Perceptions of ESRD patients' satisfaction with, and abilities in, 13 life-areas were assessed in 50 patients and their spouses by means of a series of linear rating scales. Patient and spouse views were congruent on 10 dimensions. Noncongruence existed only on perception of the patient's satisfaction with his/her medical situation, perception of the patient's ability to care for self, and perception of the patient's satisfaction with feelings about self. This noncongruence was especially pronounced in certain patient subgroups, and explanations related to the nature of ESRD and its treatment are suggested. We recommend that future studies examine not only perceptions of patients' satisfaction but also perceptions of the spouse's own satisfaction with various life-areas.

The value of infusion cholecystography 99mTc HIDA cholescintigraphy and ultrasonography was compared in 51 patients presenting with a clinical diagnosis of acute cholecystitis. Of the 35 patients with proven gallbladder disease, the presence of gallstones was correctly predicted in 31 (88 per cent) by infusion cholecystography, 32 (90 per cent) by 99mTc HIDA cholescintigraphy and 27 (77 per cent) by ultrasonography. There were no false positive investigations. Infusion cholecystography may be of particular interest to surgeons with no ready access to isotope scanning techniques.
An open, multicenter study was conducted to evaluate the efficacy and adverse effects of ketoconazole in patients with cutaneous fungal infections seen in typical dermatology practices. One hundred twelve American investigators entered 954 patients into this trial. Responses based on clinical and mycologic assessments were available on 672 patients. Mycologic evaluations were conducted using pre- and post-therapy wet mounts in potassium hydroxide. Eighty percent of these patients were mycologically clear and clinically rated as either excellent (cured) or marked improvement. Adverse effects were mild to moderate in severity and only rarely caused therapy to be interrupted. Gastrointestinal effects were the most frequently reported complaints.
There has been little research on social environments created by spatial arrangements in health care and how these social environments may affect the nature of health care. This article describes changes in the social environment of a renal dialysis clinic that were created by an alteration in the spatial arrangements. It was found that changes occurred in the communication between staff and patients, the communication among patients, and the communication between patients and visitors. These changes in turn raised questions about the quality of care available to patients.
Nine patients with myeloma were studied over 13 oral administrations of 10 mg melphalan and 5-10 mg prednisolone. Plasma melphalan concentrations were estimated by high-pressure liquid chromatography, prednisolone concentrations by quantitative thin-layer chromatography. The mean plasma half-life of unchanged melphalan was 0.9 +/- 0.5 (SD) h. The 'lag-time' before melphalan was detected in the plasma varied from 1 to 4 h, the mean peak concentration was 96 +/- 21 ng/ml, and the mean area under the plasma concentration by time curve was 160 +/- 78 ng h/ml. This variability was consistent with observations made elsewhere following much higher oral doses of melphalan and illustrates the relatively wide interindividual variability of absorption. Observations made in the same subjects on two separate occasions showed lower variability. The melphalan elimination rate was not significantly affected by moderate impairment of creatinine clearance (to 31 ml/min). Absorption of prednisolone in five of these patients was apparently normal and unaffected by concurrent administration of melphalan.
Triglycerides (TG), total cholesterol (TC) and HDL-cholesterol (HDL-C) were determined in 48 long-distance runners (group I) and in 34 sedentary males of similar age-range and weight serving as controls (group II). Blood samples were taken before and after racing in runners and at rest in controls, but comparisons were made between baseline values only. Taken globally, group I subjects had significantly higher HDL-C levels (p less than 0.01) and lower TC/HDL-C ratios (p less than 0.05) than group II subjects. However, considerable changes were found within and between the groups when subjects were divided according to age. Thus, there was no significant difference in blood lipids between long-distance runners and controls in the 20-34 years subgroups, whereas in the 35-55 years subgroups HDL-C values tended to decrease in controls but remained high in long-distance runners, the difference being highly significant (p less than 0.001). Consequently, there also was a significant difference (P less than 0.01) between group 1 and group II subjects in TC: HDL-C ratios. The increase in TG with age was much greater in controls than in runners. It is suggested that high HDL-C levels might be one of the mechanisms though which intensive physical training exerts a protective effect against coronary disease.
Fetal lung maturity has been studied both by silica gel chromatography of phospholipids and gas liquid chromatography of fatty acids. Lecithin/sphingomyelin (L/S) ratios and palmitic/stearic (P/S) ratios were compared. A good correlation was obtained. However, when the L/S ratio has an intermediate value (1.5 less than or equal to L/S less than 2) making it difficult to judge the fetal lung maturity, the P/S ratio expresses the lung maturity more accurately.

A new case of analbuminaemia is described in a 6 month old child of algerian origin. The condition was discovered fortuitously, the clinical manifestations consisting only of slight oedema. Serum albumin concentration was 64 mg/l and its immunochemical behavior identical to that of normal albumin. The body react by an increase in the synthesis of globulins. In the propositus, levels of alpha, antitrypsin, caeruloplasmin, haptoglobin, jalpha2 macroglobulin, transferrin, immunoglobulin M were more than 3 times normal. Analysis of non-esterified fatty acids normally carried by albumin was normal. By contrast, it was shown that the presence of free bilirubin not bound to proteins was detected from 17 micronmol/l. Study of the family showed a normal distribution of albumin and globulins. The genetic origin seen in previously reported cases was confirmed by the co-sanguinity of the parents.
