The field of cryosurgery in proctology can be seen in therapy of inoperable anal and rectal carcinomas. It is aimed at improving life quality. The role of microcirculation as a factor causing a contrary heating effect can be proved by intravital microscopy. Cryodestruction is limited by this factor. By means Of measurements of temperature we know the boundaries of complete tissue destruction which is reached in a distance of 4-8 mm. Our own number of cases consists of 122 patients with a mean age of 72 years.

The authors represent lifelike exercise models for the training without patient of diagnostic and therapeutic interventions in the rectosigmoid and in the colon. The two models are used for the practical training of the rectosigmoidoscopy with the rigid rectoscope and the coloscopy with the fibre-optical examination technique. In the excorporated animal rectum may be performed polypectomies by means of removal of the loop, exploratory excisions from the intestinal wall as well as electro-surgical and cryosurgical interventions. The experiences got up to now in training classes are described.

Both tibiae were osteotomised after medullary nailing in 40 rabbits. In each case the right fracture was treated with ultrasound of 0.1 W/cm2 for 2 minutes 4 times every second day after on week. The left extremity is used as control. An accelerated and increased callus formation and callus maturation were found by ultrasonic therapy. This was possible to be shown on a higher level of the alkaline phosphatase, a higher temperature, and an accelerated calcium and phosphate incorporation in the callus in comparison with the untreated side. The mineralisation by means of ultrasound showed a characteristical three phase behaviour. The mechanism of action is probably the piezo-electric effect of the local tension induction by ultrasound with resultant activation of the metabolism. The advantage of the mineralisation amounted to 2 weeks in contrast to the untreated extremity in the 4th week after the fracture. These findings confirm numerous positive clinical results.

Our rectoscopies were made in patients for various indications. Venous blood samples before rectoscopy as well as 1, 5, 10, 20 and 30 minutes after its beginning and blood cultures were taken. No transient bacteriaemia could be identified. Only in patients suffering from carditis an internist should be consulted before performing the rectoscopy.
After a short survey of the literature concerned, it is pointed out that early findings of rectal carcinoma (T1 or Dukes' disease A orCSI + II according to Mason) can be removed locally in the unaffected tissue under observation of the strictest indication by transanal surgery or by posterior rectomy. In this procedure the surgical mortality is 0 per cent, the ten-year-survival can go up to 90 per cent. The own material described comprises 97 patients with an average age of 64.5 years.
11 patients suffering from acquired anal insufficiency underwent a relative simple myoplastic surgical procedure, dealt with in detail. 10 patients of them regained full continence and full working capacity. They also recovered completely from a social point of view.
Outline of the pathophysiological basis of defaecation is given and also classification of anal incontinence. In the operative technique described the defective sphincters are replaced by drawing down the puborectal muscle, i.e. by a displacement of the muscle caudally. This method is simple, physiological, and appears superior to all others. It has been used in 15 patients, in 14 of whom it was completely successful. The lesion had been present in these patients for 1-13 years. Their average age was 40. There is a fundamental distinction between congenital and acquired anal incontinence. The congenital lesions fall within the domain of paediatric surgery, and will not be discussed.
