Chemiluminescence and immune cell activation. I. Early activation of rat thymocytes can be monitored by chemiluminescence measurements.

Immediately after the addition of concanavalin A, rat thymocytes respond, in the presence of luminol, with a burst of chemiluminescence (CL) that can be conveniently monitored in an ordinary liquid scintillation spectrometer. Peak CL is reached after 50 sec. Addition of catalase suppresses 65% of the CL, suggesting that H2O2 generation may be its major source. CL with different kinetic characteristics can also be generated by the calcium ionophore A23187. Our rat thymocytes contain approximately 0.1% endogenous macrophages. Bone marrow-derived rat macrophages also respond to concanavalin A or A23187 stimulation with a burst of CL. However, the kinetic properties of CL as well as the inhibition of CL by catalase in these cells differ markedly from those of thymocyte preparations, suggesting that the major portion of the CL in rat thymocytes actually originates in T lymphocytes. Because CL measurements allow the monitoring of very early events in cell activation and because of the simplicity of the technique, CL measurements may become a useful method for the study of lymphocyte activation, of macrophage-lymphocyte interactions, as well as for the rapid screening for specificity and reactivity of T lymphocyte populations.

[Diagnosis and therapy of megaloblastic anemias].

In most cases megaloblastic anaemias are the sequel of a deficiency of vitamin B12, more infrequently of a deficiency of folic acid of different etiology. Oriented to frequency and anamnesis the diagnostics follows the leading symptoms of pernicious anaemia (straw colour, glossitis, achlorhydria) and on the basis of special findings in the peripheral blood (hyperchromacia, megalocytosis, much decreased number of reticulocytes, increased iron and bilirubin level) it leads to the proved suspicion of a megaloblastic anaemia. This suspicion is ascertained by the investigation of the bone-marrow, with the help of aimed investigations the anaemia is further clarified differential-diagnostically. An unclear anaemia should not be treated ex juvantibus with vitamin B12 and/or folic acid. The therapy, always taking into consideration a possible basic disease, is carried out by parenteral application of vitamin B12, possibly in form of hydroxocobalamine or by folic acid. In persisting disturbance of the resorption of vitamin B12 on account of the threatening complication of a funicular spinal disease the long-term therapy must never be interrupted, unless in normal haematological findings.

[Concerning the casuistics by aspiration of waste oil sediments (author's transl)].

When cleaning an underground waste oil dump, an 18 year old worker suffered loss of consciousness by inhaling vapours of trichloroethylene and then suffocated by aspiration of waste oil sediments. Histological examination of the lungs proved the copious presence of partly granulous darkish-brown and of rather light coloured crystalloid foreign substances up to the multiple size of erythrocytes. "Chemical examination of blood, brain and urine showed the presence of trichloroethylene and its metabolites, respectively. The trichloroethylene concentration has been determined in the brain."

[Leukocyte agglomeration as an activity criterion of inflammatory processes].

The agglomeration of leucocytes serves for the proof of activation of the leucocytic system, the so-called leucergia, which among other occurs in inflammatory diseases of different genesis. On the basis of 1,000 examinations in patients with rheumatoid arthritis the sensitivity of the method was tested compared with the usual criteria of inflammation. In agroup of patients with slight activity with the agglomeration of leucocytes an inflammatory activity could be proved in 67.5% (electrophoresis 53.5%, BSR 49.2%, CrP 35.4%, leucocytosis 27.9%). The chi2-test resulted in a significant correspondence between the examinations mentioned and the agglomeration (p less than 0.001). In contrast to the usual criteria of activity the proof of leucergy is a new principle ofthe diagnostics of activity. High sensitivity and simple methodics recommend the use of the agglomeration of leucocytes in routine diagnostics.
