[Surgical method of retropubic prostatectomy].

The retropubic prostatectomy is performed mostly for prostatic hypertrophy. By performing the preventive suture-ligation of the bilateral prostatic arteries which are running among the vesicoprostatic pedicle before prostatectomy, massive bleeding from bladder neck and prostatic bed can be controlled well, and ligation of bilateral spermatic cord prevents retrograde epididymitis. This surgical procedure makes retropubic prostatectomy easy.

[clinical studies on primary renal pelvic tumors].

The 15 cases of the primary renal pelvic tumors treated at our Hospital between 1974 and 1983, were reviewed retrospectively. The patients ranged in age from 41 to 74 years old (average: 58.3 years old). There were 11 males and 4 females, the ratio being 2.8:1.0. The affected side was left in 9 cases and right in 6 cases. The most frequent symptom was macrohematuria, which was seen in 12 cases (80%). The major finding of IVP was non-functioning kidney, which was seen in 8 cases (53.3%). Positive urinary cytology was obtained in 8 cases (53.3%). As the surgical method, total nephroureterectomy with bladder cuff was performed in 8 cases, nephroureterectomy in one case and nephrectomy in 6 cases. Histologically, 14 cases were transitional cell carcinoma and one case was squamous cell carcinoma. Simultaneous urothelial tumors were seen in the bladder of 2 patients. A subsequent ureteral tumor was found in one of the 7 cases in which ureters were resected incompletely, and subsequent bladder tumors were found in 8 of the 15 cases receiving surgical treatment in the follow-up period. All of tumors were found within 2 years after operation. Over-all actual survival rates at 1, 3 and 5 years were 87%, 67%, 48%, respectively. Three and 5 year actual survival rates were 100%, 100% respectively for the low stage group and 59%, 29% respectively for the high stage group. Three and 5 year actual survival rates were 100%, 78%, respectively for the low grade group and 44%, 27% respectively for the high grade group. Among several factors, stage and grade of the tumor were the most influencing factors for prognosis.

[Clinical studies on primary ureteral tumors].

The 15 cases of the primary ureteral tumors treated at our Hospital between 1974 and 1983, were reviewed retrospectively. The incidence of primary ureteral tumors among the outpatients in our urologic clinic was 0.15%. The patients ranged in age from 50 to 75 years old (average: 65.5 years old). There were 11 males and 4 females, the ratio being 2.8:1.0. The right ureter and the lower third of the ureter were involved more frequently than other areas. The most frequent symptom was macrohematuria which was seen in 12 cases (80%). The major finding of IVP was non-functioning kidney, which was seen in 11 cases (73.3). Positive urinary cytology was obtained in 9 cases (60%). Twelve patients underwent nephroureterectomy with bladder cuff or total cystectomy. Histologically, all cases were transitional cell carcinoma. Simultaneous urothelial tumors were seen in the bladder in 4 cases (26.7%). The over-all actual survival rates at 1, 3 and 5 years were 59%, 42%, 42%, respectively. The 5-year actual survival rate was 63% for the low grade group and 0% for the high grade group. The 5-year actual survival rate was 82% for the low stage group and 0% for the high stage group. Among several factors, grade and stage of the tumor were the most influencing factors for prognosis.

[Clinical evaluation of lymphoscintigraphy in urological disease].

Clinical efficacy of lymphoscintigraphy was evaluated in 31 cases of urological disease. To diagnose metastatic change of malignant tumor, 99mTc-rhenium colloid (5mCi) was administrated from bilateral pedal region. Of these patients 12 had true positive finding, 2 had false positive finding, 2 had false negative finding, and 8 had true negative finding defined by lymphoscintigram and pathological finding. Overall accuracy of lymphoscintigraphy revealed 83% in correlation of pathological finding, 78% in lymphangiography. Accuracy between lymphangiography and pathological finding was 80%. The overall clinical efficacy of lymphoscintigraphy was the same as lymphangiography. The merit of this method was that we can examine easily, painlessly, and frequently.

[Two cases of primary urethral carcinoma in females].

Two cases of primary female urethral carcinoma are presented and discussed. The patients were 70 years old and 65 years old, and the pathological diagnoses were squamous cell carcinoma and adenocarcinoma, respectively. The patient with squamous cell carcinoma had metastasized inguinal lymph nodes at the first consultation, despite the administration of bleomycin, and the tumor had widely metastasized to skin of the lower extremities. She died of cachexia and disseminated intravascular coagulopathy (DIC). The other patient with adenocarcinoma was treated by postoperative cobalt radiation therapy in a total dose of 6,000 rads. After 18 months, she had recurrence of tumor in urethra, and developed Virchow's node metastasis, she died of acute renal failure. Both patients died within two years.

[A case of paraganglioma of the urinary bladder].

A very rare case of paraganglioma of the bladder is reported. The patient was a 51-year-old man. He was admitted to our hospital on June 18th, 1981, with the complaint of gross hematuria. He had no attack of hypertension and we could not find in him any parasympathetic symptom except hematuria. Upon urological examination, he was suspected to have stage B cancer of bladder. He received partial cystectomy with uneventful course during the operation on June 30th, 1981 and was discharged on the 15th postoperative day. The postoperative histological diagnosis was paraganglioma of the urinary bladder. He was confirmed to have normal levels of catecholamine and its metabolites in the urine after 4 months postoperatively. At present, he is asymptomatic.

[The concentrations of 60Co and stable cobalt in the migratory marine organism (author's transl)].

In order to investigate both the contamination level and distribution of radioactive nuclides in the marine environment, the concentrations of both 60Co and stable cobalt were determined. The samples collected from the sea around Japan were fishes, squids. The analytical method for 60Co was consisted of the radiochemical separation using ion exchange resins followed with the low background beta counting, and that for stable cobalt was the spectrophotometric determination. The results obtained were as follows: (1) The viscera of squids was found to enrich both 60Co and stable cobalt. (2) No 60Co was detected in fishes except the liver of bonito taken from the north west Pacific. (3) The correlation between 60Co and stable cobalt was rather good in the squids samples. (4) The enrichment factors for squids and fishes agreed well with the values reported by B. Patel or S.E. Tompson.

[The concentrations of 60Co and stable cobalt in the coastal marine organisms (author's transl)].

The concentration of 60Co and stable cobalt in the shellfishes and seaweeds which were collected from the coast around Japan were obtained by low background beta counting for 60Co and spectrometric determination of stable cobalt. The results obtained were as follows: (1) The 60Co was found in the shellfishes which were enriched in the stable cobalt. (2) The correlation between 60Co and stable cobalt was rather good in the shellfishes. (3) The activities of 60Co was found in the seaweeds taken from seashore near the nuclear power plant. (4) There was great differences in the specific activities between samples collected from contaminated and not contaminated area.

Dissolution of cystine stones by irrigated tiopronin solution.

A new method involving the introduction of tiopronin solution directly into the renal pelvis or bladder has been developed for the treatment of cystine stones. Therapeutic irrigation by tiopronin solution was done in 4 cases of residual cystine kidney stones using a nephrostomy tube, and in an infant with bladder cystine stones using a Foley 3-way catheter. In every case all of the cystine stones were dissolved completely. During the course of the treatment no abnormality was noted on blood, liver function or immunological tests. Neither have there been any negative side effects, such as eruption, anorexia, nausea, diarrhea or general fatigue. The significance of this irrigation process is that whereas nephrectomy, heminephrectomy or nephrolithotomy was necessary previously to remove kidney cystine stones, now only pyelolithotomy and nephrostomy tube insertion are necessary. Furthermore, an operation is unnecessary in cases of bladder cystine stone dissolution by tiopronin irrigation.

Pholipomycin, a new member of phosphoglycolipid antibiotics. I. Taxonomy of producing organism and fermentation and isolation of pholipomycin.

Pholipomycin is a new member of the phosphoglycolipid family of antibiotics. Taxonomic studies of the producing organism revealed that it has morphologically characteristic aerial mycelia in which two to three spores are borne usually on short and clavate side branches. The species name, Streptomyces lividoclavatus, has been proposed. Pholipomycin is produced mainly in the solid residue of the fermentation culture broth and is isolated by methanol extraction of the mycelial cake followed by purification on ion-exchange resin and silica gel chromatography.
