Contribution of the microvessel network to the clonal and kinetic profiles of adrenal cortical proliferative lesions.

Monoclonal adrenocortical lesions have been characterized by an inverse correlation between proliferation and apoptosis, and polyclonal lesions show a direct correlation. Their relationship with the vascular pattern remains unknown in adrenocortical nodular hyperplasias (ACNHs), adenomas (ACAs), and carcinomas (ACCs). We studied 20 ACNHs, 25 ACAs, and 10 ACCs (World Health Organization classification criteria) from 55 women. The analysis included X-chromosome inactivation assay (on microdissected samples), slide and flow cytometry, and in situ end labeling. Endothelial cells were stained with anti-CD31, and the blood vessel area and density were quantified by image analysis in the same areas. Appropriate tissue controls were run in every case. Regression analyses between kinetic and vascular features were performed in both polyclonal and monoclonal lesions. Polyclonal patterns were observed in 14 of 18 informative ACNHs and 3 of 22 informative ACAs, and monoclonal patterns were seen in 4 of 18 ACNHs, 19 of 22 ACAs, and 9 of 9 ACCs. A progressive increase in microvessel area was observed in the ACNH-ACA-ACC transition but was statistically significant between benign and malignant lesions only (191.36 +/- 168.32 v 958.07 +/- 1279.86 microm(2); P < .0001). In addition, case stratification by clonal pattern showed significant differences between polyclonal and monoclonal benign lesions; 6% of polyclonal and 57% of monoclonal lesions had microvessel area >186 microm(2) (P = .0000008). Monoclonal lesions showed parallel trends (but with opposite signs) for microvessel area and density in comparison with proliferation and apoptosis, whereas polyclonal lesions showed inverse trends. In conclusion, the kinetic advantage of monoclonal adrenal cortical lesions (increased proliferation, decreased apoptosis) is maintained by parallel increases in microvessel area and density.

Clonal patterns in phaeochromocytomas and MEN-2A adrenal medullary hyperplasias: histological and kinetic correlates.

The relationship among histological features, cell kinetics, and clonality has not been studied in adrenal medullary hyperplasias (AMHs) and phaeochromocytomas (PCCs). Thirty-four PCCs (23 sporadic and 11 MEN-2A (multiple endocrine neoplasia type 2A)-related tumours, the latter associated with AMH) from females were included in this study. Representative samples were histologically evaluated and microdissected to extract DNA and evaluate the methylation pattern of the androgen receptor alleles. At least two tissue samples (from the peripheral and internal zones in each tumour) were analysed with appropriate tissue controls run in every case. The same areas were selected for MIB-1 staining and in situ end labelling (ISEL). Malignant PCCs were defined by histologically confirmed distant metastases. All monoclonal AMH nodules from the same patient showed the same X-chromosome inactivated. Six sporadic PCCs revealed liver metastases (malignant PCC) and eight additional sporadic PCCs showed periadrenal infiltration (locally invasive PCC). All informative PCCs were monoclonal, except for five locally invasive PCCs and one benign PCC that revealed polyclonal patterns. Those cases also showed a fibroblastic stromal reaction with prominent blood vessels, focal smooth muscle differentiation, and significantly higher MIB-1 (126.8+/-29.9) and ISEL (50.9+/-12.8) indices. Concordant X-chromosome inactivation in nodules from a given patient suggests that MEN-2A AMH is a multifocal monoclonal condition. A subgroup of PCCs characterized by balanced methylation of androgen receptor alleles, high cellular turnover, and stromal proliferation also shows locally invasive features.

Clonality as expression of distinctive cell kinetics patterns in nodular hyperplasias and adenomas of the adrenal cortex.

Although histopathologic criteria for adrenal cortical nodular hyperplasias (ACNHs) and adenomas (ACAs) have been developed, their kinetics and clonality are virtually unknown. We studied 20 ACNHs and 25 ACAs (based on World Health Organization criteria) from 45 females. Representative samples were histologically evaluated, and the methylation pattern of the androgen receptor alleles was analyzed on microdissected samples. Consecutive sections were selected for slide cytometry, flow cytometry, and in situ end labeling (ISEL). Apoptosis was studied by flow cytometry (nuclear area/DNA content plotter analysis) and by ISEL. Appropriate tissue controls were run in every case. Polyclonal gel patterns were revealed in 14/18 informative ACNHs and in 3/22 informative ACAs, whereas monoclonal gel patterns were observed in 4/18 ACNHs and 19/22 ACAs. Overlapping proliferation rates (PRs) were observed in both clonal groups, and apoptosis was detected only in G(0)/G(1) cells, especially in monoclonal ACNHs (3/4; 75%) and in polyclonal ACAs (2/3; 67%). Significantly higher PRs were observed in ACNHs with polyclonal patterns and G(0)/G(1) apoptosis and in ACAs regardless of clonality pattern and presence of G(0)/G(1) apoptosis. All except one ACNH (19/20; 95%) and 15/25 ACAs (60%) showed diploid DNA content, whereas the remaining cases were hyperdiploid. A direct correlation between PR and ISEL was observed in polyclonal lesions (PR = 29.32 ISEL - 1.93), whereas the correlation was inverse for monoclonal lesions (PR = -9.13 ISEL + 21.57). We concluded that only simultaneous down-regulated apoptosis and high proliferation result in selective kinetic advantage, dominant clone expansion, and unbalanced methylation patterns of androgen receptor alleles in ACNHs and ACAs.

Prognostic factors in follicular carcinoma of the thyroid.

Although there are numerous publications on the nomenclature, morphological diagnostic criteria, and prognostic factors of follicular carcinoma of the thyroid, these issues remain controversial. We present the findings of a retrospective and comparative study of 82 patients who underwent thyroid surgery for thyroid neoplasms. Of these patients, 58 had follicular carcinoma, 12 had atypical adenoma, and the remaining 12 patients had adenoma with partial capsular invasion in the surgical specimen. The goal of the study was to determine diagnostic criteria and prognostic factors in follicular carcinoma. Our results showed that encapsulated thyroid follicular neoplasms should be considered malignant when they demonstrate total capsular invasion and/or vascular invasion. Statistical analysis of the cases of follicular carcinoma showed that higher morbidity rates were seen in the patients with metastasis at the time of diagnosis, with extensively invasive tumors, with age older than 35 years, and with tumors with simultaneous vascular and capsular invasion. The accumulated survival rate over 5 years based on mortality was 91%, and based on morbidity it was 71% (84% for patients with encapsulated tumors and 56% for patients with extensively invasive tumors). In the univariate survival study based on morbidity, we found a relationship with the following parameters: metastasis at the time of diagnosis, grade of local invasion, presence of capsular and vascular invasion, cellular atypia, histologic pattern (insular vs others), presence of thyroid nodule, and age of the patient at the time of diagnosis. In the multivariate survival study based on morbidity, the factors that showed prognostic value were metastasis at the time of diagnosis, grade of local invasion, and age of the patient at the time of diagnosis.

Immunocytochemical study of the ultimobranchial tubule in Wistar rats.

A systematic immunohistochemical study of the ultimobranchial tubule (UBT) has been carried out in 45 Wistar rats of different ages (0, 5, 10, 15, 20, 25, 30, 60 and 120 days). The existence of calcitonin immunoreactive cells in the UBT wall has been demonstrated in a 5-days old rat. In addition, immunohistochemical studies for thyroglobulin revealed positive staining in follicular cells connected to the UBT and, occasionally, in isolated cells lying within solid clusters from the UBT. These last results together with the continued and repeated existence of numerous mitosis and PAS (+) microfollicles, apparently rising from the UBT, support the hypothesis that the ultimobranchial body (UBB) may contribute partially to the formation of a part of the follicular component.

The effect of HeNe laser radiation on the thyroid gland of the rat.

Although laser irradiation is becoming common practice in medicine, there is not always a clear understanding of the possible side-effects. The present report is a light and electron microscopic study of the effects of fixed low intensity doses of soft HeNe laser on the thyroid of Wistar rats. The immediate effects are mild multifocal degenerative changes; these lesions recover in less than 3 months. Long-term lesions are identified only by electron microscopy; they consist of an increased number of peroxisomes and free or intramitochondrial crystalline structures. We discuss the laser's hypothetical functions.

Effects of treatment with noradrenaline and isoproterenol on the excretory portion of the submaxillary gland in the rat: an ultrastructural study.

The aim of this study is to evaluate the modification of the excretory portion and its comparison with the acinus, on stimulating the rat submaxillary gland with noradrenaline and isoproterenol. Stimulation of the submaxillary gland with these drugs for a period of 10 min produces different effects in the acinus and excretory portion. The secretory granules do not show reorganisation of their contents after stimulation with isoproterenol, while in the acinus the material is reorganised forming concentric laminae. After stimulation with noradrenaline, the acinar lumen increases its diameter with respect to the control groups, while this same stimulus does not modify the diameter of the ductal lumen. After stimulation with noradrenaline, the extracellular spaces are consistently obliterated in all ducts except the striated duct, in which the behaviour is variable as in the acinus. In these two latter cases, the extracellular spaces are visible in some cases and not in others. Stimulation with noradrenaline is a potent stimulus of synthesis in both the granular undulated duct and the striated undulated duct, while in the acinus this effect is produced by isoproterenol.

A morphometric study of the secretory granules of the granular duct in the submaxillary gland of the rat following stimulation with noradrenaline and isoproterenol.

In the present work, we carry out a morphometric analysis, at ultrastructural level, of the secretory granules of the granular undulated duct of the submaxillary gland of the rat, under basal conditions (Control Group or I), following stimulation for 10 minutes with 2 mg/100 g weight of Isoproterenol (Group II), and following stimulation with 2 gammas/100 g weight of Noradrenalin for the same time as in the former case. It is seen that in general, Noradrenalin produces the appearance of a greater number of small granules than does Isoproterenol or the control group; and that Isoproterenol induces the presence of larger-sized granules than does Noradrenalin.

Demonstration of sugar residues in the ultimobranchial tubule and thyroid C-cells of the rat using peroxidase labelled lectins.

The distribution of various sugars, that contribute to the composition of the carbohydrate components of the glycoprotein in the ultimobranchial tubule and the thyroid C-cells has been studied, using eight peroxidase-labelled lectins (Dolichos biblorus, Glycine max, Bauhinia purpurea, Helix pomatia, Arachis hipogaea, Ulex europaeus I, Limax flavus and Sophora japonica). The results obtained seem to show that there are receptors for Dolichos biflorus, Glycine max, Bauhinia purpurea, Helix pomatia, Arachis hipogaea, Ulex europaeus I and Limax flavus in the ultimobranchial tubule, suggesting the existence of N-acetylgalactosamine, galactose, L-fucose and sialic acid. However, the C-cells show only a positive reaction with Ulex europaeus I indicating the existence of fucosyl residues in those cells. All these findings support the contention that the ultimobranchial body might contribute decisively not only to the formation of C-cells but also, partially, to the constitution of certain follicular cells.

[Study of isochromosomes of the long arms of the X chromosome].

A clinical and cytogenetic study of 10 patients with chromosome formula of isochromosome in the long arms of the X in all the cells or in a mosaic with another cell line is made. Cases os isochromosomes, so far reported, differentiating them into monocentric and dicentric, after applying techniques of C-banding to them, have been review. Finally a clinical study of the cases 46,Xi (Xq) is made.

Hürthle cell and mitochondrion-rich cell tumors. A clinicopathologic study.

Hürthle cells are large eosinophilic thyroid cells that contain a large number of mitochondria with a high content of oxidative enzymes. In the last 10 years several reports have emphasized the disagreement over the morphologic features, biologic behavior and treatment of Hürthle cell tumors. The authors reviewed the clinical and pathologic features of 28 patients with Hürthle cell and mitochondrion-rich cell tumors (16 adenomas, 10 follicular carcinomas, and 2 papillary carcinomas) and present electron microscopic, immunohistochemical, and morphometric data. The results suggest that there is a correlation between biologic behavior and pathologic findings, that tumor size should not be considered a special conditioning factor in order to assign a biologic behavior, that nuclear size and anisokaryosis are not an absolute criteria for diagnosing malignancy, and finally, that electron microscopic examination is not useful in separating benign from malignant Hürthle cell tumors.

Ultrastructural study of the epithelial mucous cells in lizards (Lacertilia).

An ultrastructural study of the mucous cells of the intestinal epithelium of four lacertilian species (Lacerta lepida, Lacerta hispanica, Psammodromus algirus and Acanthodactylus erythrurus) is here reported. Two types of mucous cells have been found in these species: common mucous cells and granular mucous cells. Immature and mature forms of both types have been observed. The common mucous cells or typical goblet cells have the same characteristics in all four species. The granules of the granular mucous cells of the two species of lacerta are similar but differ from those of the other two species.

Partial trisomy for the long arm of chromosome 7. Case report and review.

We reviewed partial trisomy of the long arm of chromosome 7 after a new case was brought to our attention. The clinical differences between the various types of trisomies 7q were evaluated by statistical analysis, and three groups were defined. These groups correspond to the segments q22 or q21 leads to q31, q31 leads to qter, and q32 leads to qter, and would seem to represent three different syndromes, of which one is more serious than the other two.
