Effects of training on lactate kinetics parameters and their influence on short high-intensity exercise performance.

The purpose of the present study was to relate the training-induced alterations in lactate kinetics parameters to the concomitant changes in time to exhaustion (T(lim)) at a work rate corresponding to maximal oxygen uptake (Pa(peak)). Eight subjects performed before and after training i) an incremental exercise up to exhaustion to determine Pa(peak), ii) a 5-min 90 % Pa(peak) exercise followed by a 90-min passive recovery to determine an individual blood lactate recovery curve fitted to the bi-exponential time function: La(t) = La(0) + A1(1 - e -gamma1 x t) + A2(1 - e -gamma2 x t), and iii) a time to exhaustion at Pa peak to determine T lim. A biopsy of the vastus lateralis muscle was made before and after training. The training programme consisted in pedalling on a cycle ergometer 2 h a day, 6 days a week, for 4 weeks. Training-induced increases (p < 0.05) in Pa(peak), muscle capillary density, citrate synthase activity, gamma2 that denotes the lactate removal ability (from 0.0547 +/- 0.0038 to 0.0822 +/- 0.0071 min (-1)) and T(lim) (from 299 +/- 23 to 486 +/- 63 s), decreases (p < 0.05) in activities of lactate dehydrogenase (LDH) and muscle type of LDH, the phosphofructokinase/citrate synthase activities ratio and the estimated net amount of lactate released (NALR) during exercise recovery (from 66.5 +/- 8.6 to 47.2 +/- 11.1 mmol) were also observed. The improvement of T (lim) with training was related to the increase in gamma2 (r = 0.74, p = 0.0367) and to the decrease in NALR (r = 0.77, p = 0.0250). These results suggest that the post-training greater ability to remove lactate from the organism and reduced muscle lactate accumulation during exercise account for the concomitant improvement of the time to exhaustion during high-intensity exercise performed at the same relative work rate.

Infant immunization and the occurrence of atopic disease in Dutch and German children: a nested case-control study.

Our goal was to assess the role of early childhood vaccination in the occurrence of respiratory symptoms and allergic sensitization in 7-8-year-old Dutch and German children. A nested case-control study was conducted among children participating in a large longitudinal study on respiratory health, to study the relationship between vaccination (bacille Calmette-Guérin (BCG), pertussis, measles/mumps, rubella, and Haemophilus influenza type b (Hib)) and respiratory symptoms and allergic sensitization. Parents of 510 7-8-year-old children with respiratory complaints and an equal number of randomly selected children without respiratory complaints were asked to complete a questionnaire. Blood samples were collected for specific serum IgE analysis. Vaccination status was assessed through the records of the participating Municipal Health Services. No association between vaccination against pertussis, measles, rubella, or Hib and respiratory symptoms or allergic sensitization was found. For sensitization against house dust mite, BCG vaccination resulted in an increased risk (OR, 2.28; 95% CI, 1.05-4.96). Birth order was inversely associated with allergic sensitization, but was not related to respiratory symptoms. We found an association between BCG vaccination and the subsequent risk for sensitization against house dust mite. No evidence was found for an association between vaccination and respiratory symptoms. Earlier reports of an association of birth order with atopic disease were supported by the results of the present study.

Differences in lactate exchange and removal abilities in athletes specialised in different track running events (100 to 1500 m).

The purpose of this study was to investigate whether track running specialisation could be associated with differences in the ability to exchange and remove lactate. Thirty-four male high-level runners were divided into two groups according to their specialty (100 - 400 m/800 - 1500 m). All performed a 1-min 25.2 km x h -1 event, followed by a 90-min passive recovery to obtain individual blood lactate recovery curves which were fitted to a bi-exponential time function: [La](t) = [La](0) + A 1 (1-e -gamma1t) + A 2 (1-e -gamma2t). The velocity constant gamma 1 which denotes the ability to exchange lactate between the previously worked muscles and blood was higher (p < 0.001) in middle-distance runners than in sprint runners. The velocity constant gamma 2 which reflects the overall ability to remove lactate did not differ significantly between the two groups. gamma 1 was positively correlated with the best performance over 800 m achieved by 16 athletes during the outdoor track season following the protocol (r = 0.55, p < 0.05). In conclusion, the lactate exchange ability seems to play a role on the athlete's capacity to sustain exercise close to 2-min-duration and specifically to run 800 m.

Time to exhaustion at VO(2)max is related to the lactate exchange and removal abilities.

The aim of the present study was to investigate the relationships between lactate exchange and removal abilities and the capacity to prolong exercise, as assessed by the time to exhaustion (Tlim) at a work rate corresponding to VO(2)max (Pa max ). The individual blood lactate recovery curves obtained for 13 untrained subjects after 5 min 90 % Pa max exercise were fitted to the biexponential time function: La(t) = La(0) + A(1) (1-e (-gamma(1) x t) + A(2) (1-e (-gamma(2) x t), where t is time into the recovery, La(0) is the arterialized lactate concentration measured at the end of the exercise, gamma(1) and gamma(2) are velocity constants denoting the lactate exchange and removal abilities, respectively. Tlim was positively related to gamma(1) and gamma(2) (r = 0.60, p < 0.05 and r = 0.56, p < 0.05, respectively) but was negatively related to La(0) (r = 0.75, p < 0.01). gamma()1 was positively related to the capillary density (r = 0.69, p < 0.01) and to the number of capillaries per type I fiber area (r = 0.62, p < 0.05). It was concluded that 1) high lactate exchange and removal abilities would allow continuing a high-intensity exercise for a longer duration, and 2) a high capillary density may explain the associated high lactate exchange ability.

Blood lactate exchange and removal abilities after relative high-intensity exercise: effects of training in normoxia and hypoxia.

The effects of 4 weeks of endurance training in conditions of normoxia or hypoxia on muscle characteristics and blood lactate responses after a 5-min constant-load exercise (CLE) at 90% of the power corresponding to the maximal oxygen uptake were examined at sea-level in 13 sedentary subjects. Five subjects trained in normobaric hypoxia (HT group, fraction of oxygen in inspired gas = 13.2%), and eight subjects trained in normoxia at the same relative work rates (NT group). The blood lactate recovery curves from the CLE were fitted to a biexponential time function: La(t) = La(0) + A1(1 - e- gamma 1.t) + A2(1 - e- gamma 2.t), where the velocity constants gamma 1 and gamma 2 denote the lactate exchange and removal abilities, respectively, A1 and A2 are concentration parameters that describe the amplitudes of concentration variations in the space represented by the arterial blood, La(t) is the lactate concentration at time t, and La(0) is the lactate concentration at the beginning of recovery from CLE. Before training, the two groups displayed the same muscle characteristics, blood lactate kinetics after CLE, and gamma 1 and gamma 2 values. Training modified their muscle characteristics, blood lactate kinetics and the parameters of the fits in the same direction, and proportions among the HT and the NT subjects. Endurance training increased significantly the capillary density (by 31%), citrate synthase activity (by 48%) and H isozyme proportion of lactate dehydrogenase (by 24%), and gamma 1 (by 68%) and gamma 2 (by 47%) values. It was concluded that (1) endurance training improves the lactate exchange and removal abilities estimated during recovery from exercises performed at the same relative work rate, and (2) training in normobaric hypoxia results in similar effects on lactate exchange and removal abilities to training in normoxia performed at the same relative work rates. These results, which were obtained non-invasively in vivo in humans during recovery from CLE, are comparable to those obtained in vitro or by invasive methods during exercise and subsequent recovery.

Local Lyapunov exponents detect epileptic zones in spike-less interictal MEG recordings.

OBJECTIVE: In terms of dynamical system theory the rapid alteration of electromagnetic brain signal properties observed with transition from interictal into ictal epileptic activity implies an alteration between at least two dynamical states (attractors). We explored whether such a multistability is reflected also in the dynamical characteristics of the interictal signal. METHODS: A combined method consisting of structural MRI, multichannel magnetoencephalography (MEG) and the non-linear dynamics was applied for the detection of subthreshold interictal activity in temporal lobe epilepsy. Employing multichannel MEG we calculated local Lyapunov exponents (ILE) as a measure of the probability of state transitions in patients with mesiotemporal and extra-mesiotemporal focal epilepsy. RESULTS: The spatial distribution of the variability of ILE was found to be correlated to the side of the epileptic focus, thus, providing a reliable estimate of its localization. CONCLUSION: Our results demonstrate that interictal epileptic activity is characterized by an increase of the number of transitions between different attractors.

[Choice of infusion site--differences in the references of staff (nurses and residents) and patients].

Insertion of an intravenous (i.v.) line is a common and routine procedure in hospitalized patients. The literature recommends not to insert an i.v. line in a limb fold, but other than that there are no guidelines regarding optimal locations for the i.v. line. In this study we attempted to elucidate whether there are other possible preferences and guidelines for choosing the location of an i.v. line. We studied four groups: patients at the time of hospitalization, hospitalized patients, nurses and surgical interns on a surgical ward. We found that 91.8% of the patients request to take part in choosing the location of their i.v. line. Moreover, we found that most patients have a hand preference for the i.v., whereas, the ward staff has little location preference. Despite these findings, we discovered that, in practice, most patients were not asked to participate in the decision making process regarding their i.v. line location. Analysis of the results showed that there is a need to include the patient's preference in choosing the location for the i.v. line, whenever possible. If the patient doesn't have a preference, the i.v. line should be inserted in the non-dominant hand or the hand in the bed which is more accessible to the medical & nursing staff.

[Residents training in gynecologic operative endoscopy: a review of Israeli and world experience].

Two decades ago, Gynecologic procedures performed by endoscopy were basically limited to diagnostic procedures. Since 1990, operative endoscopy is increasingly used to treat a growing variety of gynecologic conditions, resulting in shorter hospital stay and smaller, less painful incisions. One of the greatest tasks facing today's academic gynecologic surgeons is training residents and attendant physicians to perform endoscopic surgery adequately and safely. Clearly, there is a need to provide basic knowledge of endosurgical principles, and understanding of equipment and technologies used in operative endoscopy. Also, information on minimization, early recognition and treatment of endoscopic complications is of great importance as well as hands-on experience in these new procedures. We have reviewed the world as well as the Israeli experience in the field and out of this growing base of knowledge we have tried to delineate and forge guidelines for adequate teaching, training and accreditation.

Bridging the pressure and materials gaps: high pressure sum frequency generation study on supported Pd nanoparticles

Infrared-visible sum frequency generation vibrational spectroscopy is applied for the first time to monitor CO stretching vibrations on alumina supported Pd nanoparticles in a pressure range from 10(-7) to 200 mbar. The adsorption behavior of Pd aggregates with 3 and 6 nm mean size is dominated by surface defects and two different adsorption sites (twofold bridging and on-top) were identified. The CO adsorption site occupancy on Pd nanocrystals is mainly governed by the gas phase pressure while the structure of the particles and their temperature have a smaller influence.

Photon emission spectroscopy of individual oxide-supported silver clusters in a scanning tunneling microscope

Photon emission spectra of individual alumina-supported silver clusters have been measured for the first time. The light emission stimulated by electron injection from the tip of a scanning tunneling microscope can be assigned to the (1,0) mode of the Mie-plasmon resonance in small silver particles. As cluster sizes decrease, the resonance position shifts to higher energies and the linewidth increases. In the size range examined (1.5-12 nm), intrinsic size effects are discussed as possible origins for the observed size dependence of the Mie resonance.

Cardiorespiratory and metabolic responses to exercise in HbSC sickle cell patients.

PURPOSE: Relative to healthy control individuals with normal hemoglobin (Hb), patients carrying the double heterozygous form of sickle cell disease (HbSC) display an impaired oxygen transport capacity. The present study was undertaken to determine the influence of the decreased oxygen availability associated with the presence of HbSC on the cardiorespiratory and metabolic responses to endurance exercise. METHODS: Eleven black men affected by the double heterozygous form of the sickle cell disease (HbSC group) and seven healthy subjects with normal Hb (HbAA group) of the same ethnic origin submitted successively to an incremental exercise test to exhaustion on a cycle ergometer for the determination of their maximal tolerated power and to a 20-min endurance exercise. RESULTS: The HbSC had a significantly lower exercise tolerance than the HbAA. During the endurance exercise, they exhibited furthermore significantly lower VO2, VCO2, and minute ventilation V(E) than the HbAA. Despite the fact that the HbSC exercised at a significantly lower mean absolute work rate than the HbAA, except for the ventilatory equivalent for CO2 (V(E)/VCO2), which was higher (P < 0.001) in the HbSC group, the other parameters recorded during the 20-min endurance exercise (heart rate, arterial PaO2, PaCO2, pH, lactate, and VE/VO2, the ventilatory equivalent for O2) and during the subsequent recovery (blood lactate) were similar for both groups. CONCLUSION: The study underscores the importance of considering relative work rate as well as absolute work rate to arrive at a correct interpretation of exercise and recovery data. The results give evidence that the modifications of homeostasis brought into play by exercise were shifted toward distinctly lower absolute work rates in HbSC patients.

Native language, gender, and functional organization of the auditory cortex.

Whole-head magnetoencephalography was employed in 40 normal subjects to investigate whether the basic functional organization of the auditory cortex varies with linguistic environment. Robust activations of the bilateral supratemporal auditory cortices to 1-kHz pure tones, maximum at about 100 ms after stimulus onset, were studied in Finnish and German female and male subject groups with monolingual background. Activations elicited by the tones were mutually indistinguishable in German and Finnish women. In contrast, German men showed significantly stronger auditory responses to pure tones in the left, language-dominant hemisphere than Finnish men. We discuss the possibility that the prominent left-hemisphere activation in German males reflects higher frequency resolution required for distinguishing between German than Finnish vowels and that the clear effect of native language in male but not in female auditory cortex derives from more pronounced functional lateralization in men. The present data suggest that the influence of native language can extend to auditory cortical processing of pure-tone stimuli with no linguistic content and that this effect is conspicuous in the male brain.

Measuring in vivo myelination of human white matter fiber tracts with magnetization transfer MR.

Precise characterization of white matter pathways is important for the understanding of structural-functional relationships in the human brain. While it is known from postmortem studies that the connectivity of cortical areas is conveyed by projection, commissural, and association fibers, most clinical studies disregard useful information about specific fiber tracts. Magnetization transfer (MT) MR detects the relative proportion of free mobile protons and immobile protons bound to macromolecules. MT values correlate with histopathology and it has been proposed that in the white matter, the amount of magnetization transfer correlates with the degree of myelination. Thus, MT-MR provides measures that may reflect more accurately the physiology and natural course of diseases involving the white matter. We applied this quantitative in vivo method to five children at different ages to determine whether the maturational changes of distinct fiber tracts could be measured. All regions of interest were localized by means of Brodmann's original descriptions and the additional use of reconstructed 3D-matched data from 10 myelin-stained human brain specimens. With this atlas-guided approach we localized and measured 26 supratentorial white matter fiber tracts in each hemisphere, connecting to primary as well as association cortices. All fiber tracts showed consistent age-related MT changes and the strongest effects were found in those tracts projecting to the primary cortical areas. These results suggest that our method is suitable for in vivo MT measurements in specific fiber tracts. It can provide data that relate to myelination during ontogenesis or myelination delays in myelin disorders. In the clinical domain, the focus on specific fiber tracts appears to be advantageous over standard approaches, because such system of parcellation is based on the functional anatomy of the white matter. Consequently, it may be especially useful for topical analysis in neurology allowing the assessment of the functional consequences of white matter damage as well as the effectiveness of treatments in patients with any lesion that can be visualized by MRI.

A fronto-parietal circuit for object manipulation in man: evidence from an fMRI-study.

Functional magnetic resonance imaging (fMRI) was used to localize brain areas active during manipulation of complex objects. In one experiment subjects were required to manipulate complex objects for exploring their macrogeometric features as compared to manipulation of a simple smooth object (a sphere). In a second experiment subjects were asked to manipulate complex objects and to silently name them upon recognition as compared to manipulation of complex not recognizable objects without covert naming. Manipulation of complex objects resulted in an activation of ventral premotor cortex [Brodmann's area (BA) 44], of a region in the intraparietal sulcus (most probably corresponding to the anterior intraparietal area in the monkey), of area SII and of a sector of the superior parietal lobule. When the objects were covertly named additional activations were found in the opercular part of BA 44 and in the pars triangularis of the inferior frontal gyrus (BA 45). We suggest that a fronto-parietal circuit for manipulation of objects exists in humans and involves basically the same areas as in the monkey. It is proposed that area SII analyses the intrinsic object characteristics whilst the superior parietal lobule is related to kinaesthesia.

Etiology and pathogenetic mechanisms of optic disc swelling with visual loss. An interdisciplinary prospective pilot study of 102 cases.

A nonrandomized, prospective, interdisciplinary pilot study of 102 patients with noncompressive optic disc swelling with visual loss (ODSWVL) was performed in order to investigate etiologic and pathogenetic mechanisms. Forty-six patients suffered from underlying inflammatory disease. Seventeen patients suffered from highly probable cardiogenic embolization, 16 patients from multiple vascular risk factors. The remaining patients of the noninflammatory disease group suffered from leukemia, previously unknown or severely decompensated diabetes mellitus, acute arterial hypertension, different kinds of coagulopathies and others. Ninety-six of the 102 patients required medical treatment according to general medical standards. Inhomogeneity of the underlying disease processes explains the ineffectiveness of different monotherapies in previous studies. Interdisciplinary search for the underlying causes allows causative treatment. ODSWVL and anterior ischemic optic neuropathy in particular seem to be a common final pathway of various pathogenetic mechanisms due to different etiologies rather than a disease entity by itself.

Handedness and asymmetry of hand representation in human motor cortex.

The cortical representation of five simple hand and finger movements in the human motor cortex was determined in left- and right-handed people with whole-head magnetoencephalography. Different movements were found to be represented by spatially segregated dipolar sources in primary motor cortex. The spatial arrangement of neuronal sources for digit and wrist movements was nonsomatotopic and varied greatly between subjects. As an estimator of hand area size in primary motor cortex, we determined the smallest cuboid volume enclosing the five dipole sources within the left and right hemisphere of each subject. Interhemispheric comparison revealed a significant increase of this volume in primary motor cortex opposite to the preferred hand. This asymmetry was due to a greater spatial segregation of neuronal dipole generators subserving different hand and finger actions in the dominant hemisphere. Mean Euclidean distances between dipole sources for different movements were 10.7 +/- 3.5 mm in the dominant and 9.4 +/- 3.5 mm in the nondominant hemisphere (mean +/- SD; P = 0. 01, two-tailed t-test). The expansion of hand representation in primary motor cortex could not simply be attributed to a greater number of pyramidal cells devoted to each particular movement as inferred from current source amplitudes. The degree of hemispheric asymmetry of hand area size in the primary motor cortex was correlated highly with the asymmetry of hand performance in a standardized handedness test (r = -0.76, P < 0.01). These results demonstrate for the first time a biological correlate of handedness in human motor cortex. The expansion of hand motor cortex in the dominant hemisphere may provide extra space for the cortical encoding of a greater motor skill repertoire of the preferred hand.

Lactate exchange and removal abilities in rowing performance.

The relationships between individual performance and lactate exchange and removal abilities were studies in 12 male rowers all subjected to three measurements on a rowing ergometer. An incremental exercise carried out to determine the maximal oxygen uptake (VO2max) and the corresponding maximal aerobic power (Pamax), a 2500-m all-out test where the mean work rate (P2500) represented the individual performance, and a 6-min 90% Pamax exercise designed to assess the lactate kinetics during the following 90 min passive recovery were performed. The lactate recovery curves were fitted to the bi-exponential time function: La(t) = La(O) + A1(1-e-gamma 1.t) + A2(1-e-gamma 2.t). The velocity constants gamma 1 and gamma 2 denote the lactate exchange and removal abilities, respectively. The mean value of P2500 sustained by the rowers was 376 +/- 41W (106 +/- 5% of Pamax (P2500%). P2500 was positively correlated with gamma 2 (P < 0.05). gamma 1 and gamma 2 explained 67% of the P2500 variance. P2500% was also correlated with gamma 2 (P < 0.01). These results suggest that a better performance on the rowing ergometer is associated with improved lactate exchange and removal abilities. Furthermore, the ability to row at high relative work rates was correlated with an increased lactate removal ability. Training-induced adaptations could explain the high gamma 1 and gamma 2 displayed by the present rowers.

Short endurance training improves lactate removal ability in patients with heart transplants.

Eight male patients with heart transplants at least a year after the operation were submitted to a 6-wk endurance training program and explored for their blood lactate kinetics before and after exercise. The tests consisted of a bicycle exercise upgraded by 20 W every 2 min until volitional fatigue. Training induced a significant (P < 0.025) decrease in lactate concentrations from the 40-W to the 120-W exercise step and a significant increase (P < 0.025) in the time into exercise (9.87 +/- 0.87 min vs 7.17 +/- 0.90 min) at which a lactate concentration of 2 mmol.l-1 was reached. Lactate recovery curves were significantly lower (P < 0.036) after training than before training, except at minutes 1, 2, 8, and 60. The fits of a biexponential mathematical model to the lactate recovery curves reveal a significant (P < 0.036) training-induced increase (+71%) in the slow-velocity constant gamma 2v of the model. In view of the functional meaning given to this parameter, namely the ability to remove lactate, it is concluded that training lowers blood lactate concentrations during exercise and recovery in patients with heart transplants at least in part by raising the efficiency with which lactate is removed, and that the ability to remove lactate can be a valuable criterion to evaluate physical fitness.
