Epidermal growth factor contents in seminal plasma as a marker of prostatic function.

Epidermal growth factor (EGF), first identified in extracts from submaxillary salivary glands of adult male mice, acts as a potent mitogen in a wide variety of cells and tissues in culture. Human EGF, a polypeptide of 53 amino acids isolated from human urine, has been identified in several biological fluids, including prostatic fluid and seminal plasma. Epidermal growth factor content in seminal plasma was measured by RIA. The EGF content in seminal fluid did not change after bilateral vasectomy and the first fraction of sample obtained by split ejaculation contained a significantly larger amount of EGF than the second fraction. Seminal EGF originates from the prostate. There was a close correlation between EGF and zinc contents in seminal fluid. The seminal fluid EGF content was lowered in patients with prostatitis and systemic androgen deficiency. The EGF content in seminal fluid seems to be a reliable indicator of prostatic function.

Hypoosmotic swelling test with a medium of distilled water.

The functional competence of the human sperm membrane is assessed by studying the swelling reaction of sperm when suspended in a medium of distilled water (water test). Eighty-seven patients with idiopathic infertility were investigated by the water test and the results were compared with various semen parameters. High correlations with the percentage of b-g type swollen sperm in the water test were observed for sperm concentration (r = .53, p < .05) and percent motility (r = .62, p < .01). The sperm swelling values obtained by the water test correlated well with those obtained by the hypoosmotic swelling test (HOST). Correlation coefficients in b-g and g swelling pattern were 0.89 and 0.71, respectively. The percentage of g-type swelling obtained by the water test was significantly larger than that obtained by the HOST (p < .05), although the two hypoosmotic procedures gave similar percentages of b-g swelling pattern. The percentage of g swelling pattern obtained by the water test correlated well with percent motility (r = .70, p < .001). These findings suggest that the water test is more useful for assessing the integrity of the sperm membrane than the HOST. The relationship between this test and subsequent fertility, as well as to other sperm function tests, needs further investigation.

Acrosome-reacted sperm in infertile and fertile men using the triple-stain technique.

By means of a triple-stain technique, the acrosome reaction in infertile and fertile males was investigated. The highest correlations with the percentage of acrosome-reacted sperm were found for motile sperm concentration (r = .60, p < .01) and percent motility (r = .58, p < .05). Significant correlations were also demonstrated between acrosome reactivity and both sperm concentration (r = .50, p < .05) and velocity (r = .40, p < .05). There was only a weak correlation between the percentage of acrosome-reacted sperm and that of swollen sperm. The ratio of acrosome-reacted sperm demonstrated in oligoasthenozoospermic patients was significantly lower than that observed in normozoospermic or fertile males (both, p < .05). There was no significant difference in the ratio of acrosome-reacted sperm between normozoospermic and fertile males. However, all the cases in the latter group had 15% or more acrosome-reacted sperm, whereas the parameter was widely distributed in the former group and some in this group had a decreased ratio. It would appear that a disturbance of the acrosome reaction is one of the causes of reduced fertility potential in normozoospermic males.

Effect of pentoxifylline on sperm motion.

The ability of pentoxifylline to stimulate sperm motility characteristics was assessed. The effect of pentoxifylline was immediate and lasted for 240 min. The percent motility was increased 1.5-fold when compared with the control at 120, 180, and 240 min after pentoxifylline treatment. Significant increases in velocity and amplitude of lateral head displacement by the agent were observed at 120 min after the start of incubation. Conversely, linearity was reduced by pentoxifylline. Beat-cross frequency did not show any significant change by the agent at any time point. Sperm swelling was not influenced by pentoxifylline throughout the experiment. It would appear that pentoxifylline stimulates sperm motion in vitro with no change of the sperm membrane.

Varicocelectomy: changes in Resazurin Reduction test.

The Resazurin Reduction Test was performed on semen specimens from 27 patients with varicocele before and after varicocelectomy. The increases in both sperm concentration and motility were slight 3 months after surgery, but sperm concentration significantly improved 6 months after surgery (P < 0.01). The positive rate in the Resazurin Test significantly increased 3 and 6 months after surgery (each, P < 0.05) and the resazurin grade also increased at these times (each, P < 0.01). Of 13 patients with improved seminal findings, 7 had negative reactions during the preoperative Resazurin Test and 3 of these 7 patients continued to have negative reactions, while the other 4 had positive reaction after surgery. Of 14 patients with no improvement of semen characteristics, 6 continued to have negative reactions after surgery, but 4 with negative reactions before treatment showed positive reactions postoperatively. The Resazurin Test seems to assess fertility potential accurately after varicocelectomy.

Changes in seminal plasma transferrin concentration following administration of clomiphene citrate.

This study was designed to investigate the functional changes in Sertoli cells following clomiphene citrate administration to patients with idiopathic infertility. The agent was administered to 44 oligozoospermic patients. Patients with improvement in sperm concentration after treatment (improvement group) showed significantly lower pretreatment follicle-stimulating hormone (FSH) levels in serum. Serum FSH and testosterone levels significantly increased in the improvement group after treatment (both p < 0.05). Seminal transferrin concentration significantly increased in the improvement group after treatment (p < 0.05), although there was no significant difference between pretreatment seminal transferrin concentration in the improvement group and that in patients with no improvement in sperm concentration. Seminal plasma transferrin concentration in patients in whom pretreatment luteinizing hormone level in serum was below 10 mIU/mL, and in those in whom the ratio of post- to pretreatment values was 1.5 or more, tended to increase after treatment. This group also contained a few more patients with improvement in sperm concentration than the other groups. The patients showing low pretreatment FSH levels and a high ratio of post- to pretreatment values demonstrated a significant increase in seminal plasma transferrin concentration after treatment (p < 0.01), and this group contained more patients with improvement than the other groups. It would appear that Sertoli cell stimulation may induce an increase in sperm concentration after clomiphene citrate administration, and the elevated transferrin secretion seems to be associated with the increase of serum FSH level after treatment.

Correlation of seminal plasma transferrin concentration and hypoosmotic swelling of the sperm.

To investigate whether there is a relationship between seminal plasma transferrin concentration and hypoosmotic swelling of the sperm, semen samples from 46 infertile patients were examined. Seminal plasma transferrin concentration in oligozoospermic subjects (sperm concentration < 20 x 10(6)/mL) was significantly lower than that in normozoospermic subjects (sperm concentration > or = 20 x 10(6)/mL) (p < .05). Patients with > or = 50% swollen sperm showed significantly higher transferrin concentration in seminal plasma than those with < 50% swollen sperm (p < .05), while in oligozoospermic patients no difference was observed between seminal plasma transferrin concentration and hypoosmotic swelling. No relationship between seminal plasma transferrin concentration and serum gonadotropin levels was demonstrated. The results indicate that transferrin synthesis is important to sperm membrane integrity and functional competence. However, subsequent fertility must be examined to determine whether seminal plasma transferrin levels are predictive of fertilizing capacity.

Acrobeads test using monoclonal antibody-coated immunobeads: clinical application.

Acrosome-reacted sperm was examined in infertile and fertile males using the Acrobeads test. The Acrobeads score was significantly higher in fertile males than in infertile males with normal or abnormal seminal findings (p < .005 and p < .001, respectively). The Acrobeads score was 2 or more in all the fertile males, but 62% of the infertile males with abnormal seminal findings (p < .02). Therefore, a disturbance in the acrosome reaction is suggested to be one of the causes of decreased fertility potential. A high correlation was found between Acrobeads score and sperm motility (r = .46, p < .001) or motile sperm concentration (r = .51, p < .0001). The Acrobeads test, the results of which reflect the acrosome condition of the sperm, is useful in the assessment of sperm function related to fertility.

Effect of varicocele on fertility potential: comparison between impregnating and nonimpregnating groups.

Sixty patients with varicocele, complaining of infertility, underwent a corrective operation. Eighteen (30%) of the 60 patients impregnated their mates (impregnating group). Sperm concentration significantly increased after the operation in the impregnating group (p < .05), but not in the patients who failed to impregnate their mates (nonimpregnating group). Sperm motility was not altered after the operation in these two groups. Sperm velocity significantly increased postoperatively in the impregnating group (p < .05), while linearity was not altered in either group. Sperm swelling significantly increased postoperatively in the impregnating group (p < .05), but not in the nonimpregnating group. Preoperative sperm parameters were not different between the two groups. Varicocele may disturb sperm function including sperm motion besides spermatogenesis and may impair the fertility potential. However, a prospective large-scale controlled study is required to obtain a definitive conclusion.

[Clinicopathological study of the testicular microlithiasis].

PURPOSE: Testicular microlithiasis (TM) is a relatively rare condition characterized by calcific concref1p4 within the seminiferous tubules. Little has been reported on the incidence or the clinical implication of TM among Japanese. To address the problem, we evaluated pathologic specimens from biopsies and orchiectomies, of testes with various conditions. MATERIALS AND METHODS: Pathologic specimens of the testes of 200 cases, 56 from orchiectomy and 144 from testicular biopsy, were investigated. RESULTS: The pathological diagnosis of TM was confirmed in seven (3.5%) cases, four of which were associated with germ cell tumors and the other three were obtained from testicular biopsies performed for examination of infertile men. Of the 41 patients with germ cell tumors, four (9.8%) were found to have TM, and another three (2.5%) were identified among 122 patients with infertility. The prevalence of TM is significantly higher in specimen with germ cell tumors than those without germ cell tumors (p < 0.05). CONCLUSIONS: Although TM is rarely encountered, this condition is relatively often accompanied by testicular malignancy. Further investigation would be fundamental to ascertain the relationship between TM and testicular malignancy.

Effect of interferon-alpha A/D in combination with the Japanese and Chinese traditional herbal medicine juzen-taiho-to on lung metastasis of murine renal cell carcinoma.

Several studies have shown that the Kampo medicine Juzen-taiho-to (Si-Quan-Da-Bu-Tang in Chinese) has various biological activities, including anti-tumor effects when combined with surgical excision or with chemotherapeutic drugs. Here we investigated the effect of combined therapy with interferon (IFN)-alpha A/D and Juzen-taiho-to on experimental lung metastasis of murine renal cell carcinoma (Renca) cells. Five consecutive administrations of IFN-alpha A/D to Renca-bearing mice resulted in dose-dependent inhibition of lung metastasis. IFN-alpha A/D at the dose of 100,000 IU/mouse significantly inhibited the metastasis, but a marked loss of body weight was observed during and after the administration. In contrast, oral administration of Juzen-taiho-to (50 mg/mouse) alone tended to inhibit the metastasis, but the effect was not statistically significant. The combination treatment of suboptimal doses of IFN-alpha A/D and Juzen-taiho-to markedly augmented the antimetastatic effect without causing any loss of body weight, as compared with either treatment alone. Similar results were also obtained by treatment with IFN-gamma in combination with Juzen-taiho-to. Clinically, immunotherapy with IFNs has been primarily approved for the treatment of patients with metastatic renal cell carcinoma, but sufficient efficacy has not yet been obtained. Therefore, the combination of IFNs with Juzen-taiho-to may provide a means to increase the therapeutic potential of IFNs and to decrease their toxicity for the treatment of metastatic renal cell carcinoma.

Serum soluble Fas level for detection and staging of prostate cancer.

To evaluate the clinical usefulness of measuring serum soluble Fas (sFas) for differentiation between prostate cancer and benign prostate hyperplasia (BPH) and for staging of prostate cancer, serum sFas and PSA were determined in 38 and 20 men with prostate cancer and BPH, respectively, before treatment. In 17 patients, sFas and PSA were measured one hour after transrectal ultrasound-guided sextant biopsy in order to examine the leakage of sFas into the circulation after prostatic injury. Patients with prostate cancer had a significantly higher level of sFas than those with BPH. The serum sFas level was statistically elevated in patients with metastatic prostate cancer. There was a statistically significant correlation between sFas and PSA in patients with prostate cancer but not in those without cancer. The serum sFas did not change one hour after systematic prostatic biopsy although PSA levels were markedly elevated. sFas levels might be useful as a discriminator between prostate cancer and BPH while sFas might indicate the tumor burden in patients with prostate cancer.

Effect of a matrix metalloproteinase inhibitor (ONO-4817) on lung metastasis of murine renal cell carcinoma.

We examined the anti-metastatic effect of a newly developed inhibitor of synthetic matrix metalloproteinase (MMP), ONO-4817, on experimental pulmonary metastasis of murine renal cell carcinoma (Renca) cells and on tumor cell invasion, through reconstituted basement membrane (Matrigel) in vitro using the same cells. Oral administration of ONO-4817 (50-200 mg/kg/day) to Renca-bearing mice resulted in a dose-dependent inhibition of lung metastasis without a loss of body weight. ONO-4817 at the high dose of 200 mg/kg showed a tendency to prolong the survival of the mice. We also found that oral administration of ONO-4817 significantly inhibited the angiogenic response (number of vessels oriented towards the tumor mass) and the growth of tumors inoculated i.d. in syngeneic mice. In addition, ONO-4817, at non-cytotoxic concentrations of less than 10 microM, caused a marked inhibition of the invasion of Renca cells as compared to the vehicle control. Gelatin zymography revealed that ONO-4817 inhibited the enzymatic activity of MMP-2 produced by Renca cells in a concentration-dependent manner. In conclusion, ONO-4817 effectively inhibited lung metastasis of Renca cells through its anti-invasive and anti-angiogenic properties. These results suggest that use of the MMP inhibitor (MMPI) ONO-481 7 may provide a therapeutic basis for preventing lung recurrence and metastasis of renal cell carcinoma.

Transurethral treatment of ejaculatory duct obstruction in infertile men.

Ten cases of ejaculatory duct obstruction with midline cyst, complaining of male infertility, were treated with transurethral incision. They had azoospermia or oligozoospermia, and physical examination did not show any abnormal findings. Serum levels of testosterone, LH, and FSH were within normal limits. The diagnosis of ejaculatory duct obstruction with midline cyst was made by transurethral ultrasonography and vasography. Thereafter, patients underwent transurethral incision of the verumontanum with a cold knife. Semen volume increased in all patients, and sperm concentration and/or motility improved in 7 patients (70%). Pregnancy was achieved by 3 couples (30%). Incision of the ejaculatory duct via an endoscopic technique could improve seminal findings and subsequent fertility.

Cytoprotective action of roxatidine acetate HCl.

The cytoprotective action of roxatidine acetate HCl (roxatidine) was investigated. We also studied the involvement of endogenous prostaglandins (PGs) in the cytoprotective action of roxatidine and the effect of roxatidine on SRS content in pleurisy induced by A23187. Simultaneously, these effects of roxatidine were compared with those of other histamine H2-receptor antagonists at the same anti-secretory activity level. Roxatidine prevented formation of the gastric mucosal lesions induced by abs. ethanol, 0.6 N HCl and 0.2 N NaOH, but it failed to prevent 30% NaCl-induced gastric mucosal lesions. Cimetidine, ranitidine and famotidine failed to prevent formation of the gastric mucosal lesions induced by necrotizing agents. The cytoprotective action of roxatidine was not abolished by pretreatment with indomethacin. Roxatidine did not greatly influence SRS production. Consequently, it appears that roxatidine has a cytoprotective action and that this action is not associated with endogenous PGs and SRS.

Correlation between the hypoosmotic swelling test and various sperm function tests.

OBJECTIVE: To compare the hypoosmotic swelling test (HOST) with other sperm function assays. MATERIALS AND METHODS: The HOST was performed on 105 patients with idiopathic infertility. The results were compared with those of various other sperm function tests, including the sperm penetration assay (SPA), triple-stain technique for evaluating acrosomal reactions, and the in vitro cervical mucus penetration test (Penetrak). RESULTS: A strong positive correlation was observed between percent sperm motility and the percent swollen sperm, between the motile sperm concentration and sperm swelling, and between sperm swelling and penetration values. Only a weak correlation was detected between sperm swelling and the percent sperm penetration. However, most of the cases with a sperm swelling percentage of 50% or more had good sperm penetration into ova. CONCLUSIONS: The HOST was useful to evaluate sperm tail function and was found valuable as a screening test prior to the SPA.

Comparison of resazurin test results with various sperm parameters.

The resazurin reduction test was performed on semen samples from 83 untreated infertile and 9 pregnancy-confirmed fertile males, and the results were compared with other sperm parameters. The highest correlations with resazurin grade were found for motile sperm concentration (r = 0.65, P < 0.001), per cent motility (r = 0.58, P < 0.01), and percentage of swollen sperm (r = 0.57, P < 0.01). There were also significant but weak correlations between resazurin grade and both sperm concentration and linearity. There was a significant difference in the rate of positive tests between samples from normozoospermic and fertile males, and samples from patients exhibiting abnormal semen parameters (P < 0.001). These findings indicate that the resazurin test may be able to predict those males who may be fertile despite an abnormal semen analysis, and therefore, the test may be useful in the evaluation of the infertile patient as a part of a comprehensive male fertility assessment.
