We describe a sensitive method for measuring the concentration of the new antineoplastic drug ET-18-OCH3 in plasma. After plasma lipids are extracted, ET-18-OCH3 is separated from the excess of endogenous lipids by thin-layer chromatography and specific enzymatic hydrolysis of sphingomyelin by the action of sphingomyelinase. Analytical recovery after the complete isolation was 73.5% (CV = 9.8%, n = 15). [3H]-ET-18-OCH3 is used as internal standard. A densitometric method in which 8-anilino-1-naphthalenesulfonate, Mg salt, is used as fluorescent agent (excitation at 367 nm and emission greater than 390 nm) allows the sensitive determination of ET-18-OCH3 down to 0.1 mg/L (CV greater than 30%). The day-to-day CV is 25% for concentrations of 0.15 to 0.625 mg/L, 12% for 1.5 to 5.0 mg/L. Preliminary pharmacokinetic data reveal gastrointestinal absorption of ET-18-OCH3 after multiple oral administration.
This series describes some common and uncommon cutaneous findings in twelve patients with ataxia-telangiectasia (A-T). All patients had the characteristic telangiectasia as described previously. However, the telangiectases did not conform to a classic photodistribution . Our series did include one patient with a pathologic reaction to light-simulating hydroa aestivale or vacciniforme . In addition, there were three patients with acanthosis nigricans in two unrelated families. Vitiligo, impetigo, recurrent herpetic gingivostomatitis, hirsutism, lipoatrophy, gray hair, progeroid changes and hyper- and hypopigmented macules were noted in our patients. Many of our patients had one or more caf'e au lait like-lesions in a dermatomal distribution. In addition, several of the family members showed one or more café au lait-like lesions, suggesting the possibility that this finding may represent a phenotypic expression in the skin of carriers of this cancer-prone syndrome.
Measurements of the vapour concentrations delivered by the EMO and Oxford Miniature Vaporizers (OMV) were made with both continuous (plenum mode) and intermittent (drawover mode) air flows. Leakage of ether, halothane and trichloroethylene vapours through the corrugated elephant tubing was also measured. Both vaporizers performed most consistently with the intermittent flows for which they were designed. Outputs were minimal at very low carrier gas flows, reached their greatest at the higher settings in the middle flow range and tended to be low at the highest flows. These effects were far more notable with continuous than with intermittent flows. Minimal amounts of ether were lost through the tubing but halothane losses were appreciable, while losses of trichloroethylene were enough to reduce the concentrations available to the patient. The EMO is not suitable for plenum use with carrier gas flows below about 10 litres/min. The OMV is a useful plenum vaporizer although the outputs are generally lower than indicated at higher flows.
