low casein diet was faster than absorption of OM added to a low soybean protein isolate diet in early stages of feeding in chronic portal-cannulated rats. In the present study, the gastric digestion of 14C-labeled OM in rats fed a casein-based diet was higher than that in rats fed an soybean protein isolate-based diet 30 min and 3 hr after feeding. In rats with chronic bile-pancreatic juice diversion from the proximal small intestine, the higher gastric solubilization of OM in the stomach of the casein group was also observed, but the contents of soluble digest of OM in the stomach were lower than those in the normal rats. The portal absorption of OM in the casein group was higher than in the soybean protein isolate group both 30 min and 4 hr after feeding in the bile-pancreatic juice-diverted rats, and the difference of the portal absorption between the diet groups corresponded to the difference of the amount of solubilized OM in the upper small intestine; this, in turn, depends on pepsin digestion in the stomach. These findings suggest that the difference between the two diet groups in the ability to digest OM in the stomach can at least partly explain the higher portal absorption of OM in the casein group in the early stages of feeding.

[Motility of isolated vestibular hair cells induced by potassium].

Many studies of the outer hair cells in cochlea have demonstrated active motility. However, very few studies have been done on vestibular hair cells. This study was designed to demonstrate the motile responses of isolated vestibular hair cells of the chick, induced by potassium promoting contraction. Reversible cell shape changes were observed in 4 of 6 type I hair cells and 3 of 5 type II hair cells by applying the contraction solution. The cell shape changes were revealed mainly in the cuticular plate and infracuticular region. It was suggested that contraction in the cuticular plate of the isolated hair cells might be converted into tension which increases the stiffness of the sensory hairs and restricts their motions, based on the results of the present study, and the structure of contractile proteins and hair behaviors reported by previous investigators.

[Paranasal sinusitis due to Pasteurella multocida].

A case of paranasal sinusitis due to Pasteurella multocida (P. multocida) is reported. A 39-year-old woman presented with chief complaints of rhinorrhea and headache. The patient kept a cat in her house and kept such close contact with it as to wake up by being licked every morning. Bacteriological examination revealed P. multocida isolated from her nasal discharge and also from the saliva of the cat kept by the patient. The two isolates were compatible with respect to biochemical properties, serotype and drug susceptibility. Therefore, P. multocida infection in this case was considered to have originated from the pet cat. P. multocida infection has been increasing recently. One of the reasons is a pet boom. In order to prevent acquiring the infection from a pet animal, we should have knowledge about this infection, advise the patient to avoid close contact with pets, and provide valuable information concerning these problems to society from the viewpoint of zoonosis.

[Changes in the utricular otoconia of the chick embryo developed under 2G-gravity].

Whether or not vestibular adaptation occurs upon exposure to altered gravity is an important human problem. Studies have been performed to ascertain whether morphological changes in vestibular organs occur in animal models upon exposure to microgravity. On the other hand, the morphological changes which occur in vestibular organs under hypergravity have not yet been fully elucidated and morphological changes in the otoconia of the chick embryo exposed to hypergravity during the embryonic period remain to be thoroughly examined. The purpose of this paper is to clarify the effects of hypergravity on vestibular organs, especially its effects on otoconial formation in the developing chick embryo. Thus, we designed an apparatus which creates artificial gravity, creating approximately 2G gravity by means of centrifugation. The utricular macula was chosen for study because it can be prepared for examination with a minimum of dissection artifacts. The data presented show that morphological changes were confined to otoconial size alterations according to scanning electron microscopic observations. Giant otoconia were observed along the marginal zones of utricle .

Effects of mast cell growth factor, interleukin-3, and interleukin-6 on human primitive hematopoietic progenitors from bone marrow and cord blood.

Phycoerythrin (PE)-labeled murine monoclonal antibodies (MAB) to CD2, CD10, CD19, CD20, and CD33 were used as lineage markers, as were PE-labeled anti-DR MAB and fluorescein isothiocyanate (FITC)-conjugated MAB to CD34. One hundred CD34+Lin+DR+ or CD34+Lin-DR- cells were individually sorted and incubated without adherent cell layer in alpha-medium with or without cytokines such as 100 U/mL recombinant human interleukin-3 (rhIL-3), 100 U/mL rhIL-6, and/or 500 ng/mL mast cell growth factor (MGF). The incubated cells were harvested and cultured in medium containing methylcellulose every 2 weeks. The numbers of colonies from colony-forming units-granulocyte/macrophage (CFU-GM), burst-forming units-erythroid (BFU-E), and mixed colony-forming units (CFU-Mix) were scored on day 14. In the incubation, CD34+Lin-DR- cells from bone marrow and cord blood produced more CFU-GM and BFU-E, and for longer periods, than did CD34+Lin+DR+ cells. In addition, CD34+Lin-DR- from cord blood supplied more CFU-GM and BFU-E than did CD34+Lin-DR- from bone marrow. Although these data demonstrate that MGF, IL-3, and IL-6 synergistically stimulate the production of CFU-GM and BFU-E, this study indicates that CD34+Lin-DR- cells contain more primitive hematopoietic progenitors and that CD34+Lin-DR- cells are unable to maintain their self-renewal capacity in the presence of IL-3, IL-6, and MGF without adherent cell layer.

[Biopsy diagnosis for gastric lymphoma--usefulness and limitation of AgNOR staining].

Sixteen cases of resected gastric lymphoma were classified into 3 groups based on the easiness of the diagnosis; mucosa associated lymphoid tissue lymphoma (MALToma) 4 cases, superficial type lymphoma 5 cases and ulcerative (mass forming) type lymphoma 7 cases. Sixty seven of pre-operative biopsy reports and 44 biopsy specimens were analyzed. Although lymphoma cells retrospectively obtained in 92%, 69% and 88% of biopsy specimens, the definite diagnosis was only made in 7%, 24% and 38% of endoscopic biopsy. Mean AgNOR number per nucleus in resected specimens were as follows; chronic inflammation 1.20 +/- 0.02, MALToma 1.39 +/- 0.23, superficial lymphoma 2.44 +/- 0.50 and ulcerative lymphoma 3.44 +/- 0.55. They correlated well to proliferating cell nuclear antigen positive rates (r = 0.776, p < 0.01). Supposing that lesions showing 2.0 or more AgNOR dots in biopsy specimens are lymphoma, diagnostic rate improves to 69% in superficial lymphoma and to 88% in ulcerative lymphoma. Biopsy specimens from MALToma never showed 2.0 or more AgNOR dots. These results suggest that application of AgNOR to the biopsy diagnosis is useful in superficial and ulcerative lymphoma and has limitation in MALToma of the stomach.

Proliferating cell nuclear antigen (PCNA), immunostaining and flow cytometric DNA analysis of renal cell carcinoma.

Nuclear PCNA immunoreactivity is found in the proliferating compartment of normal and tumor cells. The purpose of the present study was to examine immunohistochemical staining of PCNA in renal cell carcinoma and its relationship to tumor grading, flow cytometric DNA analysis and prognosis. The resected kidneys from 81 patients with renal cell carcinoma were used. Anti-PCNA monoclonal antibody was applied to paraffin sections to identify proliferating cells by the SAB technique. The proportion of PCNA-positive cells to negative cells was determined by counting the tumor cells stained by PCNA at 400x magnification and expressed as percentage of positive cells. In parallel with the PCNA immunostaining DNA flow cytometry was performed, using the cell suspension prepared from paraffin sections and stained with propidium iodide. 79 tumors were evaluable for PCNA immunohistochemistry and 54 tumors for DNA flow cytometry. Immunohistochemically, there was no significant difference in percentage of PCNA-positive cells between tumor grades G 1 and G 2 (1.4 +/- 1.3% in G 1 and 1.6 +/- 1.4% in G 2), while G 3 tumors contained high percentage of PCNA positive cells, the mean value being 7.6 +/- 5.3%. The percentage of PCNA-positive tumor cells was found to be correlated with histological grading of tumor and also with the S+G2+M phase fraction and the S phase fraction measured by flow cytometry (p < 0.01). The mean percentage of PCNA-positive tumor cells was higher in aneuploid tumors than in diploid tumors. There was no significant relationship between the percentage of PCNA positive cells and tumor staging. Sixty-one cases were available for survival analysis.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of mutations of ras oncogene in human pancreatic exocrine and endocrine tumors.

BACKGROUND: Ras oncogene mutations have been found in many human cancers; however, pancreatic endocrine tumors have rarely been studied. The purpose of this study was to analyze ras mutations in pancreatic endocrine tumors and to compare these results with the incidence of ras mutations in pancreatic exocrine cancers studied in our laboratory. METHODS: Ras oncogene mutations were studied in 33 foregut endocrine tumors (pancreatic 31, duodenal submucosa 2). Eleven were insulinomas, 12 gastrinomas, 2 glucagonomas, and 11 others were nonfunctioning islet cell carcinomas. Thirteen were benign and 20 were malignant. These were compared with 65 pancreatic exocrine cancers. Tumors were microdissected from paraffin-embedded sections. DNA was extracted and amplified by polymerase chain reaction. Mutations were detected by a oligonucleotide hybridization method with sequence-specific phosphorus 32-radiolabeled probes. RESULTS: No ras mutations were identified among the 33 pancreatic endocrine tumors. In contrast, 51 of 65 (78.5%) pancreatic exocrine cancers exhibited a ras mutation. Fifty were K-ras mutations and one unusual tumor exhibited a N-61 ras mutation. CONCLUSIONS: Ras oncogene mutations do not play a role in the tumorigenesis of pancreatic endocrine tumors.

[A case of mucinous cystic adenocarcinoma of the lung with signet-ring cells].

A 70-year-old male case of mucinous cystic adenocarcinoma of the lung with signet-ring cells was reported. Patient was operated with diagnosis suspicious lung cancer by the specimen from TBLB. But, intraoperative rapid pathological-diagnosis was confused us for no cellular component, finally we performed right upper lobectomy and R1 LN dissection. Postoperative histological examination showed that the tumor was entirely filled with mucin and there are a few tumor cells with the signet-ring shape in its marginal area. So, this tumor was diagnosed as mucinous cystic adenocarcinoma of the lung with signet ring cells. This type of the lung adenocarcinoma was rare case, but we need to have much attentions because of the clinical differences from the other type of adenocarcinoma.

[A case of exogenous lipoid pneumonia probably due to domestic insecticide].

A 61-year-old female was admitted to our hospital because of cough, hemosputum and right chest pain. The P-A and L-R chest films revealed focal consolidation in the right upper filed and mass-like shadow in the right S3. Transbronchial lung biopsy specimens showed collections of lipid-laden macrophages. The lipid material of macrophages in broncholaveolar lavage fluid was stained yellow-orange with Sudan IV. The patient had no difficulty in swallowing and had no history of regular use of oily drugs. Fiberoptic bronchoscopy and chest CT showed no obstruction of bronchi. She had used much domestic insecticide with the doors closed before going to bed every summer and autumn for 10 years. We concluded that the domestic insecticide containing mineral oil had resulted in exogenous lipoid pneumonia. Her symptoms and abnormal shadows in chest X-ray film disappeared soon after antimicrobial chemotherapy was started. She is now healthy without recurrence.

[Assay of erythropoietin in serum with short term enzyme linked immunosorbent assay method--the clinical significance, Part 1: Relation to anemia in renal failure and hematological disorders].

With a newly developed enzyme linked immunosorbent assay kit TOYOBO Co. in which 2 anti-EPO monoclonal antibodies were used, we assayed EPO concentration in sera from normal adults, 168 patients with renal failure and 333 patients with hematological disorders. In the patients with renal failure, serum EPO level was normal (52.9%) or reduced (42.9%), and there was no correlation to their hematocrits. However, there was an increment in EPO concentration correlated to their severity of anemia in the most patients with hematological disorders, such as iron deficiency anemia (correlation coefficient r = -0.74), aplastic anemia (r = -0.89), leukemia (r = -0.81), and MDS (r = -0.65). On the other hand, EPO concentration in sera from all the untreated patients with polycythemia vera were significantly low level. But the concentrations of EPO from the patients successfully treated, with normal hematocrit were recovered to normal level. In the patients with secondary polycythemia, there were much varieties in EPO level. Assay of EPO in blood is important not only for diagnosis of polycythemia but also for the analysis of anemia and clinical use of EPO in vivo. The method described here is accurate and technically not complicated, and could be widely induced in most laboratories.

[Assay of erythropoietin in serum with short term enzyme linked immunosorbent assay method--the clinical significance: Part 2--:Relation to serum iron, UIBC and ferritin in renal failure and hematological disorders].

With a newly developed short term enzyme linked immunosorbent assay kit (TOYOBO Co.), in which 2 kinds of anti-EPO monoclonal antibodies were used, we assayed EPO concentration in sera from patients with renal failure and hematological disorders. In this report, the EPO data were analysed in relation to serum iron concentrations, with ferritin and UIBC. In the patients with renal failure, there was no significant correlation between EPO concentration and serum iron, ferritin, nor UIBC concentration. On the other hand, in the patients with hematological disorders, there were two types. One was in patients with iron deficiency anemia, whose serum EPO was negatively correlated to serum iron (r = -0.64) and ferritin (r = -0.59), but positively related to UIBC (r = 0.27). The another was the pattern in patients with aplastic anemia, leukemia and MDS, whose serum EPO positively correlated to iron and ferritin but negatively correlated to UIBC. In the patients with aplastic anemia serum EPO had good correlation to serum iron (r = 0.62), ferritin (r = 0.60) and UIBC (r = -0.46). The relationship of EPO to iron in the patients with leukemia (r = 0.54), and EPO to ferritin in the patients with MDS (r = 0.42) show significantly positive correlation coefficient.(ABSTRACT TRUNCATED AT 250 WORDS)

[An evaluation of a therapy with antibacterial and antifungal agents infections with hematological disorders].

The primary objective of this study is to confirm the efficacy and safety of combination therapy with antibacterial and antifungal drugs for patients with hematological disorders complicated with infections of unknown causative organisms. The subjects consisted of 55 patients with hematological disorders, 28 males and 27 females, ranging from 1 to 78 years of age, with an average age of 34 years. The breakdown of the patients by disease was: 41 leukemias, 10 malignant lymphomas, 3 myelodysplastic syndromes and an aplastic anemia. All patients were treated with sulbactam/cefoperazone (SBT/CPZ) and aminoglycoside (AG) plus fluconazole (FLCZ) as an empiric therapy. Overall efficacy rate was 54.5% for all the patients: 57.7% for patients with suspected sepsis and 0% for those with pneumonia from which unknown organisms were detected. The efficacy rate was 59.4% for patient who had been previously treated with other antibiotics and 47.8% for those who had not previously been treated. The above results suggested that this combination therapy might be effective for severe infections of unknown causative organisms associated with hematological disorders.
