Precocious puberty due to a hypothalamic hamartoma.

A case of precocious puberty due to a hypothalamic hamartoma is presented. Concentrations of plasma LH and FSH, testosterone and its derivatives were found to be elevated. Circadian rhythms of LH were also observed. After removal of the mass, plasma LH and FHS concentrations declined to nearly half the preoperative levels.

[Autosensitization of neurosurgical cases by normal brain tissue antigens (author's transl)].

The leukocyte reactions of 106 neurosurgical cases, including 63 brain tumors, 10 intracerebral hematomas, 10 cerebral infarctions, 10 subarachnoidal hemorrhages, 8 cerebral injuries and 5 chronic subdural hematomas, against the extracts of gliomas and normal brain tissues were tested by capillary migration (LMI) and adherence inhibition (LAI) assays. Both tests showed specific responses with autochthonous and allogeneic glioma extracts in glioma patients. The sensitivity of LAI was superior to that of LMI, although LAI also showed adherence enhancement in the presence of weakly sensitized leukocytes or weak antigenic stimuli. Leukocytes from glioma patients showed positive inhibition with normal brain tissues from patients with glioma and intracerebral hematoma. Positive leukocyte reactions with normal brain tissues were also confirmed in patients with intracerebral hematomas, cerebral infarctions and severe cerebral lacerations, but not in those with subarachnoidal hemorrhages, minor cerebral contusions and chronic subdural hematomas. These results suggest that the leukocytes of patients with destructive brain lesions were autosensitized by normal brain antigens. The autosensitization has some advantages in that destroyed brain tissues are eliminated, but the hyperimmune state might cause postictal brain edema and should be properly controlled by steroids.

Experimentally induced cerebral aneurysms in rats: Part III. Pathology.

Experimentally induced cerebral aneurysms in rats treated with unilateral ligation of the common carotid artery, deoxycorticosterone and salt hypertension, and beta-aminopropionitrile, were studied macroscopically and with light-microscopy. The material consisted of 13 aneurysms located on the anterior cerebral-anterior communicating arterial complex and one on the posterior cerebral artery. The findings were generally in accordance with that of spontaneous lesions in man. The results show the validity of these experimental aneurysms as a model for saccular cerebral aneurysms in man.

Embolization and superselective angiography by means of balloon catheters.

Three types of thin flexible balloon catheters were made. One of them is a releasable balloon catheter, which is prepared for embolization of intracranial aneurysms, arteriovenous malformations and fistulas. The other two are nonreleasable balloon catheters that are utilized for temporary occlusion, catheterization, and superselective angiography of the intracranial arteries. These catheters are useful experimentally for the above mentioned purposes. Our clinical experiences with catheterization and superselective angiography with our balloon catheter are reported.

Intracranial germinomas: treatment and results.

Eighty-nine patients with intracranial germinomas were treated from 1954 through 1975. Radiotherapy was given in 62 patients. The five-year survival rate was 61% and the ten-year survival rate was 55%. In unbiopsied patients the five-year survival rate was 66% and the ten-year survival rate was 59%, whereas in biopsied ones it was 56% and 47%, respectively. Prophylactic irradiation to the spinal axis in the presence of germinoma cells in the cerebrospinal fluid may not be justified.

Rim enhancement in computed tomography with chronic epidural hematoma.

Three cases of epidural hematoma with delayed clinical course are presented. In each patient, computed tomography (CT) initially failed to show typical biconvex, high density extra-axial lesion. The correct diagnosis was reached only after injection of contrast medium revealed enhancement of the border of the hematoma. Importance of contrast enhancement in selected patients with head injury is stress, and the mechanism of rim enhancement is discussed.

Experimentally induced cerebral aneurysms in rats: part II.

An experimental condition that permits easy inducement of cerebral aneurysms in rats is reported. Previously, we demonstrated that saccular cerebral aneurysms were inducible in experimental animals and suggested that adult rats might be more preferable for inducing the lesion. Therefore, a longer-term study was performed intending to increase the incidence of cerebral aneurysms, using only adult rats. Adult rats were treated with ligation of one common carotid arter, deoxycorticosterone and salt hypertension, and beta-aminopropionitrile. Cerebral aneurysms developed in eleven of 30 rats which died 11-21 weeks after the start of the beta-aminopropionitrile feeding. All aneurysms were located on the anterior cerebral-anterior communicating arterial complex. Contributory mechanisms are discussed.

[Cerebrospinal fluid (CSF) cytology using "cytospin" in patients with brain tumors (author's transl)].

Cerebrospinal fluid (CSF) cytology was performed using cytocentrifugation method ("Cytospin") in 88 preoperative patients who had been known to have deeply seated brain tumors by CT. 20 cases (22.7%) among them showed positive findings and 11 cases (12.5%) probably positive findings. High incidence of positive results were obtained in germinomas (60.9%), ependymomas (50.0%), medulloblastomas (40.0%), followed by malignant lymphomas (20.0%), pontine gliomas (16.7%) and metastatic tumors (16.7%). In contrast, it was persistently negative in cerebral astrocytomas, glioblastomas, teratomas, epidermoids, meningiomas and neurinomas. This method was quickly prepared in 30 minutes and was superior in retaining detailed cytomorphology. It was concluded that the CSF cytology is a less invasive diagnostic investigation, can provide an additional information as regards the nature of some kinds of brain tumors and is still useful even in the CT era.

Intracavernous giant aneurysm associated with Marfan's syndrome.

A case of intracavernous giant aneurysm associated with Marfan's syndrome is reported. The patient was treated with a superficial temporal artery-middle cerebral artery bypass and a ligation of the internal carotid artery in the neck. Although they are common in aorta and pulmonary artery, the vascular abnormalities in Marfan's syndrome are very rarely reported in intracranial arteries. The patient also had an aberrant internal carotid artery which presented as a red mass behind the eardrum.

Balloon embolization of a giant aneurysm using a newly developed catheter.

A new catheter with a releasable balloon has been developed for the controllable embolization of intracranial vascular lesions located beyond many arterial curves such as the carotid siphon. Embolization is accomplished by introduction of the balloon to the lesion, followed by inflating the balloon with a polymerizable liquid. After this liquid sets to a gel, the catheter is twisted off to detach the balloon at a specially designed joint. This balloon catheter has been used clinically to embolize an aneurysm of the middle cerebral artery.

[Nonsuture microvascular anastomosis--experimental arterial end-to-end anastomosis using plastic adhesive and a soluble PVA tube (author's transl)].

Microvascular anastomosis is now widely applied and many improved methods of the nonsuture anastomosis have been developed instead of the suture anastomosis for the purpose of saving time and making the reconstruction easier. We introduced a new nonsuture method of microvascular anastomosis using plastic adhesive and a soluble tube made of polyvinylalcohol (PVA). PVA, which had been utilized as a plasma expander, is a water-soluble polymer and its solubility is changeable depending on the degree of polimerization and percent saponification. We have made two kinds of soluble PVA tubes, the one has monolayer wall and the other double layered wall. The inner wall of the latter is more soluble than the outer wall. As plastic adhesives, we employed ethyl 2--cyanoacrylate, isopropyl 2--cyanoacrylate, and isobutyl 2--cyanoacrylate which were much superior to methyl 2--cyanoacrylate. Common carotid arteries of rats of 1.0 to 1.3 mm external diameter range were reconstructed and re-exploration was carried out at intervals of more than 7 days after operation. The anastomotic technique was very easy and it took about five minutes to reconstruct. In our last series, approximately 98 percent patency rate was achieved. The advantage of our method is that the blood stream is regained in the small soluble at the anastomotic site immediately after the release of hemostatic clamps.
