The overexpression of two members of the erbB oncogene family-the epidermal growth factor receptor protein (EGF-R) and the HER-2/neu protein-in breast cancer has been investigated by numerous authors. Their exact role in breast cancer is still not defined. The simultaneous investigation of EGF-R and HER-2/neu in the same study population has only rarely been performed in breast cancer. Therefore, we analyzed the EGF-R and the HER-2/neu protein expression in 142 breast cancer specimens and 12 benign breast samples by immunohistochemistry. Results of the different expression analyses were compared with established clinicopathological parameters including estrogen and progesterone receptor status. In our study group EGF-R expression correlated with advanced tumor stage, axillary lymph node status and histological grade. HER-2/neu expression correlated with larger tumor size. Neither the evaluation of a single parameter of the erbB family nor the combination of both parameters showed a significant correlation with the disease-free and the overall survival of the individual patient with breast cancer. Only the expression of the progesterone receptor correlated with a better overall survival. Steroid hormone receptor expression and the expression of EGF-R and HER-2/ neu seem to be two independent phenomena in our samples of breast cancer. The simultaneous evaluation of EGF-R and HER-2/neu expression did not lead to new information of prognostic relevance.
The expression of distinct variant isoforms of the cell surface glycoprotein CD44 (CD44v) has been found to be associated with metastatic potential of rodent adenocarcinoma cells and with an altered prognosis in several types of human cancer. In hormone-dependent gynecological cancers, different CD44v expression patterns have been observed. The influence of ovarian steroid hormones and their antagonists on CD44v expression is still unclear, since there are only retrospective correlation studies so far. Therefore, we examined the CD44 mRNA expression in a standardized stimulation experiment in a number of breast and endometrial carcinoma cell lines varying in estrogen receptor (ER) status. Higher CD44 overall expression was observed in ER positive endometrial and breast carcinoma cell lines when compared to corresponding ER negative cell lines. The number and composition of alternatively spliced isoforms showed no clear correlation to the ER expression status. Three CD44v isoforms were detected in all cell lines expressing CD44v, two of which have not been reported previously in normal endometrial cells. These isoforms may have specific functions in this type of carcinoma. In the second part of the study, the influence of (anti-) hormones on CD44 expression in endometrial carcinoma cell lines was examined. CD44 overall expression showed an increase when the cells were grown in medium containing fetal calf serum (FCS) as compared to cells maintained in medium-free of FCS. CD44 expression was transiently increased by estradiol (1 h). The CD44 splice pattern of endometrial cancer cell lines RL95-2 and Hec-1-A, after treatment with (anti-) hormones showed constant and high expression rates for distinct CD44v-isoforms such as CD44E (CD44v8-v10). Only certain weakly expressed isoforms changed their expression level during the experimental period, but no direct correlation to hormone treatment was observed. In conclusion, estradiol or FCS increase CD44 overall expression, but there seems to be no direct influence of ovarian steroid hormones on the CD44v splice machinery in endometrial carcinoma cell lines.
We retrospectively analyzed the frequency and details of blood transfusions in a group of 12,578 obstetrical patients. We found 250 patients who received red blood cell transfusions resulting in a transfusion rate of 2 in 100. In most of these cases, there were no risk factors placing patients at high risk for blood transfusions. The administration of one or two units of blood was adequate in 76% and of up to three units adequate in 87% of cases. Our results do not support a commercial blood bank's recent claim of frequent transfusions in obstetrical patients. We feel that the routine storage of blood for autologous transfusions is not warranted in obstetrical patients.
In 50 pregnancies morphometric parameters of the placenta and the serum estriol values in the third trimester were correlated. The results show that there is no correlation between the placental parameters as determined by a semi-automatic method and the estriol values in maternal serum in late pregnancy. The placental parameters probably do not reflect the biochemical activity of the trophoblast. A sufficient capacity of the placenta for estriol synthesis must be taken into account independently of its morphological substrate.
Gene amplification is a common mechanism of proto-oncogene activation and contributes to tumor progression. Analysis of such genetic alterations is relevant to our understanding of tumor genetics and can provide prognostic information for the patients. A rapid, non-radioactive approach based on qdPCR and fluorescent DNA technique was applied for determination of int-2 and c-erbB2 gene amplification and correlated with other prognostic factors in 70 breast cancer samples. ER and PgR were analysed by immunohistochemistry. The mixed template assay showed 96% concordance between calculated and measured gene copy number. int-2 gene and c-erbB2 amplification were both found in 24% of the tumors. The amplification did not correlate with any of the other prognostic factors. 8% of the tumors showed amplification of both genes without significant correlations to any of the other parameters. The fd-PCR assay is a valuable tool for determination of amplification of int-2 and c-erbB2 genes. Therefore, more detailed information about individual tumour biology and outcome may be acquired by this routine assay and probably provide prognostic impact.
