Cell kinetic study of murine jejunal crypts during multiple doses per day.

We studied cell kinetics of mouse jejunal crypts during multiple fractionation using double labeling methods. Mice were irradiated to the total body once a day or twice a day. Fraction size was 2 Gy. Overall time was 1, 3 and 5 days. Labeling index (LI), duration of S phase (Ts) and potential doubling time (Tpot) were assessed next morning following the irradiation schedule. Mice were administered both bromo-deoxyuridine (BrdUrd) and 3H-Thymidine ([3H]dT) with a constant interval. Removed jejunum were stained with immuno-histochemistry and, then, processed for autoradiography. BrdUrd and [3H]dT LIs were the same and were increased up to 5 days despite the fractionation schedule. After 1 day following 2 fx/day, both Ts and Tpot decreased significantly. For 1 fx/day treatment, neither Tpot nor Ts changed. After 5 day following 1 and 2 fx/day, both Ts and Tpots were the same and Tpots were significantly shorter than that obtained from control. It seems that proliferative responses between 1 and 2 fx/day were different.

N-(F-18)-fluoroacetyl-D-glucosamine: a new positron labeled pharmaceutical for cancer study.

In order to evaluate the role of hexosamine metabolism in tumor tissue, we studied the biodistribution of N-(F-18)-fluoroacetyl-D-glucosamine (FAGlu) in male Donryu rats bearing poorly differentiated hepatomas (AH109A and AH272). Compare with the former result of the high tumor uptake of FAGlu in C3H/He mice bearing well differentiated spontaneous hepatoma, the tumor uptakes of FAGlu in these tumors showed the lower values. This suggested that spontaneous hepatoma maintained a high activity of glucosamine metabolism, while poorly differentiated hepatoma had little activity. Metabolism of glucosamine in tumor tissue may be another marker for characterizing tumors. We also discuss the tissue distribution of new F-18 labeled hexosamines, N-(F-18)-fluoroacetyl-D-mannosamine and N-(F-18)-fluoroacetyl-D-galactosamine in tumor bearing rats.

Polysplenia syndrome with various visceral anomalies in an adult: embryological and clinical considerations.

A 26-year-old female with polysplenia syndrome is reported. She had numerous visceral anomalies including polysplenia, a short pancreas, a preduodenal portal vein, malrotation of the bowel, azygos continuation of the inferior vena cava, bilateral hyparterial bronchi and symmetrical liver lobation. Embryological and clinical considerations of polysplenia syndrome are described.

Marked increase of CD57+CD16- cells in long-term survivors of graft-versus-host disease after allogeneic bone marrow transplantation.

We have evaluated long-term serial changes in the immunological state from soon after allogeneic bone marrow transplantation (BMT) In 44, mainly leukemia patients with respect to changes in lymphocyte surface markers. Absolute numbers of cluster designation (CD)2+, CD20+ and human lymphocyte antigen-DR+ (HLA-DR+) cells recovered to within their normal ranges three months, one year and two years, respectively, after BMT. The reversal of the CD4+: CD8+ ratio persisted for five years or more but returned to normal after six years. CD57+CD16- cells were markedly increased from three mo up to a maximum of five years after transplantation; they were increased between three and six months after transplantation irrespective of graft-versus-host disease (GVHD), but changes after one year or more differed among patients without GVHD, with acute GVHD, with acute and chronic GVHD or with chronic GVHD. Absolute numbers of CD57+CD16- cells tended gradually to return to normal after one year or more in the group without GVHD but only after six years in patients in the other three GVHD groups.

[Postoperative pain relief by continuous epidural infusion of bupivacaine and buprenorphine].

Continuous postoperative pain relief produced by epidural block with bupivacaine and buprenorphine was evaluated in 12 patients after thoracotomy, 19 patients after upper abdominal surgery, and 14 patients after lower abdominal surgery. Patients initially received 8 ml of 0.25% bupivacaine and 0.1 mg of buprenorphine at recovery room in operating theater and continuously received the mixture of 0.25% bupivacaine and 5 micrograms.ml-1 buprenorphine at a rate of 1 ml.h-1 using a portable pump. About fifty percent of the patients did not need additional narcotics during 48 postoperative hours. About ninety percent of the patients needed one additional narcotics during 48 postoperative hours. The authors conclude that epidural analgesia with the mixture of bupivacaine and buprenorphine produces satisfactory postoperative pain relief.

[Pulmonary kinetics of 13N-ammonia in smoking subjects--a quantitative study using dynamic PET].

To evaluate the effect of smoking on nonrespiratory function in the lung, a dynamic PET with 13NH3 in the lung was performed in 18 normal volunteers without lung disease nor congestion (9 non-smokers, 4 exsmokers, 5 smokers). After intravenous bolus injection of 13NH3, twenty serial 5.5-second scans followed by six 30-second scans were performed. Regions of interests were assigned on the ventral, lateral and dorsal part of the right lung and time-activity curves were generated through 26 images. The activity curve demonstrated a biexponential clearance from the lung with fast and slow component. The retention fraction (RF), fractional size of the slow component, was calculated, and the half-times (t 1/2) of both components were also evaluated. A significant increase of RF and prolongation of t 1/2 of slow component were observed in smokers compared to non-smokers and exsmokers. However, no significant difference of RF nor t 1/2 of both components was observed between non-smokers and exsmokers. These results suggest that long term smoking may modify the pulmonary kinetics of 13NH3, but the change is reversible after cessation of smoking for one year or longer.

[Continuous epidural infusion of bupivacaine and buprenorphine for postoperative pain relief].

The efficacy for postoperative analgesia and side-effect of combined epidural infusion of bupivacaine and buprenorphine in comparison with each of these drugs alone were evaluated in 150 patients. All patients received initially bupivacaine 8 ml and buprenorphine 0.1 mg. In a random order, epidural infusion of 5 micrograms.ml-1 buprenorphine 1 ml.h-1 (group A; n = 50), 0.25% bupivacaine 1 ml.h-1 (group B; n = 50), or a combination of the two drugs 1 ml.h-1 (group C; n = 50) was continued for 48 h by a portable disposable device. The analgesic efficacy in group C was superior to that in group A or group B. No significant difference in the incidence of side-effect was found among the three groups. We conclude that epidural analgesia with the combination of buprenorphine and bupivacaine is safe, and easy to manage, giving pain relief superior to that provided by each of these drugs alone.

[Sevoflurane anesthesia for asthmatic patients: report of five cases].

We anesthetized five patients receiving antiasthmatics with sevoflurane-nitrous oxide-oxygen. Induction with sevoflurane inhalation was conducted uneventfully in all patients. Mild asthmatic attacks occurred in three patients during light anesthesia. The asthmatic attacks subsided with elevating the concentration of sevoflurane or the administration of aminophylline in two patients. In only one patient, inhalation therapy with salbutamol was required to treat an asthmatic attack during deep anesthesia. The results indicate that sevoflurane was a proper inhalation anesthetic for our asthmatic patients.

[Intramuscular degeneration process in Duchenne muscular dystrophy--investigation by longitudinal MR imaging of the skeletal muscles].

Intramuscular degeneration process of Duchenne dystrophy skeletal muscles was investigated by longitudinal skeletal muscle imaging with high-field-strength NMR-CT of 1.5 Tesla. Thigh muscles in 10 cases ranging in age from 4 to 19 years were examined by T1-weighed longitudinal images (TR = 215-505 ms, TE = 19-20 ms). Following results were obtained: (1) Skeletal muscle degeneration was depicted as high signal intensity area reflecting its high fat contents. (2) These high signal intensity areas had a longitudinally streaky appearance in parallel direction with myofibers. (3) These findings were more prominent toward myotendon junction than muscle bellies. (4) Skeletal muscle degeneration progressed rapidly between 7 to 10 years of age, and reached a plateau after that.

Diurnal variations of lipogenic enzyme mRNA quantities in rat liver.

The diurnal variations in mRNA quantities of lipogenic enzymes (acetyl-CoA carboxylase, fatty acid synthase, malic enzyme and glucose-6-phosphate dehydrogenase) in rat livers were detected. When the rats began feeding actively after lights out at 1900 h, the mRNA quantities were high from 0500 h to 0900 h in the morning. The variation in fatty acid synthase mRNA quantities was the most dramatic. However, no measurable variation in any enzyme levels including fatty acid synthase was detected. It may be because the half-lives of the enzymes are too long to be effected by the mRNAs which were high for several hours.

5-HT receptor-mediated regulation of thyrotropin-releasing hormone release in rat spinal cord.

5-Hydroxytryptamine (5-HT, 10 microM) increased the veratridine-evoked release of thyrotropin-releasing hormone (TRH) from spinal cord slices. This effect of 5-HT was reduced by pre-exposure of the tissue to 10 microM ketanserin and methysergide, which both have 5-HT2 antagonistic activity. A 5-HT1 agonist, 8-hydroxy-2-(di-n-propylamino)tetralin (10 microM), reduced the release of TRH. These results imply that 5-HT released from the terminals of descending fibers modulates the release of TRH, a cotransmitter contained in 5-HT fibers.

Chemoselective one-step purification method for peptides synthesized by the solid-phase technique.

The specific reaction between SH and iodoacetamide groups has been explored as the basis of an affinity-type purification procedure for peptides synthesized by the solid-phase technique. For this affinity-type purification procedure, we synthesized an SH precursor reagent bearing an acid-labile S-protecting group, pMB-SCH2CONHCH2CH2-SO2CH2CH2OCO2pNP (compound I), in which pMB is p-methoxybenzyl and pNP is p-nitrophenyl. Using this reagent, the procedure involves the following sequence of four reactions: (i) attachment of the SH function of compound I to the alpha-amino group of a peptide-resin through a base-labile sulfonylethoxycarbonyl linkage in the final step of solid-phase peptide synthesis, (ii) acid treatment to remove the S-pMB and side-chain-protecting groups employed and cleave the modified peptide from the resin, (iii) immobilization of the derived SH-peptide on an iodoacetamide-resin column, and (iv) base (5% NH4OH) treatment to release the desired peptide from the resin in nearly pure form. To facilitate this purification procedure, unreacted amino groups were acetylated in each step during solid-phase synthesis. The usefulness of this method was demonstrated by the purification of several peptides (18 to approximately 44 amino acids in length) synthesized by the 9-fluorenylmethoxycarbonyl (Fmoc)-based solid-phase technique. The principle of this affinity-type purification procedure may also be applied to the tert-butoxycarbonyl (Boc)-based solid-phase technique.

[MR imaging of the thymoma--differentiation of invasive thymoma from noninvasive thymoma].

Magnetic resonance imaging (MRI) findings were reviewed in nine patients with histologically confirmed thymoma. The morphologic findings obtained by MRI were useful in distinguishing invasive from noninvasive thymoma. Invasive thymomas (2 cases) showed irregular contours and broad obliteration of the fatty plane between the mass and great vessels on T1-weighted coronary images. Perivascular infiltration shadow was considered to be characteristic of invasive thymoma. Noninvasive thymomas (7 cases) were round or oval in shape and showed slight obliteration of the fatty plane. In conclusion, MRI was helpful in differentiating invasive from noninvasive thymoma and defining the extent of thymoma.

High-performance liquid chromatographic determination of histamine N-methyltransferase activity.

A method for the determination of histamine N-methyltransferase (HMT) activity by high-performance liquid chromatography based on post-column derivatization with omicron-phthalaldehyde is described. The determination involves the separation of the substrate, histamine, from its product. N tau-methylhistamine, using a weak cation exchanger, followed by on-line derivatization of these imidazoleamines with omicron-phthalaldehyde and their detection and quantitation with a fluorimetric detector. This assay method is suitable for the measurement of HMT activity during enzyme purification.

In vivo distribution of opioid receptors in man in relation to the cortical projections of the medial and lateral pain systems measured with positron emission tomography.

In vivo opioid receptor binding in the cortical projections of the medial (cingulate and prefrontal cortex) and lateral pain system (primary somatosensory cortex) in male volunteers has been quantitated using [11C]diprenorphine and positron emission tomography. High levels of opioid receptor binding were seen in the cortical projections of the medial pain system in the cingulate and prefrontal cortex as has previously been observed in post-mortem studies. However, a focal reduction of opioid receptor binding was observed and quantitated in the primary motor/sensory strip when compared to surrounding parietal cortex. This new finding suggests that the medial pain system is likely to be more susceptible to exogenous and endogenous opioid neuromodulation than the so-called lateral pain system.

[Evaluation of hepatic tumors by contrast ultrasonography with carbon dioxide].

For evaluating hepatic tumors, ultrasonograms using carbon dioxide by arterial injection (CO2US) was performed in 37 patients, including 28 hepatocellular carcinomas, 11 hepatic metastases, and 3 hepatic hemangiomas. Hepatic tumors were enhanced by CO2US and easily identified. CO2US of hepatic tumors were classified into six patients. Generally, on CO2US hepatocellular carcinomas were well enhanced, and hepatic metastases had ring-like enhancement. Hepatic hemangiomas showed peripheral patchy enhancement. In the patients with hepatocellular carcinoma, the primary tumors were better identified on CO2US than conventional sonography. The daughter nodules with hepatocellular carcinoma were also better demonstrated than other examinations. CO2US is helpful in evaluating and differentiating hepatic tumors.
