212 patients with calcareous tendinitis of the shoulder were retrospectively analysed with a pain questionnaire after treatment with needle irrigation. Age, sex, number of needle irrigations and the time interval between treatment and interview were recorded from a file-card, also the side of affection. 149 questionnaires could be analysed by relational data bank processing on a personal computer. The median pain duration previous to the irrigation was 24 months; the median time interval between chalk deposition in x-ray studies of the shoulders and irrigation was 6 months. The sex ratio was 2:1 with preference given to females. The mean age at intervention was 47 years among the females and 49 years among the males; this age difference was not significant. There was no relationship between the preferred hand for working and the side of subacromial chalk deposition. We found in 54.5% painful calcareous depositions in the right shoulder and in 33.5% in the left. Both sides were affected in 12%. The median interval between needle irrigation and questionnaire analysis was 5 years. At this time 60% of all patients were free of pain, 34% stated a marked relieve of pain and 6% were unchanged.
Five out of eight consecutive cases with initial symptoms of a 'midline granuloma' were identified as malignant histiocytosis (histiocytic sarcoma) which within 5 months to 4 years led to generalization and death. The three remaining cases also fulfilled the morphological criteria of this type of neoplasia, though these patients are still alive 1/2 to 8 years after diagnosis, possibly as a result of local radiotherapy. The age of the individuals ranged from 18 to 71 years and there was a male preponderance of 7:1. The histiocytic nature of the atypical cells was primarily documented by intense activity of NaF-inhibitable non-specific esterase, of acid phosphatase and of beta-glucuronidase as demonstrated in cryostat sections of formaldehyde-saccharose-fixed fresh biopsy specimens and by the detection of alpha-1-antichymotrypsin, alpha-1-antitrypsin, and lysozyme antigens, in that order of constancy (immunohistochemical examination of formaldehyde-fixed paraffin sections, using the avidin-biotin-peroxidase complex method). There was among the reported cases a considerable heterogeneity with regard to these 'markers'. We conclude that malignant histiocytosis is a (the?) major cause of the 'midline granuloma syndrome'.
An ELISA was used to measure soluble interleukin-2 receptor (sIL-2R) in the sera and synovial fluid (SF) of patients with rheumatoid arthritis (RA). Patients with seropositive, as well as seronegative RA had raised levels of sIL-2R in serum and SF compared to patients with osteoarthritis and sex-age matched healthy subjects. According to literature, sIL-2R levels in the sera of RA-patients may be a useful marker of disease activity. Increased sIL-2R levels in SF show new aspects in pathophysiology of RA.
Avascular necrosis of the hip was diagnosed early with magnetic resonance (MR) imaging in a 50-year-old man with pain and slightly diminished function of his right hip. Conservative treatment (non-weight bearing) of the hip led to complete recovery at one year follow-up. This case emphasizes the importance of MR imaging as first evaluation for avascular necrosis of the hip.
Rabies in Central Europe is maintained and spread by red foxes (Vulpes vulpes). The epidemiology is explained by a variety of parameters of the main host. Certain natural and artificial topographic features may function as barriers to the spread of the virus within fox populations. Taking into account epidemiological barriers greatly facilitated the strategic application of oral immunization of foxes for freeing Switzerland from rabies.
In the official method for rodent filth in corn meal, filth and corn meal are separated in organic solvents, and particles are identified by the presence of hair and a mucous coating. The solvents are toxic, poor separation yields low recoveries, and fecal characteristics are rarely present on all fragments, especially on small particles. The official AOAC alkaline phosphatase test for mammalian feces, 44.181-44.184, has therefore been adapted to determine the presence of mammalian feces in corn meal. The enzyme cleaves phosphate radicals from a test indicator/substrate, phenolphthalein diphosphate. As free phenolphthalein accumulates, a pink-to-red color develops in the gelled test agar medium. In a collaborative study conducted to compare the proposed method with the official method for corn meal, 44.049, the proposed method yielded 45.5% higher recoveries than the official method. Repeatability and reproducibility for the official method were roughly 1.8 times more variable than for the proposed method. The method has been adopted official first action.
