Cellular localizations and processing of the two molecular forms of the Hodgkin-associated Ki-1 (CD30) antigen. The protein kinase Ki-1/57 occurs in the nucleus.

The Ki-1 antibody not only detects a Hodgkin-associated membrane molecule of 120 kd (Ki-1/120 = CD30), but also reacts with an independently synthesized molecule of 57 kd (Ki-1/57) that only occurs intracellularly. Hodgkin's disease-derived cell lines L428 and L540 contain both Ki-1-reactive antigens, whereas others, e.g., U266/Bl myeloma cells, only express the intracellular Ki-1/57. The present immunoelectronmicroscopic analysis detected the Ki-1/57 antigen of U266/Bl cells not only in the cytoplasm, but also in association with the nuclear envelope, chromatin structures, and nucleoli. This Ki-1/57-specific type of labeling also was observed in L428 and L540 cells that, in contrast to U266/Bl cells, showed an additional staining of cell membranes and cytoplasmic vesicles. These results were confirmed by two independent methods: 1) cytocentrifuge preparations of isolated nuclei of L540 cells showed a spotted Ki-1-specific labeling, 2) immunoprecipitations demonstrated that the Ki-1/57, but not the Ki-1/120 antigen, was transferred into the nuclei of L540 and U266/Bl cells, whereas the Ki-1/120 antigen with its 90-kd precursor remained in the non-nuclei fraction of L540 cells.

[Neonatal mortality in infants with very low birth weights in the county of North Jutland. Retrospective study].

The object of this investigation was to review the neonatal mortality among infants with very low birthweights (less than or equal to 1,500 gram) in the County of North Jutland and to determine the distribution of the main causes of death. It is important to note that, in a review of this type, the material was unselected. The period involved was 1988-1989. During this period, 86 infants with very low birthweights were born and 63 of these survived for longer than 28 days, corresponding to a neonatal survival of 73%. The survival increased from 11% in infants weighing 500-700 gram at birth to 93% for infants with birthweights between 1,250-1,500 gram from 0% with gestational ages of 24-25 weeks to 98% with gestational ages greater than or equal to 30 weeks. No significant differences in survival were observed as regards sex, place of delivery, method of delivery and singleton/twin delivery but survival was markedly dependent on whether the infants had asphyxia on delivery. The commonest causes of death were the respiratory distress syndrome, periventricular haemorrhage and asphyxia, followed by sepsis, enterocolitis necrotans and immaturity. The incidence of immediate sequelae in the form of chronic pulmonary disease and the retinopathy of prematurity were low.

[Periventricular cerebral hemorrhage in children with very low birth weights in the county of North Jutland. Retrospective study].

The object of this investigation was to determine the frequency of intracerebral periventricular haemorrhage in neonates with very low birthweights in an unselected material from the County of North Jutland. During the years 1988-1989 a total of 86 infants with low birthweights were born in the county. Intracerebral periventricular haemorrhage was diagnosed by means of ultrasonic scanning of the cerebrum and/or postmortem examination. Seven infants had to be excluded as neither of these examinations had been performed. The material thus consisted of 79 infants with a median weight of 1,196 gram (range 565-1,500 g) and a median gestational age of 30 weeks (range 25-37 weeks). Periventricular haemorrhage was found in a total of 13 infants, corresponding to 16% and the haemorrhages were found mainly in infants with gestational ages less than or equal to 29 weeks (32%). Haemorrhages with involvement of the parenchyma and/or dilation of the ventricular system comprised more than half of the cases of haemorrhage (8 out 13). The frequency of periventricular haemorrhage fell with gestational ages less than or equal to 30 weeks and the haemorrhages were less extensive. The respiratory distress syndrome and asphyxia were found to important risk factors for the development of periventricular haemorrhage.

[The distribution effects of the Danish 800 crown rule. Preliminary results of the DIKE study. Choice of drugs and drugs used by the population].

During the period 1 July 1989 to 31 December 1990, patients had to pay the full price for subsidized medicine up to DDK 800 (66 pounds) per calendar year. The consequences for the population were examined by a representative national survey (January 1990) and for the patients by interviews at pharmacies (December 1989 and April 1990). Fourteen percent of the population had exceeded the limit of DDK 400 (33 pounds) for the second half of 1989. By the end of December 1989, 53% of patients at pharmacies had exceeded the limit. According to the national health survey (1986) 51% of the population took medicine during a period of two weeks; 58% female, 42% male. In 1990, 41% of the population had no medicine expenses. Among the other fraction 15% had expenses of DDK 200 (16 pounds) or more, monthly. Among patients at pharmacies 22-25% had expenses to that amount. More than 90% of the patients at pharmacies bought the prescribed medicine or the medicine normally used. Less than 10% asked the physician to prescribe a cheaper medicine. The new regulation on medicine reimbursement has not yet induced greater price-consciousness in the users. Whether this is possible by economic means, is an open question.

[Diagnostic problems of purulent meningitis in children in general practice].

Previous investigations have demonstrated that considerable problems may exist in the diagnosis of purulent meningitis (PM) in general practice. Referrals from general practitioners/duty roster doctors concerning 97 children discharged the diagnosis of PM were reviewed retrospectively. The patients were subdivided into two groups according to whether the diagnosis was established by the referring doctor or not. Only 35% of the children under one year were admitted for suspected PM, whereas 65% of the children between one year and 15 year were hospitalized with the correct diagnosis. The commonest positive findings in both age groups were alterations in consciousness which were found in more than 80% of the children. Children in whom the diagnosis was not established by the referring doctor had fewer classical signs of meningitis (neck-stiffness, Kernig's sign, bulging fontanelle and petecchia) than children in whom the diagnosis was established. Children with negative cultures from the cerebro-spinal fluid were significantly more frequently treated with antibiotics prior to hospitalization. Approximately half of the children admitted with suspected meningitis were not treated according the guidelines issued by the Danish Board of Health, without this having any effect on the survival rate.

Development of IL-2-independent feline lymphoid cell lines chronically infected with feline immunodeficiency virus: importance for diagnostic reagents and vaccines.

Two interleukin 2 (IL-2)-independent feline immunodeficiency virus (FIV) producer cell lines (FL-4 and FL-6) were produced by selecting cells from an IL-2-dependent culture of mixed peripheral blood lymphocytes infected with FIV. The new cell lines have been stable for over 1 year and spontaneously produce FIV with an average reverse transcriptase titer of 300,000 cpm/ml and an average sucrose gradient purified viral protein concentration of 1 mg/l. FIV produced from these cultures is highly infectious in vitro and in vivo. The FL-6 cell line was phenotyped as expressing the feline CD8 and Pan-T antigens, while the FL-4 cell line expressed the CD4, CD8, and Pan-T antigens. Both cell lines, however, express high levels of viral core and envelope proteins. Paraformaldehyde-inactivated whole virus and similarly inactivated whole-cell virus preparations induced a strong antibody response to core and envelope antigens in immunized cats. The establishment of FIV-producing feline IL-2-independent peripheral blood lymphocyte lines should be valuable for the development of FIV-diagnostic reagents and vaccines and also as a model for human acquired immunodeficiency syndrome vaccine development.

Were Langfeldt's schizophreniform psychoses really affective?

Langfeldt's cases of schizophreniform psychoses were reclassified according to the ICD-9 and DSM-III-R diagnostic systems. The main purpose was to reexamine the validity of his concept of 'schizophreniform psychoses' to see whether it supported the existence of a 'third psychosis', and whether his material could be helpful in identifying good prognostic features in schizophrenia. Most of the schizophreniform psychoses turned out to be affective disorders with psychotic features. The number of other psychotic disorders was too small to facilitate a more thorough examination.

Observer variation in the radiographic classification of ankle fractures.

We recorded inter- and intra-observer variations in the classification of ankle fractures by the Lauge Hansen and Weber systems. Radiographs of 94 patients were classified independently by four observers. The observer variation was calculated by kappa statistics, which corrects the obtained values for the agreement expected by chance. There was an acceptable level of agreement for the overall classification into both systems. For the staging of supination-adduction and supination-eversion fractures in the Lauge Hansen system the agreement was poor. The results indicate that future classification systems should be subject to reliability analysis before they are accepted.

Effect of increased donor age on kidney transplant outcome.

Donors over the age of 50 years provided kidneys for 28 of our 226 recipients (12.4%) transplanted from January 1, 1987 to December 31, 1988. Immediate function following transplantation occurred in 36% of the kidneys from donors both over and under the age of 50. The overall 3-month graft survival rate for transplants from donors over 50 years was 89%, compared with 78% for transplants from donors under 50 years (p greater than 0.05). Thus kidneys from well-selected older donors make an important contribution to the total pool of organs available for transplantation.

Protein kinase activity of the intracellular but not of the membrane-associated form of the Ki-1 antigen (CD30).

The Hodgkin-associated Ki-1 antigen (CD30) consists of a 120-kDa membrane-associated glycosylated phosphoprotein (Ki-1/120) and a 57-kDa non-glycosylated phosphoprotein (Ki-1/57) which only occurs intracellularly. Both molecules are phosphorylated at serine residues. An analysis of the peptide fragments resulting from staphylococcal V8-protease digestion of the Ki-1/57 molecule revealed identical bands irrespective of the cell source. Only a few bands of the Ki-1/57 digests appeared among the peptide fragments of the Ki-1/120 membrane antigen. The protein kinase activity was tested for both forms of the Ki-1 antigen. The Ki-1/120, devoid of the Ki-1/57 molecule, was immunoprecipitated from cell lysates of Hodgkin-analogous cell lines L428 or L540, which had been loaded with the Ki-1 or the Ki-1-analogous antibodies Ber-H2, HSR-1, -2 and -3 (method 1). These other antibodies reacted with the Ki-1/120, but not with the Ki-1/57 antigen. The latter, devoid of the Ki-1/120, was isolated from L540 cells after removal of the membrane form by method 1 or from U266/Bl myeloma or Raji Burkitt lymphoma cells which only contain the smaller form. Effects of non-specific adsorption were eliminated by various control precipitates. The Ki-1/57 intracellular antigen showed autophosphorylation and could phosphorylate histones as well. In contrast, a protein kinase activity of the membrane-associated Ki-1/120 could not be demonstrated.

Predictive value of the four good prognostic features in DSM-III-R schizophreniform disorder.

DSM-III-R divides schizophreniform disorder into 2 subtypes with and without good prognostic features. The 4 prognostic features have been selected based on the literature, and the presence of at least 2 should indicate a good prognosis. The predictive value of the good prognostic features was tested in a sample of 16 untreated patients with DSM-III-R schizophreniform disorder with known long-term outcome based on personal follow-up examination. No correlation between the presence of 2 or more features and favorable outcome was observed. Confusion, disorientation or perplexity at the height of the psychotic episode was the only feature consistently associated with a favorable outcome in this sample. The introduction of good prognostic features of schizophreniform disorder by DSM-III-R has been done without due consideration of the methodological problems of prediction research.

Langfeldt's schizophreniform psychoses fifty years later.

As a result of follow-up studies published in 1937 and 1939, Langfeldt divided schizophrenia into two groups; 'typical schizophrenia' which had a poor outcome, and the 'schizophreniform psychoses' which had a less typical clinical picture of schizophrenia and a good outcome. Langfeldt's cases of schizophreniform psychoses were reclassified according to the ICD-9 and DSM-III-R diagnostic systems. Most of the schizophreniform psychoses did not appear 'schizophrenia-like' at all, but turned out to be mainly affective disorders. Those included in Langfeldt's diagnosis of 'schizophreniform psychoses' were found to be too heterogenous to validate the existence of this syndrome.

Misclassification of childhood homicide on death certificates.

Suspect classification of homicide deaths of Connecticut residents under 20 years of age was noted for 29 percent of cases examined. Misclassification was attributed to incomplete or erroneous information recorded on the death certificates, rather than errors in the designation of ICD-9 homicide codes. The results have important implications in the interpretation of vital statistics when homicide is listed as the cause of death and underscore the value of record linkage systems.
