New technique for knife and radial keratotomy.

BACKGROUND: Radial keratotomy is the most popular method of surgical correction of myopia, but requires further improvements. This study presents a new knife design for radial keratotomy. METHODS: Radial keratotomy was performed on 120 eyes of 60 patients with myopia. We utilized two diamond knife designs. The new knife allows a surgeon to perform corneal incisions of varying programmed depth with one movement of the hand, deeper at the limbus. As a control, we used the standard Fyodorov knife. One eye was operated on with one type of knife, the other eye with the other type. The number of incisions was the same with both techniques. RESULTS: After a 2-year follow up of the group of eyes operated on with the new knife, the mean keratometric power changed from 43.20 diopters (D) to 39.30 D, and the mean amount of myopia from -4.37 to -0.32 D. In the group of eyes operated on with the standard knife, the mean keratometric power changed from 43.20 to 39.50 D, and the mean amount of myopia from -4.36 to -0.40 D. CONCLUSIONS: The final refractive effect of the new knife was equal to that of standard knives. However, the new knife was more convenient: duration of surgery was shorter, and there was no need to change the length of the blade to deepen the incisions during surgery.

[Laser-Doppler velocimetry on the optic nerve head and hemorheology. Animal experiments. First results in humans].

We study capillary microcirculation on the optic nerve head and its interrelation with blood rheological modifications. The mean maximal velocity has been evaluated in the capillaries of the rabbit optic nerve with a laser doppler velocimeter, in physiological conditions and after intravenous Dextrane 250 injections. Dextrane 250 increases red blood cells aggregation and induces blood hyperviscosity at low flow rates. This hyperaggregation is determined with a reflectometric method. Experimental results show a significant reduction of blood velocity when blood viscosity increases. In our present study, we compare blood capillaries flow rate in the optic nerve, between two groups: healthy humans and subjects suffering from glaucoma. The aggregation level is simultaneously measured for the two groups. First results seem to prove that blood flow rate decrease is associated with red blood cells aggregation increase for the second group.

[Ethambutol and the optic nerve].

Author describe two clinical observations of optic neuropathy due to the systemic absorption of antibiotics used against tuberculosis. Two items are pointed out: swelling of the optic disc and functional improvement of electro-physiologic and colorimetric tests 6 months after the end of the treatment. He think that optic disc swelling is the clinical manifestation of an anatomical and volumetrical increase of the neuronal axon due to blockade of the axoplasmic transport without demyelinating lesions. This could explain the functional improvement.

[Difficulties in the surgical treatment of hyperthyroid myopathy].

Surgery can help patients affected with Graves' myositis. This surgery is very delicate because it is performed on muscles modified with retractile sclerosis transforming muscular fibers into fibrous and rigid strings. The surgical treatment must be decided with endocrinologists. Many surgical stages are usually necessary. Reunion procedures must be chosen, while unpredictable muscular resections must be banished.
