Failure in placement of trocar-mounted catheters.

Some commercially available catheters have a trocar with a small knob at the tip. If the catheter is pushed retrograde on the trocar such that the tip lies behind the knob, it may be impossible to advance it again. To avoid this problem, we suggest dilating the track to two French sizes larger than the catheter, using a larger stylet, or using a more rigid catheter.

Crohn's disease involving the proximal stomach.

We report an unusual case of Crohn's disease of the esophagus and stomach. Unlike previously described cases, the disease in this patient affected the fundus and body while sparing the antrum of the stomach, and produced a mass effect simulating malignancy.

Percutaneous abscess drainage.

We have drained 50 abscesses in 40 patients. The success rate was 100% for entering the abscess cavities and 98% for establishing catheter drainage. The success rate for treating the abscess (i.e., no surgery required) was 88%. We drained all abscesses for which a safe access route was available, regardless of the abscess's characteristics. Abscesses which are not unilocular may be successfully treated by percutaneous drainage. In critically ill patients and those unsuitable for surgery, catheter drainage is useful until the patient becomes stable. When surgery is not feasible, percutaneous procedures may be the patient's only hope for survival.

The uses of corpus cavernosography. A review.

In a series of 18 corpus cavernosograms (CC) we encountered 12 patients with Peyronie's disease, four normals, one with hypoplastic corpora and one with penile fracture. All examinations were successful and there was one complication, a case of post-procedural priapism. CC is a valuable procedure in the evaluation of impotence.

Ilio-psoas abscess in Crohn's disease.

We encountered 12 patients with psoas abscess in a two-year period. Of these, four were due to Crohn's disease making this the most important single cause of such abscesses in our series. Psoas abscess may be the presenting complaint in Crohn's disease and it should be considered as a possible cause of any psoas abscess from which enteric organisms are cultured.

[Renal angiomyolipoma in computer tomography].

Renal angiomyolipoma ia a rare benign lesion. It commonly presents with features suggestive of renal cell carcinoma when unilateral and solitary. It may mimic polycystic kidneys when bilateral and multiple. Angiomyolipomas with a high fat content are easily detected by computed tomography. Preoperative diagnosis of the lesion is possible and may help to obviate nephrectomy.

Postoperative residual ovary syndrome: an uncommon cause of pelvic mass.

We report two patients with an ovarian mass following hysterectomy. These represent part of the spectrum of the "residual ovary syndrome." The retroperitoneal incarceration of a residual ovary and the presence of adhesions, preventing free access to the peritoneal cavity, result in an accumulation of follicular products locally and, thus, a mass with cystic and solid components. Differential diagnostic considerations include functional and dysfunctional ovaries, benign and malignant ovarian tumors, and endometriosis.
