Cytogenetic diagnosis of malignant pleural effusions: culture methods to supplement direct preparations in diagnosis.

Pleural effusions from 100 unselected patients were studied cytogenetically by direct preparation, 24- and 48-hour cultures, and long-term cultures. The study included benign pleural effusion in 17 patients, malignant pleural effusion in 76, and an indeterminate cause in 7. Two cytogenetic criteria for detecting a malignant lesion were tested: (1) three or more metaphases that were either hyperdiploid or had markers and (2) two or more metaphases with the same markers (clone). Among the malignant pleural effusions, 51 (67%) were positive by at least one method when the first criterion was used and 34 (45%) were positive by at least one method when the second criterion was used. By the first criterion, at least one of the culture methods was positive in 13 cases (17%) in which direct preparations were negative; there was 7 such cases (9%) by the second criterion. Apparently, culture methods alone are not better than direct preparations but are useful in supplementing direct preparations. There was one false-positive long-term culture by the first criterion. Thus, the clone criterion of malignancy may be more accurate but is less sensitive. There was no evidence that therapy had any effect on the cytogenetic studies.

Is your enema really necessary?

Two hundred and seventy-four women admitted for delivery of singleton infants were studied for the effects of a preparatory enema on faecal contamination, duration of labour, and the incidence of infection in the newborn. Altogether 149 of the women were given an enema (controls) and 125 were not. The two groups showed no significant difference in the degree of faecal contamination during the first and second stages of labour, and the incidences of gross contamination were similar. Contamination after an enema was especially difficult to control, since it was more likely to be fluid. Seven neonates in each group showed evidence of infection, bowel organisms being isolated from four in the no-enema group and two in the control group. Durations of labour, though not strictly comparable, were similar in the two groups. The findings suggest that when preparing for normal labour the enema should be reserved for women who have not had their bowels open in the past 24 hours and have an obviously loaded rectum on initial pelvic examination.

Hypertrophic obstructive cardiomyopathy and lentiginosis: a little known neural ectodermal syndrome.

Eleven patients, 10 male, with classic hypertrophic obstructive cardiomyopathy and lentiginosis are described. Physical examination showed differences from the few previously reported cases in that (1) this condition was not confined to children; (2) mental retardation, sensorineural deafness and gonadal and somatic infantilism were either rare or absent; and (3) detailed family studies provided no evidence that this condition was inherited. Nine patients underwent cardiac catheterization and left ventricular angiography; all had left ventricular outflow obstruction and three had concomitant right ventricular outflow obstruction with a pressure gradient in excess of 100 mm Hg. Ten of the 11 patients were severely symptomatic, and 7, each with a left ventricular pressure gradient of more than 70 mm Hg, underwent successful septal myotomy/myectomy that resulted in marked symptomatic improvement that was maintained after long-term follow-up.

Ovarian cancer: the ten year experience of a district general hospital.

Ninety-seven cases of ovarian cancer were diagnosed and treated at Northwick Park Hospital between 1970 and 1980. In this retrospective study data are presented about symptomatology and certain aetiological factors. The survival with different treatment regimens is compared, with particular reference to the improved figures resulting from a policy of aggressive surgery followed by multiple chemotherapy.

Coronary artery disease in twins.

In two pairs of monozygotic twins with coronary artery disease who were studied by coronary angiography, there were striking similarities in the clinical course, response to exercise, and distribution of coronary artery disease. Though it is difficult to distinguish between heredity as an independent risk factor and the other major risk factors, the similarities in each twin pair may be best explained by identical heredity.

Fabry cardiomyopathy in the female confirmed by endomyocardial biopsy.

A 58-year-old woman with cardiac enlargement and heart failure was a member of a family with Fabry disease. In this patient, endomyocardial biopsy permitted microscopic examination of the myocardium which revealed cellular abnormalities characteristic of Fabry cardiomyopathy. It is suggested that unfavorable lyonization of the mutant-bearing X chromosome accounted for the expression of this disorder in specific organs of a female patient.

Familial pheochromocytoma and islet cell tumor of the pancreas.

The occurrence of pheochromocytoma(s) or pancreatic islet cell tumor(s), or both, in two or more members of three unrelated families in a manner consistent with autosomal dominant inheritance suggests that this tumor association is a genetically determined syndrome. Among 11 affected patients (aged five to 53 years), 10 had pheochromocytoma (bilateral in six), four had islet cell tumor (multicentric in one), and three had both tumors. Clinical presentation was due to pheochromocytoma in 10 cases (symptoms or signs commencing before age 10 years in three patients) and islet cell carcinoma in one case. Four patients are dead as a result of the tumors--two from pheochromocytoma and two from islet cell carcinoma.

The causes of low oestrogen excretion in pregnancy: the development of diagnostic methods for the antenatal detection of familial congenital adrenocortical hypoplasia.

A pregnancy producing a boy with congeital adrenocortical hypoplasia is described. Consistently low oestrogen excretion, less than 10 umol/24 h, was not associated with any anatomical abnormality or diminished growth of the fetus as judged by ultrasound examination. Fetoplacental steroid sulphatase definciency was excluded by finding normal maternal excretion of oestrogen precursors, the 3Beta-hydroxy-5-ene steroid sulphates. Generalized adrenocortical hypoplasia was proven in the baby boy by analysis of steroid netabolites excreted in his urine. He suffered from salt loss and progressive jaundice. Both were responsive to gluco- and mineralo-corticoid replacement therapy. It is suggested that amniocentesis may be required for the antenatal diagnosis of congenital adrenocortical hypoplasia.

An assessment of the Mi-Mark endoemtrial sampling technique.

One hundred and twenty cytological smears and 106 histological specimens were obtained from 115 patients using a new technique of endometrial sampling. It proved an atraumatic procedure and was well accepted by the patients. Some problems were encountered in the preparation of satisfactory cytological specimens and in their interpretation. The method was not completely reliable for detecting endometrial pathology and is therefore considered unsuitable for monitoring patients on hormone replacement therapy. It was found to be useful as a gynaecological outpatient technique for sampling the endometrium when formal curettage was unsuccessful, in avoiding the necessity for a preoperative curettage to confirm suspected carcinoma, and in the investigation of infertility.

Surgical day care: a review of 5,427 cases.

A surgical day care unit was established at the Kaplan Hospital, Rehovot, in 1975. This study includes 5,427 patients who underwent ear, nose and throat, gynecologic, eye and plastic surgery as well as other surgical procedures in the unit. The organization and staffing of the unit are also described. The feasibility of performing minor to intermediate surgical procedures requiring general anesthesia on a day basis is demonstrated. The experience gained shows that high medical standards are achieved with low resource expenditure. The cost per patient is significantly reduced. Absenteeism from school and word due to disability during long waiting times and hospitalization is decreased. There is less demand for hospital beds and the risk of cross infection is reduced. The psychological trauma of hospitalization, particularly in children, is reduced, and the waiting period for surgical intervention is much shorter.
