Influence of fluid removal rate during hemodialysis on left ventricular performance and exercise tolerance in patients with coronary artery disease.

The effect of hemodialysis (HD) on left ventricular (LV) function and exercise tolerance were measured at rest and during exercise using gated equilibrium radionuclide ventriculography in seven patients with confirmed coronary artery disease (CAD). To separate the effects of fluid removal rate on LV function in CAD, we investigated the same patients with identical overall volume loss of 4 liters during two different treatment times (4 hr and 2 hr). HD significantly increased resting LV ejection fraction (EF) from 55.7 +/- 8% to 64.7 +/- 8% (P less than 0.01) during the 4 hr HD and from 58.1 +/- 9 to 68.1 +/- 10 (P less than 0.05) during the 2 hr HD. Indicating ischemia, EF decreased at pre- and postdialysis peak exercise without differences between both treatments. HD also resulted in an improved segmental wall motion score. Exercise duration as well as S-T segment depression and angina score improved during HD, whereas heart rate, blood pressure and double product remained unchanged. We conclude that HD improves global and regional resting LV function and exercise tolerance in patients with CAD. The degree of interdialytic hydration and not the degree of fluid removal per time affects LV performance in CAD. Since LV function is the major prognostic factor in CAD, those patients require volume restriction and/or shorter interdialytic phases.

[Disseminated intravascular coagulation and circulatory shock].

The authors describe the pathophysiology of disseminated intravascular coagulation and the coagulopathy of utilization. The clinical and laboratory data on different types of shock are described. The efficacy of different antithrombotic agents in shock with disseminated intravascular coagulation is shown.

Blood Coagulation Disorders↗

[Pathophysiology of hemodynamic and metabolic disturbances following circulatory shock (author's transl)].

Circulatory shock is defined as a syndrome of hemodynamic and metabolic disturbances. Impairment of macro- and microcirculation initiated by different causes is followed by a reversible and later on by an irreversible disturbance of cell metabolism. Breakdown of organ function may be the consequence. The shock specific discomposure of the hemostatic system is characterized by an inactivation of blood coagulation. This leads first to hypercoagulability accompanied by a tendency to thrombosis; later on the impairment of the hemostatic balance is followed by the consumption of coagulation factors and by hypocoagulability with a more or less pronounced bleeding tendency. Treatment of shock is the main prerequisite to avoid and overcome shock induced coagulation disorder. Heparin administration, fresh blood and fresh plasma are adjuvant measures. Substitution of concentrates of clotting factors are indicated only in states of total consumption of the coagulation system.

[The effects of "relaxing music" on patients, doctors and nursing staff of a medical intensive care unit (author's transl)].

With the intention of reducing the psychological stress and anxiety of patients during their admission to the medical intensive care unit, specially selected and prepared instrumental music was played over a loud speaker to the patient cubicles. The patients' opinions on the programme and their subjective state of health were determined by a questionnaire. 78% of the patients felt that their well-being was improved by the music. With the exception of 7%, who were disturbed by the music, all other patients found it reassuring, diverting, hypnotic or entertaining. In addition to the patients 11 doctors and 16 nurses from the intensive care unit as well as 38 doctors and 80 nurses from 39 medical intensive care units in German university clinics and large hospitals were asked for their opinions on the use of music for intensive care patients.

[Clinical and in-vitro studies of the therapeutical range of acetylsalicylic-acid to prevent deep vein thrombosis (author's transl)].

In three consecutive studies, acetylsalicyliclysine using the dosages 0.9 g/48 h, 1.8 g/24 h and 3.6 g/24 h, was examined in order to see its effect in preventing deep vein thrombosis (DVT) after elective hip joint surgery. DVT was diagnosed by the 125-i-fibrinogen-test. No reduction of postoperative DVT was found under any of the administered AS-lysine dosages. The collagen-induced platelet aggregation was significantly decreased in all three groups. However, under the low AS-lysine dosages, aggregation was less inhibited with the incidence of DVT than without DVT. In the group with the 3.6 g-dosage, aggregation was maximally inhibited, without any significant difference related to the occurrence of DVT. The results reveal no therapeutic range of AS-lysine to prevent postoperative venous thrombosis.
