Intralymphocytic sodium and free calcium concentration in relation to salt sensitivity in patients with essential hypertension.

In order to clarify the relation between salt sensitivity and changes in intracellular sodium ([Na]i) and free calcium concentration ([Ca2+]i) after salt loading, [Na]i and [Ca2+]i were determined in lymphocytes of twenty patients with essential hypertension under a low salt diet (3 g/day) and a high salt diet (20 g/day) for seven days, respectively. They were classified as "salt-sensitive" (n = 10) or "nonsalt-sensitive" (n = 10) on the basis of the changes in blood pressure after salt loading. Both lymphocytic [Na]i and [Ca2+]i were significantly increased with salt loading in salt-sensitive patients (p less than 0.05 for both), while they were not affected by salt loading in nonsalt-sensitive patients. Lymphocytic [Ca2+]i showed a positive correlation with lymphocytic [Na]i under both low salt diet (r = 0.62, p less than 0.01) and high salt diet (r = 0.70, p less than 0.01) in all patients in both groups. In addition, a close and positive correlation was observed between the changes in lymphocytic [Na]i and those in lymphocytic [Ca2+]i after salt loading in all patients in both groups (r = 0.80, p less than 0.001). These results suggest that the increase in [Ca2+]i, possibly linked with the increase in [Na]i, may be involved in elevation of blood pressure in the salt-sensitive patients after salt loading.

The outcome of idiopathic Bence Jones proteinuria.

The authors studied two patients with idiopathic Bence Jones proteinuria (BJP) that fulfill all the criteria proposed by Kyle and Greipp. None had evidence of overt multiple myeloma, of its variants, of primary systemic amyloidosis, or of other lymphoid tumors. In a patient with kappa type idiopathic BJP an elevation of a labelling index was found when an evolving myeloma developed 2 years later. The other had benign lambda type BJP until he died of bronchogenic carcinoma after 14 years. In most of patients with idiopathic BJP overt multiple myeloma or systemic amyloidosis have developed after a long period. An elevation of labelling index in the course of illness is expected to be a premonitory sign for malignant transformation. Idiopathic BJP may be characterized by less nephrotoxicity or amyloidogenicity of Bence Jones protein synthesized as well as a slow growth rate of tumor cells.

An unsuccessful attempt to isolate human immunodeficiency virus (HIV) from patients with primary immunodeficiency diseases.

Isolation of the human immunodeficiency virus (HIV) was performed in 5 seronegative patients with primary immunodeficiency diseases (IDDs) who had no or little ability to produce antibodies and received a large amount of intravenous gammaglobulin (IVG) for a long period. In this study cultured cells were considered to be infected if the following 2 criteria were met: (1) They produced reverse transcriptase (RT) activity in the supernatant fluid and (2) expressed viral antigens. Under experimental conditions, virus was demonstrated by both RT activity and viral antigen expression detectable by immunofluorescence technique in lymphocyte cultures from 2 seropositive patients with acquired immunodeficiency syndrome-related complex who were examined as controls. There was no evidence of HIV infection in cultures of lymphocytes from all of the 5 patients with primary IDDs and from 2 healthy subjects. The above patients did not show the extremely low numbers of target T4+ cells, which make isolation of the virus in lymphocytes difficult. The present results suggest that current methods of IVG preparation either remove or inactivate HIV.

Benzo[a]pyrene metabolism by purified cytochrome P-450 from 3-methylcholanthrene-treated rats.

The metabolism of benzo[a]pyrene in reconstituted pulmonary mono-oxygenase systems has been studied. Metabolites formed by pulmonary cytochrome P450MC, a major form of pulmonary cytochrome P-450 isolated from 3-methylcholanthrene-treated rats, were analysed by h.p.l.c. The profiles of benzo[a]pyrene metabolites formed by the reconstituted P-450MC systems were different from that obtained with rat-lung microsomes, indicating the presence of several unknown metabolites in the reconstituted systems containing NADPH-cytochrome P-450 reductase and epoxide hydrolase. 3-Hydroxybenzo[a]pyrene was a major product formed by pulmonary cytochrome P-450MC, in the absence or presence of epoxide hydrolase. The addition of purified epoxide hydrolase to the reconstituted systems increased the formation of dihydrodihydroxy-benzo[a]pyrenes, particularly 7,8-dihydro-7,8-dihydroxybenzo[a]pyrene. The 9,10-dihydro-9,10-dihydroxybenzo[a]pyrene was the major dihydrodiol formed by pulmonary cytochrome P-450MC. By the addition of epoxide hydrolase the total amount of phenols decreased in parallel with an increased production of dihydrodiol, but the amount of quinones was not changed. Similar results concerning the related production of phenols and dihydrodiols, in the absence and presence of epoxide hydrolase, were obtained in reconstituted systems of hepatic cytochrome P-450MC, the major form of hepatic cytochrome P-450 from 3-methylcholanthrene-treated rats.

[Treatment of hepatocellular carcinoma with tumor thrombus in the main portal branch--analysis of five cases showing regression of the tumor thrombus].

Twenty-five cases of hepatocellular carcinoma with tumor thrombus in the first branch or trunk of the portal vein were treated with one-shot chemotherapy or transcatheter arterial embolization (TAE). Five of them showed regression of the tumor thrombus and survived 14 months on average. In these five cases, residual liver function was maintained at a good level. It was revealed that TAE was effective not only for tumors but also for tumor thrombi. But in cases with such tumor thrombus, normal liver tissue would be damaged by TAE. Therefore, residual liver function seems to be the most significant factor in choosing therapy.

Manifestation of cerebral malaria-like symptoms in the WM/Ms rat infected with Plasmodium berghei strain NK65.

Conditions required for the induction of cerebral malaria (CM)-like symptoms were investigated using 12 strains of rats and 5 murine malaria strains. Among various combinations, only inbred WM/Ms rats infected with P. berghei (NK65) developed neuropathological complications that closely resembled human CM cases. When young WM/Ms rats were infected with the parasites, neurologic signs were induced followed by death in 5-10 days with almost 100% incidence, whereas aged hosts revealed strong resistance. Histologically, edematous changes, occlusion of vessels, and petechial hemorrhages were found in the brain. There was an optimum dose of parasites to induce the manifestations, and a low incidence was obtained by increased or decreased inoculum size. No correlation was found between the level of parasitemia and incidence of the disease. The other 11 rat strains inoculated with this parasite showed high levels of parasitemia, but most of their infections were self-limiting or malarial death occurred without CM-like signs. Inoculation into WM/Ms rats with other murine malaria parasites, including P. chabaudi, P. vinckei, P. yoelii (17X), and P. yoelii (nigeriensis) failed to induce CM-like manifestations irrespective of the inoculation size and the degree of parasitemia. These results indicated that P. berghei (NK65)-infected WM/Ms rats represent an experimental model for CM and certain appropriate conditions are needed for its development in both parasite and host sides.
