[Statistical study of epulis, especially in general pathology].

This report contains a statistical study of 874 cases of epulis diagnosed by the Department of Pathology of Tokyo Dental College from 1966 to 1986. 1. Of the 874 cases, 344 were epulis fibrosa, 217 were epulis granulomatosa, 78 were epulis fibromatosa, 74 were epulis fibrosaosteoplastica, 51 were epulis hemangiomatosa, 43 were epulis fibrosa teleangiectaticum, 15 were epulis cementoplastica, 14 were epulis osteomatosa, 3 were congenital epulis, 2 were giant cell epulis, and 1 was epulis cementomatosa. 2. As has been reported in other literature, there is a marked tendency for this condition to occur in females (331 male cases and 539 female cases). 3. Our data indicate a higher occurrence rate in people in their fifties, although the occurrence rates were similar in people in their twenties and in people in their sixties. 4. The epulis was observed most frequently in the maxillary incisor region.

Sensitive serum markers for detecting pancreatic cancer.

Serum amylase, immunoreactive elastase (IRE), carcinoembryonic antigen (CEA), and carbohydrate antigen (CA 19-9) were measured in 40 patients with pancreatic cancer. IRE and CA 19-9 were further assayed in nonpancreatic malignancies (n = 98) and chronic nonmalignant disease (n = 194). In pancreatic cancer, elevated values were observed for amylase in 30% of the patients, for IRE in 70%, for CEA in 28%, and for CA 19-9 in 68%. Elevation of IRE and/or CA 19-9 was found in 95% of the 40 patients. Elevated serum IRE was observed more frequently in head cancer and resectable cancer, whereas elevation in CA 19-9 occurred more often in body-tail cancer and unresectable cancer. Elevation of serum IRE tends to occur at earlier stages of pancreatic cancer. No one test is adequate for the accurate diagnosis of pancreatic cancer. However, the two assays complement each other, and their combined use would provide a sensitive clue for tentative diagnosis of pancreatic cancer.

Chronic pancreatitis at early age of onset presenting interesting findings through endoscopic retrograde pancreatography and chemical analysis of nonopaque pancreatic concretion.

This case concerns a 20-year-old male patient with an approximate 10-year history of recurrent and severe abdominal pain radiating to the back. Endoscopic retrograde cholangiopancreatography revealed a short obstructing stenosis of the main pancreatic duct in the head of the pancreas, marked and tortuous dilatation of the prestenotic portion of the main pancreatic duct and its side branches, and a filling defect in the side branch in the body of the gland. Pancreaticojejunostomy was performed to induce decompression of the pancreatic duct. Histology of the pancreas showed advanced chronic pancreatitis. Three nonopaque concretions were obtained at operation. The largest one, which was milky white in appearance and elastic and soft in consistency, proved to be made up of protein. The concretion was rich in acidic amino acids, but poor in basic or aromatic residues. The molar composition of amino acids in the concretion was, in decreasing order, aspartic acid, serine, valine, glycine, and glutamic acid. Powder x-ray diffractometry revealed no crystalline structures.

Serum immunoreactive elastase in diagnosis of pancreatic diseases. A sensitive marker for pancreatic cancer.

Diagnostic significance of serum immunoreactive elastase-1 was studied in 137 patients with pancreatic disease, 335 with various nonpancreatic diseases, and 416 healthy controls by using radioimmunoassay. Frequency of abnormally high serum elastase values exceeding 410 ng/dl was 100% in acute pancreatitis (N = 14), 40% in chronic pancreatitis (N = 80), and 72% in pancreatic cancer (N = 43). In pancreatic cancer the mean value of serum elastase in resectable cancer (N = 19) was significantly higher than that in unresectable cancer (N = 24). Sensitivity, specificity, and efficiency of serum elastase in pancreatic cancer were 72.1%, 98.3%, and 95.9% against healthy controls; 72.1%, 85.9%, and 83.6% against nonpancreatic digestive diseases; and 72.1%, 60.0%, and 64.2% against chronic pancreatitis at a cutoff level of 410 ng/dl, respectively. High serum elastase could be a diagnostic clue to detect pancreatic duct obstruction due to pancreatic cancer, although further examination should be done by endoscopic retrograde pancreatography and other imaging studies.

Effects of chronic sultopride treatment on endocrine systems in psychotic women.

The effects of chronic sultopride treatment on endocrine systems were studied using five schizophrenic women. Sultopride, an antipsychotic drug, was administered orally three times daily for 5 weeks in a daily dose of 300-600 mg. The serum prolactin levels increased significantly after 1 day of treatment, reaching a maximum at 1 week and remaining elevated during treatment. The serum GH levels declined temporarily after 1 week of treatment and then returned to normal values after 3-5 weeks of treatment. Sultopride had no significant effects on LH, FSH, TSH, insulin, estradiol-17 beta and cortisol basal levels. Serum sultopride levels measured by radioimmunoassay remained steady during treatment. These results showed that sultopride stimulates prolactin secretion in schizophrenic women, probably by blocking pituitary dopamine receptors.

The excretion of dimethadione in pure pancreatic juice and bile in postoperative patients.

The excretion of dimethadione (DMO) in pure pancreatic juice and bile was studied in postoperative patients undergoing external drainage of pancreatic juice and in those who had undergone percutaneous transhepatic cholangiodrainage. During and after oral administration of trimethadione, which is the precursor of DMO, pancreatic DMO concentration closely paralleled plasma DMO concentration. Pancreatic juice/plasma concentration ratio for DMO exceeded 1.0. In a patient with a large pancreatic cyst communicating with the main pancreatic duct, pancreatic DMO concentration after single rapid injection of secretin inversely correlated with the flow rate, while its output depended directly on the rate. Biliary DMO output after secretin injection was closely dependent on the flow rate, but was extremely low. The present results are essentially compatible with the findings previously observed in dogs.

Effects of contact lens wear on mitosis of corneal epithelium and lactate content in aqueous humor of rabbit.

Various types of contact lenses, i.e., poly-methyl-methacrylate hard lenses, poly-2-hydroxyethyl-methacrylate soft lenses and two kinds of gas-permeable hard lenses with different permeabilities to oxygen, were placed on one eye of the albino rabbit for varying lengths of time. Using the fellow eye as the control, studies were carried out of the effects of the lens wear on the mitosis in the corneal epithelium and the lactate concentration in the aqueous humor. The contact lens wear suppressed the mitosis and increased the concentration of aqueous lactate, the degree of changes being greater as the permeability of the lens to oxygen decreased and the period of wear became longer.

[Preparation of highly purified cephamycin C].

Preparation of highly purified cephamycin C (CM-C) is described in this report. The purification of CM-C was carried out by Sephadex LH-20 chromatography of the N-BOC-CM-C developed with MeOH followed by removal of the protecting group. Partially purified CM-C was further refined by Sephadex LH-20 chromatography developed with MeOH and Diaion HP-20 SS chromatography developed with H2O. CM-C thus obtained showed the UV absorption at 265 nm [E1% 1cm 184.4 (H2O)] which is the strongest absorption ever appeared in the literatures.
