Immunohistochemical distribution of intestinal 15 kDa protein in human tissues.

The distribution of human intestinal 15 kDa protein (I-15P), a new fatty acid-binding protein (FABP), was observed in normal tissues using immunohistochemical techniques. The antiserum against human I-15P intensely reacted with the villous epithelium of the terminal ileum but not with the enterocytes of the crypts. Although the surface epithelium of the stomach and villous epithelium of the duodenum showed weak reactivities, the epithelial cells of the jejunum, proximal ileum, colon and rectum, and also glandular epithelia with intestinal metaplasia of the stomach were not immunostained. The other human tissues examined were negative for anti-human I-15P antibody. Human I-15P thus represents a distinctive, confined tissue distribution different from the other FABPs and is expected to serve as a useful cellular marker of terminal ileal enterocytes.

Adult T cell leukemia clinically manifested with ascites and pleural effusion.

A 62-year-old man with adult T cell leukemia (ATL) presented with complaints of anorexia and abdominal fullness. Evaluation revealed ascites and pleural effusion, but no lymph node swelling, hepatosplenomegaly, or skin involvement. The diagnosis of ATL was made by the detection of specific surface markers for T lymphocytes in pleural effusion and ascitic fluid, and by determination of human T cell leukemia virus type I (HTLV-I) proviral DNA integration in mononuclear cells of pleural effusion. This case was considered a rare type of ATL with infiltration of the pleura and peritoneum.

Anatomic comparison of the ipsilateral and the contralateral testis in intravaginal torsion.

Sixteen cases of intravaginal testicular torsion were studied to compare the intrascrotal anatomy with that of the contralateral testis and to determine possible anatomic predisposition to testicular torsion. The anatomy of the contralateral testis was not always similar to that of the ipsilateral testis, and four of the contralateral testes were judged to be without risk of torsion.

[Effects of chronic low-frequency electrical stimulation on the external urethral sphincter of male rabbits--electrophoretic analyses of myosin light and heavy chain isoforms].

The present study investigates the effects of chronic low-frequency electrical stimulation on the external urethral sphincter (EUS) of rabbits through the biochemical analysis of the isoforms of myosin light and heavy chains. Twenty-eight adult male rabbits were used in the test. Of these, the EUS of 14 rabbits were continuously stimulated directly at 1.8 Hz, 0.8 msec, 5.0 V with an implantable electrical cardiac pacemaker for more than 10 weeks. The other 14 rabbits were used as the control group and were nurtured under the same conditions as the stimulated group but without the electrical stimulation. Upon conclusion of the stimulation program, the urethra was removed from all 28 rabbits. The portion of the urethra containing the EUS (from the middle of the prostate to the pelvic diaphragm) was cut transversely into thin serial sections and glycerinated. The glycerinated muscle fibers were then isolated under a stereomicroscope and samples for electrophoretic analysis were prepared. Two-dimensional electrophoresis according to the procedure of O'Farrell for myosin light chains and SDS-PAGE containing 40% glycerol for myosin heavy chains were carried out. The molar ratio of myosin subunits was determined by quantification through the dye elution process. The average percentages of slow myosin light chain molecules in 8 unstimulated and 8 stimulated EUS were 33.4 +/- 8.9% (mean +/- SD) and 70.1 +/- 12.8%, respectively. The average percentages of slow myosin heavy chain molecules in 6 unstimulated and 6 stimulated EUS were 17.6 +/- 5.7% and 40.2 +/- 7.1%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Peripheral blood stem cell collection with high dose etoposide].

Peripheral blood stem cells (PBSC) were collected from 24 patients who were treated with high dose etoposide. Studied patients included one with acute lymphoblastic leukemia, 4 with acute myeloid leukemia (AML), 1 with myelodysplastic syndrome, 13 with lymphoma, 1 with malignant histiocytosis, 2 with myeloma, and 4 with testicular tumor. Etoposide was infused at a dose of 500 mg/m2 for 4 days, followed by subcutaneous injection of recombinant human granulocyte-colony stimulating factor from the nadir of leukocyte. PBSC were collected by processing 15-20 liters of blood apheresis in the recovery phase of chemotherapy. In all patients, the number of CFU-GM collected per aphereresis ranged from 0.01 to 59.4 x 10(5)/kg, and more than 5 x 10(5)/kg CFU-GM were collected in 19 of the patients (73%). All leukemia patients treated along with our protocols have remained in complete remission, but one patient with AML relapsed within 1 month after the treatment. Ten lymphoma patients were assessable for antitumor effect, and complete response (CR) was observed in 2, partial response (PR) was 7, and no change (NC) in one patient. Two patients with myeloma were classified to be NC. Three of the 4 patients with testicular tumor were PR, and the other one was NC. Eleven patients subsequently underwent PBSCT. The number of days required to achieve an absolute granulocyte count of 0.5 x 10(9)/l was 7 to 11 days, with a mean of 8.6.

[A case of secondary valve replacement caused by pulmonary bioprosthetic valve endocarditis].

A 42-year-old male with pulmonary bioprosthetic valve endocarditis accompanied by residual minor leakage through a previously closed patch for ventricular septal defect (VSD), underwent reoperation with a Carpentier-Edwards bioprosthetic valve. The patient had a history of pulmonary valve replacement and VSD in 1973. A massive vegetation on the pulmonary valve was demonstrated by echocardiography. Five repeated blood cultures yielded Eikenella corrodens. After medical treatment, reoperation was performed. The patient was free of complications after the procedure. Although bioprosthetic valves have potential problems of dysfunction and calcification in long-term use, these problems develop at a significantly slower rate in right-sided positions compared with left-sided positions and bioprosthetic valve thrombosis in the pulmonary position has apparently not been reported. Mechanical prostheses for pulmonary valve replacement have a poor prognosis, with a high incidence of valve thrombosis despite adequate anticoagulant therapy. For the replacement of prosthetic valves in right-sided positions (tricuspid and pulmonary), bioprosthetic valves are now our first choice.

[Glucose-6-phosphate dehydrogenase].

Glucose 6-phosphate dehydrogenase (G6PD) plays a key role in the generation of NADPH which is essential for maintaining glutathione in the reduce state, and in the production of ribose 5-phosphate for the synthesis of nucleotides. G6PD in its active form is either a dimer or tetramer consisting identical subunits. The gene for G6PD is located on the X chromosome. Human red cell G6PD consists of 515 amino acids with a molecular weight of 59,265 daltons. Deficiency of G6PD is the most common metabolic disorder, and is associated with chronic and drug- or infection-induced hemolytic anemia. It is estimated that 400 million people in the world are affected. The mutations responsible for about 73 variants have been determined. Some of them have polymorphic frequencies in different populations. Except for seven kinds of variants with small gene deletion, splice site deletion of intron or three nucleotide substitutions, all of those were found to be produced by one or two nucleotide substitutions. Molecular studies disclosed that all the class 1 variants associated with chronic hemolysis have the mutations surrounding either the substrate or the NADP binding site.

Nuclear proteins involved in cell division in the ciliate, Tetrahymena.

For the purpose of clarifying the presence of nuclear proteins which may induce cell division, we conducted experiments using ciliate Tetrahymena which can be synchronized at the G2 phase in the cell cycle easily by periodic heat shock treatment (HST): We obtained proteins from the nuclei isolated from the cells grown at the early mid log-phase, at the G2 phase (60 min) and at the G1 phase (150 min), after HST. The proteins were studied by comparing the spots separated by isoelectric point, 10-20% gradient SDS two-dimensional electrophoresis. As a result, in comparison with the intranuclear protein at the log-phase, the proteins at the G2 phase showed a marked increase, but with no great change in the electrophoretic pattern. Meanwhile, the proteins at the G1 phase differed greatly from those at the G2 phase not only in the quantitative changes, but in the electrophoretic patterns. It is considered that the level of the accumulation of the nuclear proteins which should be closely involved in cell division must increase markedly at the G2 phase and decrease at the G1 phase. We confirmed the presence of four proteins: pI 5.8 MW 68 kDa, pI 6.1 MW 75 kDa, pI 8.6 MW 48 kDa and pI 6.6 MW 57 kDa, and then prepared monoclonal antibodies using these nuclear proteins as antigens. Among them, the antibody (IgM) against the pI 8.6 MW 48 kDa polypeptide (p48) was recognized the nuclei by indirect immunofluorescence in ancellular system at the S, G2 and the mid-M phases. However, nuclei at the late M and G1 phases were not stained.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of focal encephalitis with psychological symptoms similar to chorea minor].

We reported a patient with choreic movements, emotional lability, and compulsive-obsessive behavior that developed 4 weeks after onset of fever and lasted for several years. There was no evidence of streptococcal infection or rheumatic fever. T2-weighted MRI showed hyperintense lesions in the bilateral caudate nuclei and putamina. A diagnosis of focal encephalitis was made according to initial fever, convulsion, and CSF pleocytosis. Treatments with haloperidol and prednisolone were effective in some degree. The neurobehavioral syndrome as well as the involuntary movements in this patient can be attributed to the striatal damage, which may disrupt the basal ganglia-limbic-frontal lobe tracts. A symptomatic similarity between the syndrome in this patient and chorea minor suggests a striatal damage in chorea minor, where the causative lesions remain unknown.

[Treatment of multiple myeloma with peripheral blood stem cell transplantation].

Dose intensive chemoradiotherapy with peripheral blood stem cell transplantation (PBSCT) has emerged as a useful treatment for patients with multiple myeloma. High-dose melphalan (140 mg/m2) combined with total body irradiation is usually an effective regimen for refractory myeloma. Based on the greater efficacy of the tandem autoplants with high-dose melphalan (200 mg/m2) for refractory myeloma, a similar approach is currently under investigation in untreated patients. A considerable relapse rate is still observed after high-dose chemoradiotherapy supported with PBSCT. It is unknown whether these relapses are the consequence of inadequate tumor reduction or tumor cells contamination in the grafts or a combination of both.

[Noninvasive two-dimensional analysis of iridial circulation using the laser speckle phenomenon].

We have developed an apparatus using the laser speckle phenomenon for noninvasive two-dimensional measurement of the peripheral circulation of the iris. The normalized blur of the speckle pattern (NB-value) was adopted as an index of the blood velocity in the tissue. It was found that the average NB-value of the pigmented rabbit iris was gradually increasing from pupil margin to periphery. The coefficient of variation of the NB-value in the middle area of the iridial width was the smallest. The coefficient of reproducibility of the NB-value measured in the same area twice at 5-minute intervals was 8.8%, and that at 24-hour intervals was 14.1%. The NB-value of the iris showed a good correlation (r = 0.57, p < 0.0072 n = 20) with the iridial blood flow rate measured at the same time by the microsphere technique when intraocular pressure was adjusted to 20 mmHg or 40 mmHg. This result suggests that the NB-value can be useful for quantitative assessment not only of the blood velocity but also of the blood flow rate in the iridial tissues.

[Effect of topical timolol on the blood velocity of the iris microcirculation and the aqueous veins in human subjects].

To clarify the effect of topical timolol on the microcirculation in human eyes, the blood velocity changes of the iris microcirculation and the aqueous veins were measured before and 90 minutes after instillation of timolol in 8 eyes of 8 healthy volunteers by laser speckle flowgraphy. Systemic blood pressure, heart rate and intraocular pressure were measured before and 90 minutes after the instillation. The fellow eyes were measured as controls. The blood velocity decreased in the iris microcirculation (10% decrease, p < .05) and in the aqueous veins (8% decrease, p < .02) 90 minutes after instillation of timolol. The diameter of the aqueous veins did not change, so that the blood flow in the aqueous veins decreased (6% decrease, p < .02) after instillation. The intraocular pressure and pulse rate decreased (20 and 8% decrease, p < .002 and < .001, respectively) 90 minutes after the instillation. The blood velocity, vessel diameter, and intraocular pressure of fellow eyes and systemic blood pressure showed no significant change. These findings indicate that topical instillation of timolol induces decrease of blood flow in the iris, and, possibly, in the ciliary body. The instillation of timolol may decrease aqueous humor formation, and blood flow in aqueous veins.

[A Japanese family with congenital abnormal plasminogen].

A Japanese family with congenital abnormal plasminogen is reported. The patient was a 44-year-old male with no past history of thrombosis. Since only the plasminogen (PLG) activity was reduced on laboratory tests before surgery for lumbar disc herniation, coagulation and fibrinolysis studies were performed in the patient and his family. The patient underwent resection of the nucleus pulposus and posteriorlateral fixation of the lumbar spine. The PLG activity was 8% in the patient and his sister, 55% in his father, and 53% and 48% in his nephew brothers. The PLG antigen level was normal in all members of his family examined. IEF of PLG antigen showed abnormal patterns in which all bands were shifted slightly to the cathode side in the patient and his sister, but his father and nephew brothers exhibited duplicated bands showing combinations of normal and abnormal patterns. From these results, the proband and his sister were considered to be homozygotes, and his father and nephew brothers to be heterozygotes for congenital abnormal plasminogen. Acute reactant substances (fibrinogen, CRP, CPK, C1IN, alpha 1AT, etc.) and PIC (plasmin, alpha 2-plasmin inhibitor complex) increased after the operation due to the surgical insult, but the surgery did not trigger thrombosis. This patient is considered not to have developed thrombosis although he was a homozygote for congenital abnormal plasminogen, because the anticoagulation process until thrombogenesis was normal.

[An apparatus using laser speckle phenomenon for noncontact two-dimensional analysis of microcirculation in optic nerve head or choroid in human eyes].

The previously reported apparatus using the laser speckle phenomenon for noncontact two-dimensional analysis of the fundus peripheral circulation was improved so that peripheral circulation in the optic nerve head (ONH) or choroid in human eyes can be measured on a real-time basis. The fundus was illuminated by a diode laser spot and its image speckle was detected with an area sensor. The NB value, a quantitative index of blood velocity, was calculated by the logic board every 0.125 sec successively for 7 sec. Using this apparatus, the NB averaged over the measurement field located in the temporal site of ONH, free of visible surface vessels (NBONH), and that located in the posterior choroid (NBCh) were measured in normal human eyes. The coefficients of reproducibility of 5-minute interval measurements were 11.7% for the NBONH averaged over 5 pulses (mean NBONH) and 8.7% for the NBONH averaged over 5 pulses (mean NBCh), and those of 24-hour interval measurements were 13.0% for the former and 9.7% for the latter, respectively. The average of pulsatile component of NBONH was 38.4% of mean NBONH and that of NBCh was 26.6% of mean NBCh.

Intussusception of the cecum with calcified fecaliths.

We treated a patient with a complete invagination of the cecum that contained two large laminated calcified fecaliths. Colonofiberscopy showed a dimpling submucosal tumor that was palpated as a bony hard tumor at laparotomy. This finding suggested that the fecaliths caused intussusception of the cecum and that such an intussusception should be suspected when a bony hard, dimpling submucosal tumor is found in the cecum. To our knowledge, this is the first reported case of a cecal invagination accompanied by calcified fecaliths without an appendical intussusception.

The confusion associated with breast cancer chemotherapy in Japan: the first year's experience at the Division of Oncology and Hematology, National Cancer Center Hospital East.

The current status of breast cancer chemotherapy in Japan was examined and compared with internationally accepted standard therapy, by reviewing the previous treatment of patients with advanced or metastatic breast cancer who had been referred to the National Cancer Center Hospital East. Forty patients were referred mainly from middle-sized or large hospitals between July 1992 and June 1993. The most commonly used regimen for adjuvant therapy was a combination of long-term and oral low-dose fluoropyrimidine compounds (LDFU), which is rarely used in Western countries, and tamoxifen. Some patients had received perioperative intravenous mitomycin C. The adjuvant polychemotherapy most commonly used internationally has been given to only a few patients. The first-line treatment for advanced or metastatic disease in the previous hospitals varied, but myelosuppressive chemotherapy including anthracyclines was given to half of the patients. Oral LDFU was also used for some patients alone or in combination as the first-line chemotherapy. Local therapy for metastasis to soft tissue was performed in 9 patients. Oral LDFU therapy that is frequently used for cancer treatment in Japan should be evaluated in a well designed controlled trial.

[Surgical treatment of acute aortic dissections--a clinical study using a ringed intraluminal graft].

From 1984 to 1994, surgery was performed using a ringed intraluminal graft (RIG) in 75 patients with acute aortic dissection (DeBakey's type I in 37 cases, type II in 10 cases and type III in 28 cases). The operative death rate was 24% for type I, 30% for type II and 21% for type III. The average time from onset to surgery was short (47 +/- 67 hours for type I, 34 +/- 36 hours for type II and 47 +/- 77 hours for type III). The outcome of these cases indicated that this technique was effective for saving the lives of patients in the acute early stage of aortic dissection. No characteristic complications developed after this surgery. The postoperative course of patients was followed by using CT scans, MRI, angiography. No patients developed aneurysmal formation in the ligated area or dislocation of the RIG. There were no deaths directly attributable to the RIG. Patients who were autopsied in the late postoperative period showed no aneurysm of the ligated area or necrosis of the aortic wall. In conclusion, RIG surgery effectively saved the life of patients with acute aortic dissection and the RIG could be used as prosthetic graft for long-term periods.
