Rational therapeutic drug monitoring.

The simple act of ordering a serum drug level does not guarantee that the information will be meaningful or useful. The interpretation of serum concentrations can be profoundly influenced by such factors as the timing of the sample, the patient's clinical state, the drug's pharmacokinetics and metabolism, and the tube type and analytic methodology used. The likelihood of obtaining clinically meaningful and useful results can be maximized when these factors are taken into account.

Necrotizing lesions in Sjögren's syndrome. I. An immunofluorescent study.

Necrotizing lesions were found in labial salivary glands from patients bearing Sjögren's syndrome associated to rheumatoid arthritis, systemic sclerosis and systemic lupus erythematosus. In small and well circumscribed areas, parenchyma necrosis was accompanied by oedema, polymorphs and plasma cells. Immunoglobulins and complement deposition was demonstrated in serial sections. The immunological basis of these lesions and their significance in Sjögren's syndrome are discussed.

Necrotizing lesions in Sjögren's syndrome. II. An electron microscopic study.

Further evidences about necrotizing lesions in Sjögren's syndrome are presented. Electron microscopic studies disclosed acinar replacement by a mucus-containing lake surrounded by remnant myoepithelial cells. Interepithelial lymphocytes infiltrate the acinar walls in adjacent areas and macrophages bearing mucoid material within cytopasmic vacuoles were found in the interstitium. The significance of these findings in the natural history of Sjögren's syndrome is discussed.

[Biofeedback. I. Current status of its clinical application].

The aim of this article is to restate the question of the clinical application of biofeedback and its prospects after having defined it. After the huge success that biofeedback has known, some decantation is occurring. One is stricken by the absence of methodological closeness of too many studies, in this field, which very much limits the conclusions one can make presently. The best and most reliable results have been obtained by the biofeedback training of electromyogramme (elimination of subvocalization in reading, direct paralyzed muscles training, tension headaches treatment). The results are encouraging, but not yet enough backed up, as to regards to the improvement of various forms of epilepsy by the biofeedback training of the EEG sensorimotor rhythm, the premature ventricular contractions, sinus tachycardia and migraine headaches. In the other fields, the research status does not yet allow any valid conclusion.

[Biofeedback. II. Reflections. Perspectives].

The Biofeedback is a new field in full evolution where the clinical application is still at an experimental stage. But its interest is not only in the prospects which it is offering on the practical side, but also in the theoretical field in the sense of an improvement of our understanding of the rules and mechanisms of the learning at the visceral level. Besides, it is obvious that progress could only be made at the same time on the practical side and in theoretical field, and in a true psychosomatic outlook.
