A novel biologically active selenooorganic compound--VII. Biotransformation of ebselen in perfused rat liver.

Ebselen (PZ 51) is a selenoorganic compound with antioxidant and antiinflammatory properties, and its metabolism was studied in isolated perfused rat liver (hemoglobin-free, open system). 75Se-labelled ebselen was taken up into liver cells and radioactivity was excreted into bile. Biliary excretion of 75Se-compounds reached maximal values of 4 nmol/min per g wet wt. HPLC analysis of bile and effluent perfusate as well as identification of separated metabolites by mass spectrometry were carried out. The biliary metabolites were (a) an interesting novel Se-glucuronide, 2-glucuronylselenobenzanilide, (metabolite IV), as the major metabolite, and (b) an O-glucuronide, N-(4'-glucuronyloxyphenyl)-2-methylselenobenzanilide (metabolite III). The major effluent perfusate metabolites were Se-methylated derivatives (metabolites I and II). There was no evidence for sulfated metabolites. The selenodisulfide with glutathione, S-(2-phenyl-carbamoyl-phenylselenyl)-glutathione, was not detected, probably because of low steady-state concentrations and/or its biochemical lability. The selenium in ebselen is not bioavailable (e.g. for the synthesis of glutathione peroxidase), in contrast to selenite, for example, thus explaining the very low ebselen toxicity. However, the enzymatic steps in Se-methylation could be similar to those in the metabolism of selenite which include hydrogen selenide methylation. Se-glucuronides constitute a novel category of compounds in addition to the O-, N-, C- and S-glucuronide classes known in biology.

A novel biologically active selenoorganic compound--IV. Protective glutathione-dependent effect of PZ 51 (ebselen) against ADP-Fe induced lipid peroxidation in isolated hepatocytes.

PZ 51 (Ebselen) is capable of inhibiting ADP-Fe-induced lipid peroxidation in isolated hepatocytes, assessed by generation of low-level chemiluminescence, and the formation of alkanes (ethane, n-pentane) and malondialdehyde. The sulphur analog PZ 25 is much less active. PZ 51 is ineffective in glutathione-depleted hepatocytes that were obtained after phorone pretreatment of the animals. However, other antioxidants, like (+)-cyanidanol-3 or diethyldithiocarbamate are effective in inhibiting lipid peroxidation also in glutathione-depleted cells. The results are attributed to a GSH peroxidase-like activity of PZ 51. Addition of the chemically synthetized compound, PZ 51-SG, a glutathione adduct of PZ 51, does not provide extra protection in GSH-depleted cells in comparison to the parent compound, PZ 51. Dithioerythritol exhibits a protective effect, and PZ 51 is more efficient than in the absence of this thiol reductant. Dithioerythritol also exerts an effect in glutathione-depleted cells. A peroxidase-like activity using DTE as reductant is demonstrated in vitro.

[Occlusive syndromes in pregnancy and puerperium. Apropos of 2 cases].

The authors recall the aetio-pathogenesis of intestinal obstruction in pregnancy and stress the difficulty of the diagnosis. They insist upon the importance of a simple and rapid surgical procedure which can be completed in the post-partum period if need be to prevent further episodes. From the obstetric point of view, the pregnancy usually progresses favourably, provided that an early delivery is induced routinely. Vaginal delivery is permissible, but should be assisted. The maternal prognosis depends on how early the diagnosis is made, but usually seems to be favourable. However, foetal mortality still remains high, between 10 and 15%.

[A thousand obstetrical sutures of the perineum using a slow absorption synthetic suture material (author's transl)].

A thread which is absorbed slowly and which has a polyglycolic acid base has been used since 1975 in the Sainte-Foy-les-Lyon Maternity Unit to sew up episiotomies and perineal tears. The authors had 98.70 p. 100 of excellent results out of 1082 cases from the anatomic, aesthetic and functional angle. This technique appears to be a useful one in early mobilisation in obstetrics.

Value of three-dimensional echocardiography as an adjunct to conventional transesophageal echocardiography.

Three-dimensional imaging of cardiac structures could enhance the functional understanding and the interpretation of pathologies. Limited processing capabilities, relocation problems and inadequate two-dimensional image quality have previously limited its applicability. Recently, an integrated echocardiographic computerized tomography unit (echo-CT) which uses a transesophageal approach has been developed. This system is capable of sampling and processing multiple echocardiographic images and, thus, provides three-dimensional views. To evaluate the feasibility and potential of this technique, we studied 69 patients with various cardiac disorders. All but 3 patients (96%) tolerated the procedure well allowing at least one scan to be performed. No complications were encountered. The indication for echo-CT included coronary artery disease (n = 4), mitral valve disease (n = 18), suspected arterial embolism (n = 19), masses (n = 8), congenital malformation (n = 10), postcardiac surgery (n = 8), aortic aneurysm (n = 1) and suspected left-to-right shunt (n = 1). Conventional transesophageal echocardiography revealed a pathology in 45 patients. Of these pathologies, 37 (82%) could be reconstructed and displayed in three-dimensional views. Three-dimensional imaging provided an improved spatial understanding of the pathology in 21 cases (39%). Echo-CT was especially valuable in diseases of the mitral value (i.e. mitral valve prolapse, flail leaflets, mitral stenosis) where it had the potential to delineate the location, type and morphology of defects. In conclusion, three-dimensional transesophageal imaging enhances image interpretation and understanding. This could be of value in complex morphologies and cardiac disorders in which surgical repair is attempted.
