Some aspects of prophylactic oophorectomy and ovarian carcinoma.

The early diagnosis of ovarian carcinoma is as difficult and rare as ever before. Despite the appearance of sophisticated drugs and combinations of these with surgery and radiotherapy the prognosis still remains the same as during the last 30 years, i.e. overall survival between 20 to 30%. This study concerns the impact of prophylactic oophorectomy in Sweden. 4.6% of ovarian carcinoma patients had received pelvic surgery at some time prior to the diagnosis of ovarian cancer and after the age of 40. With the help of statistical data it is estimated that about one hundred out of the approximately thousand women per year who now develop ovarian cancer in Sweden could be saved if prophylactic oophorectomy were practised in our country. Various aspects of prophylactic oophorectomy and its effects are discussed. It is our opinion that every gynaecologist/surgeon should seriously take into consideration and discuss the possibility of prophylactic oophorectomy in the perimenopausal patient before operation. Not to perform prophylactic oophorectomy during pelvic surgery after the menopause seems to us to be an unreasonable practice.

Adriamycin-methotrexate combination chemotherapy of advanced carcinoma of the cervix: a third look.

Twenty-four consecutive women with recurrence and/or metastasizing carcinoma of the cervix were treated with adriamycin and methotrexate. All patients had previously been treated with external irradiation and 2 intracavitary radium applications. The median recurrence-free interval was 22 months. All patients were evaluable. Five patients (21%) had partial remission for a median duration of 5 months (2 to 9 months), and 7 patients had static disease. When used in dosages that do not precipitate dangerous toxicity this therapy is not effective and in the authors' opinion should be abandoned.

Histological grading and treatment failures in carcinoma of the uterine body.

In a material of 777 patients with carcinoma of the uterine body, Stages I and II, 115 (15%) had poorly differentiated tumours, and 662 (85%) moderately-to-well differentiated tumours. The five-year tumour death rate was 34% and 12%, respectively. No differences in local recurrences and metastases in the pelvis were recorded in the two tumour groups. On the other hand, distant metastases occurred in 23% of patients with poorly differentiated tumours, and only in 5% among patients with moderately-to-well differentiated tumours. Whether chemotherapy integrated in the treatment-scheme would reduce the frequency of distant metastases and improve the survival rate remains an open question.

Phase II study of Leo 1031 (prednimustine) in advanced ovarian carcinoma.

Thirty-six patients with advanced ovarian carcinoma, 21 who had not responded to conventional surgery and irradiation therapy and 15 who had not responded to conventional surgery, irradiation therapy, and chemotherapy, were treated in a phase II study with Leo 1031. Objective responses were obtained in ten (28%) of the 36 patients (two complete and eight partial responses). An additional 11 (32%) patients had stable disease. The two complete responders were in remission for 4 months. During remission, all of the patients had an improved quality of life. Myelosuppression of varying severity occurred in 11 patients; three of these patients died from their complications. Two patients developed reversible confusion and one patient became psychotic. Eight patients became euphoric during treatment. Leo 1031 might be worthwhile as a palliative chemotherapy for patients with advanced ovarian carcinoma, but the risk of euphoria must always be considered since it can lead to voluntary overdosage.

Vaginal smear history in patients with invasive cervical carcinoma.

In 23 (about 43%) patients cytologically screended within 4 years before the diagnosis invasive cervical cancer 11 had a benign smear; when reviewed, two were borderline cases and three were misinterpreted. Twelve patients had different degrees of squamous cell dysplasia. In three of these the cancer was detected during the follow up procedures. Four did not respond to calls for follow up. Five were checked without any active measure but sampling new smears. Atypical smears must be clinically evaluated and treated in order to detect invasive carcinomas. The therapeutic results, however, may be controlled by repeat smears.

Levels of free and protein-bound progesterone in plasma during late pregnancy.

Using a filter method, levels of free (unbound) and protein-bound progesterone in blood samples were measured serially from the 23rd week of pregnancy until delivery in 16 healthy women. In addition, total estrogen and progesterone concentrations were determined by radioimmunoassay. The levels of both free and total progesterone increased steadily until the end of pregnancy. Free progesterone increased with advancing pregnancy more in proportion to the total progesterone, as it rose from 6% (of total) at week 24 to 13% at week 40. Both free and total progesterone decreased markedly very soon (2 h) after delivery. However, the corresponding decrease in the level of free progesterone was considerably less, with the result that it rose to 19% of the total, the greatest proportion of free progesterone observed at any time. The levels of both total estrogen and progesterone in plasma increased with advancing pregnancy, and at no time, prior to delivery, was there a significant fall in progesterone or an abrupt rise in estrogen.

The effect of a copper IUD and various "inert" Iuds on the histological picture of the rabbit endometrium during early pseudopregnancy and pregnancy.

The influence of a copper IUD and two "inert" IUDs on the histological picture of the rabbit endometrium during early pseudopregnancy and early pregnancy was studied. The copper IUD markedly inhibited the endometrial maturation during the first five days of both pseudopregnancy and pregnancy. The "inert" IUDs had no such effect. The copper IUD prevented the normal response of the rabbit uterus to 17-beta-estradiol in castrated animals. The observed effects of the copper IUD might seriously interfere with the normal process of implantation.

Effect of a copper and a plastic IUD on the histochemistry of endometrial enzymes in the rabbit during early pseudopregnancy.

The influence of a copper and a plastic IUD on the localization and intensity of alkaline and acid phosphatase and beta-glucuronidase activity in the rabbit endometrium during early pseudopregnancy was investigated. The activities of the two lysosomal enzymes, acid phosphatase and beta-glucuronidase were increased in surface epithelial cells in the presence of both types of IUDs. The alkaline phosphatase activity was almost completely abolished in surface and glandular epithelial cells in uterine horns.

Effect of copper and plastic intrauterine devices on the norepinephrine content of rabbit oviduct and uterus.

An intrauterine device (IUD) consisting of either a plain or a copper-coated polyethylene catheter was inserted in one of the uteri of rabbits. Sham-operated uteri served as controls. The norepinephrine (NE) content was compared only between uteri within the same animal. Both copper and plastic IUDs markedly reduced the NE content in the uterus, but not in the oviduct, within 1 week after insertion. NE in both organs was present exclusively in the adrenergic nerve system. There was no difference in the action of the two types of IUDs. The effect of an IUD was probably eliminated by interruption of the preganglionic nerve connections. The results showed that the uterine adrenergic neurons are influenced by the presence of an IUD.

Effect of a copper IUD on the incorporation of (3H)thymidine and (5-3H)uridine into the endometrium of the rabbit during early pregnancy.

The influence of a copper IUD on the incorporation of [3H]thymidine and [5-3H]uridine into the endometrium of the rabbit was studied during early pregnancy. The effect of the copper device was compared with those of polyethylene and platinum IUDs. The comparison showed that the presence of a copper IUD resulted in a significantly lower incorporation of [3H]thymidine on the 3rd day of pregnancy and in a higher incorporation of [3H]thymidine and [5-3H]uridine on the 5th day of pregnancy. The increase in the total amount of DNA in the endometrium in early pregnancy was markedly reduced in the copper IUD-containing horn, and autoradiography showed that the labelling frequency of epithelial cells in the endometrium was significantly reduced. A possible effect of the copper IUD on the stromal cells is discussed. These effects of copper could be due either to an altered hormonal responsiveness of the endometrium in the presence of copper or to a 'direct' effect of copper on nucleic acid metabolism.
