[Research in general practice. Plea for organized and high quality clinical research by general practice physicians in Switzerland].

The clinical research in the practitioner's office is indispensable for the teaching of the medicine of first recourse. On the basis of the analysis of 109 publications, the authors propose to structure this research under the shape of investigator's networks while guaranteeing an interface between the primary care physicians and the academic specialists. To realise this network, an institutional support is necessary.

Dietary influence on the urinary excretion of polyamines.

The amino groups of amino acids, the constituents of proteins, are catabolized in the urea cycle. One intermediate of this cycle, ornithine, is a precursor molecule of polyamines. The influence of dietary protein intake on the production and excretion of polyamines in the urine is yet unclear. The aim of this study was to investigate the excretion of polyamines in the urine following three days of creatine-free, creatine-free and low-polyamine diet in four persons. On the fourth day they were loaded with creatine-free, creatinine-free and low-polyamine high-protein diet (80 g/70 kg body weight). High-protein diet resulted in no increase of urinary polyamine excretion. The low-polyamine diet caused a non-significant decrease in urinary polyamine excretion (by 15%).

[Oral anticoagulants in clinical practice].

Oral anticoagulation (OA) in ambulatory practice is a controversial issue, and the efficacy and safety of its performance is an essential condition for the use of such sophisticated therapy in this setting. The efficacy and use of OA was studied in 24 participating practices over one year (1.4 1987 to 31.3. 1988). Overall, 538 patients were treated by OA, accounting for 22.4 patients/medical practice. OA was initiated in 207 patients, while 287 patients were on longterm (greater than 2 years) anticoagulation therapy. Most patients had polymorbidity, and many presented several indications for OA. On average, thromboplastin time was determined 2.7 times in the first 2 weeks of therapy and every 21 days during the whole period. 72% of measured thromboplastin times were within the target and 90.6% within the therapeutic range. Bleeding complications occurred in 38 patients (7.6%; 11.6/100 treatment years). OA had to be interrupted only 14 times (2.6%; 4.3/100 treatment years), and 7 patients (2.3%; 2.3/100 treatment years) were admitted to hospital. 6 patients died, as compared with 7.5 deaths expected from national mortality rates; none died from complications of OA. Based on these results it can be concluded that OA was highly effective in the 24 participating practices and had a low rate of complications.

[Morbidity detection in general practice].

A system of morbidity registration in the domain of primary health care is presented on the basis of experiences made abroad. A pilot project (Sentinella) functions in the environments of Berne since November 1984. Its extension to the whole of Switzerland is scheduled for the middle of 1986.
