Experimental study of pulpotomy with calcium hydroxide-iodoform paste in dogs' immature permanent teeth.

To investigate the healing process and continuous root development following pulpotomy with improved calcium hydroxide-iodoform paste, the authors performed an experimental study of pulpotomy in immature permanent teeth. Thirty five incompletely formed root canals in dogs were used, and radiographical and histopathological observations were made. The results lead to the conclusion that pulpotomy with improved calcium hydroxide-iodoform paste is a very effective treatment for immature permanent teeth.

[Comparative anatomical study on the ductal systems of rabbit and cat parotid glands].

A comparative study on the ductal systems of rabbit and cat parotid glands was performed by scanning and transmission electron-microscopies, and by light microscopy. Stereological structures of both ductal systems, which were observed as resin casts, resembled one another closely. Each branch showed repeated tree-like ramifications and the striated duct and the intercalated duct took a short linear course. The development of these ducts was poor. Numerous micromorphological differences and resemblances between rabbit and cat were found: Acinus 1. Rabbits had serous acini; and cats, seromucous ones. 2. In both specimens, secretory material was present in the intercellular spaces and these spaces were enlarged. 3. Secretory granules surrounded by a limiting membrane were present in the terminal lumen in cats, but not in rabbits. Intercalated duct 4. Small secretory granules occupying the supranuclear portion were present in rabbits and cats except the proximal region of the striated duct in rabbits. 5. Spherical secretory granules were present in the lumen in cats, but not in rabbits. Striated duct 6. The walls of the duct in both animals were composed of three types of cells; the light cell, the dark cell, and the basal cell. 7. Small secretory granules occupied the apical portion of the light cell in cats. 8. Spherical secretory granules were observed in the lumen in cats, but not in rabbits.

[Idiopathic enlargement of the right atrium with persistent superior vena cava].

A 69-year-old male, who had been found (during a routine roentgenogram of the chest at an annual check-up) to have cardiomegaly, had been asymptomatic until 63 years of age, when DOE and swelling of the legs appeared. He visited our hospital because these symptoms has become more severe. Physical examination revealed JVD, third heart sound and a grade 3 holosystolic murmur, hepatomegaly and edema in the lower legs. His ECG showed atrial fibrillation. His chest X-P showed marked cardiomegaly and rt pleural effusion. His echocardiography and MRI revealed a marked enlargement of the right atrium and a slight enlargement of the right ventricle. The latter also showed persistent left superior vena cava. The cardiac catheterization, angiocardiography and intracavitary electrocardiography revealed no organic cardiac disease which induced enlargement of the right atrium. The idiopathic enlargement of the right atrium is a rare disease. Patients suffering from this disease are asymptomatic in most cases. We reported the idiopathic enlargement of the right atrium with persistent left superior vena cava in this paper.

[Acute myocardial infarction induced by lung cancer chemotherapy with cisplatin and etoposide].

From June of 1986 to May of 1989, we encountered 78 cases of lung cancer and administered CDDP and etoposide, orally and intravenously. We found 3 cases of acute myocardial infarction (AMI) onset after this chemotherapy regimen. It is important to detect AMI onset as soon as possible. We must take prophylactic procedures for patients with coronary risk factors who have had radiation therapy previously.

[Persistent left superior vena cava to left atrial communication detected with radionuclide angiography: a case report].

A persistent left superior vena cava (PLSVC) is the common clinical congenital abnormality of the greater central veins. This anomaly may occur as an isolated lesion, but rarely it is significantly associated with other congenital cardiac lesions, particularly it drain into the left atrium. We recently encountered a patient in whom PLSVC to left atrial communication with atrial septal defect was diagnosed on the basis of radionuclide angiography with Tc-99m pertechnetate. Radionuclide angiography might be the noninvasive study of choice in patients suspected of having PLSVC by the clinical findings or chest roentgenography.

[Progress of imaging diagnosis for the cancer therapy--breast cancer].

As two main modalities for the breast cancer, ultrasonography (US) and mammography (MMG) were discussed about the efficacy and limitation of qualitative diagnosis. The accuracy rate of US and MMG for the breast cancer were 90.0% and 92.7% at the Aichi Cancer Center Hospital during these 2 years (1986-1988). The number of patients with the breast cancer is increasing recently in Japan. This cancer, however, is one of the curable cancer at the early stage. Therefore, it is very important thing to detect a small malignant mass less than 1cm in a diameter by using these modalities. In addition, the establishment of the mass screening system for breast cancer is desired. In order to make a precise diagnosis, it is necessary to add some devices on the ordinary modalities. For examples, mechanical improvement of the Moribuden tube with 0.1mm focus, operation of ductgraphy and cystography by double contrast method in MMG and application of 10MHz transducer in US. Furthermore, needle biopsy under US or X-ray is expected positively to make a fast diagnosis. On the other hand, DMR (Digital Mammo-Radiography) system has a bright prospect for the mass screening of the breast cancer in the respect of efficacy, cost and radiation hazard. DMR system has many advantages as follows, 1. The system is capable of bringing out clearly very minor differences of contrast. 2. Tumor shadows can be easily displayed. 3. The lateral projection only is sufficient (allowing examination of the chest wall also). 4. The X-ray dose is small: 0.03-0.05mSv. 5. The equipment is inexpensive to use. 6. Patient positioning is simple, and examinations take little time. 7. Long operator training programs are unnecessary. 8. The system uses digital signals, and so automatic diagnosis is possible. 9. Theoretically, at least, tumors as small as 0.2 mm in diameter can be detected.

[Clinical evaluation of cefuzonam of severe infections in leukemia and related disorders].

Cefuzonam (CZON) which has a broad spectrum on both Gram-negative and Gram-positive bacteria including methicillin-resistant Staphylococcus aureus was evaluated in severe infections associated with hematological disorders. Sixty five patients were treated with CZON. Among them, 56 patients were evaluable for effectiveness. Nine patients were not evaluable because 3 patients were treated with combination of other antibiotics such as ceftizoxime, norfloxacin, ofloxacin, 1 patient was subjected to additional therapy of G-CSF and gamma-globulin, 4 were the patients with other disease than hematologic disorder (3 malignant mesotheliomas, 1 ovarian cancer), and the remaining one was prophylactically treated. Excellent responses were observed in 21 (37.5%) patients, good responses in 11 (19.6%) patients, with an overall efficacy rate of 57.1%. The efficacy rate in septic patients was 80% (4/5), and that in patient whose peripheral granulocytes were continuously below 100/microliters was 60% (3/5). Three patients who suffered from malignant mesothelioma, one patient who suffered from ovarian cancer, one patient who was treated prophylactically were included in the final evaluation of side effects. Side effects were observed in 2 patients (2/61, 3.3%). In a patient of 7 years, mild liver disfunction (GOT/GPT, 46/55) was found in 10 days after CZON treatment was started. In a patient of 65 years, mild appetite loss was identified in 2 days after CZON administration was begun. The liver disfunction was improved soon after the cessation of the treatment. The mild appetite loss disappeared while the treatment was continued. These results showed that CZON was an effective and safe antibiotic for the treatment of severe infections in patients with hematological disorders.

[A Japanese family with hereditary HbH disease--a case report and its gene analysis].

Hereditary HbH disease was found in a Japanese family. The propositus showed hypochromic microcytic anemia and chronic hemolysis. HbH inclusion bodies were detected in red cells, and an abnormal band corresponding HbH was found in an isoelectric focusing of the hemolysate. Gene analysis of the propositus revealed double heterozygosity for alpha + and alpha zero thalassemias. Four of six his siblings shared the alpha zero thalassemia and one the alpha + thalassemia. Another one was normal. The alpha + thalassemia was of 3.7 kb-deletion type and alpha zero was close to Southeast Asian type. This is the fourth Japanese family with hereditary HbH disease.

[Tumor imaging using 99mTc-hexamethylpropyleneamine oxime].

Tumor imagings have been performed in 32 tumor sites in 28 patients with a variety of malignancies by using 99mTc-HM-PAO. They have been compared with radionuclide angiography and conventional tumor imagings such as 67Ga citrate or 201TlCl scanning, and then their possible values have been evaluated for the investigation of tumor blood flow. Tumors were clearly visualized in 19 out of 32 sites, with 59% of positive rate. Good correlation was observed between the radioactivity in the tumor site on static image of 99mTc-HM-PAO and the radioactivity at arterial phase on radionuclide angiography, which represents tumor blood flow. The radioactivities in the marginal regions tended to be stronger than those in the center of large tumors. These results indicated that tumor imaging by 99mTc-HM-PAO mainly reflected tumor blood flow. On the other hand, 99MTc-HM-PAO imagings were not well correlated with 67Ga-studies, while they were coincided with all 201Tl scannings performed. It suggested that the affinity of 99mTc-HM-PAO to capillary wall also involved in its accumulation in the tumor. In conclusion, tumor imaging by 99MTc-HM-PAO was able to estimate tumor blood flow, and this examination would be helpful for the management of malignancies.

[Invasive fungal infections in hematologic malignancies--a retrospective study of 61 autopsied cases].

In a review of 61 consecutive autopsy cases with a hematologic malignancy, said cases extending from April, 1984 to August, 1989, 34 cases were documented to have had an invasive fungal infection. The highest rate of incidence was found in various leukemia cases (69 to 100%), followed by those who had had a malignant lymphoma (50%) and a multiple myeloma (33%). Cultures from autopsy materials that determined the presence of a fungus were positive in 21 cases, including 13 cases of candidiasis, 8 cases of aspergillosis, and 2 cases of geotrichosis. The most frequent site of the fungal infection was in the lungs (76%), followed by the GI tract, the kidneys, the liver, and the spleen. Of 36 cases that had been treated with an empiric antifungal therapy, an invasive fungal infection was documented in 22 cases, half of them being fatal. In contrast, of 20 cases that had not received any antifungal treatment prior to death, an invasive fungal infection was found in 8 cases and three of these were fatal.

[A primary malignant melanoma of the small intestine].

In October 14, 1982, a 30-year-old male was admitted to the hospital because of a left flank pain and melena. On undergoing testing, an upper gastrointestinal series revealed barium retention in the small bowel, in which an abdomen X-p showed an abnormal gas shadow. Endoscopic examinations revealed depressive lesions, and the histological diagnosis was malignant neoplasm with a duodenal wall infiltration. A superior mesentric arteriogram showed the dilatation of the media colic artery and neovascular lesions. Thus a leiomyosarcoma of the small intestine was suspected, and the patient underwent surgery on November 15. The tumor was sack-like and looked black; histologically it was diagnosed as a small intestine, because the primary origin was only detected in the small intestine. Four months after the operation, the tumor recurred and the patient died in June, 1983.

[Acute myelogenous leukemia (M2) simultaneously associated with multiple myeloma with special reference to chromosome abnormality of t(6; 14) (p21.1; q32.3)].

A 73-year-old male was admitted to our hospital in October 1987 because of severe anemia, anorexia, and loss of weight. The hemoglobin level was 5.7 g/dl, the white blood cell count 2,500/microliters with 5% myeloblasts positive for peroxidase, and the platelet count 8.6 x 10(4)/microliters. The LDH was 656 mU/ml, the total protein in the serum 7.4 g/dl, IgG 419 mg/dl, IgA 104 mg/dl, IgM 10 mg/dl, and urine Bence Jones (BJ) protein 8.8 g/day. The X-ray survey of the bones showed multiple osteolytic lesions. A bone marrow aspirate was hypercellular with 91.4% plasma cells, and was cultured a whole day for chromosome study. It revealed an abnormal karyotype of 46, XY, -15, t(6; 14) (p21.1; q32.3), +der(15)t(1; 15) (q23; q24). Immunoelectrophoresis demonstrated lambda type BJ protein. He was treated with melphalan and prednisolone. Proteinuria and marrow plasma cells decreased in amount. In December a white cell count was 6,030/microliters with 80% myeloblasts. A bone marrow aspirate revealed an increase of 82.6% myeloblasts or promyelocytes. The patient was refractory to chemotherapy and died of sepsis in April 1988. An unrelated abnormal karyotype; 48, XY, +8, +13 appeared concomitant with an increase of the leukemic cells, but no cells showed the t(6; 14). We cytogenetically discussed the simultaneous presence of multiple myeloma with acute myelogenous leukemia.

[Eel calcitonin treatment on patients with urogenital carcinoma for relief of pain from metastatic bone lesions].

Forty-seven patients of advanced urogenital carcinoma with bone metastasis were treated with eel calcitonin (CT) to relieve severe pain from metastatic bone lesions. Patients were 45 males and 2 females with a mean age of 72.9. CT was administered intravenously at a daily dose of 160 units for 10 days. The efficacy of CT on relief of pain was estimated using a mark sheets filled by each patient and his or her doctor. And also the amount of analgesics given to patients before and after the administration of CT were checked. CT was effective on 77% of patients to reduce severe bone pain, especially on osteoblastic lesions metastasized from prostatic carcinoma. CT administration decreased the amount of analgesics in 37% of the cases. As toxicity, nausea and vomiting which stopped the CT administration were observed in only one From these results, we conclude that CT is quite useful drug for relief of severe bone pain from metastatic lesions in patients with urogenital carcinoma.

A 165-base pair sequence between the dihydrofolate reductase gene and the divergently transcribed upstream gene is sufficient for bidirectional transcriptional activity.

The dihydrofolate reductase gene encodes a key enzyme of one-carbon metabolism and is constitutively expressed in all cells. Recently, transcripts initiated at 89 base pairs upstream from the transcriptional initiation site of the dihydrofolate reductase gene and transcribed from the opposite strand have been identified and shown to encode for a protein with homology to a bacterial DNA mismatch repair enzyme (Fujii, H., and Shimada, T. (1989) J. Biol. Chem. 264, 10057-10064). Therefore, the two genes are organized in a head-to-head configuration separated by an 89-base pair segment. The promoter activities of this short spacer sequence were studied in a transient assay using the chloramphenicol acetyltransferase and the guanine phosphoribosyltransferase genes as reporters. A 165-base pair fragment from -111 to +54 relative to the dihydrofolate reductase initiation site was shown to be sufficient for transcriptional activity in either direction, suggesting that expression of the two divergent genes is regulated by a bidirectional promoter that may use common regulatory elements.

Cloning and structure of a yeast gene encoding a general transcription initiation factor TFIID that binds to the TATA box.

The TATA sequence-binding factor TFIID plays a central role both in promoter activation by RNA polymerase II and other common initiation factors, and in promoter regulation by gene-specific factors. The sequence of yeast TFIID, which seems to be encoded by a single gene, contains interesting structural motifs that are possibly involved in these functions, and is similar to sequences of bacterial sigma factors.

Human adenylate kinase deficiency associated with hemolytic anemia. A single base substitution affecting solubility and catalytic activity of the cytosolic adenylate kinase.

Adenylate kinase deficiency in the erythrocyte is a rare genetic disorder associated with hemolytic anemia. To determine the molecular basis of this disorder, we first cloned the normal gene encoding human cytosolic adenylate kinase (AK1) and determined the structure. The gene was 12 kilobase pairs long and was split into 7 exons. The structures of 5'- and 3'-flanking regions were determined by primer extension and RNA blot analysis. The results showed that two species of mRNA with 0.9 and 2.5 kilobases, which differed at the 3'-end portion, were generated by the AK1 gene. Alu sequences were found in the largest intron (intron 5) and in the noncoding region of exon 7. Next, both alleles of the AK1 gene were cloned from DNA of a patient bearing the adenylate kinase deficiency and their nucleotide sequences determined. A transition (C----T) was found in exon 6 on an allele, which resulted in an Arg to Trp (CGG----TGG) substitution at the 128th residue of AK1. Since chicken AK1 is highly homologous to human AK1 with respect to the amino acid sequence, we introduced an Arg to Trp substitution to chicken AK1 at the same position by oligodeoxynucleotide-directed mutagenesis. The mutant chicken AK1 expressed in Escherichia coli showed a reduced catalytic activity as well as a decreased solubility and a change in affinity to phosphocellulose. Thus it was considered that the observed C----T transition was a cause of the decreased AK1 activity of the patient's erythrocyte. Analysis on phosphocellulose chromatography of erythrocyte AK1 of the patient and parents revealed that the patient's mutant allele was derived from the mother.
