[Non-radiologic measures for the diagnosis of ovarian carcinomas].

The ovarian carcinoma is still a problematic carcinoma. There are no early symptoms and no early diagnostics. Those carcinomas which are detected in an initial stage are generally found by chance during an operation indicated otherwise. Gynecologic palpation is the most important among the non-radiologic diagnostic means. Every ovarian tumor with a size of more than 5 cm is suspicious to be a carcinoma and has to be further investigated. Age is the most important risk factor. After the age of 35, the incidence of the ovarian carcinoma increases continuously until the age of 70. A reliable confirmation of the diagnosis "ovarian carcinoma" is only possible by laparoscopy and/or laparotomy. Laparotomy should be planned in such a manner that therapeutic surgery can be immediately performed if the diagnosis is confirmed.

[Prevention of deep-vein thrombosis in patients with gynaecological cancer undergoing radiotherapy. A comparison of calcium heparin and a semisynthetic heparin analogue (author's transl)].

Patients treated for gynaecological cancer by combined external irradiation and intravaginal radium application participated in two randomized, controlled, and prospective trials. In the first trial the incidence of deep vein thrombosis (DVT) diagnosed by the 125I-fibrinogen test was 43% in the control group. The injection of 7,500 i.u. of subcutaneous calcium heparin twice daily prevented DVT in the test group. An increase of soluble fibrin monomer complexes (SFMC) and fibrinogen in the plasma of patients with DVT suggested that the patients were hypercoagulable. No correlation between AT III values and DVT was observed. In the second trial, the effect of the same dose of heparin was compared with a twice daily s.c. injection of 5,000 U of a semi-synthetic heparin analogue (SSHA). The incidence of DVT was reduced to 15% by heparin; in the patients given SSHA the incidence was 25%, which was not significantly different from the heparin-treated group.

[A comparative study of fibrinogenolysis of adult and cord vein fibrinogen (author's transl)].

The streptokinase induced fibrinogenolysis of cord vein fibrinogen follows a slower degradation rate than adult fibrinogen prepared from plasma in the same manner. However, no difference in the degradation rate could be observed when plasmin was used. It is suggested that the lower plasminogen content in the preparation of the cord vein fibrinogen is responsible for the slower degradation rate observed after streptokinase induction.

Evaluation of hypercoagulability after remission induction chemotherapy of acute leukemia.

In 20 patients with acute leukemia the thrombin-mediated fibrin monomer complexes (SFMC) were determined before and after remission induction chemotherapy for several weeks to assess the extent of hypercoagulability. Increased levels of SFMC were observed in patients with high leukocyte cell counts after induction chemotherapy which may reflect the release of thromboplastic material from destructed leukemic cells. Patients with low leukocyte counts showed only a moderate increase in SFMC. A low concentration of antithrombin III (< 20 mg/100 ml) or a drop from a previously normal value was a prognostic unfavorable sign. An elevated level of plasmin-mediated fibrin-split-products could not be related to the course of therapy.

[Disseminated intravascular coagulation (DIC) after endotoxin infusion into the common bile duct of rabbits (author's transl)].

Endotoxin from E. coli was infused into the distally ligated common bile duct of rabbits under the static pressure of 25 cm H2O. Fibrinogen, soluble fibrin monomer complexes, antithrombin III, leukocyte and platelet counts were estimated before, and 2, 4, and 6 h after endotoxin infusion. All parameters were found significantly changed 2 h after endotoxin infusion. While fibrinogen level, AT III, leukocyte and platelet counts decreased after the endotoxin infusion the amount of SFMC increased. The change of hematological parameters showed a pattern characteristic of disseminated intravascular coagulation (DIC). In accordance with this microclots in the glomeruli of the kidneys could be demonstrated in all endotoxin-treated animals by pathological study. The findings suggest that by endotoxin infusion into the common bile duct, as a focal origin, DIC can be produced.

The effect of heparinized blood exchange transfusion on endotoxin induced disseminated intravascular coagulation (DIC).

Sepsis of the newborn induced by gram negative bacteria, especially E. coli is often accompanied by a severe coagulation disorder. It can be treated by blood exchange transfusion (ET) with heparinized blood. In this study the hematological effect obtained by the exchange transfusion was investigated in rabbits after induction of a generalized Shwartzman reaction by two spaced injections of endotoxin (75 microgram/kg) 24 hrs. apart. Three groups of 6 animals each were investigated: group I: without endotoxin but with ET (controls); group II: endotoxin without ET; group III: endotoxin with ET. Fibrinogen, soluble fibrin monomer complexes (SFMC), fibrin(ogen) degradation products (FDP), platelet- and leukocyte counts and urine volume (ml/hr) were estimated. In group II a decline in the fibrinogen level, and in platelet and leukocyte count, as well as an increase in SFMC and FDP could be observed from 6 hrs. on after the second endotoxin injection. In group III 6 hrs. after the second endotoxin injection, exchange transfusion with heparinized blood was performed. Variance analysis showed significant differences in all parameters, except in the urine volumes after exchange transfusion between group III and group II. By exchange transfusion an approach of the values towards the values of the controls could be recognized. The findings indicate, that by blood exchange transfusion the hematological consequences of the endotoxin induced DIC can be corrected, while the dysfunction of the kidneys can be improved only slightly.
