[The effects of prostaglandin synthetase inhibitor on male infertility].

From August 1979 to August 1981, 272 patients with the chief complaints of sterility visited our University Hospital. Flurbiprofen was administered to 40 of the 164 cases of idiopathic male sterility. Flurbiprofen of 120 mg was administered daily for 3 consecutive months. Drug use was discontinued in 2 cases because of severe digestive symptoms. Semen findings were compared before and after the medication. In the cases with sperm concentration over 10 X 10(6)/ml, sperm concentration and motility were improved in 26.9% and 23.1%, respectively. In the cases with sperm concentration under 10 X 10(6)/ml, sperm concentration and motility were effective in 8.3% and 25.0%, respectively. LH and FSH levels in blood were not different between before and after the administration. On the other hand, testosterone level in blood tended to increase a little, but not with statistically significance. Digestive symptoms and skin eruption were found in 4 cases and one, respectively.

[A clinical observation on urogenital tuberculosis].

A clinical observation was made on 25 cases of urogenital tuberculosis experienced at the urological department of Chiba Prefectural Sawara Hospital between January 1973 and December 1982. They accounted for 0.28% of the 8,945 outpatients. There were 17 cases of urinary tuberculosis and 13 cases of genital tuberculosis. In urinary tuberculosis, the age distribution showed a peak in the fifties. There was a predominance in males. The patients in their twenties and fifties comprised 60% of the cases of genital tuberculosis. The kidney affected was predominantly the left kidney. Bladder tuberculosis was found in 7 cases. The majority of the epididymal tuberculosis were found in the right side. Bladder and renal symptoms were the chief complaints for 41.2% and 17.6%, of the cases, respectively. Thirty two percent of the patients had a history of tuberculosis. In cystoscopy, typical findings of tuberculosis were found in only 25% of the cases. Mycobacterium in urine could be detected in 33.3%. Accelerated blood sedimentation rate was detected in 65% of the cases. Pyelography revealed ureteral abnormality in the majority of the cases. Three of the 16 patients with renal tuberculosis underwent nephrectomy, and orchiectomy was performed on 6 of the 13 patients with epididymal tuberculosis.

Changes in cytosolic androgen receptor after administration of testosterone of androgen-dependent mouse mammary tumor (Shionogi carcinoma) and its sublines of altered androgen dependency.

The effect of testosterone treatment on the cytosolic androgen receptor in an androgen-dependent tumor (Shionogi Carcinoma 115, SC 115) and its sublines of altered hormone-dependency (CS 1, less androgen-dependent, and CS 2, androgen-independent) was examined. In all tumors examined a single, high-affinity binding to 3H-R 1881 was detected in cytosols. The size of the maximum binding sites was in the order SC 115, CS 1 and CS 2. Inhibition experiments revealed that the binding to 3H-R 1881 in cytosols of these tumors was attributable to the androgen receptor. The amount of the androgen receptor in cytosols was decreased at 1 hr after testosterone injection, then replenished gradually towards the pretreatment level. Depletion of the androgen receptor was dependent on the dose of testosterone administered. Multiple injections of testosterone depressed the replenishment of the receptor. Treatment with actinomycin D or cycloheximide did not alter the dynamics of the androgen receptor. Therefore, it was concluded that the depletion and replenishment of the androgen receptor in cytosols of the tumors were caused by receptor recycling. Because a similar pattern of depletion and replenishment of the androgen receptor was also observed in the less androgen-dependent tumors, the growth of tumors was felt to be unrelated to the receptor dynamics.

Properties of two sublines derived from rat prostatic adenocarcinoma (Dunning R 3327 tumor).

Rat prostatic tumor (Dunning R 3327 tumor) is a well-differentiated adenocarcinoma of Copenhagen strain rat; its growth is extremely slow and androgen-dependent. Two new sublines (Chiba University A and B; CUA, CUB) were obtained during passages; CUA, squamous cell carcinoma, grew at moderate speed, and CUB grew rapidly, being composed of spindle-shaped cells and large bizarre polygonal cells. Growth of CUA was slightly androgen-dependent, while that of CUB was independent of sex hormones. Activities of acid phosphatase were increased in both CUA and CUB as compared with the original R 3327, so the less differentiated sublines of CUA and CUB showed unusual progression. In contrast, activities of alkaline phosphatase in CUA and CUB were diminished as compared with that of the original tumor. Both androgen and estrogen receptors were detected in the cytosol from R 3327, but there was no detectable amount of androgen and estrogen receptors in CUA. CUB did not show any androgen binding but estrogen binding was observed in the cytosol, though estrogen did not have any detectable effect on the growth of this tumor.

[A case of true hermaphroditism].

The patient was a two years and seven months old, who was legally a female. She visited our University Hospital with the chief complaint of ambiguous genitalia. The hypertrophied clitoris was 13 mm in length and 11 mm in diameter. The external urethral orifice and vagina were observed separately. The testis-like mass was palpable in the right labium. She had a karyotype of 46XX . The Urinary excretion of 17 KS and pregnanetriol and serum 17 alpha-hydroxyprogesterone were within the normal range. Vaginography showed the vagina, uterus and left fallopian tube. At exploratory laparotomy, we found the uterus and left fallopian tube; the left and right gonads were in the intraperitoneal cavity and the inguinal canal, respectively. To build female characteristics, clitoroplasty was performed, the right gonad was removed the testis-like portion of the left gonad was resected, and the ovary-like portion in the left gonad was biopsied. Both gonads were confirmed to be ovotestis histologically.

[A case of malignant lymphoma with the chief complaint of right intrascrotal tumor].

A 65-year-old male visited the Sawara Hospital with the chief complaint of right intrascrotal mass. The mass without tenderness, in which right testis and spermatic cord appeared to be involved, reached the right inguinal region. Right high orchiectomy was performed under the diagnosis of a testicular tumor. Histological diagnosis was malignant lymphoma. Intravenous pyelogram revealed right hydronephrosis and 67Ga-scintigram showed an abnormal accumulation of radioisotopes in the pelvic and paraaortic region. Chemotherapy with vincristine, cyclophosphamide, 6-mercaptopurine and prednisolone was performed under the diagnosis of stage IV lymphoma. Two months after chemotherapy, right hydronephrosis disappeared and no abnormal accumulation in 67Ga-scintigram was found. He has been doing well three months after treatment.

Internal spermatic vein prostaglandins in varicocele patients.

Blood was collected from 20 patients with varicocele (15 in the infertile group and 5 in the incidental varicocele group) at the time of high ligation by insertion of a catheter into the internal spermatic vein toward the renal side. Simultaneously, blood was obtained from the antecubital vein. The levels of cortisol, prostaglandin (PG)(A + B), PGE, and PGF in these blood samples were measured. Although no differences in cortisol and PG(A + B) levels were observed between internal spermatic and peripheral vein bloods in varicocele patients examined, PGE and PGF levels were higher in internal spermatic venous bloods than in peripheral venous bloods. Differences in PGF levels between the samples of blood from the internal spermatic vein and from the peripheral vein in cases of the infertile group were larger than those of the incidental varicocele group. PGE and PGF in internal spermatic vein blood samples were correlated. The possibility that PGE and PGF, refluxed from the renal vein to the testes via the internal spermatic vein in varicocele patients, disturbs spermatogenesis is discussed.

Low molecular weight component of androgen receptor in cytosols from benign hypertrophic human prostate treated with high KCl solution.

Cytosol of the benign hypertrophic human prostate was prepared in a low salt medium and then the concentration of salt was increased to 0.4 M with KCl (0.4 M KCl-cytosol). This preparation showed a high affinity binding to R 1881 and the binding was specific for androgens. These results suggest that the binding of the preparation to R 1881 was due mainly to the cytosolic androgen receptor. The R 1881 binding component in the 0.4 M KCl-cytosol was sedimented at 3S by sucrose density gradient centrifugation. The small sedimentation coefficient of the binder seems to be due to the high concentration of salt and not to degradation by proteolytic enzymes in the preparation. The molecular weight, Stokes radius and frictional ratio of this binding component were 32,000, 25.9 A and 1.24, respectively.

Citric acid concentration in subclinical mastitic milk.

The effect of subclinical mastitis on the citric acid concentration of cow's milk was studied by comparing milk from the mastitic quarter with that from a healthy quarter of the same udder. Forty-two pairs of fore-milk from the morning milking of 17 cows were examined and the citric acid content was measured by an automated carboxylic acid analyzer. The citric acid concentration deceased in subclinical mastitic milk and the extent of decrease was proportional to the degree of mastitis as expressed by the quarter difference value in concentration of Na + Cl in mM/l, i.e. the increase in the Na and Cl concentrations in the abnormal milk over that found in normal milk from the same udder. The correlation coefficient between the quarter difference value of Na + Cl and the value for citric acid was -0.77, (d.f. = 46, P less than 0.001) and a linear regression equation was obtained. The result showed clearly that subclinical mastitis is one of the causes of variation in the citric acid content of cow's milk. The quantitative change in citric acid concentration in subclinical mastitic milk is discussed with reference to the admixture theory for the formation of abnormal milk.
